Data from: Incidence of cancer-associated thromboembolism in Japanese gastric and colorectal cancer patients receiving chemotherapy: a single-institutional retrospective cohort analysis (Sapporo CAT study)

Objective: Few data regarding the incidence of cancer-associated thromboembolism (TE) are available for Asian populations. We investigated the incidence of TE (TEi) and its risk factors among gastric and colorectal cancer (GCC) patients who received chemotherapy in a daily practice setting. Design: A retrospective cohort study. Setting: A single institutional study that used data from Sapporo City General Hospital, Japan, on patients treated between January 2008 and May 2015. Participants: Five hundred Japanese GCC patients who started chemotherapy from January 2008 to May 2015. Primary and secondary outcome measures: TE was diagnosed by reviewing all the reports of contrast-enhanced computed tomography (CT) performed during the follow-up period. All types of thrombosis detected by CT or additional imaging tests, such as venous TE, arterial TE, and cerebral infarction, were defined as TE. Medical records of all identified patients were reviewed and potential risk factors for TE including clinicopathological backgrounds were collected. We defined the following patients as 'active cancer'; patients with unresectable advanced GCC, cancer recurrence during or after completing adjuvant (Adj) chemotherapy, and/or presence of other malignant tumours. Results: Of the 500 patients, 70 patients (14.0%) developed TE during the follow-up period. TEi was 9.2% and 17.3% in gastric and colorectal cancer patients, 18.1% and 3.5% in active and non-active cancer patients, and 24.0% and 12.9% in multiple and single primary, respectively. Multivariate logistic regression analysis showed that colorectal cancer (odds ratio [OR], 2.371; 95% confidence interval [CI], 1.328 to 4.233), active cancer (OR 7.593; 95% CI 2.950 to 19.543), and multiple primary (OR 2.527; 95% CI 1.189 to 5.370) were independently associated with TEi. Conclusion: TEi was 14.0% among Japanese GCC patients received chemotherapy, and was significantly higher among patients with colorectal cancer, active cancer, and multiple primary than among those with gastric cancer, non-active cancer, and single primary, respectively.

Antibody features towards VAR2CSA and CSA binding infected erythrocytes in a cohort of pregnant women from PNG

<p><i>Plasmodium falciparum </i>causes placental malaria, which results in adverse outcomes for mother and child. <i>P. falciparum</i> infected erythrocytes that express the parasite protein VAR2CSA on their surface can bind to placental chondroitin sulfate-A.  It has been hypothesized that naturally acquired antibodies towards VAR2CSA protect against placental infection, but it has proven difficult to use measures of antibody to identify individuals protected from disease. We used a systems serology approach to identify naturally acquired antibody features mid pregnancy that were associated with protection from placental malaria at delivery.  Machine learning techniques selected six out of 169 measured antibody features towards VAR2CSA that could predict (with 86% accuracy) whether a woman would subsequently have active placental malaria infection at delivery. Selected features were associated with inhibition of placental binding and/or opsonic phagocytosis of infected erythrocytes, and network analysis indicated that there are not one but multiple pathways to protection from placental malaria.</p>

EEG abnormalities and their radiographic correlates in a COVID-19 inpatient cohort

