Optimizing Outcomes in Phacoemulsification: Addressing Intraoperative and Postoperative Complications in a Case Series from Iraq

Comparing full-field data from structural components with complicated geometries

<p>A new decomposition algorithm based on QR factorisation is introduced for processing and comparing irregularly shaped stress and deformation datasets found in structural analysis. The algorithm improves the comparison of two-dimensional data fields from the surface of components where data is missing from the field of view due to obstructed measurement systems or component geometry that results in areas where no data is present. The technique enables the comparison of these irregularly shaped datasets without the need for interpolation or warping of the data. This ensures comparisons are only made between the available data in each dataset and thus similarity metrics are not biased by missing data. The decomposition and comparison technique has been applied during an impact experiment, a modal analysis, and a fatigue study, with the stress and displacement data obtained from finite element analysis, digital image correlation and thermoelastic stress analysis. The results demonstrate that the technique can be used to process data from a range of sources and suggests the technique has the potential for use in a wide variety of applications.</p>

Clinical characteristics, risk factors and complications of COVID-19 among critically ill older adults – A case control study

<p><strong>Background</strong>: The older population is often disproportionately and adversely affected during humanitarian emergencies, as has also been seen during the COVID-19 pandemic. Data regarding COVID-19 in older adults is usually over-generalised and does not delve into details of the clinical characteristics in them. This study was conducted to analyse clinical and laboratory characteristics, risk factors, and complications of COVID-19 between older adults who survived and those who did not.</p> <p><strong>Methods</strong>: We conducted a case-control study among older adults(age &gt; 60 years) admitted to the Intensive Care Unit(ICU) during the COVID-19 pandemic. The non-survivors (cases) were matched with age and sex-matched survivors (control) in a ratio of 1: 3. The data regarding socio-demographics, clinical characteristics, complications, treatment, laboratory data, and outcomes were analysed.</p> <p><strong>Results</strong>: The most common signs and symptoms observed were fever (cases vs controls) (68.92 vs. 68.8%), followed by shortness of breath (62.2% Vs. 52.2%), and cough (47.3% Vs. 60.2%). Our analysis found no association between the presence of any of the comorbidities and mortality. At admission, laboratory markers such as LDH(Lactate Dehydrogenase), WBC(White Blood Count), creatinine, CRP(C-Reactive Protein), D-dimer, ferritin, and IL-6(Interleukin-6) were found to be significantly higher among the cases than among the controls. Complications such as development of seizure, bacteremia, acute renal injury, respiratory failure, and septic shock were seen to have a significant association with non-survivors.</p> <p><strong>Conclusions</strong>: Hypoxia, tachycardia, and tachypnoea at presentation were associated with higher mortality. The older adults in this study mostly presented with the typical clinical features of COVID-19 pneumonia. The presence of comorbid illnesses among them did not affect mortality. Higher death was seen among those with higher levels of CRP, LDH, D-dimer, and ferritin; and with lower lymphocyte counts.</p>

Tunneled antibiotic-impregnated versus bolt-connected non-coated external ventricular drainages: a comparison of complications.

<p><strong>Background:&nbsp;</strong>External ventricular drainage (EVD) is a common emergency neurosurgical procedure, but it is not free of adverse events. The aim of this study is to compare the complication rate of two frequently used EVD types: tunneled antibiotic-impregnated catheters (Bactiseal&Oacute;) and bolt-connected non-coated devices (Camino&Oacute;).</p> <p><strong>Methods:</strong>&nbsp;All EVDs placed between March 1<sup>st</sup>, 2015 and December 31<sup>st</sup>, 2017 were registered. Procedures performed with any catheter different from Bactiseal&Oacute;&nbsp;or Camino&Oacute;&nbsp;EVD, with incomplete follow-up and those EVDs placed due to an infectious disease were excluded. Demographic and clinical variables were collected, as well as overall complication rate (infection, hemorrhage, obstruction, malposition of the catheter, involuntary pull-out of the device) and need of replacement of the EVD.&nbsp;</p> <p><strong>Results:&nbsp;</strong>A total of 77 EVDs were finally considered for analysis (40 Bactiseal&reg;, 37 Camino&reg;). There was a statistically significant difference in diagnosis and also in the location of the procedure, as more bolt-connected EVD were place outside the operating room (97.3% vs 23.5%,&nbsp;<em>p</em>&lt;0.001) due to emergent pathologies like vascular diseases and spontaneous hemorrhages. In the univariate analysis, a statistically significant higher rate of catheter involuntary pull-out (29.7% vs 7.5%,&nbsp;<em>p</em>=0.012) and need of EVD replacement (32.4% vs 12.5%,&nbsp;<em>p</em>=0.035) was found in the Camino cohort. However, those differences could not be confirmed with multivariable analysis, which showed no association between the type of catheter and any of the studied complications. Ventriculostomy duration was identified as a risk factor for infection (OR 1.09, CI 95% 1.02-1.18).</p> <p><strong>Conclusion:&nbsp;</strong>No significant differences were observed regarding infection, hemorrhage, obstruction, malposition, involuntary catheter pull-out, and need for EVD replacement when comparing non-impregnated bolt-connected EVDs (Camino&reg;) with tunneled antibiotic-impregnated catheters (Bactiseal&reg;). Duration of EVD was associated with an increased risk of infection.&nbsp;</p>
