A case report on intensive, robot-assisted rehabilitation program for brainstem radionecrosis
<p>Radiotherapy is a valid treatment option for nasopharyngeal carcinoma. However, complications can occur following irradiation of the closest anatomical structures, including brainstem radionecrosis (BRN). The rehabilitation is poorly described in patients with BRN, despite its usefulness in improving functional independence in patients with brain tumors. We aimed at testing the usefulness of intensive, robot-assisted neurorehabilitation program to improve functional independence in a 57-year-old male with BRN.</p> <p>Patient concerns: </p> <p>A 57-year-old male diagnosed with a nasopharyngeal carcinoma, received a radiation total dose of 72 Gy. Owing to the appearance of a severe symptomatology characterized by dysphagia, hearing loss, and left sided hemiparesis, the patient was hospitalized to be provided with intensive pharmacological and neurorehabilitation treatment.</p> <p>Diagnosis: </p> <p>Follow-up brain magnetic resonance imaging disclosed no residual cancer, but some brainstem lesions compatible with BRN areas were appreciable.</p> <p>Intervention: </p> <p>The patient underwent a 2-month conventional, respiratory, and speech therapy. Given that the patient only mildly improved, he was provided with intensive robot-aided upper limb and gait training and virtual reality-based cognitive rehabilitation for other 2 months.</p> <p>Outcomes: </p> <p>The patient reported a significant improvement in functional independence, spasticity, cognitive impairment degree, and balance.</p> <p>Conclusion: </p> <p>Our case suggests the usefulness of neurorobotic intensive rehabilitation in BRN to reduce functional disability. Future studies should investigate whether an earlier, even multidisciplinary rehabilitative treatment could lead to better functional outcome in patients with BRN.</p>
Improving neuropsychiatric symptoms following stroke using virtual reality: A case report
<p>Post-stroke cognitive impairment occurs frequently in patients with stroke, with a 20% to 80% prevalence. Anxiety is common after stroke, and is associated with a poorer quality of life. The use of standard relaxation techniques in treating anxiety in patients undergoing post-stroke rehabilitation have shown some positive effects, whereas virtual reality seems to have a role in the treatment of anxiety disorders, especially when associated to neurological damage. A 50-year-old woman, smokers, affected by hypertension and right ischemic stroke in the chronic phase (i.e., after 12 months by cerebrovascular event), came to our observation for a severe anxiety state and a mild cognitive deficit, mainly involving attention and visuo-executive processes, besides a mild left hemiparesis. Anxiety in a patient with ischemic stroke. Standard relaxation techniques alone in a common clinical setting or the same psychological approach in an immersive virtual environment (i.e., Computer Assisted Rehabilitation Environment – CAREN). The patient's cognitive and psychological profile, with regard to attention processes, mood, anxiety, and coping strategies, were evaluated before and after the 2 different trainings. A significant improvement in the functional and behavioral outcomes were observed only at the end of the combined approach.The immersive virtual reality environment CAREN might be useful to improve cognitive and psychological status, with regard to anxiety symptoms, in post-stroke individuals.</p>
The successful treatment of Enterocytozoon bieneusi Microsporidiosis with nitazoxanide in a patient with B-ALL: A case report
<div> <strong>Introduction</strong>: <em>Enterocytozoon</em> <em>bieneusi</em> (<em>E</em>. <em>bieneusi</em>) Microsporidia can cause opportunistic infections in immunocompromised patients and is also an emerging disease in these individuals. Its clinical manifestations are chronic diarrhea and severe wasting syndrome, these can be extremely debilitating and carry a significant risk of death for immunocompromised patients. Often, microsporidia cannot be confirmed immediately by routine examination and culture. Effective and available treatment options are limited for infections caused by <em>E</em>. <em>bieneusi</em> in humans. Such cases are very rare in the Chinese Mainland.</div> <div> </div> <div> <strong>Case Presentation</strong>: A 47-year-old male had recurrent, profuse watery diarrhea and abdominal discomfort for more than 7 months, with a fever for 5 days. Two years earlier, he received treatment with a modified BFM-90 protocol for acute B cell lymphoblastic leukemia and is currently in the final stages of maintenance therapy with oral methotrexate and mercaptopurine. The leukemia was assessed as still in remission two months ago. PET/CT showed massive peritoneal fluid accumulation and a high uptake area in the diffused peritoneum (SUVmax 12.57), suggesting tumor invasion or microbial infections. However, broad-spectrum antibacterial therapies were ineffective. Metagenomic sequencing of plasma and peritoneal fluid showed no suggestion of the existence of a tumor but instead showed a high sequence number of DNA and RNA of the Microsporidia. His albendazole treatment failed and subsequent treatment with nitazoxanide successfully resolved the infection.</div> <div> </div> <div> <strong>Conclusion</strong>: This case shows that we should consider the possibility of atypical pathogen infection in patients with hematologic malignancy who repeatedly develop unexplained diarrhea with wasting. mNGS can help rule out malignant neoplasms and diagnose infections. Our results suggest that nitazoxanide effectively treats <em>E</em>. <em>bieneusi</em> microsporidia infections. </div>
