Factors influencing the sustainability of homestead vegetable production intervention in Rufiji, Tanzania: A cross-sectional mixed methods study

<p><strong>Background </strong></p> <p>There is growing evidence that home vegetable gardening interventions improve food security and nutrition outcomes at the family level. This study assessed factors influencing the sustainability of homestead vegetable production intervention, one year after the cessation of external support.</p> <p><strong>Methods</strong></p> <p>This was a cross-sectional study using both quantitative and qualitative data collection methods. A total of 247 randomly selected households that participated in the homestead vegetable intervention were interviewed using a structured questionnaire. The study held four focus group discussions with households that participated in the intervention, and four In-Depth interviews with two extension workers, one community health worker, and one agriculture district officer. Multiple logistic regression for quantitative data and thematic analysis for qualitative data was conducted.</p> <p><strong>Results</strong></p> <p>About 20.24% (50/247) of households sustained homestead vegetable production for one year after the intervention phased out. Lack of seeds (adjusted OR=1.26: CI=0.39-0.89) and either manure or fertilizers (adjusted OR=1.69: CI =1.08-2.63) were significant factors influencing the sustainability of homesteads vegetable production. In the Focus Group discussions (FGDs) and In-Depth Interview (IDIs), all participating women and extension workers reported high cost of water, destruction from free-grazing animals, agriculture pests and diseases, poor soil fertility, shortage of seeds, and lack of capital affected homestead vegetable production sustainability.</p> <p><strong>Conclusion</strong></p> <p>Existing individual, community, and system challenges influence the sustainability of external-funded agriculture and nutrition interventions. The study findings underscore the importance of community authorities, scientists, and policymakers in having a well-thought sustainability plan in all promising external-funded interventions.</p>

Satisfaction with Telemedicine for Cancer Pain Management: A Model of Care and Cross-Sectional Patient Satisfaction Study

<p>Background: Since cancer pain requires complex modalities of care, the proper strategy for addressing its telemedicine-based management should be better defined. This study aimed to trace a pathway for a progressive implementation of the telemedicine process for the treatment of pain in the setting of cancer patients. Methods: The features of the model were investigated to dissect the dropout from the telemedicine pathway. A cross-sectional patient satisfaction study was conducted. The degree of satisfaction was evaluated through a developed 22-item questionnaire (Likert scale 0&ndash;7). Results: A total of 375 video consultations for 164 patients (mean age 62.9 years) were performed through remote consultations for cancer pain management between March 2021 and February 2022. After the exclusion of 72 patients, 92 (56.1%) were included in the analysis. The dropout ratio was 8.7%. The number of visits and pharmacological therapies for neuropathic pain correlated with the risk for readmission (p&nbsp;&lt; 0.05). Overall, the satisfaction was very high (mean &gt; 5.5 for all items). Conclusion: Feedback from patients reflected high satisfaction rates with the care provided. A methodological approach based on the degree of satisfaction combined with the analysis of the pathways can help to implement the quality of a service provided through telemedicine. While not without limitations, our hybrid protocol can be useful for addressing cancer pain through a patient-centered approach.</p>

Determinants of QuantiFERON Plus-diagnosed tuberculosis infection in adult Ugandan TB contacts: A cross-sectional study

<p>Background</p> <p>The tuberculin skin test is commonly used to diagnose latent tuberculosis infection (LTBI) in resource-limited settings, but its specificity is limited by factors including cross-reactivity with BCG vaccine and environmental mycobacteria. Interferon-gamma release assays (IGRA) overcome this problem by detecting <em>M</em>. <em>tuberculosis</em> complex-specific responses, but studies to determine risk factors for IGRA-positivity in high TB burden settings are lacking.</p> <p>Methods</p> <p>We conducted a cross-sectional study to determine factors associated with a positive IGRA by employing the QuantiFERON-TB® Gold-plus (QFT Plus) assay in a cohort of asymptomatic adult TB contacts in Kampala, Uganda. Multivariate logistic regression analysis with forward stepwise logit function was employed to identify independent correlates of QFT Plus-positivity.</p> <p>Results</p> <p>Of the 202 participants enrolled, 129/202 (64%) were female, 173/202 (86%) had a BCG scar, and 67/202 (33%) were HIV-infected. Overall, 105/192 (54%, 95% CI 0.48–0.62) participants had a positive QFT Plus result. Increased risk of QFT-Plus positivity was independently associated with manual employment/unemployment vs. professional employment (adjusted odds ratio (aOR) 2.18, 95% CI 1.01–4.72), a family vs. non-family relation to the index patient (aOR 2.87, 95% CI 1.33–6.18), living in the same vs. a different house as the index (aOR 3.05, 95% CI 1.28–7.29), a higher body mass index (BMI) (aOR per additional kg/m2 1.09, 95% CI 1.00–1.18) and tobacco smoking vs. not (aOR 2.94, 95% CI 1.00–8.60). HIV infection was not associated with QFT-Plus positivity (aOR 0.91, 95% CI 0.42–1.96).</p> <p>Conclusion</p> <p>The prevalence of IGRA positivity in this study population was lower than previously estimated. Tobacco smoking and raised BMI are hitherto unappreciated determinants of LTBI risk in this setting.</p>

