Data from: Empirical targets for acute hemodynamic management of individuals with spinal cord injury

Objective: To determine the hemodynamic conditions associated with optimal neurological improvement in acute traumatic spinal cord injury (SCI) individuals who had lumbar intrathecal catheters placed to measure cerebrospinal fluid pressure (CSFP). Methods: Ninety-two acute SCI individuals were enrolled in this multi-center prospective observational clinical trial. We monitored mean arterial pressure (MAP) and CSFP during the first week post-injury, and assessed neurologic function at baseline and six-months post-injury. We used relative risk iterations to determine transition points at which the likelihood of either improving neurologically or remaining unchanged neurologically was equivalent. These transition points guided our analyses where we examined the linear relationships between time spent within target hemodynamic ranges (i.e., clinical adherence) and neurological recovery. Results: Relative risk transition points for CSFP, MAP, and spinal cord perfusion pressure (SCPP) were linearly associated with neurological improvement and directed the identification of key hemodynamic target ranges. Clinical adherence to the target ranges was positively and linearly related to improved neurological outcomes. Adherence to SCPP targets, not MAP targets, was the best indicator of improved neurological recovery, which occurred with SCPP targets of 60-65 mmHg. Failing to maintain the SCPP within the target ranges was an important detrimental factor in neurologic recovery, particularly if the target range is set lower. Conclusion: We provide an empirical, data-driven approach to aid institutions in setting hemodynamic management targets that accept the real-life challenges of adherence to specific targets. Our results provide a framework to guide the development of widespread institutional management guidelines for acute traumatic SCI.

Data from: Sex-dependent differences in central artery hemodynamics in normal and fibulin-5 deficient mice: implications for ageing

Mouse models provide unique opportunities to study vascular disease, but they demand increased experimental and computational resolution. We describe a workflow for combining in vivo and in vitro biomechanical data to build mouse-specific computational models of the central vasculature including regional variations in biaxial wall stiffness, thickness and perivascular support. These fluid–solid interaction models are informed by micro-computed tomography imaging and in vivo ultrasound and pressure measurements, and include mouse-specific inflow and outflow boundary conditions. Hence, the model can capture three-dimensional unsteady flows and pulse wave characteristics. The utility of this experimental–computational approach is illustrated by comparing central artery biomechanics in adult wild-type and fibulin-5 deficient mice, a model of early vascular ageing. Findings are also examined as a function of sex. Computational results compare well with measurements and data available in the literature and suggest that pulse wave velocity, a spatially integrated measure of arterial stiffness, does not reflect well the presence of regional differences in stiffening, particularly those manifested in male versus female mice. Modelling results are also useful for comparing quantities that are difficult to measure or infer experimentally, including local pulse pressures at the renal arteries and characteristics of the peripheral vascular bed that may differ with disease.

A Dataset of Reconstructed Carotid Bifurcation Lumen and Plaque Models with Centerline Tree and Simulated Hemodynamics

<p><code>carotid_bifurcation_database.zip</code> contains 79 cases of left and right-side carotid bifurcations (152 inner wall models). For each case, inner wall (lumen) and plaque models were extracted from computed tomography angiography (CTA) scans. The models were segmented, reconstructed, and a centerline tree was created for each geometry using the&nbsp;<a href="https://github.com/PepeEulzer/CarotidAnalyzer">CarotidAnalyzer</a> pipeline. The geometries include varying degrees of internal carotid stenosis. Bifurcations with 100% stenosis were omitted, as the vessel is not discernible in the scan.</p> <p><code>carotid_flow_database.zip</code> contains hemodynamic flow simulations of the above models. Fluid data (velocity, pressure) and surface data (wall shear stress) are given in seperate files for each case. Hemodynamic data from the Inspired Therapeutics NeoMate Mechanical Circulatory Support System for neonates and infants as tested in static mock circulatory loops, dynamic mock circulatory loops, and acute animal studies

<p>Inspired Therapeutics (Merritt Island, FL) is developing a mechanical circulatory support (MCS) system designed as a single driver with interchangeable, extracorporeal, magnetically levitated pumps. The NeoMate system design features an integrated centrifugal rotary pump, motor, and controller that will be housed in a single compact unit.  Conceptually, the primary innovation of this technology will be the combination of disposable, low-cost pumps for use with a single, multi-functional, universal controller to support multiple pediatric cardiopulmonary indications.  In response to the paucity of clinically available pediatric devices, Inspired Therapeutics is specifically targeting the underserved neonate and infant heart failure (HF) patient population first.  In this article, we present the development of the prototype Inspired Therapeutics NeoMate System for pediatric left ventricular assist device (LVAD) support, and feasibility testing in static mock flow loops (H-Q curves), dynamic mock flow loops (hemodynamics), and in an acute healthy ovine model (hemodynamics and clinical applicability).  The resultant hydrodynamic and hemodynamic data demonstrated the ability of this prototype pediatric LVAD and universal controller to function over a range of rotary pump speeds (500-6000 RPM), to provide pump flow rates of up to 2.6 L/min, and to volume unload the left ventricle in acute animals.  Key engineering challenges observed and proposed solutions for the next design iteration are also presented.</p>
