Data from: Requirements and access needs of patients with chronic disease to their hospital electronic health record: results of a cross-sectional questionnaire survey
Objectives: To identify patient's views on the functionality required for personalised access to the secondary care EHR and their priorities for development. Design: Quantitative analysis of a cross-sectional self-complete survey of patient views on required EHR functionality from a secondary care EHR, including a patient ranking of functionality. Setting: Secondary care patients attending a regional cystic fibrosis unit in the north of England Participants: 201 adults [106 (52.7%) male], median age 29 years (range 17-58 years) entered and completed the study. Inclusion criteria; a confirmed diagnosis of CF, aged 16 years and over, at a time of clinical stability Outcome measures: Quantitative responses within 4 themes; 1) value placed on aspects of the EHR; 2) access requirements to functions of the EHR; 3) views on information sent to the EHR 4) patient feedback entered into the EHR. A ranked score for 15 functions of the EHR was obtained Results: Highest ratings (% reporting item as very important/important) were reported for access to clinical measures [lung function (94%), CRP (84%), sputum microbiology (81%) and blood results (80%)], medication changes (82%) and lists (83%) and sending repeat prescription (83%) and treatment requests (80%), whilst sending symptom diaries was less so (62%). Email contact with clinicians was the most valuable communication element of the EHR (84% very important/important). Of 15 features of the EHR [1=most desirable to 15=least desirable) patients identified 'clinical measures' [2.62 (CI 2.07-3.06)], and 'access to medication lists' [4.91 (CI 4.47-5.44)], as highest priority for development and the ability to comment on errors/ommissions [11.0 (CI 10.6-11.5)] or experience of care [11.8 (CI 11.4-12.2)] as lowest. Conclusions: Patients want extensive personal access to their hospital EHR, placing high importance on the viewing of practical clinical measures and medication management. These influence routine day to day care and are priorities for development
"Natural" solutions could speed up climate change mitigation, with risks, compared to emissions reductions alone. Additional options are needed.
<p>Mitigation of climate change by intentionally storing carbon in tropical forests, soils and wetlands, and by reducing greenhouse gas fluxes from these settings has been promoted as rapidly deployable and cost-effective. This approach, sometimes referred to as "natural" mitigation, could keep post-industrialization warming below 1.5°C, when coupled with reductions in fossil fuel emissions, as confirmed here with a simple numerical model of future emissions. However, such mitigation could cease in response to changes in future climate, land use or natural resource policies, or there could be CO<sub>2</sub> released from reservoirs of stored carbon. Model simulations suggest cumulative emissions could be similar, under scenarios where carbon storage ceases, or stored carbon is released, to emissions expected in the absence of any natural mitigation. If climate change is to be minimized, low-risk natural mitigation (e.g. by reducing deforestation) should be considered, as emissions targets that could limit warming to 1.5°C cannot be met without mitigation of this magnitude. However, additional mitigation options should also be considered that can reduce CO<sub>2</sub> emissions and remove CO<sub>2</sub> from the air (and store it permanently) and/or reduce the temperature of the atmosphere or the ocean.</p>
Data from: Resource-Area-Dependence Analysis: inferring animal resource needs from home-range and mapping data
An animal's home-range can be expected to encompass the resources it requires for surviving or reproducing. Thus, animals inhabiting a heterogeneous landscape, where resource patches vary in size, shape and distribution, will naturally have home-ranges of varied sizes, so that each home-range encompasses a minimum required amount of a resource. Home-range size can be estimated from telemetry data, and often key resources, or proxies for them such as the areas of important habitat types, can be mapped. We propose a new method, Resource-Area-Dependence Analysis (RADA), which uses a sample of tracked animals and a categorical map to i) infer in which map categories important resources are accessible, ii) within which home range cores they are found, and iii) estimate the mean minimum areas of these map categories required for such resource provision. We provide three examples of applying RADA to datasets of radio-tracked animals from southern England: 15 red squirrels Sciurus vulgaris, 17 gray squirrels S. carolinensis and 114 common buzzards Buteo buteo. The analyses showed that each red squirrel required a mean (95% CL) of 0.48 ha (0.24-0.97) of pine wood within the outermost home-range, each gray squirrel needed 0.34 ha (0.11-1.12) ha of mature deciduous woodland and 0.035-0.046 ha of wheat, also within the outermost home-range, while each buzzard required 0.54 ha (0.35-0.82) of rough ground close to the home-range center and 14 ha (11-17) of meadow within an intermediate core, with 52% of them also relying on 0.41 ha (0.29-0.59) of suburban land near the home-range center. RADA thus provides a useful tool to infer key animal resource requirements during studies of animal movement and habitat use.
Data from: Phylogenetic structural equation modelling reveals no need for an 'origin' of the leaf economics spectrum
The leaf economics spectrum (LES) is a prominent ecophysiological paradigm that describes global variation in leaf physiology across plant ecological strategies using a handful of key traits. Nearly a decade ago, Shipley et al. (2006) used structural equation modelling to explore the causal functional relationships among LES traits that give rise to their strong global covariation. They concluded that an unmeasured trait drives LES covariation, sparking efforts to identify the latent physiological trait underlying the 'origin' of the LES. Here, we use newly developed phylogenetic structural equation modelling approaches to reassess these conclusions using both global LES data as well as data collected across scales in the genus Helianthus. For global LES data, accounting for phylogenetic non-independence indicates that no additional unmeasured traits are required to explain LES covariation. Across datasets in Helianthus, trait relationships are highly variable, indicating that global-scale models may poorly describe LES covariation at non-global scales.
