BMP/SMAD1/5 signaling in the endometrial epithelium is essential for receptivity and early pregnancy

<p>The biological processes that control endometrial receptivity and embryo implantation are critical for the successful outcome of pregnancy. The endometrium is the complex inner lining of the uterine wall that is under the cyclical control of estrogen and progesterone and is a site of intimate contact between mother and blastocyst. The bone morphogenetic signaling (BMP) pathway is a highly conserved signaling pathway that controls key cellular processes throughout pregnancy and exerts intracellular effects via the SMAD1/5 transcription factors. To delineate the endometrial compartment–specific roles of BMP signaling, we generated mice with epithelial-specific conditional deletion of SMAD1/5 using Lactoferrin-icre (Smad1flox/flox;Smad5flox/flox;Lactoferrin-cre, "Smad1/5 cKO"). Histological analysis of the reproductive tracts showed that Smad1/5 cKO mice were developmentally normal and displayed no defects in glandular morphology. In fertility analyses, single SMAD1 or SMAD5 deletion had no effect on fertility; however, double-conditional deletion of SMAD1 and SMAD5 resulted in severe subfertility. Timed mating analyses revealed endometrial receptivity defects in the Smad1/5 cKO mice beginning at 3.5 days post coitum (dpc) that perturbed embryo implantation at 4.5 dpc, as demonstrated by the detection of unattached blastocysts in the uterus, decreased COX2 expression, and FOXO1 cytoplasmic mislocalization. We also found that defects that arose during peri-implantation adversely affected embryonic and decidual development at 5.5 and 6.5 dpc. Thus, uterine epithelial BMP/SMAD1/5 signaling is essential during early pregnancy and SMAD1/5 epithelial-specific deletion has detrimental effects on stromal cell decidualization and pregnancy development.</p>

Pregnancy-induced hypertension and its associated risk factors among pregnant garment workers attending an urban health center in Bangladesh

<p><strong><span>ABSTRACT</span></strong></p> <p><strong><span>Background: </span></strong><span>Pregnancy-induced hypertension (PIH) can lead to severe morbidity, long-term disability, and mortality among mothers and their infants<strong>. </strong>PIH is increasing worldwide, disproportionately affecting more mothers in developing countries. <span>&nbsp;</span><span>This study aims to determine the prevalence of PIH and its associated risk factors among pregnant garment workers attending an urban health center in Bangladesh</span>.</span></p> <p><strong><span>Methods:</span></strong><span> This cross-sectional study was conducted among pregnant garment workers at &ge;20 weeks of gestation, who visited a health center of a garment in Narayanganj, Bangladesh between July and October 2023. <span>Data on socio-demographics, clinical characteristics including body weight and blood pressure, and family history of chronic diseases were gathered. </span>Logistic regression was done to calculate crude and adjusted odds ratios (OR) to identify factors associated with PIH.</span></p> <p><strong><span>Result:</span></strong><span><span>&nbsp; </span><span>With an average age of 27.3 years (standard deviation 3.2) and nearly two-thirds of the 150 participants having completed secondary education or higher, the mean gestational age was 21.7 weeks. One out of ten had a family history of high blood pressure and 2% had a family history of PIH. The overall prevalence of PIH was 7.3%. Higher risk for PIH was linked to family history of PIH (aOR 18.1; 95% CI 1.3, 251.3). </span></span></p> <p><strong><span>Conclusion:</span></strong><span> <span>The study identified that one out of every 14 pregnant garment workers had PIH. The findings highlight that a family history of PIH significantly increases the likelihood of developing PIH in this population. Therefore, pregnant women with this risk factor should be prioritised by health care professionals. </span></span></p>

