Data from: Provision of care for chronic kidney disease by non-nephrologists in a developing nation: a national survey

Objectives The prevalence of chronic kidney disease (CKD) in developing countries has increased dramatically. This study aimed to explore the practice patterns of non-dialysis-dependent CKD care in an affluent developing country. Settings Primary and specialised healthcare facilities of public and private sectors in the United Arab Emirates. Participants 159 non-nephrologist physicians practising in the United Arab Emirates. Interventions A 28-item online self-administered questionnaire based on CKD clinical practice guidelines. Primary and secondary outcome measures The physicians' approach to identifying and managing patients with CKD. Results The survey was completed by 159 non-nephrologists, of whom 135 reported having treated patients with CKD. Almost all the respondents screen patients with hypertension and diabetes for CKD, but one-third of them do not screen patients with cardiovascular disease and elderly patients for CKD. The use of accurate CKD screening tests (estimated glomerular filtration rate and albumin/creatinine ratio) was suboptimal (77% and 59% of physicians used the procedures, respectively). One-third of the physicians do not offer treatment with inhibitors of the renin–angiotensin system to patients with CKD, and only 66% offer antilipid treatment. In general, the primary healthcare physicians are more familiar than secondary healthcare physicians with the diagnosis and management of patients with CKD. Conclusions We identified substantial physician-declared deficiencies in the practice of identifying and managing early CKD. Integration of quality CKD care within the healthcare system is required to face the increasing burden of CKD in the United Arab Emirates and possibly in other developing nations.

Data from: A nation-wide survey of neonicotinoid insecticides in agricultural land with implications for agri-environment schemes

1. Neonicotinoids are the most widely used class of insecticides globally. However, the link between farming practices and the extent of contamination of soils and crops by neonicotinoid insecticides, as well as and the extent of such contamination in organic fields and ecological focus areas (EFAs) are currently unclear. 2. We measured the concentrations of five neonicotinoid insecticides (imidacloprid, clothianidin, thiamethoxam, thiacloprid, acetamiprid) in 702 soil and plant samples in 169 cultivated fields and EFAs from 62 conventional, integrated production and organic farms distributed over the entire lowland of Switzerland. 3. We detected neonicotinoids in 93% of organic soils and crops, and more than 80% of EFA soils and plants – two types of arable land supposedly free of insecticides. We also tested 16 samples of organic seeds, of which 14 were positive for neonicotinoids. 4. Finally, we calculated hazard quotients (HQs) and potentially affected fractions for 72 beneficial and 12 pest species. Under a field-realistic scenario, we found that between 5.3 and 8.6% of above-ground invertebrate species may be exposed to lethal concentrations of clothianidin, and 31.6 to 41.2% to sublethal concentrations, in "integrated production" and conventional fields. We also found that 1.3 to 6.8% (up to 12.5% based on HQs) of the beneficial invertebrate species may be exposed to sublethal concentrations of neonicotinoids in EFAs and organic fields. In contrast, no pest species would be exposed to lethal concentrations, even under a worst-case scenario. 5. Synthesis and applications. Our study suggests that diffuse contamination by neonicotinoids may harm a significant fraction of non-target beneficial species. The use of neonicotinoids on crops may threaten biodiversity in refuge areas, while also potentially jeopardizing the practice of organic farming by impeding the biological control of pests. Based on our results, we call for a reduction in the dispersion and overuse of neonicotinoid insecticides in order to prevent any detrimental effects on biodiversity and ecosystem services associated with agroecosystems.

Data from: Gynaecological cancer follow-up: national survey of current practice in the UK

Objective: To establish a baseline of national practice for follow-up after treatment for gynaecological cancer. Design: Questionnaire survey. Setting: Gynaecological cancer centres and units. Geographical location: UK Participants: Members of the British Gynaecological Cancer Society and the National Forum of Gynaecological Oncology Nurses. Interventions: A questionnaire survey. Outcome measures: To determine schedules of follow-up, who provides it and what routine testing is used for patients who have had previous gynaecological cancer. Results: A total of 117 responses were obtained; 115 (98%) reported hospital scheduled regular follow-up appointments. Two involved General Practitioners. Follow-up was augmented or replaced by telephone follow-up in 29 (25%) and patient initiated appointments in 38 responses (32%). A total of 80 (68%) cancer specialists also offered combined follow-up clinics with other specialties. Clinical examinations for hospital based follow-up were mainly performed by doctors (67% for scheduled regular appointments and 63% for patient initiated appointments) while telephone follow-up care was provided in the majority by nurses (76%). Most respondents provided routine tests (76/117 (65%)), from which 66/76 (87%) reported carrying out surveillance tests for ovarian cancer, 35/76 (46%) for cervical cancer, 8/76 (11%) for vulval cancer and 7/76 (9%) for endometrial cancer. Usually patients were discharged after five years (82/117 (70%)), whereas three (3%) were discharged after four years, nine (8%) after three and one (1%) after two years. Conclusions: Practice varied but most used a standard hospital based protocol of appointments for five years and routine tests were performed usually for women with ovarian cancer. A minority utilised nurse-led or telephone follow-up. General Practitioners were rarely involved in routine care. A randomised study comparing various models of follow-up could be considered.
