Assessing the Impact of an Intervention to Improve the Quality of Emergency Obstetric Care on Maternal Health in Nigeria

<p>Nigeria suffers some of the highest rates of maternal and perinatal mortality in the world. Poor quality of obstetric care is a key factor, and although not quantified at a national level, local studies suggest that high rates of maternal and perinatal deaths are due to clinical management errors in hospitals. This project therefore explored the nature of existing practices and quality of clinical management of three leading causes of maternal and perinatal deaths (primary postpartum haemorrhage, eclampsia, and obstructed labour); implement an intervention package aimed at improving the quality of clinical management of these causes of deaths; and test the effectiveness of this package on quality of care indicators and maternal and perinatal outcomes. </p> <p> </p> <p>The study follows a quasi-experimental research design, using interrupted time series analysis for assessment over a three year period.  Six public secondary care hospitals and two public tertiary care hospitals in both north and south of Nigeria were selected randomly. Formative research was conducted in all eight hospitals, and thereafter the intervention will be carried out in four hospitals, with the remaining four hospitals functioning as control sites. The package consists of training in use of evidence-based clinical protocols on postpartum haemorrhage (PPH), preeclampsia/eclampsia and obstructed labour, audit and feedback, reminders, commodity supply monitoring and replenishments and patients’ education. </p> <p> </p> <p>Clinical outcomes will be compared between intervention and control sites in a total of 2334 cases of PPH, preeclampsia/eclampsia and obstructed labour (based on an anticipated 50% reduction in case fatality rates, significance level of 0.05 and 80% power). This will consist of 1167 cases in the intervention sites and 1167 in the control sites.  Maternal and perinatal mortality rates will serve as primary outcome measures. Secondary outcomes involve quality of care drawn from United Nations process indicators, skilled attendance index and data derived from the formative audit phase of the project.</p> <p> </p> <p>The results will enable the identification of a system-wide quality of care framework for improving the delivery of emergency obstetric care for the prevention of maternal and perinatal morbidity and mortality in Nigeria.</p>

Raw Data for the article: Post-crisis debriefing: A tool for improving quality in the medical emergency team system

<p><strong>Objectives:&nbsp;</strong>To examine clinicians&#39; perception of quality of technical and non-technical response to emergencies and application of post crisis debriefing.</p> <p><strong>Design:&nbsp;</strong>Descriptive, anonymous, self-reporting survey on the needs and perception of a post-crisis debriefing implementation.</p> <p><strong>Setting:&nbsp;</strong>Multi-specialist medical institute in Italy focused on solid organ transplantation and organ failure support.</p> <p><strong>Main outcomes:&nbsp;</strong>Perception of application of guidelines and evaluation of debriefing implementation during in-hospital emergencies.</p> <p><strong>Results:&nbsp;</strong>Response rate to the survey was 25% (148 health care workers). Of all respondents, 86% were employed &gt;10 years, 75% were involved in &le;5 emergencies over the previous year. Resuscitation guidelines were considered fully applied by 55%; 64% of respondents considered the teaching programme as sufficient. Of all participants, 97% were aware of the importance of teamwork dynamics, 79% were aware of the importance of the personal performance, and 52% considered emergencies as valid opportunities for professional growth. Leadership was considered important by 45% of respondents; debriefing implemented by 41%, and considered a potentially useful tool by 85%.</p> <p><strong>Conclusion:&nbsp;</strong>Post-crisis debriefing is a way to diffuse self-reflective and life-long learning culture; it is perceived as a powerful tool for improving quality of the rapid response system by the vast majority of those surveyed.</p> <p><strong>Keywords:&nbsp;</strong>Communication; Crisis resource management; Emergency; ICU; Intensive care medicine; Leadership; Patient safety; Teamwork.</p>

Data from: Improving governance outcomes for water quality: insights from participatory social network analysis for chalk stream catchments in England

<p>Globally important chalk streams in England are in poor ecological health, in part due to inadequate water quality. Addressing this issue requires an understanding of the governance systems that surround water quality. The complexity and uncertainty inherent in hydrological systems has led to the emergence of integrated and adaptive forms of governance. In these multi-actor governance systems, the structure of the relationships between actors (the social network) has been shown to affect governance processes and outcomes.</p> <p>Using participatory social network analysis, we mapped and analysed the social networks for the River Test and River Itchen in Hampshire, UK, to identify actors and their roles, determine the network characteristics, and identify interventions to improve governance.</p> <p>Although the results suggest a well connected network of actors from the state, private sector and civil society, we find that decision making is not decentralised. Bureaucratic governance by central state actors dominates. However, trust in these central state actors and private actors in the networks is low, which undermines collaboration and co-ordination in the network.</p> <p>Devolving authority to local actors, building trust in the networks, and improving connections to important actors could help to improve governance outcomes for water quality.</p>