Features of the tactics of pregnancy and childbirth in pneumonia

<p><span><span>Несмотря на достижения современного акушерства, пневмония, как причина заболеваемости и смертности среди беременных женщин, продолжает оставаться важной проблемой. Цель: оценить тактику лечебных мероприятий у беременных женщин с пневмонией по триместрам. Материал и методы исследования:. 85 беременных с верифицированным диагнозом пневмония были разделены на 3 группы : I-ую &ndash; составили 35 (41,1%) женщин в I триместре (до 13 недель) беременности; II-ую &ndash;25 (29,4%) беременных во II триместре (в сроке 13-22 недель); III-ью &ndash; 25(29,4%)<span>&nbsp; </span>пациенток в III триместре беременности. Результаты. Наиболее тяжелую группу составили беременные в III группы.У 3 (15%) беременных в сроке 31&mdash; 32 недели беременности было обострение пневмонического статуса, развилась острая дыхательная недостаточность, отек легкого. Выводы. После консервативного лечения 70,6% беременных со сроком гестации до 22 недель и<span>&nbsp; </span>17,7% беременных в сроки 32-33 недели были выписаны в удовлетворительном состоянии.. В срок при спонтанной родовой деятельности родоразрешены через естественные родовые пути 11,8%, по акушерским показаниям<span>&nbsp;&nbsp; </span>оперативное родоразрешение произведено у 5,9% больных пневмонией, в послеоперационном периоде летальный исход произошел в 3,5% случаев.<span>&nbsp; </span>Пневмония у беременных является угрожающим для жизни состоянием, наиболее опасной для жизни является тяжелая пневмония, развившаяся в поздние сроки III триместра беременности, перед родоразрешением</span></span><span>.</span></p> <p><strong><span>Ключевые слова:</span></strong><span> беременность, пневмония, антикоагулянтная терапия.</span></p>

Data from: Characteristics and outcomes of women utilizing emergency medical services for third-trimester pregnancy-related complaints in India: a prospective observational study

Objectives: Characterize the demographics, management, and outcomes of obstetric patients transported by emergency medical services (EMS). Design: Prospective observational study. Setting: Five Indian states utilizing a centralized EMS agency that transported 3.1 million pregnant women in 2014. Participants: This study enrolled a convenience sample of 1684 women in third trimester of pregnancy calling with a "pregnancy-related" complaint for free-of-charge ambulance transport. Calls were deemed "pregnancy-related" if categorized by EMS dispatchers as "pregnancy", "childbirth", "miscarriage", or "labor pains". Interfacility transfers, patients absent upon ambulance arrival, and patients refusing care were excluded. Main outcome measures: Emergency medical technician (EMT) interventions, method of delivery, and death. Results: The median age enrolled was 23 years (IQR 21-25). Women were primarily from rural/tribal areas (1550/1684 (92.0%)) and lower economic strata (1177/1684 (69.9%)). Time from initial call to hospital arrival was longer for rural/tribal compared to urban patients (66 min (IQR 51-84) vs 56 min (IQR 42-73), respectively, p&lt;0.0001). EMTs assisted delivery in 44 women, delivering the placenta in 33/44 (75%), performing transabdominal uterine massage in 29/33 (87.9%), and administering oxytocin in none (0%). There were 1411 recorded deliveries. Most women delivered at a hospital (1212/1411 (85.9%)), however 126/1411 (8.9%) delivered at home following hospital discharge. Follow-up rates at 48 hours, 7 days, and 42 days were 95.0%, 94.4%, and 94.1%, respectively. Four women died, all within 48 hours. The cesarean section rate was 8.2% (116/1411). On multivariate regression analysis, women transported to private hospitals versus government primary health centers were less likely to deliver by cesarean section (odds ratio 0.14 (0.05 to 0.43)). Conclusions: Pregnant women from vulnerable Indian populations use free-of-charge EMS for impending delivery, making it integral to the health care system. Future research and health system planning should focus on strengthening and expanding EMS as a component of EmONC.

Data from: Superfetation reduces the negative effects of pregnancy on the fast-start escape performance in live-bearing fish

<p>Superfetation, the ability to simultaneously carry multiple litters of different developmental stages <i>in utero</i>, is a reproductive strategy that evolved repeatedly in viviparous animal lineages. The evolution of superfetation is hypothesized to reduce the reproductive burden and, consequently, improve the locomotor performance of the female during pregnancy. Here, we apply new computer-vision based techniques to study changes in body shape and three-dimensional fast-start escape performance during pregnancy in three livebearing fishes (family Poeciliidae) that exhibit different levels of superfetation. We found that superfetation correlates with a reduced abdominal distension and a more slender female body shape just before parturition. We further found that body slenderness positively correlates with maximal speeds, curvature amplitude and curvature rate, implying that superfetation improves the fast-start escape performance. Collectively, our study suggests that superfetation may have evolved in performance-demanding (e.g. high flow or high predation) environments to reduce the locomotor cost of pregnancy.</p>

Twin pregnancy outcome following teriflunomide treatment in a relapsing-remitting multiple sclerosis patient

