Temporal and regional trends of antibiotic use in long-term aged care facilities across 39 countries, 1985-2019: systematic review and meta-analysis

<p><b>Background</b></p> <p>Antibiotic misuse is a key contributor to antimicrobial resistance and a concern in long-term aged care facilities (LTCFs). Our objectives were to:  i) summarise key indicators of systemic antibiotic use and appropriateness of use, and ii) examine temporal and regional variations in antibiotic use, in LTCFs (PROSPERO registration CRD42018107125).</p> <p><b>Methods &amp; Findings</b></p> <p>Medline and EMBASE were searched for studies published between 1990-2021 reporting antibiotic use rates in LTCFs. Random effects meta-analysis provided pooled estimates of antibiotic use rates (percentage of residents on an antibiotic on a single day [point prevalence] and over 12 months [period prevalence]; percentage of appropriate prescriptions). Meta-regression examined associations between antibiotic use, year of measurement and region. </p> <p>A total of 90 articles representing 78 studies from 39 countries with data between 1985-2019 were included. Pooled estimates of point prevalence and 12-month period prevalence were 5.2% (95% CI: 3.3-7.9; n=523,171) and 62.0% (95% CI: 54.0-69.3; n=946,127), respectively. Point prevalence varied significantly between regions (Q=224.1, df=7, p&lt;0.001), and ranged from 2.4% (95% CI: 1.7-2.7) in Eastern Europe to 9.0% in the British Isles (95% CI: 7.6-10.5) and Northern Europe (95% CI: 7.7-10.5). Twelve-month period prevalence varied significantly between region (Q=15.1, df-3, p=0.002) and ranged from 53.9% (95% CI: 48.3-59.4) in the British Isles to 68.3% (95% CI: 63.6-72.7) in Australia. Meta-regression found no association between year of measurement and antibiotic use prevalence. The pooled estimate of the percentage of appropriate antibiotic prescriptions was 28.5% (95% CI: 10.3-58.0; n=17,245) as assessed by the McGeer criteria. Year of measurement was associated with decreasing appropriateness of antibiotic use over time (OR: 0.78, 95% CI: 0.67-0.91).  The most frequently used antibiotic classes were penicillins (n=44 studies), cephalosporins (n=36), sulphonamides/trimethoprim (n=31), and quinolones (n=28). </p> <p><b>Conclusions</b></p> <p>Coordinated efforts focusing on LTCFs are required to address antibiotic misuse in LTCFs. Our analysis provides overall baseline and regional estimates for future monitoring of antibiotic use in LTCFs.  </p>

"COVID toes": a meta-analysis of case and observational studies on clinical, histopathological and laboratory findings.

<p><strong>Background: </strong>Coronavirus disease (COVID-19) is related to several extrapulmonary disorders; however, little is known about the clinical, laboratory and histopathological characteristics of pernio-like skin lesions associated with COVID-19 infection. <strong>Objective: </strong>To evaluate and summarize the clinical, laboratory and histopathological characteristics of pernio-like lesions reported in the literature. <strong>Methods: </strong>We conducted a search of the PubMed, SciELO and ScienceDirect databases for articles published between 1 January 2020 and 30 November 2020, following the PRISMA recommendations (PROSPERO registration ID: CRD42020225055). The target population was individuals with suspected or laboratory-confirmed COVID-19 with pernio-like lesions. Observational studies, research letters and case/series reports were all eligible for inclusion. Observational studies were evaluated using a random-effects model to calculate the weighted mean prevalence, overall mean and 95% confidence interval. We evaluated case studies using the chi-square test for dichotomous variables and the Mann-Whitney test for continuous variables. <strong>Results:</strong> A total of 187 patients from case reports and 715 patients from 18 observational studies were included. The mean age of patients was 16.6 (14.5&ndash;18.8) years. Feet were affected in 91.4% (87.0&ndash;94.4%) of patients in observational studies. The proportion of patients with a positive RT-PCR test was less than 15%. Lesion topography and morphology were associated with age. <strong>Conclusion:</strong> Lesions mostly occurred in pediatric patients, and the morphological characteristics tended to differ between pediatric and non-pediatric populations. There is a possible multifactorial component in lesion pathophysiology. The non-positivity of laboratory tests does not exclude an association with COVID-19. Chilblain-like lesions may be a late manifestation of COVID-19.</p>