Improving quality of life among healthy individuals through theatre activities: a theatre-in-health pilot project

<p><strong>Background</strong></p> <p>Review of previous studies regarding arts and health reveals knowledge gaps regarding the use of theatre&nbsp;activities outside a clinical setting with healthy participants. The purpose of this study was to evaluate quality of life benefits of structured theatre training improvisations among generally healthy individuals.</p> <p><strong>Methods</strong></p> <p>A mixed methods approach was used at three theatre activities workshops. Pre- and post-quality of life surveys were completed at administered at each workshop. workshop. Participants also free wrote after the workshops, reflecting on their experiences.</p> <p><strong>Results</strong></p> <p>The surveys showed that there was a statistically significant shift in a positive direction in each domain studied. Themes that emerged from the free-writing responses were those of play, social acceptance, gratitude, and community.</p> <p><strong>Conclusions</strong></p> <p>Findings demonstrate that structured theatre activities produce quality of life benefits across all domains. This pilot study indicates the potential for such theatre-in-health projects as a tool for quality of life improvement.</p>

HINC dataset from: Homeopathic treatment as an add-on therapy may improve quality of life and prolong survival in patients with non-small cell lung cancer: A prospective, randomized, placebo-controlled, double-blind, three-arm, multicenter study

<p class="CxSpFirst"><b>Background:</b> Patients with advanced non-small cell lung cancer (NSCLC) have limited treatment options. Alongside conventional anticancer treatment, additive homeopathy might help to alleviate side effects of conventional therapy. The aim of the present study was to investigate whether additive homeopathy might influence quality of life (QoL) and survival in NSCLC patients.</p> <p class="CxSpMiddle"><b>Methods:</b> In this prospective, randomized, placebo-controlled, double-blind, three-arm, multicenter, phase III study, we evaluated the possible effects of additive homeopathic treatment compared with placebo in NSCLC stage IV patients with respect to QoL in the two randomized groups and survival time in all three groups. Treated patients visited the outpatients' centers every 9 weeks. 150 Patients with stage IV NSCLC were included in the study. 98 received either individualized homeopathic remedies (n=51) or placebo (n=47) in a double-blinded fashion. 52 control patients without any homeopathic treatment were observed for survival only. The constituents of the different homeopathic remedies were mainly of plant, mineral or animal origin. The remedies were manufactured by stepwise dilution and succussion, thereby preparing stable Good Manufacturing Practice grade formulations.</p> <p class="CxSpMiddle"><b>Results:</b> QoL as well as functional and symptom scales showed significant improvement in the homeopathy group when compared with placebo after 9 and 18 weeks of homeopathic treatment (p&lt;0.001). Median survival time was significantly longer in the homeopathy group (435 days) vs placebo (257 days; p=0.010) as well as vs control (228 days; p&lt;0.001). Survival rate in the homeopathy group differed significantly from placebo (p=0.020) and from control (p&lt;0.001).</p> <p class="CxSpMiddle"><b>Conclusion:</b> QoL improved significantly in the homeopathy group compared with placebo. In addition, survival was significantly longer in the homeopathy group versus placebo and control. A higher QoL might have contributed to the prolonged survival. The study suggests that homeopathy positively influences not only QoL but also survival. Further studies including other tumor entities are warranted.</p>

Fencing farm dams to exclude livestock halves methane emissions and improves water quality