<p>Teriflunomide is a disease-modifying drug that has been approved for treatment of relapsing-remitting multiple sclerosis. Due to its teratogenic effect in animals, however, it is not recommended during pregnancy. For this reason, effective contraception must be used during its administration. When an unscheduled pregnancy occurs during therapy, patients must undergo a cholestyramine procedure for rapid flushing of the drug. We describe the case of a 35-year-old female patient suffering diagnosed with relapsing-remitting multiple sclerosis at the age of 20. The patient as a result of side effects of previous therapies started taking teriflunomide. Despite recommendations for the use of contraceptives, the patient became pregnant during drug therapy. Pregnancy occurred 12 months after initiating teriflunomide treatment. Therapy with teriflunomide was immediately suspended and cholestyramine was prescribed (8 g 3 times a day, for 11 days) to flush out any residual drug from the body. Despite an 8-week exposure to teriflumomide during gestation, the patient gave birth to healthy twin girls at 35<sup>th</sup> week. Controls carried out after birth did not reveal any malformation or genetic and chromosomal abnormality. At a 5-month pediatric specialist check both babies were healthy and growing regularly. This shows that even if there is evidence of teratogenic effects in animals, an 8-week exposure to teraflunomide &gt;0.02 mg/L did not have effects on the newborn.</p>

Anti-seizure medication use during pregnancy and risk of ASD and ADHD in children

<p class="CxSpFirst"><b>Objective: </b>To determine whether children born to women who use anti-seizure medications (ASMs) during pregnancy have higher risk of autism spectrum disorder (ASD) and attention-deficit/hyperactivity disorder (ADHD) independent of confounding factors.</p> <p class="CxSpMiddle"><b>Methods:</b> We used Swedish-register data (n=14,614 children born 1996-2011 and followed through 2013) to examine associations in children of women with epilepsy, using the largest sample to date and adjusting for a range of measured confounders. We examined maternal-reported first-trimester use of any ASM (22.7%); and the three most commonly reported individual drugs (valproic acid, 4.8%; lamotrigine, 6.8%; and carbamazepine, 9.7%). We identified ASD with ICD-10 diagnoses and ADHD with ICD-10 diagnoses or filled prescriptions of ADHD medication.</p> <p class="CxSpMiddle"><b>Results: </b>Examination of individual drugs revealed that after adjustment for confounding, use of valproic acid was associated with ASD (Hazard Ratio=2.30, 95%CI=1.53-3.47) and ADHD (HR=1.74, 95%CI=1.28-2.38). Whereas a small, non-statistically significant association with ASD (HR=1.25, 95%CI=0.88-1.79) and ADHD (HR=1.18, 95%CI=0.91-1.52) remained for reported use of carbamazepine, confounding explained all of the associations with lamotrigine (HR<sub>ASD</sub>=0.86, 95%CI=0.67-1.53; HR<sub>ADHD</sub>=1.01, 95%CI=0.67-1.53).</p> <p class="CxSpMiddle"><b>Conclusions: </b>We found no evidence of risk related to exposure to lamotrigine, whereas we observed elevated risk of ASD and ADHD related to maternal use of valproic acid. Associations with carbamazepine were weak and not statistically significant. Our findings add to a growing body of evidence that suggest that certain ASMs may be safer than others in pregnancy.</p>

Parenting costs time: Changes in pair bond maintenance across pregnancy and infant rearing in a monogamous primate (Plecturocebus cupreus)

Abstract for article:</p> <p>Relationships support social animals&rsquo; health, but maintaining relationships is challenging. When transitioning to parenthood, new parents balance pair-bond maintenance with infant care. We studied pair-bond maintenance via affiliation in 22 adult titi monkey pairs (<em>Plecturocebus cupreus</em>) for 16 months centered around&nbsp;their first offspring&rsquo;s birth. Pair affiliation peaked during pregnancy, decreased across the postpartum period, and rose after reaching minimum affiliation 32.6 weeks postpartum. Pairs in which fathers carry infants more than average had lower affiliation at the infant&rsquo;s birth and return to an increase in affiliation sooner. Parents of infants who were slow to independence had higher rates of affiliation. Titi monkey infants actively prefer their fathers; mothers may avoid their infant-carrying mate, suggesting infants play an active role in parental affiliative decline. Our data supports previous findings that affiliation between partners declines following an infant&rsquo;s birth, but demonstrates new knowledge about the extent and duration of affiliative decline.</p>