<p>Objective: To identify the prevalence of EEG abnormalities in patients with COVID-19 with neurologic changes, their associated neuroimaging abnormalities and rates of mortality.</p> <p>Methods: A retrospective case series of 192 adult COVID-19 positive inpatients with EEG performed between March and June 2020 at 4 hospitals: 161 undergoing continuous, 24 routine, and 7 reduced-montage EEG. Study indication, epilepsy history, intubation status, administration of sedatives or antiseizure medications, metabolic abnormalities, neuroimaging pathology associated with epileptiform abnormalities, and in-hospital mortality were analyzed.</p> <p>Results: EEG indications included encephalopathy (54.7%), seizure (18.2%), coma (17.2%), focal deficit (5.2%), and abnormal movements (4.6%). Epileptiform abnormalities occurred in 39.6% of patients: focal intermittent epileptiform discharges in 25.0%, lateralized periodic discharges in 6.3%, and generalized periodic discharges in 19.3%. Seizures were recorded in 8 patients, 3 with status epilepticus. Antiseizure medication administration, epilepsy history, and older age were associated with epileptiform abnormalities. Only 26.3% of patients with any epileptiform abnormality, 37.5% with electrographic seizures, and 25.7% patients with clinical seizures had known epilepsy. Background findings included generalized slowing (88.5%), focal slowing (15.6%), burst suppression (3.6%), attenuation (3.1%), and normal EEG (3.1%). Neuroimaging pathology was identified in 67.1% of patients with epileptiform abnormalities, over two-thirds acute. In-hospital mortality was 39.5% for patients with epileptiform abnormalities, 36.2% for those without. Risk factors for mortality were coma and ventilator support at time of EEG.</p> <p>Significance: This article highlights the range of EEG abnormalities frequently associated with acute neuroimaging abnormalities in COVID-19. Mortality rates were high, particularly for patients in coma requiring mechanical ventilation. These findings may guide the prognosis and management of patients with COVID-19 and neurologic changes.</p>

Is High Milk Intake Good for Children's Health? A National Population-based Observational Cohort Study

<p>Milk is widely considered as a beneficial product for growing children. This study was designed to describe the milk consumption status in Korean children aged 30&ndash;36 months and to investigate its association with the risk of obesity and iron deficiency anemia (IDA). This nationwide&nbsp;administrative study used data from the Korean national health insurance system and child health screening examinations consists of children born in 2008 and 2009. In total, 425,583 children were included, and they were divided into three groups based on daily milk consumption; low milk group (do not drink or drink &lt;200 mL milk per day, n = 139,659), reference group (drink 200&ndash;499 mL milk per day, n = 255,670), and high milk group (drink &ge;500 mL milk per day, n = 30,254). After adjusting variable confounding factors, consumption of a large amount of milk of &ge;500 mL per day at the age of 30&ndash;36 months was associated with an increased risk of obesity at the age of 42&ndash;72 months and IDA after the age of 30 months. These results may provide partial evidence for dietary guidelines for milk consumption in children that are conducive to health.</p>

Prehabilitation program in elderly patients: a prospective cohort study

<p>The preoperative period may be an opportune period to optimize the patients' physical condition with a multimodal preoperative program. The impact of a "prehabilitation" program in elderly patients is discussed. This mono-centre observational cohort study included consecutively 139 patients planned for major abdominal and thoracic surgery: 44 in the control group (age&lt;65) and 95 in the elderly group (age&gt; 65). All patients followed a "prehabilitation" program including exercise training, nutritional optimization, psychological support, and behavior change. Seventeen patients in the control group and 45 in the elderly group completed the study at six months. The 6-minute walk test (6MWT) increased in both groups from the initial evaluation to the last one (median value of 80 m [interquartile range 51] for those under 65 years; 59 m [34] for the older ones; P=0.114). The 6MWT was also similar after one month of prehabilitation for both populations. The rate of postoperative complications was similar in the two groups. Prehabilitation showed equivalence in patients over 65 years of age compared to younger patients in terms of increase in functional capabilities and of postoperative evolution. This multimodal program represents a bundle of care which can benefit the frailer population.</p>

Dabrafenib plus trametinib versus anti-PD-1 monotherapy as adjuvant therapy in BRAF V600-mutant stage III melanoma after definitive surgery: a prospectively-powered multicenter, retrospective cohort study

