Predictors of abnormal computed tomography findings for paediatric head injury: a retrospective cohort study
<p class="MDPI17abstract"><b>Objectives:</b> Head injuries in children are common causes for visits to the emergency department (ED). Computed tomography<b> (</b>CT) scans are useful for confirming head injury diagnoses. However, radiation exposure from CT scans might cause lethal malignancies. We aimed to examine predictors for the indication of performing CT scans necessary for diagnosis.</p> <p class="MDPI17abstract"><b>Design:</b> Retrospective cohort study.</p> <p class="MDPI17abstract"><b>Setting:</b> Three EDs in Japan</p> <p class="MDPI17abstract"><b>Participants</b>: Patients aged <16 years with head trauma who underwent CT.</p> <p class="MDPI17abstract"><b>Primary and Secondary Outcome Measures</b>: The primary outcome measure was abnormal CT findings that were evaluated using the area under the receiver-operating characteristic curve (AUC). We derived predictors from three existing CDRs: Canadian Assessment of Tomography for Childhood Head Injury (CATCH), Children's Head Injury Algorithm for the Prediction of Important Clinical Events (CHALICE), and Paediatric Emergency Care Applied Research Network (PECARN).</p> <p class="MDPI17abstract"><b>Results:</b> Of 1,103 eligible patients, 410 were included in this study. There were 283 (68%) boys, and the median age was 2 years. In total, 35 (9%) patients showed an abnormality, 73 (18%) were admitted, and 3 (0.7%) underwent neurosurgery. We developed a CDR consisting of 6 predictors for identifying children with abnormal CT findings: (1) severe or worsening headache; (2) GCS <15; (3) signs of skull fracture; (4) hematoma; (5) loss of consciousness; and (6) altered mental status. Our CDR had a sensitivity of 74.3%, a specificity of 75.2%, a negative predictive value of 96.9%, and a positive predictive value of 21.8%. The AUC for our rule was not inferior to those for CATCH, CHALICE, and PECARN {0.75 (95% confidence interval [CI], 0.67-0.81) versus 0.64 (95% CI, 0.56-0.73; p<0.05), 0.68 (95% CI, 0.60–0.76; p=0.28), and 0.67 (95% CI, 0.60-0.74); p=0.10}.</p> <p class="MDPI17abstract"><b>Conclusions:</b> Our findings suggest that a CDR, which lowers the frequency of CT in children with head injuries, must be developed and validated.</p>
Data from quantitative real-time PCR of corticotropin-releasing hormone and glucocorticoid receptor in the hippocampus and hypothalamus of rats subjected to midline fluid percussion injury or control sham surgery.
<p>These files contain all data from quantitative real-time PCR of corticotropin-releasing hormone (CRH) and glucocorticoid receptor (GR) measured in the hippocampus and hypothalamus of male and female rats. Rats were randomly assigned to a treatment group before the initiation of the study. Rats received a control sham surgery or were subjected to midline fluid percussion injury to induce a diffuse traumatic brain injury. Tissue biopsies were collected at 7 days post-injury and analyzed via quantitative real-time PCR. </p> <p> </p>
COVID-19 clinician moral injury survey
<p><b>Background</b></p> <p>Moral injury is an emerging explanation of burnout and suicidality, but remains poorly quantified in at-risk practitioners. We hypothesized that COVID-19 pandemic-related moral injury differs between frontline clinicians, genders, age, and country of practice.<br> </p> <p><b>Methods</b></p> <p>We conducted an online cross-sectional survey of international physicians, nurses, nurse practitioners, paramedics and respiratory therapists between April and June 2020. We included the adapted version of the Expressions of Moral Injury Scale (EMIS). The primary outcome was differences in moral injury scores between clinician roles.</p> <p><b>Results</b></p> <p>Three hundred and two clinicians participated, including physicians (61% [n=184]), nurses (28% [n=85]), and nurse practitioners (5% [n=14]). The median age was 39 (IQR 32-76), females comprised 54% of the respondents, and the majority resided in Canada (n =183 [61%]) or the United States (US; n = 106 [35%]). Emergency medicine (88% [n=265]), and intensive care (6% [n=17]) were the main specialties responding. Median moral injury scores across multiple domains were higher for nurses compared to physicians, as well as for younger, and female respondents. Moral injury scores were also significantly higher for respondents from the United States, the United Kingdom and Australia, compared to Canada.</p> <p><b>Conclusions</b></p> <p>Our research suggests that during COVID-19, measures of moral injury differ across roles, gender and place of work. Future research is warranted to better understand the impact of moral injury on clinicians' psychological well-being during the COVID-19 pandemic.</p>
The effect of methylene blue on white matter injury after MCAO in rats and mice