<p>Agricultural practices have created tens of millions of small artificial water bodies ("farm dams" or "agricultural ponds") to provide water for domestic livestock worldwide. Among freshwater ecosystems, farm dams have some of the highest greenhouse gas (GHG) emissions per m<sup>2</sup> due to fertilizer and manure run-off boosting methane production – an extremely potent GHG. However, management strategies to mitigate the substantial emissions from millions of farm dams remain unexplored. We tested the hypothesis that installing fences to exclude livestock could reduce nutrients, improve water quality, and lower aquatic GHG emissions. We established a large-scale experiment spanning 400 km across south-eastern Australia where we compared unfenced (N = 33) and fenced farm dams (N = 31) within 17 livestock farms. Fenced farm dams recorded 32% less dissolved nitrogen, 39% less dissolved phosphorus, 22% more dissolved oxygen, and produced 56% less diffusive methane emissions than unfenced dams. We found no effect of farm dam management on diffusive carbon dioxide emissions and on the organic carbon in the soil. Dissolved oxygen was the most important variable explaining changes in carbon fluxes across dams, whereby doubling dissolved oxygen from 5 to 10 mg L<sup>-1</sup> led to a 74% decrease in methane fluxes, a 124% decrease in carbon dioxide fluxes, and a 96% decrease in CO<sub>2</sub>-eq (CH<sub>4</sub> + CO<sub>2</sub>) fluxes. Dams with very high dissolved oxygen (&gt;10 mg L<sup>-1</sup>) showed a switch from positive to negative CO<sub>2</sub>-eq. (CO<sub>2</sub> + CH<sub>4</sub>) fluxes (i.e., negative radiative balance), indicating a positive contribution to reducing atmospheric warming. Our results demonstrate that simple management actions can dramatically improve water quality and decrease methane emissions while contributing to more productive and sustainable farming.</p>

SUPPLIMANTRY [Vacuum Ohmic Heating: A Promising Technology for the Improvement of Tomato Paste Processing, Safety, Quality and Storage Stability]

<p><strong>Abstract</strong>: Ohmic heating (OH) is an electrothermal technology used to inactivate enzyme and microbial activities. This work aimed to study the impact of Ohmic Heating Under Vacuum (OHUV) which compared to conventional heating (CH) as well as storage stability at 5&deg;C and 25 &deg;C on microbial safety, and nutritional quality. The evaluation parameters were pH, titratable acidity, TSS, lycopene, ascorbic acid, PME, HMF, and microbiological activity. The obtained results showed that tomato paste samples that were treated by OHUV are significantly superior to CH in terms of all physicochemical and microbiological characteristics, as well as being the least harmful during storage in both transparent and dark packages. The results showed the changes in ascorbic acid, lycopene, and HMF values that were treated by OHUV at 25 &deg;C and filled in transparent package are most affected compared to other treated samples. On the other hand, tomato paste samples stored in dark packages at 5 &deg;C performed significantly better than those subjected to CH under the same conditions and activated PME the most had higher ascorbic acid and lycopene and fewer changes in HMF during storage time for 90 days. OHUV found to be a good alternative treatment in the production of tomato paste.</p>

Data from: Novel methods to define invasive procedures at the end-of-life were developed to improve quality of end of life care research: A population-based cohort study in colorectal cancer

<p><strong>Background</strong></p> <p>Understanding the use of invasive procedures (IPs) at the end-of-life (EoL) is important to avoid under- and overtreatment, but epidemiologic analysis is hampered by limited methods to define treatment intent and EoL phase. This study applied novel methods to report IPs at the EoL using a colorectal cancer (CRC) case study.</p> <p><strong>Methods</strong></p> <p>An English population-based cohort of adult patients diagnosed between 2013 and 2015 was used with follow-up to 2018. Procedure intent (curative, non-curative, diagnostic) by cancer site and stage at diagnosis was classified by two surgeons independently. Joinpoint regression modelled weekly rates of IPs for 36 sub-cohorts of patients with incremental survival of 0-36 months. EoL phase was defined by a significant IP rate change before death. Zero-inflated Poisson regression explored associations between IP rates and clinical/sociodemographic variables.</p> <p><strong>Results</strong></p> <p>Of 87,731 patients included, 41,972 (48%) died. 9,492 procedures were classified by intent (interrater agreement 99.8%). Patients received 502,895 IPs (1.39 and 3.36 per person year for survivors and decedents). Joinpoint regression identified significant increases in IPs four weeks before death in those living 3-6 months, and eight weeks before death in those living 7–36 months from diagnosis. 7,908 (18.8%) patients underwent IPs at the EoL, with stoma formation the most common major procedure. Younger age, early-stage disease, men, lower comorbidity, those receiving chemotherapy and living longer from diagnosis were associated with IPs.</p> <p><strong>Conclusions</strong></p> <p>Methods to identify and classify IPs at the EoL were developed and tested within a CRC population. This approach can be now extended and validated to identify potential under- and overtreatment. </p>