<p>Mesenchymal stromal cells (MSCs) are widely used in preclinical models of traumatic brain injury (TBI). Results are promising in terms of neurological improvement but are hampered by wide variability in treatment responses. We made a systematic review and meta-analysis: 1) to assess the quality of evidence for MSC treatment in TBI rodent models; 2) to determine the effect size of MSCs on sensorimotor function, cognitive function and anatomical damage; 3) to identify MSC-related and protocol-related variables associated with greater efficacy; 4) to understand whether MSC manipulations boost therapeutic efficacy.</p> <p>The meta-analysis included 80 studies. After TBI, MSCs improved sensorimotor and cognitive deficits, and reduced anatomical damage. Stratified meta-analysis on sensorimotor outcome showed similar efficacy for different MSC sources and for syngeneic or xenogenic transplants. Efficacy was greater when MSCs were delivered in the first week post-injury, and when implanted directly into the lesion cavity. The greatest effect size was for cells embedded in matrices or for MSC-derivatives.</p> <p>MSC therapy is effective in preclinical TBI models, improving sensorimotor, cognitive and anatomical outcomes, with large effect sizes. A meta-analysis of pre-pregnancy maternal body mass index and placental DNA methylation identifies 27 CpG sites with implications for mother-child health

<p>Higher maternal pre-pregnancy body mass index (ppBMI) is associated with increased neonatal morbidity, as well as with pregnancy complications and metabolic outcomes in offspring later in life. Placenta is a key organ in fetal development and has been proposed to act as a mediator between the mother and different health outcomes in children. The overall aim of the present work is to investigate the association of ppBMI with epigenome-wide placental DNA methylation (DNAm) in 10 studies from the PACE consortium, amounting to 2,631 mother-child pairs. We identify 27 CpG sites at which we observe placental DNAm variations of up to 2.0% per 10 ppBMI-unit. The CpGs that are differentially methylated in placenta do not overlap with CpGs identified in previous studies in cord blood DNAm related to ppBMI. Many of the identified CpGs are located in open sea regions, are often close to obesity-related genes such as GPX1 and LGR4 and altogether, are enriched in cancer and oxidative stress pathways. Urbanization decreases species richness, and increases abundance in dry climates whereas decreases in wet climates: A global meta-analysis

<p>Soil invertebrates have an essential role in decomposition, nutrient turnover and soil structure formation, all of which are strongly threatened by urbanization. Sealing, compaction by trampling and pollution destroy and degrade city soils and potentially damage soil-living invertebrates. The existing literature on how urbanization affects soil invertebrates is inconsistent, presenting both negative and positive effects. Therefore, here we aimed to synthesize the effects of urbanization on soil invertebrates considering their taxonomic (Acari, Annelida, Carabidae, Collembola, Gastropoda, Isopoda, Myriapoda, Nematoda) and functional (soil living vs. soil-related; mobility) identities, as well as to examine how the overall effect is modulated by climatic conditions (total annual precipitation, annual mean ambient temperature), urban heat island effect (based on ambient temperature differences between urban and rural areas) and city population. In a systematic review using hierarchical and categorical meta-analyses, we extracted 158 effect sizes from 75 studies on abundance and 125 effect sizes from 84 studies on species richness. Invertebrate abundance showed an increase, whereas species richness significantly decreased with increasing urbanization. The reason behind this could be that a few generalist species can adapt well to the urban environment and achieve strongly elevated densities. The species richness of annelids, springtails, and snails&nbsp;decreased with advancing urbanization, most probably because these animals are sensitive to soil compaction and pollution, both of which are common consequences of urbanization. The temperature did not modify the effects of urbanization, but precipitation modified the effects on abundance. Abundance increased with advancing urbanization in drier climates, probably because irrigation increased soil moisture, whereas it decreased in wet climates, as urban areas were drier than their surroundings. Making future cities more climate-neutral could better sustain soil biodiversity.</p>

The effect of polyphenol compounds on Helicobacter pylori eradication: a systematic review with meta-analysis