A case report of an Eurasian Jay (Garrulus glandarius) attacking an incubating adult and depredating the eggs of the Japanese Tit (Parus minor)
<p>In May 2021, we opportunistically observed one Eurasian Jay (<em>Garrulus</em> <em>glandarius</em>) attacking an adult incubating Japanese Tit (<em>Parus</em> <em>minor</em>) and depredating nine tit eggs at a nestbox where a woodpecker had greatly enlarged the entrance. After the predation event, the Japanese tits abandoned the nest. We recommend that when using artificial nest boxes to protect hole-nesting birds, the appropriate entrance size should be proportional to the body size of the target species. This observation gives us a better understanding of the potential predators of secondary hole-nesting birds.</p>

The analysis of the gathered survey data provided strong evidence that the NRGF, including its process-guiding Web-platform (available at <a href="http://nanorigo.europeanprojects.net/platform/login">http://nanorigo.europeanprojects.net/platform/login</a>), represents a beneficial resource collection (data, tools, information) to support knowledge acquisition and enhancement of the understanding of a best practice approach of risk governance, going beyond conventional risk assessment and covering the whole life cycle of risk.</p> <p><strong>2_NanoRIGO_CS2 Setting up an In Vitro Aerosol Exposure Workplace</strong></p> <p>This case study aimed at testing whether the NanoRIGO risk governance framework (NRGF) is helpful for setting up a new workplace with potential hazards, obeying mandatory regulations and the principles of best practice. The test is valuated as successful if the facility setup and the definition of user practices was better, cheaper (<em>e.g.,</em> by faster and easier development) or safer than would have been possible without following the RGF guidance. For this case study PLUS applied as workplace a specific new facility, including the NAVETTA device for delivering airborne micro- and nanoparticles with high efficiency to lung cell cultures in an air-liquid interface (ALI) setup, using lateral flow and electrostatically enhanced deposition techniques.</p> <p><strong>3_NanoRIGO_CS3 Occupational Risk Management for Nanomaterials Production</strong></p> <p>The focus of the case study is to analyze, from the point of view of nanomaterial risk management, how useful is the NRGF framework developed, and its implementation in the NanoRIGO web platform. The scenarios considered have been defined according to key parameters such as: common types of processes in the handling of nanomaterials, most common types of nanomaterials and their characteristics, normal risk mitigation and containment conditions applied. In addition, risk communication tools have been tested.</p> <p><strong>4_NanoRIGO_CS4 Rubber Tyre Tear & Wear </strong></p> <p>This case study tested the usefulness and added value of the NRGF with the real-life use of tyres: the use of manufactured nanomaterials in the rubber tyre tread and the environmental release of tyre wear particles (TWP) from this tread during car driving. With a focus on the release and hazards of nano-TWP, a selected group of real-life stakeholders defined an approach for a risk governance process, and a group of scientific experts assessed the usefulness of the available tools and methods.</p> <p><strong>5_NanoRIGO_CS5 Carbon Nanofibers in Production, Use and End-of-life</strong></p> <p>This case study focused on the production, use and end of life of two Carbon Nanofibers (CNFs) produced by Grupo Antolin. These CNFs are the only known industrially produced carbon nanofilaments grown by chemical vapour deposition based on the floating catalyst technique, using a Ni compound as catalyst. They are used in different applications: automotive, sport equipment, aeronautics, etc. Due to their high aspect ratio, they are usually grouped with carbon nanotubes thus, raising some concerns about their safety at production, use and end of life. The main goal of the Grupo Antolin case study is to verify the RGF in a real industrial setting, following the main steps defined for the NRGF.</p> <p><strong>6_NanoRIGO_CS6 TiO2 Risk Classification and Data Readiness</strong></p> <p>This case study aimed to showcase the Classification, Labelling and Packaging (CLP) classification process of TiO<sub>2</sub> as a way to test the applicability and operability of the NRGF: (i) how the NRGF could support the CLP regulatory process in the future, (ii) whether any steps of the NRGF are missing or not conducted optimally, and (iii) how a council-like body could participate in operationalizing the NRGF. This issue has been examined by applying the Data Readiness Levels (DRLs; referred to as Knowledge, information, and data Readiness Levels [KaRLs] here) elaborated within the project to each step of the NRGF process.</p> <p><strong>7_NanoRIGO_CS7 Economic Benefits from the Insurance Industry Perspective</strong></p> <p>This case study aimed at the insurance industry as they are clearly an important stakeholder focusing on the economic perspectives, <em>i.e.</em> benefits vs. cost analysis. The “underwriting moment” has a quasi-regulatory function. It may also reside in with the “soft law” mechanisms suggested by Marchant (2014, <a href="https://doi.org/10.1080/13669877.2014.889200">https://doi.org/10.1080/13669877.2014.889200</a>). The NRGF allows better information flows, more effective risk communication (which has been largely absent) and crucially a better understanding of liability regimes on the part the nanotechnology sector. These are all important advances from the perspective of the insurance industry.</p>