<p><strong>Summary </strong></p> <p>Background Both dabrafenib/trametinib (D/T) and anti-PD-1 monotherapy (PD-1) are approved adjuvant therapies for patients with stage III BRAF V600-mutant melanoma. However, there is still a lack of head-to-head comparative data. We aimed to describe efficacy and toxicity outcomes for these two standard therapies across melanoma centers.</p> <p>Methods This prospectively-powered multicenter, retrospective cohort study was conducted in 15 melanoma centers in Australia, China, Germany, Italy, Japan, UK, and US. We included adult patients with resected stage III BRAF V600-mutant melanoma who received either adjuvant D/T or PD-1. The primary endpoint was recurrence-free survival (RFS). Secondary endpoints included overall survival (OS), recurrence pattern and toxicity.</p> <p>Findings We included 598 patients with stage III BRAF V600-mutant melanoma who received either adjuvant D/T (n=393 [66%]) or PD-1 (n=205 [34%]) post definitive surgery between Jul 2015 and Oct 2022. At a median follow-up of 33 months (IQR 21-43), the median RFS was 51∙0 months (95%CI 41∙0-not reached [NR]) in the D/T group, significantly longer than 44∙8 (95%CI 28∙5-NR) in the PD-1 group (univariate: HR 0∙658, 95%CI 0∙498-0∙869, P=0∙003; multivariate: HR 0∙687, 95% CI 0∙512-0∙922, P = 0∙012), with comparable OS with PD-1 (multivariate, HR 1∙011, 95%CI 0∙637-1∙604, P&gt;0∙95). Similar findings were observed using a restricted-mean-survival-time model. D/T survival benefits were consistent in most subgroups, except for older, male patients and those with V600K mutation. Among those who experienced recurrence, the proportion of distant metastases was higher in the D/T cohort. D/T had a higher incidence of treatment modification due to adverse events (AEs) than PD-1, but fewer persistent AEs.</p> <p>Interpretation In patients with stage III BRAF V600-mutant melanoma post definitive surgery, D/T yielded better RFS than PD-1, with higher transient but lower persistent toxicity, and comparable OS. D/T is potentially a superior adjuvant option compared with PD-1. &nbsp;</p>

Intensive training programme improves handwriting in a community cohort of people with Parkinson's disease

Blood cell differential count discretization modeling predicts survival in adults reporting to the emergency room: a retrospective cohort study

<p><strong>Objectives</strong>: to assess survival predictivity of baseline blood cell differential count (BCDC), discretized according to two different methods, in adults visiting the Emergency Room (ER) for illness or trauma over one-year. </p> <p><strong>Design</strong>: Retrospective cohort study of hospital records. </p> <p><strong>Setting</strong>: Tertiary care public hospital in northern Italy. </p> <p><strong>Participants</strong>: 11052 patients aged &gt; 18 years, consecutively admitted to the ER in one year, and for whom BCDC collection was indicated by ER medical staff at first presentation.</p> <p><strong>Primary outcome</strong>: Survival was the referral outcome for explorative model development. Automated BCDC analysis at baseline assessed hemoglobin, red cell mean volume (MCV) and distribution-width (RDW), platelet distribution-width (PDW), plateletcrit (PCT), absolute red blood cells, white blood cells, neutrophils, lymphocytes, monocytes, eosinophils, basophils, and platelets. Discretization cutoffs were defined by Benchmark and Tailored methods. Benchmark cutoffs were stated on laboratory reference values (CLSI). Tailored cutoffs for linear, sigmoid-shaped and for U-shaped distributed variables were discretized by Maximally Selected Rank Statistics and by Optimal-Equal Hazard Ratio respectively. Explanatory variables (age, gender, ER  admission during SARS-CoV2 surges, in-hospital admission) were analyzed using Cox multivariable regression. ROC curves were drawn by sum of Cox-significant variables for each method.</p> <p><strong>Results</strong>: Of 11052 patients (median age 67 years, IQR 51–81, 48% female), 59% (n=6489) were discharged and 41% (n=4563) were admitted in hospital. After a 306-day median follow up (IQR 208–417 days), 9455 (86%) patients were alive and 1597 (14%) deceased. Increased HRs were associated with age &gt;73-years (HR=4.6 CI=4.0–5.2), in-hospital admission (HR=2.2 CI=1.9–2.4), ER admission during SARS-CoV2 surges (Wave-I HR=1.7 CI=1.5–1.9); Wave-II HR=1.2 CI=1.0–1.3). Gender, hemoglobin, MCV, RDW, PDW, neutrophils, lymphocytes and eosinophils counts were significant in overall. Benchmark-BCDC model included basophils and platelet count (AUROC 0.74). Tailored-BCDC model included monocyte counts and plateletcrit (AUROC 0.79).</p> <p><strong>Conclusions</strong>: baseline discretized BCDC provides meaningful insight regarding Emergency Room patients survival.</p>
