Metabolic syndrome and risk of lung cancer: An analysis of Korean National Health Insurance Corporation database
<p>Introduction: Metabolic syndrome is known to increase the risk of several cancers. However, the association between lung cancer and metabolic syndrome remains unclear. Thus, we investigated the impact of metabolic syndrome on the incidence of lung cancer.</p> <p>Methods: This study enrolled participants in a health screening program provided by the Korean National Health Insurance Service between January 2009 and December 2012. The incidence of lung cancer was observed until December 2016. We analyzed the risk of lung cancer according to the presence of metabolic syndrome, metabolic syndrome components, and number of metabolic syndrome components.</p> <p>Results: During the study, 45,635 new cases of lung cancer were recorded among 9,586,753 participants. The presence of metabolic syndrome and all its components was positively associated with the risk of lung cancer in males after multivariate adjustment (hazard ratio (HR) of metabolic syndrome 1.15, 95% confidence interval (CI) = 1.12–1.18). The risk of lung cancer increased with the number of components present. The effect of metabolic syndrome on the increasing risk of lung cancer is may be higher in underweight male ever smokers than in other participants.</p> <p>Conclusion: Metabolic syndrome was associated with an increased risk of lung cancer in males. Moreover, the higher the number of metabolic syndrome components, the higher the risk of lung cancer.</p>
Data from: Validation of a non-invasive method for the early detection of metabolic syndrome: a diagnostic accuracy test in a working population
Objectives. A non-invasive method for the early detection of Metabolic Syndrome (NIN-MetS) using only Waist to Height Ratio (WHtR) and Blood Pressure (BP) has recently been published, with fixed cut-off values for gender and age. The aim of this study was to validate this method in a large sample of Spanish workers. Design. A diagnostic test accuracy to assess the validity of the method was performed. Setting. Occupational Health Services. Participants. The studies were conducted in 2012-2016 on a sample of 60,799 workers from the Balearic Islands (Spain). Interventions. The NCEP-ATP III criteria were used as the gold standard. NIM-MetS has been devised using classification trees (the CHAID, Chi-squared Automatic Interaction Detection method). Main outcome measures. Anthropometric and biochemical variables to diagnose MetS. Sensitivity, specificity, validity index and Youden Index were determined to analyse the accuracy of the diagnostic test (NIM-MetS). Results. Regarding the validation of the method, sensitivity was 54.7%, specificity 94.9% and validity index 91.2%. The cut-off value for WHtR was 0.54, ranging from 0.51 (lower age group) to 0.56 (higher) in the age groups. Variables more closely associated with MetS were WHtR (AUC=0.85; 95% CI: 0.84-0.86) and Systolic Blood Pressure (AUC=0.79; 95% CI: 0.78-0.80). The final cut-off values for the non-invasive method were WHtR≥0.56 and BP≥128/80 mmHg, which includes four levels of MetS risk (very low, low, moderate and high). Conclusions. The analysed method has shown a high validity index (higher than 91%) for the early detection of MetS. It is a non-invasive method easy to apply and interpret in any health care setting. This method provides a scale of MetS risk which allows a more accurate detection and a more effective intervention.
Data from: Metabolic syndrome is a strong risk factor for minor ischemic stroke and subsequent vascular events
Background Minor ischemic stroke (MIS) represents a major global public health problem worldwide due to high incidence. The aim of this study was to investigate whether metabolic syndrome (MetS) is a strong risk for MIS and subsequent vascular events (SVE). Methods A retrospective cohort study was performed examining symptomatic MIS in a Chinese neurologic outpatient population aged over 25 years without history of stroke. MetS was defined using the International Diabetes Federation criteria. MIS was diagnosed by magnetic resonance imaging-diffusion weighted images or fluid-attenuated inversion recovery. Results Of 1361 outpatients, a total of 753 (55.3%) patients were diagnosed with MIS; of them, 80% had a score of 0 using the MIS had a 0 score on the National Institutes of Health Stroke Scale. Among these, 303 (40.2%) individuals with MIS were diagnosed with MetS. Diagnosed of MIS with MetS significantly correlated with abdominal obesity (30.7% v.s 18.0%), hypertension (91.1% v.s 81.6%), increased blood glucose (6.9±2.4 v.s 5.0±0.4), dyslipidemia (78.2% v.s 48.2%), and SVE (50.5% v.s 11.3%) when compared with the controls group. On adjusted analysis, the risk of SVE was also significantly associated with three additional MetS criterion (RR,9.0; 95% CI, 5.677–14.46). Using Cox proportional analysis, risk of SVE in patient with MIS was significantly associated with MetS (RR, 3.3; 95% CI, 1.799–6.210), older age (RR, 1.0; 95% CI, 1.001–1.048), and high blood glucose (RR,1.1; 95%CI, 1.007–1.187). Conclusions The MetS is a strong risk factor for MIS, and patients presenting with MIS and MetS are at a high risk of SVE. Further studies are required to determine the improvement of Mets prevention in the reduction of MIS and SVE.