MEDDOPROF Silver Standard: Predictions for Occupations, Working Statuses and Activities in Clinical Case Reports
<p><strong>MEDDOPROF Silver Standard</strong></p> <p>This repository contains the <strong>MEDDOPROF Silver Standard</strong>, a collection of clinical case reports of multiple specialties that have been automatically annotated with occupations, working statuses and activities using named entity recognition models trained on the MEDDOPROF corpus. MEDDOPROF and this Silver Standard have been created by the Barcelona Supercomputing Center's NLP for Biomedical Information Analysis.</p> <p><strong>About the MEDDOPROF Corpus</strong></p> <p><a href="https://temu.bsc.es/meddoprof/">MEDDOPROF</a> is a corpus and shared task focused on the detection and normalization of occupations and related information such as working statuses and non-paid activities in clinical texts in Spanish. All mentions in the corpus have been normalized to SNOMED CT, with occupations also being normalized to the European Skills, Competences, and Occupations classification ESCO. MEDDOPLACE was released as part of the IberLEF 2021 workshop, which in turn was held within the SEPLN 2021 conference.</p> <p><strong>About this Silver Standard</strong></p> <p>The Silver Standard includes 13,357 documents and a total of 5,568,844 tokens. The documents belong to over 10 different medical specialties, including occupational health, oncology, radiology, infectology, dermatology, psichiatry and more.</p> <p>The corpus includes three different labels: PROFESION (occupation), SITUACION-LABORAL (working status) and ACTIVIDAD (activity/hobby). An Approach for Traceability Recovery between Bug Reports and Test Cases
<p>Scripts and data sets used in research for <em>An Approach for Traceability Recovery between Bug Reports and Test Cases</em>.</p> <p><em><strong>(Context)</strong></em> Automatic traceability recovery between software artifacts may promote early detection of issues.<br> Information Retrieval (IR) techniques have been proposed for the task, but they differ considerably in terms of input parameters and results. It is difficult to assess results when those techniques are applied in isolation, usually in small or medium-sized software projects. Also, an overview would be more comprehensive if a Deep Learning (DL) based technique is applied, in comparison with traditional IR techniques.<br> <em><strong>(Objective)</strong></em> We propose an approach to recover traceability links between bug reports and test cases, which can be instantiated with a set of IR and DL techniques.<br> <em><strong>(Method)</strong></em> For applying and evaluating our solution, we used historical data from the Mozilla Firefox quality assurance (QA) team, on which we assessed the following IR techniques: LSI, LDA, and BM25. We also experimented with a DL architecture called Convolutional Neural Networks (CNNs) through the use of Word Embeddings.<br> <em><strong>(Results)</strong></em> In this context of traceability, we noticed poor performances from three out of the four studied techniques. Only the LSI technique was effective, even standing out over the state-of-the-art BM25 technique.<br> <em><strong>(Conclusions)</strong></em> The obtained results suggest that the semi-automatic application of the LSI technique -- with an appropriate combination of thresholds -- is feasible for real-world software projects.</p>

Transvenous extraction of double dual-coil implantable cardioverter-defibrillator leads – Case report
<p>Abstract: Implantable cardioverter-defibrillator (ICD) is the standard treatment for prevention of sudden cardiac death (SCD) in high-risk patients. Transvenous lead extraction (TLE) is an integral part of management of patients with ICD, since ICD leads are prone to damage in long-term follow-up. TLE is a procedure with high risk of complications. Major complications include haemothorax , thromboembolic events, vascular laceration, cardiac avulsion, and even death, with pericardial tamponade beeing the most common major complication. We report the clinical case of a 54-year-old male patient with a history of chronically implanted ICD who presented with isolated pocket erosion. Cardiac fluoroscopy revealed one dual-coil ICD lead connected to the pulse generator and another dual-coil ICD lead was noted to be fractured, with the proximal end lead remnant in the ICD pocket. Suspicion of pocket infection was raised, therefore the device and the leads extractions were necessary. Dual-coil implantable cardioverter-defibrillator leads are difficult to extract due to fibrotic scar tissue formed around the leads especially near the coils. Transvenous lead extraction technique was used to remove the dual-coil implantable cardioverter-defibrillator leads, without neither major nor minor complications. As ICD therapy was recommended for secondary prophylaxis, a new ICD was implanted on the contralateral side.</p>