The illicit cigarette market in the DRC: datasets from a cross-sectional study of empty cigarette packs

<p>This study aimed to estimate the proportion of cigarettes consumed in the DRC that are illicit and the extent of cigarette tax evasion; and to identify the origins of and factors associated with illicit cigarettes.</p> <p>Stratified, multistage sampling was used to select 32 health areas across eight provinces in the DRC from which empty cigarette packs were collected. Each collected pack was examined and classified as licit if it complied, or illicit if it did not comply with the DRC&rsquo;s tax stamp or written health warning requirements; or the requirements to have a notice indicating the prohibition of sale by/to minors or information on tar and nicotine content. Cigarette retail price information was also collected for each brand represented within the collected empty packs. Cross-sectional and prospective data on Framingham risk score, allostatic load, and ankle brachial index among Puerto Rican adults from the Boston Puerto Rican Health Study

<p><strong>Background</strong><br> Puerto Ricans have higher odds of peripheral artery disease (PAD) compared with Mexican Americans. Limited studies have examined relationships between clinical risk assessment scores with PAD assessments.</p> <p><strong>Methods</strong><br> Using 2004-2015 data from the Boston Puerto Rican Health Study (BPRHS) (n = 370-583), cross-sectional, 5-y change and patterns of change in Framingham Risk Score (FRS) and allostatic load (AL) with ankle brachial index (ABI) at 5-y follow-up was assessed among Puerto Rican adults (45-75 y). Analysis were conducted in 2020. FRS and AL were calculated at baseline, 2-y and 5-y follow-up. Multivariable linear regression models examined cross-sectional and 5-y changes in FRS and AL with ABI at 5-y. Latent growth mixture modeling identified trajectories of FRS and AL over 5-y, and multivariable linear regression models were used to test associations between trajectory groups at 5-y.</p> <p><strong>Results</strong><br> Greater FRS at 5-y and increases in FRS from baseline were associated with lower ABI at 5-y (β = -0.149, p = 0.010; β = -0.171, p = 0.038, respectively). AL was not associated with ABI in cross-sectional or change analyses. Participants in low-ascending (vs. no change) FRS trajectory, and participants in moderate-ascending (vs. low-ascending) AL trajectory, had lower 5-y ABI (β = -0.025, p = 0.044; β = -0.016, p = 0.023, respectively).</p> <p><strong>Conclusions</strong><br> FRS was a better overall predictor of ABI, compared with AL. FRS may be a clinically feasible measure of PAD risk in Puerto Ricans, an understudied population. Additional research examining relationships between FRS and AL and development of PAD is warranted.</p>

Development and validation of the Health Belief Model questionnaire to promote smoking cessation for nasopharyngeal cancer prevention: a cross-sectional study

<p>Objective: Nasopharyngeal cancer (NPC) risk factors caused by lifestyle choices are substantial yet avoidable. Using the Health Belief Model as a conceptual framework, this study develops and validates a questionnaire to predict smokers' intentions to quit in Sarawak, Malaysia (HBM).</p> <p>Design: A cross sectional study.</p> <p>Setting: Urban and suburban areas in Sarawak, Malaysia.</p> <p>Participants: Following a thorough literature study, the preliminary items for the instrument were created. Prior to beginning the content validity by a panel of 10 experts, the instrument was translated into the Malay language utilising the forward-backwards approach. 10 smokers carried out face validity in both quantitative and qualitative ways. Exploratory factor analysis (EFA) and confirmatory factor analysis (CFA) were used to assess the instrument's concept validity. Phase 1 of the EFA involved 100 smokers, whereas phase 2 of the CFA involved 171 smokers. Cronbach's alpha coefficients were used to measure internal consistency and assess reliability.</p> <p>Results: The factor loading of each item remained within the acceptable threshold in the exploratory stage. The final revised CFA yielded the following model fit indices that suitably fitted the seven-factor model: Chi Square: 641.705; df= 500; P&lt; 0.001; CFI = 0.953; TLI: 0.948; RMSEA= 0.041. With the exception of one pairwise construct, satisfactory convergent validity and divergent validity were demonstrated. Phases 1 and 2 both have Cronbach's alpha values over 0.7, suggesting acceptable internal reliability.</p> <p>Conclusions: The study confirmed the validity and reliability of the instrument in predicting smokers' tendency to adopt healthy behaviour of smoking cessation to lower risk of developing cancer. The instrument consists of 34 items, dividing into two sections: 6 HBM components and health behavioural intention. The instrument is regarded as innovation and can be applicable in other smoking-related malignancies in different susceptible populations and geographical locations.</p>