Data from: When good neighbors don't need fences: Temporal landscape partitioning among baboon social groups
Intraspecific competition is a key factor shaping space-use strategies and movement decisions inmany species, yet how and when neighbors utilize shared areas while exhibiting active avoidance of one another is largely un- known. Here, we investigated temporal landscape partitioning in a population of wild baboons (Papio cynocephalus). We used global positioning system (GPS) collars to synchronously record the hourly locations of five baboon social groups for ∼900 days, and we used behavioral, demographic, and life history data to measure factors affecting use of overlap areas. Annual home ranges of neighboring groups overlapped substantially, as predicted (baboons are considered non-territorial), but home ranges overlapped less when space use was assessed over shorter time scales. Moreover, neighboring groups were in close spatial proximity to one another on fewer days than predicted by a null model, suggesting an avoidance-based spacing pattern. At all time scales examined (monthly, biweekly, and weekly), time spent in overlap areas was greater during time periods when groups fed on evenly dispersed, low-quality foods. The percent of fertile females in social groups was negatively correlated with time spent in overlap areas only during weekly time intervals. This suggests that broad temporal changes in ecological resources are a major predictor of how intensively overlap areas are used, and groups modify these ecologically driven spacing patterns at short time scales based on female reproductive status. Together, these findings offer insight into the economics of territoriality by highlighting the dynamics of spacing patterns at differing time scales.
Data from: Why we need more nature at work: effects of natural elements and sunlight on employee mental health and work attitudes
This study investigated the effects of natural elements and direct and indirect sunlight exposure on employee mental health and work attitudes. We recruited participants via an online panel from the United States and India, and analyzed data from 444 employees. Natural elements and sunlight exposure related positively to job satisfaction and organizational commitment, and negatively to depressed mood and anxiety. Direct sunlight was a dominant predictor of anxiety; indirect sunlight was a dominant predictor of depressed mood, job satisfaction, and organizational commitment. Natural elements buffered the relationship between role stressors and job satisfaction, depressed mood, and anxiety. We also found that depressed mood partially mediated the relationship between natural elements and job satisfaction. We discuss scientific and policy implications of these findings.
Data from: Tidal drift removes the need for area restricted search in foraging Atlantic puffins
Understanding how animals forage is a central objective in ecology. Theory suggests that where food is uniformly distributed, Brownian movement ensures maximum prey encounter rate, but when prey is patchy, the optimal strategy resembles a Lévy walk where Area Restricted Search (ARS) is interspersed with commuting between prey patches. Such movement appears ubiquitous in high trophic level marine predators. Here we report foraging and diving behaviour in a seabird with a high cost of flight, the Atlantic puffin (Fratercula arctica), and report a clear lack of Brownian or Levy flight and associated ARS. Instead, puffins foraged using tides to transport them through their feeding grounds. Energetic models suggest the cost of foraging trips using the drift strategy is 28-46% less than flying between patches. We suggest such alternative movement strategies are habitat-specific, but likely to be far more widespread than currently thought.
Data from: Study on the foraging behaviour of the European nightjar Caprimulgus europaeus reveals the need for a change in conservation strategy in Belgium
Effective nature conservation requires coherent actions based on the best available evidence concerning protected species. Recent studies have suggested that European nightjars Caprimulgus europaeus forage outside their recognized breeding habitats, yet, for Flanders (northern Belgium) information on nightjar foraging behaviour and key foraging habitats is lacking. To assess whether the foraging ecology of nightjars in Flanders is similar to that observed in other parts of Europe, we studied the crepuscular behaviour of this species in Bosland (northeastern Flanders) during a five-year radio telemetry study. Tracking of 48 individuals within a coniferous forest was standardized and home ranges were calculated using a kernel density estimator (fixed kernel). Habitat use was investigated by comparing kernel placement to available habitat. Average maximal foraging distance was 2603 ± 1094 m and home ranges extended up to 691 ha. We identified the key foraging habitats to be extensively-cultivated grasslands and recreational areas, areas that were previously assumed unsuitable for Belgian nightjars. Our results indicate the importance of foraging sites outside the breeding territory, confirming the findings of previous studies performed elsewhere in Europe. Incorporating our findings into future conservation plans could, therefore, lead to improved efficiency of EU conservation measures, designed for the protection of this bird species in Flanders.
Data from: Dental status of New Caledonian children: is there a need for a new oral health promotion programme?
Background: Before implementing a new oral health promotion program in the French overseas territory of Nouvelle Calédonie, the health authorities needed recent data about dental status of the New Caledonian child population. Objectives: This study aimed to describe the dental status of 6, 9 and 12-yr-old New Caledonian children and to investigate the environmental and behavioural risk factors related to oral health. Methods: A randomly selected sample of 2734 children (744 6-yr-olds, 789 9-yr-olds, and 1201 12-yr-olds) was examined clinically by seven calibrated investigators and participants responded to a questionnaire. The main variables were objective criteria about dental status and subjective criteria about experience of dental care, dental fear, self-perception of oral health, cultural or ethnic identity and environmental and behavioural risk factors. Results: Overall, most of the children had infectious oral diseases: more than 50% had gingivitis, and 60% of 6- and 9 yr-olds had at least one deciduous or permanent tooth with untreated caries. The mean 12-yr-old number of decayed missing and filled teeth (DMFT) was 2.09±2.82. The number of carious lesions was related to the unfavourable lifestyle, deprived social status and no preventive dental care. Kanak, Polynesians and Caledonians (respectively 27%, 18% and 45% of the study sample) were more affected by caries than metropolitan French and Asian children. Children with many untreated carious lesions had negative perceptions of their oral health; they complained of chewing difficulty and had higher scores for dental anxiety. Conclusion: This study highlights the need for new strategies aimed at improving oral health and at reducing inequalities in New Caledonia. An oral health promotion program would need to be developed in connection with other health programmes using the common risk factor approach within the context of the local environment.