<p>Objectives: Polyphenol compounds are classified as organic compounds with phenolic units exhibiting a variety of biological functions. This meta-analysis aims to assess the efficacy and safety of polyphenol compounds (curcumin, cranberry, garlic, licorice, and broccoli) in eradicating Helicobacter pylori.</p> <p>Design: Systematic review and meta-analysis.</p> <p>Data sources: These include PubMed, Embase, The Cochrane Library, Web of Science, Medline, Chinese National Knowledge Infrastructure database , Chinese Scientific Journal Database , and Wan Fang database from inception to January 2022.</p> <p>Study selection: All randomized controlled trials comparing polyphenol compounds with the placebo or used as an adjunct treatment are included in this meta-analysis.</p> <p>Data synthesis: The treatment effect for dichotomous outcomes were assessed using risk ratio, while for continuous outcomes, mean differences (MDs) both with 95% CIs, were used. Subgroup analyses were carried out for different treatment schemes and polyphenol compound species.  </p> <p>Results: 12 trials were included in the meta-analysis. The total eradication rate of H.pylori in the polyphenol compounds group was higher than in the group without polyphenol compounds. Statistical significance was also observed (RR 1.19, 95% CI:1.03 to 1.38, P=0.02). The most frequent adverse effects of polyphenol compounds included diarrhea, headache, and vomiting. However, there were no differences regarding side effects between the two groups (RR 1.47, 95% CI: 0.83 to 2.58, P=0.18). In subgroup analyses, the H.pylori eradication rate regimens with polyphenols therapy was superior to that of regimens without polyphenols therapy in the polyphenols versus placebo subgroup (RR 4.23, 95% CI:1.38 to 12.95, P=0.01), polyphenols plus triple therapy versus triple therapy subgroup (RR 1.11, 95% CI: 1.01 to 1.22, P=0.03).</p> <p>Conclusion: Polyphenol compounds can improve H.pylori eradication rates. Polyphenol compounds plus standard triple therapy can significantly improve the eradication. Marine primary producers in a darker future – a meta-analysis of light effects on pelagic and benthic autotrophs

<p><span>The availability of underwater light, as the primary energy source for all aquatic photoautotrophs, is (and will further be) altered by changing precipitation, water turbidity, mixing depth, and terrestrial input of chromophoric dissolved organic matter (CDOM). While experimental manipulations of CDOM input and turbidity are frequent, they often involve multiple interdependent changes (light, nutrients, C-supply). To create a baseline for the expected effects of light reduction alone, we performed a weighted meta-analysis on 240 published experiments (from 108 studies yielding 2,500 effect sizes) that directly reduced light availability and measured marine autotroph responses. Across all organisms, habitats, and response variables, reduced light led to an average 23% reduction in biomass-related performance, whereas the effect sizes on physiological performance did not significantly differ from zero. Especially pigment content increased with reduced light, which indicated strong physiological plasticity in response to diminished light. This acclimation potential was also indicated by light reduction effects minimized if experiments lasted longer. Nevertheless, performance (especially biomass accrual) was reduced the more the less light intensity remained available. Light reduction effects were also more negative at higher temperatures if ambient light conditions were poor. Macrophytes or benthic systems were more negatively affected by light reduction than microalgae or plankton systems, especially in physiological responses where microalgae and plankton showed slightly positive responses. Otherwise, effect magnitudes remained surprisingly consistent across habitats and aspects of experimental design. Therefore, the strong observed log-linear relationship between remaining light and autotrophic performance can be used as a baseline to predict marine primary production in future light climate.</span></p>

Tropical bat ectoparasitism in continuous versus fragmented forests: A gap analysis and preliminary meta-analysis

<p><span>Tropical regions are experiencing rapid rates of forest fragmentation, which can have several effects on wildlife, including altered parasite dynamics. Bats are a useful host group to consider these effects of fragmentation because they are abundant in the tropics, serve important ecological roles, and harbour many parasites. Nevertheless, research on the effects of fragmentation on bat ectoparasites is still limited. To help guide ongoing and future research efforts, this study had two objectives: (1) conduct a gap analysis to characterize the state of currently available research on fragmentation effects on bat ectoparasites, and (2) conduct a preliminary meta-analysis to identify current trends. We systematically highlighted several research gaps: studies comparing the effects of fragmented versus continuous forests on ectoparasites are limited and have primarily been conducted in the Neotropics, with a focus on bats in the superfamily Noctilionidea (especially frugivorous phyllostomids). Our preliminary meta-analysis suggested that ectoparasite prevalence (but not the mean or variance in intensity) was higher in fragments than in continuous forests. Moreover, prevalence increased with increasing roost duration, and mean intensity was higher for bats with higher wing aspect ratios. Intensity variance was affected by an interaction between forest type and wing aspect ratio, such that variance increased for bats with high wing aspect ratios in continuous forests but decreased in fragments. These results suggest that fragmentation can shape aspects of bat ectoparasitism and could have implications for the ecology, health, and conservation of bats in fragmented landscapes. However, existing research gaps could bias our current understanding of habitat change and bat health, and future research should thus investigate these effects in the Paleotropics and with other bat families. </span><span><br></span></p>

