Study of the medical service efficiency of county-level public general hospitals based on medical quality constraints: A cross-sectional study
<p><strong>Objectives:</strong> Since the new medical reform in 2009, county-level hospitals in China have achieved rapid development, but health resource waste and shortage issues still exist.</p> <p><strong>Design</strong>: We applied the Meta-frontier and SBM-undesirable(Slacks-based Measurement-undesirable) DEA(Data Envelopment Analysis) model to measure the medical service efficiency with or without medical quality constraints of the county-level public general hospitals (CPGHs). The assessment includes four inputs, three desirable outputs, and one undesirable output. We conducted the assessment via Max-DEA V.8.19 software. Moreover, we analyze the factors affecting CPGHs' medical service efficiency based on the FRM model. </p> <p><strong>Setting:</strong> A total of 77 sample CPGHs were selected from Shanxi province in China from 2013 to 2018.</p> <p><strong>Results</strong>: The results of this study showed that the efficiency level of county-level public hospitals in Shanxi Province is relatively low overall (the mean value of efficiency is 0.61 without quality constraints and 0.63 under quality constraints). This showed that ignoring medical quality constraints will result in lower efficiency and lower health resource utilization for high-medical-quality hospitals. The medical service efficiency of CPGHs differs greatly among different regions. Under the meta-frontier, the hospitals in the central region had the highest efficiency (efficiency score 0.70), followed by those in the south (efficiency score 0.63), and the hospitals in the north had the lowest efficiency (efficiency score 0.54). Factors that have larger impacts on the service efficiency of county public hospitals are the average length of hospital stay, per capita disposable income and financial subsidy income.</p> <p><strong>Conclusions</strong>: To improve CPGHs' medical service efficiency, the government should increase investment in the northern region, and hospitals should improve the management level and allocate human resources rationally.</p>

Current situation of the hospitalization of persons without family in Japan and related medical challenges
<p>This study aims to determine the approximate number of hospitalizations of persons without family and the medical challenges they encounter in hospitals across Japan. Self-administered questionnaires were mailed to 4,000 randomly selected hospitals nationwide to investigate the actual conditions and problems, decision-making processes, and use of the government-recommended Guidelines for the hospitalization of, and decision-making support for, persons without family. </p>

<p>Base de datos de la investigación realizada en Hospital Provincial María Eugenia Neto de la provincia de Zaire, República de Angola con el objetivo de determinar la correlación existente entre la aparición de pie diabético y la presencia o no de la retinopatía diabética durante el bienio septiembre de 2020 a septiembre de 2022. El universo del estudio quedó conformado por 181 pacientes con diagnóstico de pie diabético, que a la vez conformó la muestra del estudio. Se tomaron en consideración como variables secundarias las sociodemográficas: edad y sexo. Las variables principales del estudio fueron las clínicas, tales como: enfermedades crónicas asociadas, presencia o no de retinopatía diabética, úlcera en miembros inferiores, amputación en miembro inferior, años con la diabetes.</p>
CONCORDANCE OF BRONCHOSCOPIC AND RADIOLOGICAL FINDINGS IN SUSPECTED LUNG CANCER AND ITS OUTCOME- A CROSS SECTIONAL STUDY IN A TERTIARY CARE HOSPITAL
<p>Background</p> <p>Bronchoscopy and CT thorax are the most common investigations utilized to screen and diagnose lung cancer. Their individual utility in diagnosing lung cancer has been described affirmatively in existing literature. Studies correlating the airway characteristics of the two modalities in lung cancer are few.</p> <p>Objectives</p> <p>To analyze and characterize lung lesions bronchoscopically and correlate the same radiologically by CT of the thorax and thus assess its positive predictive value.</p> <p>Methods</p> <p>56 consecutive adults who presented to Respiratory Medicine Department at a tertiary care hospital in South India from November 2018 to June 2020, having Clinico-radiological suspicion of malignancy and fulfilling study criteria were recruited. They were subjected to CECT of thorax and bronchoscopy. All bronchoscopic procedures were performed using a protocolized number of passes for biopsy. The baseline demographic and clinical data, findings of bronchoscopy and CT and biopsy reports were recorded. Cohens kappa coefficient was used to estimate the agreement of findings on the two modalities. Statistical analysis was performed using SPSS software.</p> <p>Results</p> <p>Bronchoscopy was normal in 22.14 % of cases, the corresponding CTs also revealed normal airway. Bronchoscopy revealed airway lesions in 78.6 % cases; the corresponding CT revealed airway abnormalities in only 46.41 % of cases, among these 52.1 % of cases revealed an exophytic growth. There was fair strength of agreement for the two modalities in the detection of airway lesions of lung cancer. [k= 0.38, p = <0.001]. CECT thorax has a negative predictive value of 44 %, a sensitivity of 59.1 % and specificity of 100 % at 95 % CI in the detection of airway lesions.</p> <p>Conclusion</p> <p>CT is not definitive in the evaluation of lung cancer, especially in those with central disease. <p>Match<sup>RN</sup> Psychiatry is a multicenter observational study of Swiss psychiatric hospitals. This study's purpose is to describe health care quality by exploring hospital structures such as nurse staffing and the work environment; processes such as the rationing of care; nurse outcomes, including job satisfaction and work-life balance. The sample for this study included approximately 1200 nurses from 115 units of 13 psychiatric hospitals in Switzerland's German-speaking region. The nurse survey covered three dimensions: work environment, patient safety climate, and the rationing of care. Data were collected between September 2019 and June 2021. The Match<sup>RN</sup> Psychiatry study is funded by the participating psychiatric hospitals and the Swiss Psychiatric Nursing Leaders Association (Vereinigung Pflegekader Psychiatrie Schweiz).</p>