Humpback pregnancy rates and environmental variation: Supporting datasets

<p>The krill surplus hypothesis of unlimited prey resources available for Antarctic predators due to commercial whaling in the 20th century has remained largely untested since the 1970s. Rapid warming of the Western Antarctic Peninsula (WAP) over the past 50 years has resulted in decreased seasonal ice cover and a reduction of krill. The latter is being exacerbated by a commercial krill fishery in the region. Despite this, humpback whale populations have increased but may be at a threshold for growth based on these human-induced changes. Understanding how climate-mediated variation in prey availability influences humpback whale population dynamics is critical for focused management and conservation actions. Using an eight-year dataset (2013–2020), we show that inter-annual humpback whale pregnancy rates, as determined from skin-blubber biopsy samples (n = 616), are positively correlated with krill availability and fluctuations in ice cover in the previous year. Pregnancy rates showed significant inter-annual variability, between 29% and 86%. Our results indicate that krill availability is in fact limiting and affecting reproductive rates, in contrast to the krill surplus hypothesis. This suggests that this population of humpback whales may be at a threshold for population growth due to prey limitations. As a result, continued warming and increased fishing along the WAP, which continue to reduce krill stocks, will likely impact this humpback whale population and other krill predators in the region. Humpback whales are sentinel species of ecosystem health, and changes in pregnancy rates can provide quantifiable signals of the impact of environmental change at the population level. Our findings must be considered paramount in developing new and more restrictive conservation and management plans for the Antarctic marine ecosystem and minimizing the negative impacts of human activities in the region.</p>

<p><span>In some mammals, and particularly in cooperative breeding ones, successive bouts of reproduction can overlap so that a female is often pregnant while still nurturing dependent young from her previous litter. </span><span>Such an overlap requires females to divide their energetic budget between two reproductive activities, and pregnancy costs </span><span>would consequently be expected to reduce investment in concurrent offspring care. However, explicit evidence for such reductions is scarce, and the potential effects they may have on work division in cooperative breeders have not been explored.</span></p> <p><span>Using 25 years of data on reproduction and cooperative behaviour in wild Kalahari meerkats, supplemented with field experiments, we investigated whether pregnancy reduces contributions to cooperative pup care behaviours, including babysitting, provisioning, and raised guarding. We also explored whether pregnancy, which is more frequent in dominants than subordinates, could account for the reduced contributions of dominants to the cooperative pup care behaviours. </span></p> <p><span>We found that pregnancy, particularly at late stages of gestation, reduces contributions to cooperative pup care; that these reductions are eliminated when the food available to pregnant females is experimentally supplemented; and that pregnancy effects accounted for differences between dominants and subordinates in two of the three cooperative behaviours examined (pup provisioning and raised guarding but not babysitting). </span></p> <p><span>By linking pregnancy costs with reductions in concurrent pup care, our findings illuminate a tradeoff between investment in successive, overlapping bouts of reproduction. They also suggest that some of the differences in cooperative behaviour between dominant and subordinate females in cooperative breeding mammals can be a direct consequence of differences in their breeding frequency. </span></p>

Maternal anxiety and prenatal support predict pregnancy length and infant characteristics at birth during the pandemic

<p><strong>ABSTRACT</strong></p> <p>According to the theoretical prediction of the smoke detector principle, heightened anxiety as a personality trait could lead to better survival and reproductive success in unpredictable environments. Yet published studies suggest that maternal anxiety negatively affects gestation and birth outcomes, which may lead to decreased reproductive success. To resolve this conundrum, we studied the association between maternal anxiety and birth outcomes during the COVID-19 pandemic. Data collected from 1080 expectant mothers from the Corona Mums Project included maternal trait anxiety (STAI X2), perceived emotional support (PES), duration of gestation, and infant parameters at birth taken from hospital health records. After adjusting for covariates, multiple regression models showed that STAI X2 positively predicted infant birth weight (&beta;=0.07, p=0.023) and length (&beta;=0.08, p=0.010). PES positively predicted only birth weight (&beta;=0.06, p=0.048). Neither of these factors was significantly related to duration of gestation, although the association with PES was of borderline significance (&beta;=0.06, p=0.053). The results suggest that maternal increased ability to detect threats, indicated by her trait anxiety during the unpredictable conditions of COVID-19, was associated with better infant characteristics at birth. This may improve infant survival and maternal reproductive success, implying that increased trait anxiety could be adaptive during environmental threats.</p>