Effects of thermal fluctuations on biological processes: A meta-analysis of experiments manipulating thermal variability

<p>Thermal variability is a key driver of ecological processes, affecting organisms and populations across multiple temporal scales. Despite the ubiquity of variation, biologists lack a quantitative synthesis of the observed ecological consequences of thermal variability across a wide range of taxa, phenotypic traits, and experimental designs. Here, we conduct a meta-analysis to investigate how properties of organisms, their experienced thermal regime, and whether thermal variability is experienced in either the past (prior to an assay) or present (during the assay) affect performance, relative to the performance of organisms experiencing constant thermal environments. Our results – which draw upon 1,712 effect sizes from 75 studies – indicate that the effects of thermal variability are not unidirectional and become more negative as mean temperature and fluctuation range increase. Exposure to variation in the past decreases performance to a greater extent than variation experienced in the present and increases the costs to performance more than diminishing benefits across a broad set of empirical studies. Further, we identify life history attributes that predictably modify the ecological response to variation. Our findings demonstrate that effects of thermal variability on performance are context-dependent, yet negative outcomes may be heightened in warmer, more variable climates.</p>

Data for: Phylogenetically controlled life history trait meta-analysis in cetaceans reveals unexpected negative brain size and longevity correlation

<p>The identification of patterns in trait evolution is essential to understand the interaction of evolutionary forces, and provides useful information for species management. Cetaceans are a phylogenetically well-resolved infraorder that exhibit distinct trait variation across behavioural, molecular and life history dimensions, yet few researchers have applied a meta-analytic or comparative approach to these traits. To understand cetacean trait evolution, we used a phylogenetic generalised least squares approach to examine the cognitive buffer hypothesis (CBH). A large brain should buffer individuals against environmental challenges through increasing survival rates, and a longer lifespan should buffer individuals against the cost of extended development for larger brains according to the CBH, leading to an expected positive correlation between brain size and lifespan. In contrast to this expectation, previously observed in taxa including primates, we found a negative correlation between brain size and lifespan in cetaceans. This suggests cetaceans experience selective pressures different from most other mammals in these traits but may be more similar to some social mammalian carnivores that display alloparenting. We also provide a comprehensive dataset to explore additional aspects of trait evolution but which would greatly benefit from studies on behavioural ecology across cetaceans and increased focus on data-deficient species. </p>

Incidence and influencing factors of occupational pneumoconiosis: A systematic review and meta-analysis

