"Quacking for Noggin": Farm Animal–Assisted Therapy for Traumatic Brain Injury Survivors

<p>The aim of this study was to examine the effects of animal-assisted therapies with farm animals (AATF), in this case domesticated ducks, on depression, anxiety, and self-efficacy in patients with traumatic brain injury (TBI). Furthermore, the following hypothesis was tested: Engaging in AATF with domesticated ducks will be associated with decreased depression, decreased anxiety, and increased self-efficacy. The study examined the effects of AATF on anxiety, depression, and self-efficacy among nine patients with TBI. A time series quasi-experimental design was utilized. Participants completed the Hospital Anxiety and Depression Survey (HADS) and General Self-Efficacy (GSE) questionnaires at baseline, followed by the AAFT intervention. The AAFT intervention consisted of two one-hour sessions interacting with ducks every week for 12 weeks. Participants repeated baseline measures immediately following the intervention, and again four weeks post intervention to evaluate the residual effects of the intervention. A general linear model was employed to examine changes in anxiety, depression, and selfefficacy. Participants&rsquo; anxiety scores decreased significantly from baseline to post intervention measure (p = .009); however, there were no statistically significant differences between anxiety levels immediately post intervention and four weeks later. There were no statistically significant differences in depression or self-efficacy levels at pre-, post-, and retest. Our study hypothesis was partially supported in that statistically significant decreases in anxiety were observed from baseline to immediate posttest. Mastery of skills, vicarious experiences, and verbal persuasion may be the factors that contributed to the beneficial outcomes of the interactions between persons with TBI and domesticated ducks</p>

Trajectory Based Classification of Recovery In Sensorimotor Complete Traumatic Cervical Spinal Cord Injury – Individual Recovery Trajectory Data

<p><b>Objective</b></p> <p>To test the hypothesis that sensorimotor complete traumatic cervical spinal cord injury is a heterogenous clinical entity comprising several subpopulations that follow fundamentally different trajectories of neurological recovery.</p> <p><b>Methods</b></p> <p>We analyzed demographic and injury data from 655 patients who were pooled from four prospective longitudinal multicenter studies. Group based trajectory modeling was applied to model neurological recovery trajectories over the initial 12-months postinjury and to identify predictors of recovery trajectories. Neurological outcomes included: Upper Extremity Motor Score, Total Motor Scores and AIS grade improvement.</p> <p><b>Results  </b></p> <p>The analysis identified three distinct trajectories of neurological recovery. These clinical courses included: (1) Marginal recovery trajectory: characterized by minimal or no improvement in motor strength or change in AIS grade status (remained grade A); (2) Moderate recovery trajectory: characterized by low baseline motor scores that improved approximately 13 points; or AIS conversion of one grade point; (3) Good recovery trajectory: characterized by baseline motor scores in the upper quartile that improved to near maximum values within three months of injury. Patients following the moderate or good recovery trajectories were of younger age, had more caudally located injuries, a higher degree of preserved motor and sensory function at baseline examination and exhibited a greater extent of motor and sensory function in the zone of partial preservation.</p> <p><b>Conclusion</b></p> <p>Cervical complete SCI can be classified into one of three distinct subpopulations with fundamentally different trajectories of neurological recovery. This study defines unique clinical phenotypes based on potential for recovery, rather than baseline severity of injury alone. This approach may prove beneficial in clinical prognostication and in the design and interpretation of clinical trials in SCI.</p>

Head injuries, traumas, and neuropsychiatric sequela in asylum seekers

<p><b>Importance: </b>Asylum seekers experience a high burden of physical and psychological trauma, yet there is a scarcity of literature regarding the epidemiology and sequelae of head injury (HI) in asylum seekers.</p> <p><b>Objective: </b>To examine head injury prevalence and related neuropsychiatric comorbidities in asylum seekers.</p> <p><b>Methods: </b>A<b> </b>retrospective cross-sectional study through review of 139 medical affidavits from an affidavit database was performed. Affidavits written from 2010 – 2018 were included. Demographic and case-related data were collected and classified based on the presence of head injury. <a>Primary study outcome measurement was prevalence of head injury and headache. Secondary outcomes included the neuropsychiatric sequelae of head injury including depression, PTSD, anxiety, memory/concentration deficits, and the prevalence of abusive and traumatic history. Hypothesis and outcome measures were determined before data extraction.</a></p> <p><b>Results: </b>One hundred and thirty-nine medical affidavits of asylum seekers were included in the study. The mean age was 27.4±12.1, 56.8% identified as female, 28.1% were &lt;18 years. Almost half (42.5%) explicitly self-reported history of head injury. Compared to clients who did not report head injury, clients with head injury were older (31.4 vs 24.4, 7.0 year difference; 95%CI [2.9, 10.9 years]) and more likely to report a history of headache (62.7% vs 28.8%, 33.9% difference; 95%CI [20.0, 47.8%], p&lt;.001), physical abuse (91.5% vs 65.0%, 26.5% difference; 95%CI [12.8, 38.5%];), physical trauma (96.6% vs 66.3%, 30.4% difference; 95%CI [17.9, 41.5%]), concussion (22.0% vs 0.0%, 22% difference; 95%CI [12.2, 34.1%]), and loss of consciousness (27.1% vs 0.0%, 27.1% difference; 95%CI [16.4, 39.6%]). Moreover, clients with head injuries were more likely to suffer neuropsychological sequelae such as memory/concentration deficits (45.8% vs 28.8%, 17.0% difference; 95%CI [0.8, 33.2%]), and depression (83.1% vs 65%, 18.1% difference; 95%CI [3.0, 33.2%]).</p>

Data from: Human-relevant mechanisms and risk factors for TAK-875-induced liver injury identified via a gene pathway-based approach in collaborative cross mice

<p>Development of TAK-875 was discontinued when a small number of serious drug-induced liver injury (DILI) cases were observed in Phase 3 clinical trials. Subsequent studies have identified hepatocellular oxidative stress, mitochondrial dysfunction, altered bile acid homeostasis, and immune response as mechanisms of TAK-875 DILI and the contribution of genetic risk factors in oxidative response and mitochondrial pathways to the toxicity susceptibility observed in patients. We tested the hypothesis that a novel preclinical approach based on gene pathway analysis in the livers of Collaborative Cross mice could be used to identify human-relevant mechanisms of toxicity and genetic risk factors at the level of the hepatocyte as reported in a human genome-wide association study. Eight (8) male mice (4 matched pairs) from each of 45 Collaborative Cross lines were treated with a single oral (gavage) dose of either vehicle or 600 mg/kg TAK-875. As expected, liver injury was not detected histologically and few changes in plasma biomarkers of hepatotoxicity were observed. However, gene expression profiling in the liver identified hundreds of transcripts responsive to TAK-875 treatment across all strains reflecting alterations in immune response and bile acid homeostasis and the interaction of treatment and strain reflecting oxidative stress and mitochondrial dysfunction. Fold-change expression values were then used to develop pathway-based phenotypes for genetic mapping which identified candidate risk factor genes for TAK-875 toxicity susceptibility at the level of the hepatocyte. Taken together, these findings support our hypothesis that a gene pathway-based approach using Collaborative Cross mice could inform sensitive strains, human-relevant mechanisms of toxicity, and genetic risk factors for TAK-875 DILI. This novel preclinical approach may be helpful in understanding, predicting, and ultimately preventing clinical DILI for other drugs.</p>