<p><b>Objectives</b>: Methylene blue, the FDA-grandfathered drug was proved to be neuroprotective in ischemic stroke in rat. However, the mechanism of the protective effect was unknown. In this study, we used different animal models to investigate the effect of MB administration given within and beyond the therapeutic time window on behavioral deficits and infarct volume and related mechanism about the white matter protection.</p> <p><b>Methods:</b> Middle cerebral artery occlusion and reperfusion (MCAO) and photothrombotic middle cerebral artery occlusion (PT-MCAO) models were used. Behavioral deficits and infarct volume were measured by foot fault test, Garcia neurological score and TTC staining. Black gold staining and western blot were used to evaluate the brain white matter injury.</p> <p><b>Results:</b> We found that intraperitoneal administration of MB immediately or 24h after the MCAO or PT-MCAO surgery reduced infarct volume, improved the neurological deficits, reduced the white matter injury via myelin Basic Protein (BMP) protection.</p> <p><b>Conclusion:</b> These findings suggested that MB relieved white matter injury besides neuronal protection and has potential therapeutic effects on ischemic stroke.</p>
Data for: Male coercion and female injury in a sexually cannibalistic mantis
<p>Sexual conflict can generate male traits that enhance mating success but lead to injury in females. Pre-copulatory sexual cannibalism—where females eat males without mating—has the potential to select for harmful coercive traits as well, but few examples are known. Here, we show that males of the highly cannibalistic Springbok mantis, <i>Miomantis caffra</i>, wrestle females during pre-mating interactions. We find that most initial contacts between males and females involve a violent struggle whereby each sex tries be the first to grasp hold of the other with their raptorial forelegs. When females win the struggle, they always cannibalise males. However, when males grasp females first, they dramatically increase the chance of mating. We also find striking evidence that males wound females with their fore-tibial claws during struggles, resulting in haemolymph loss and scar tissue formation. Taken together, our results show how males can overcome the threat of cannibalism by coercively wrestling females. We argue that pre-copulatory injury in this species is likely a negative pleiotropic side-effect of selection on claws to capture prey.</p>
Fitness implications of nonlethal injuries in scorpions: females, but not males pay reproductive costs
<p>The ability to detach a body part in response to a predation attempt is known as autotomy, and it is perhaps the most intensively studied form of nonlethal injury in animals. Although autotomy enhances survival, it may impose reproductive costs to both males and females. We experimentally investigated how autotomy affects the reproductive success of males and females of a scorpion species. Individuals of Ananteris balzani autotomize the last abdominal segments ("tail"), losing the anus and leading to lifelong constipation since regeneration does not occur. Although male "tail" is used during courtship and sperm transfer, autotomy had no effect on male mating success. The combined effect of increased mortality and reduced fecundity resulted in autotomized females producing nearly 35% less offspring than intact females. In conclusion, the negative effects of "tail" autotomy are clearly sex-dependent, probably because the factors that influence reproductive success in males and females are markedly different.</p>
Concerns of surrogate decision makers for acute brain injury patients: a US population survey
<p><b>Objective:</b> To determine whether groups of surrogates for severe acute brain injury (SABI) patients with poor prognosis can be identified based on their prioritization of goals-of-care (GOC) decisional concerns, an online survey of 1588 adults recruited via a probability-based panel representative of the US population was conducted. </p> <p><b>Methods:</b> Participants acted as a surrogate for a GOC decision for a hypothetical SABI patient and were randomized to one of two prognostic scenarios: the patient likely being left with a range of severe functional disability (SD) or remaining in a vegetative state (VS). Participants prioritized a list of 12 decisional concerns via Best-Worst Scaling. Latent class analysis (LCA) was used to discover decisional groups.</p> <p><b>Results: </b>The completion rate was 44.6%; data weighting was conducted to mitigate nonresponse bias. For 792 SD respondents, LCA revealed four groups. All groups shared concerns regarding respecting patient wishes and minimizing suffering. The four groups were otherwise distinguished by unique concerns that their members highlighted: an older adult remaining severely disabled (34.4%), family consensus (26.4%), doubt regarding prognostic accuracy (20.7%), and cost of long-term care (18.6%). For the 796 VS respondents, LCA revealed five groups. Four of the five groups had similar concern profiles to the four SD groups. The largest (29.0%) expressed the most prognostic doubt. An additional group (15.8%) prioritized religious concerns.</p> <p><b>Conclusions:</b> While surrogate decision makers for patients with SABI are concerned with respecting patient wishes and minimizing suffering, certain groups highly prioritize other specific decisional factors. These data can help inform future interventions for supporting decision makers. </p>