<p><span><strong>Objectives</strong>: </span><span>To determine the incidence of pneumoconiosis worldwide and its influencing factors. </span></p> <p><span><strong>Design</strong>:</span><span> Systematic review and meta-analysis. </span></p> <p><span><strong>Setting</strong>:</span><span> Cohort studies on occupational pneumoconiosis.</span></p> <p><span><strong>Participants</strong>:</span><span> PubMed, Embase, the Cochrane Library, and Web of Science were searched until November 2021. Studies were selected for meta-analysis if they involved at least one variable investigated as an influencing factor for the incidence of pneumoconiosis and reported either the parameters and 95% confidence intervals (CIs) of the risk fit to the data, or sufficient information to allow for the calculation of those values. </span></p> <p><span><strong>Primary outcome measures</strong>:</span><span> The </span><span>pooled incidence of pneumoconiosis and risk ratio (RR) and 95% CIs of influencing factors. </span></p> <p><span><strong>Results</strong>:</span><span> Our meta-analysis included 19 studies with a total of 335,424 participants, of whom 29,972 developed pneumoconiosis. The pooled incidence of pneumoconiosis was 0.093 (95% CI: 0.085~0.135). We identified the following influencing factors: (1) male (RR=3.74; 95%CI 1.31–10.64; P=0.01), (2) smoking (RR=1.80; 95%CI 1.34</span><span>–</span><span>2.43; P=0.0001), (3) tunneling category (RR=4.75; 95%CI 1.96</span><span>–</span><span>11.53; P&lt;0.0001), (4) helping category (RR=0.07; 95%CI 0.13</span><span>–</span><span>0.16; P&lt;0.0001), </span><span>(5) age (the highest incidence occurs between the ages of 50 and 60), </span><span>(6) duration of dust exposure ((RR=4.59, 95% CI 2.41</span><span>–</span><span>8.74, P&lt;0.01), (7) cumulative total dust exposure (CTD) (RR=34.14, 95% CI 17.50</span><span>–</span><span>66.63, P&lt;0.01). A dose-response analysis revealed a significant positive linear dose-response association between the risk of pneumoconiosis and duration of exposure and CTD (P-nonlinearity=0.10, P-nonlinearity=0.16; respectively). The relative effectiveness of different grassland restoration methods: A systematic literature search and meta-analysis

<p>1. Active grassland restoration has gained in importance to mitigate the dramatic decline of farmland biodiversity. While there is evidence that such operations are generally effective in promoting plant diversity, little is known about the effectiveness of the different methods applied. Restoration methods can differ in intensity of seed bed preparation, seed source and technique of seed application.</p> <p>2. In this systematic literature search and meta-analysis, we screened the literature for studies of the restoration of mesic grasslands in temperate Europe. We focused on active restoration experiments that included a treatment and lasted more than three years. We evaluated the influence of restoration factors on plant species richness relative to non-restored controls.</p> <p>3. We found 187 articles that investigated the outcome of operations aimed at actively restoring mesic temperate grasslands. Most articles focused on plants, with only 9.6% dealing with other organisms (e.g. beetles, pollinating insects). Many papers had to be excluded due to incomplete data, too short study duration and/or lack of an adequate control. This resulted in 13 articles fulfilling our criteria for inclusion, yielding a total of 56 data points for the meta-analysis.</p> <p>4. Restoration actions increased plant species richness by, on average, 17.4%, compared to controls. The seed source explained a significant amount of variation in plant species richness: seeds originating from a speciose donor grassland had a positive effect. This effect was even enhanced when combined with a commercial seed mix, whereas commercial seed mixes alone had no significant effect. We did not observe any effect of other factors, such as the type of seed bed preparation or the seed application method.</p> <p>5. A seed-source obtained from species-rich grasslands seems to be key to efficient grassland restoration in mesic grasslands of temperate Europe. Even though seeds from a speciose donor grassland should be preferred over commercial seeds, associating natural and commercial seed mixes increases plant species richness. This systematic literature search further revealed two major research gaps in grassland restoration ecology: a deficit in long-term investigations as well as a deficit in studies focusing on non-plant organisms.</p>

Data from: Meta-analysis of the effects of insect pathogens: Implications for plant reproduction

<p>Despite extensive work on both insect disease and plant reproduction, there is little research on the intersection of the two. Insect-infecting pathogens could disrupt the pollination process by affecting pollinator population density or traits. Pathogens may also infect insect herbivores and change herbivory, potentially altering resource allocation to plant reproduction. We conducted a meta-analysis to 1) summarize the literature on the effects of pathogens on insect pollinators and herbivores and 2) quantify the extent to which pathogens affect insect traits, with potential repercussions for plant reproduction. We found 39 articles that fit our criteria for inclusion, extracting 218 measures of insect traits for 21 different insect species exposed to 25 different pathogens. We detected a negative effect of pathogen exposure on insect traits, which varied by host function: pathogens had a significant negative effect on insects that were herbivores or carried multiple functions but not on insects that solely functioned as pollinators. Particular pathogen types were  heavily studied in certain insect orders, with 7 of 11 viral pathogen studies conducted in Lepidoptera and 5 of 9 fungal pathogen studies conducted in Hymenoptera. Our results suggest that most studies have focused on a small set of host–pathogen pairs. To understand the implications for plant reproduction, future work is needed to directly measure the effects of pathogens on pollinator effectiveness.</p>