Global-scale modeling of early factors and country-specific trajectories of COVID-19 incidence: a cross-sectional study of the first 6 months of the pandemic

<p><span>Studies examining factors responsible for COVID-19 incidence </span><span>are</span><span> mostly focused at the national or sub-national level. A global-level characterization of contributing factors and temporal trajectories of disease incidence is lacking. Here we conducted a global-scale analysis of COVID-19 infections to identify key factors associated with early disease incidence. Additionally, we compared longitudinal trends of COVID-19 incidence at a per-country level and classified countries based on COVID-19 incidence trajectories and effects of lockdown responses. </span><span>Univariate analysis identified</span> <span>eleven variables as independently associated with COVID-19 infections at a global level (p&lt;1e-05). Multivariable analysis identified a 4-variable model as optimal for explaining global variations in COVID-19 (p&lt;0.01). COVID-19 case trajectories for most countries were best captured by a log-logistic model, as determined by AIC estimates. Six predominant country clusters were identified when characterizing the effects of lockdown intervals on variations in COVID-19 new cases per country.</span> <span>Globally, economic and meteorological factors are important determinants of early COVID-19 incidence. Analysis of longitudinal trends and lockdown effects on COVID-19 highlights important nuances in country-specific responses to infections. These results </span><span>provide valuable insights into disease incidence at a per-country level, possibly allowing for more informed decision making by individual governments in future disease outbreaks.</span></p>

Transition from transperitoneal to retroperitoneal approach in laparoscopic living donor nephrectomy: Team-based and individual learning curve: A cross-sectional study

<p><strong><span>Background</span></strong><span>: </span><span>Laparoscopic living donor nephrectomy (LLDN) is a standard practice for kidney donor transplantation due to fewer complications associated with the treatment. Retroperitoneal approach to LLDN is thought to be more advantageous in preventing injuries to various abdominal organs. However, the transition process from transperitoneal to retroperitoneal LLDN is important to ensure the best outcome for the patients</span><span>. The aim of this study was to investigate the learning curve for the retroperitoneal approach in LLDN among urologists in an individual and team-based approach</span></p> <p><span><strong>Methods</strong>: </span><span>A retrospective analysis</span> <span>of retroperitoneal LLDN was performed on procedures performed by a kidney donor team consisting of four (4) urologists from January 2019 to January 2022 at Cipto Mangunkusumo National General Hospital, Indonesia. The data were taken from pre-existing medical records. The learning curve for the operation time, warm ischemic time, and estimated blood loss were analyzed using cumulative sum (CUSUM) analysis. Phase 1 represents the initial learning curve, the phase 2 plateau represents the period of proficiency, and phase 3 represents the mastery period.</span></p> <p><span><strong>Results</strong>: </span><span>A total of 127 retroperitoneal LLDN procedures were done during the study period by four operators with various experience. The average procedure needed to achieve proficiency was 16.5 procedures. Meanwhile, the average procedure needed to achieve mastery was 28 procedures. A shorter learning curve was achieved by operators with more experience in other laparoscopic procedures.</span></p> <p><strong><span>Conclusions</span></strong><span>: </span><span>The learning curve for transition from transperitoneal to retroperitoneal approach to LLDN is relatively short and feasible for both individuals and teams of urologists. Therefore, the transition to a retroperitoneal approach is a feasible option to reduce the complication rate of LLDN.</span></p>

Effects of COVID-19 in Endocrine Patients: A Cross-Sectional Study.

