Comparison of planar digital radiography and helical standing computed tomography for assessment of condylar stress fracture risk in Thoroughbred racehorses

<p class="MsoNormal"><strong>Background</strong>: Catastrophic injury has a low incidence but leads to the death of many Thoroughbred racehorses.</p> <p class="MsoNormal"><strong>Objectives</strong>: To determine sensitivity, specificity, and reliability for condylar stress fracture risk assessment from fetlock digital radiographs (DR) and standing computed tomography (sCT).</p> <p class="MsoNormal"><strong>Study design</strong>: Controlled <em>ex vivo</em> experiment.</p> <p class="MsoNormal"><strong>Methods</strong>: A blinded set of thoracic limb fetlock DR and sCT images were prepared from 31 Thoroughbreds. Four observers evaluated the condyles and parasagittal grooves (PSG) of the third metacarpal bone for the extent of dense bone and lucency/fissure and assigned a risk assessment grade for condylar stress fracture. Sensitivity and specificity for detection of subchondral structural changes in the condyles and PSG and for risk assessment for condylar stress fracture were determined by comparison with a reference. Agreement between observers and the reference assessment and reliability between observers were determined. Intra-observer repeatability was also assessed.</p> <p class="MsoNormal"><strong>Results</strong>: Sensitivity for detection of structural change was lower than specificity for both imaging methods and all observers. For horses with normal risk, observer assessment often agreed with the reference. Sensitivity for risk assessment was lower than specificity for all observers. For horses with a high risk of injury, observers generally underestimated risk. Diagnostic sensitivity of risk assessment was improved with sCT imaging, particularly for horses with elevated risk of injury. Assessment repeatability and reliability was better with sCT than DR.</p> <p class="MsoNormal"><strong>Main limitations</strong>: The <em>ex vivo</em> study design influenced DR image sets.</p> <p class="MsoNormal"><strong>Conclusions</strong>: Risk assessment through screening with diagnostic imaging is a promising approach to improve injury prevention in racing Thoroughbreds. Knowledge of sensitivity and specificity of fetlock lesion detection provides the critical guidance needed to improve screening programs for racehorses. We found improved detection of MC3 subchondral structural change and risk assessment for condylar stress fracture with sCT <em>ex vivo</em>.</p>

Dataset: High-fidelity experimental model verification for flow in fractured porous media

<p>The dataset consists of five image series of tracer experiments in fractured porous media. The images have been taken using PET imaging and are available in DICOM format. Overall, three fractured geometries have been considered, called fractip-a, fractip-b, and fractip-e. Different boundary conditions have been used, generating in total four experiments. In addition, images of simple water displacement experiments in an intact core (fractip-j) are provided, useful for extracting macroscopic hydraulic properties of the core material. All experiments are based on water displacement and use 18F-FDG tracer for PET tracking. The available DICOM images are reconstructed using 1 min x 0.4mm space-time voxels.&nbsp;</p> <p>A related data analysis based on the Darcy Scale Image Analysis toolbox DarSIA is available at 10.5281/zenodo.10410227.</p>

Osteolitic vs Osteoblastic metastatic lesion: Computational modeling of fracture risk in the human vertebra after screws fixation procedure

<p>Metastatic lesions compromise the mechanical integrity of vertebrae, increasing the&nbsp;fracture risk. Screwfixation is usually performed to guarantee spinal stability and prevent dramatic fracture events. Accordingly, predicting the overall mechanical response in such conditions is&nbsp;critical to planning and optimizing the surgical treatment. This work proposes an image-basedfinite element computational approach describing the mechanical behavior of a patient-specific instrumented metastatic vertebra by assessing the effect of lesion size, location, type and shape&nbsp;on the fracture load and fracture patterns under physiological loading conditions. A specific&nbsp;constitutive model for the metastasis is integrated to account for the effect of the diseased tissue&nbsp;on the bone material properties. Computational results demonstrate that size, location, and type&nbsp;of metastasis significantly affect the overall vertebral mechanical response, and suggest better account these parameters in estimating the fracture risk. Combining multiple osteolytic lesions to&nbsp;account for irregular shape of the overall metastatic tissue has a not significant effect on fracture&nbsp;load of vertebra macroscopically. In addition, the combination of loading mode and metastasis&nbsp;type is shown for the first time as a critical modeling parameter in determining the fracture risk.&nbsp;The proposed computational approach moves towards defining a clinically integrated tool to&nbsp;improve the management of metastatic vertebrae and quantitatively evaluate fracture risk.</p>

Efficacy of switching from teriparatide to zoledronic acid or denosumab on bone mineral density and bone markers in patients with severe fractured osteoporosis: a real-life study

<p><strong>Abstract</strong></p> <p>Purpose: Osteoporosis is characterized by a loss of bone mass, increased bone fragility and susceptibility to fracture. Sequential treatment with bisphosphonates or denosumab (Dmab) after teriparatide (TPT) discontinuation may represent a valid treatment option, but the optimal sequential treatment strategy is unknown.</p> <p>Methods: This retrospective study enrolled 56 osteopenic/osteoporotic patients with fragility fractures who received 24 months of treatment with TPT followed by 24 months of treatment with zoledronic acid (ZOL) (TPT+ZOL) or Dmab (TPT+Dmab). Clinical features, incident fractures, bone mineral density (BMD) measurements, and bone marker profiles were characterized. Ordinary one-way ANOVA adjusted for multiple testing analyzed the difference between median T-scores at baseline, after 24 months of TPT, and after 2 doses of ZOL or at least 3 doses of Dmab when variables failed the normality test.</p> <p>Results: Twenty-three patients received TPT+ZOL (19 females, 4 males; median [IR] age, 74.3 [66.9, 78.6] years) and 33 patients received TPT+Dmab (31 females, 2 males; mean&plusmn;SD age, 66.6&plusmn;11.3 years). Mean lumbar and hip T-scores were significantly higher after sequential treatment with both TPT+ZOL and TPT+Dmab (all p&lt;0.05 vs baseline). In particular, the mean lumbar T-score increased in about 80% of patients by 8.5&plusmn;58% after ZOL, while after Dmab, the mean lumbar T-score was -9.6&plusmn;36%. Mean&plusmn;SD femur neck and hip T-scores increased by 12&plusmn;13% and 9&plusmn;31%, respectively, after treatment with ZOL, while the change in femur neck and hip T-scores after Dmab were 5&plusmn;41% and 27&plusmn;36%, respectively, No significant between-group differences were identified. Fragility fractures occurred in 3 (13%) patients treated with TPT+ZOL and in 5 (15%) patients treated with TPT+Dmab.</p> <p>Conclusion: Sequential therapy with TPT+ZOL increased bone mineralization at the lumbar level and stabilized bone mineralization at the femoral level. Both ZOL and Dmab are effective sequential treatments after TPT.</p>