<p>Abstract: Introduction: Home confinement due to COVID-19 lockdown led to changes in daily<br> routines, including social interactions, as well as restrictions on the possibility of playing sports<br> and eating habits. These changes could have a greater impact on patients suffering from chronic<br> diseases, such as endocrine patients, especially in emotional and behavioral dimensions. Materials<br> and Methods: This study aimed to assess the effects of COVID-19-induced quarantine on daily habits<br> in a group of patients with endocrine disorders, focusing on food consumption, eating habits and<br> sleep during the confinement. Eighty-five endocrine patients were enrolled. A structured interview<br> was administered to investigate socio-demographic information, general medical conditions, and<br> habits adopted during quarantine. All patients underwent the Spielberger State Anxiety Inventory<br> (STAI-Y1) to assess state anxiety. Result: Results showed that subjects mainly had a sedentary lifestyle.<br> We found a significant increase in the number of cigarettes in smokers and in meals consumed during<br> confinement, as well as a high rate of sleep disturbance, especially insomnia. Notably, physical<br> well-being resulted to be a predictive factor (OR = 0.38; 95%CI = [0.95,0.66]), whereas anxiety was a<br> risk factor for sleep disorder (OR = 1.22; 95%CI = [1.10,1.40]), as was working in public and private<br> offices and being a student. Conclusions: Changes in daily habits were likely due to the alterations<br> in routine, resulting in greater boredom and inactivity during the day. In addition, future research<br> should focus on the importance of patient adherence to therapy.</p>

Public health emergency operations centres in Africa: A cross-sectional study assessing the implementation status of core components and areas for improvement, December 2021

<div> <p><span><strong>Objective:</strong> </span><span>To assess implementation status of Public Health Emergency Operations Centres (PHEOCs) in all countries in Africa.</span></p> <p><span><strong>Design:</strong> </span><span>Cross-sectional.</span></p> <p><span><strong>Setting:</strong> </span><span>Fifty-four national PHEOC focal points in Africa responded to an online survey between May and November 2021. Included variables aimed to assess capacities for each of the four PHEOC core components. To assess the PHEOCs' functionality, criteria were defined from among the collected variables by expert consensus based on PHEOC operations' prioritisation. We report results of the descriptive analysis, including frequencies of proportions.</span></p> <p><span><strong>Results:</strong> </span><span>A total of 51 (93%) African countries responded to the survey. Among these, 41 (80%) have established a PHEOC. Twelve (29%) of these met 80% or more of the minimum requirements and were classified as fully functional. Twelve (29%) and 17 (41%) PHEOCs that met 60%–79% and below 60% of the minimum requirements were classified as functional and partially functional, respectively. </span></p> <p><span><strong>Conclusions:</strong> </span><span>Countries in Africa made considerable progress in setting up and improving functioning of PHEOCs. One-third of the responding countries with a PHEOC have one fulfilling at least 80% of the minimum requirements to operate the critical emergency functions. There are still several African countries that either do not have a PHEOC or whose PHEOCs only partially meet these minimal requirements. This calls for significant collaboration across all stakeholders to establish functional PHEOCs in Africa.</span></p> </div>

Data for: Evaluation of antibody kinetics and durability in health individuals vaccinated with inactivated COVID-19 vaccine (CoronaVac): a cross-sectional and cohort study in Zhejiang, China

<p><strong>Background</strong>: Although inactivated COVID-19 vaccines are proven to be safe and effective in the general population, the dynamic response and duration of antibodies after vaccination in the real world should be further assessed.</p> <p><strong>Methods</strong>: We enrolled 1067 volunteers who had been vaccinated with one or two doses of CoronaVac in Zhejiang Province, China. Another 90 healthy adults without previous vaccinations were recruited and vaccinated with three doses of CoronaVac, 28 days and 6 months apart. Serum samples were collected from multiple timepoints and analyzed for specific IgM/IgG and neutralizing antibodies (NAbs) for immunogenicity evaluation. Antibody responses to the Delta and Omicron variants were measured by pseudovirus-based neutralization tests.</p> <p><strong>Results</strong>: Our results revealed that binding antibody IgM peaked 14–28 days after one dose of CoronaVac, while IgG and NAbs peaked approximately 1 month after the second dose and then declined slightly over time. Antibody responses had waned by month 6 after vaccination and became undetectable in the majority of individuals at 12 months. Levels of NAbs to live SARS-CoV-2 were correlated with anti-SARS-CoV-2 IgG and NAbs to pseudovirus, but not IgM. Homologous booster around 6 months after primary vaccination activated anamnestic immunity and raised NAbs 25.5-fold. The neutralized fraction subsequently rose to 36.0% for Delta (p=0.03) and 4.3% for Omicron (p=0.004), and the response rate for Omicron rose from 7.9% (7/89) to 17.8% (16/90).</p> <p><strong>Conclusions</strong>: Two doses of CoronaVac vaccine resulted in limited protection over a short duration. The inactivated vaccine booster can reverse the decrease of antibody levels to prime strain, but it does not elicit potent neutralization against Omicron; therefore, the optimization of booster procedures is vital.</p>
