Objective: Studies on primary CNS lymphoma (PCNSL) patients in the "real life" are scarce. Our objective was to analyze, in a nationwide population-based study, the current medical practice in the management of PCNSL. Methods: The LOC database prospectively records all newly diagnosed PCNSL from 32 French centers. Data of patients diagnosed between 2011-2016 were retrospectively analyzed. Results: We identified 1002 immunocompetent patients (43% aged>70 years, median KPS 60). First-line treatment was high-dose methotrexate-based chemotherapy in 92% of cases, with an increasing use of rituximab over time (66%). Patients <60 years old received consolidation treatment in 77% of cases, consisting of whole-brain radiotherapy (WBRT) (54%) or high-dose chemotherapy with autologous stem cell transplantation (HCT-ASCT) (23%). Among the patients > 60 years old, WBRT and HCT-ASCT consolidation were administered in only 9% and 2%, respectively. The complete response rate to initial chemotherapy was 50%. Median progression-free survival was 10.5 months. For relapse, second-line chemotherapy, HCT-ASCT, WBRT and palliative care were offered to 55%, 17%, 10%, and 18% of patients, respectively. The median, 2-year and 5-year overall survival was 25.3 months, 51% and 38%, respectively (< 60 years: not reached (NR), 70% and 61%; > 60 years: 15.4 months, 44% and 28%). Age, KPS, sex and response to induction CT were independent prognostic factors in multivariate analysis. Conclusions: Our study confirms the increasing proportion of elderly within PCNSL and shows comparable outcome in this population-based study with those reported by clinical trials, reflecting a notable application of recent PCNSL advances in the real life.
Data from: Outcomes in patients with infections and augmented renal clearance: a multicenter retrospective study
Recently, augmented renal clearance (ARC), which accelerates glomerular filtration of renally eliminated drugs thereby reducing the systemic exposure to these drugs, has started to receive attention. However, the clinical features associated with ARC are still not well understood, especially in the Japanese population. This study aimed to evaluate the clinical characteristics and outcomes of ARC patients with infections in Japanese intensive care unit (ICU) settings. We conducted a retrospective observational study from April 2013 to May 2017 at two tertiary level ICUs in Japan, which included 280 patients with infections (median age 74 years; interquartile range, 64–83 years). We evaluated the estimated glomerular filtration rate (eGFR) at ICU admission using the Japanese equation, and ARC was defined as eGFR >130 mL/min/1.73 m2. Multivariable logistic regression analysis was performed to identify the independent risk factors for ARC and to determine if it was a predictor of ICU mortality. In addition, a receiver operating curve (ROC) analysis was performed, and the area under the ROC (AUROC) was determined to examine the significant variables that predict ARC. In total, 19 patients (6.8%) manifested ARC. Multivariable logistic regression analysis identified younger age as an independent risk factor for ARC (odds ratio [OR], 0.94; 95% confidence interval [CI], 0.91–0.96). However, ARC was not found to be a predictor of ICU mortality (OR, 0.57; 95% CI, 0.11–2.92). In addition, the AUROC of age was 0.79 (95% CI, 0.68–0.91), and the optimal cut off age for ARC was ≤63 years (sensitivity, 68.4%; specificity, 78.9%). The incidence of ARC was, therefore, low among patients with infections in the Japanese ICUs. Although younger age was associated with the incidence of ARC, it was not an independent predictor of ICU mortality.

Clinical outcomes in multifocal motor neuropathy: a combined cross-sectional and follow-up study
Objective <p>To assess the clinical course of multifocal motor neuropathy (MMN) in a large cohort of patients and to identify predictive factors of a progressive disease course.</p> Methods <p>Between May 2015 and February 2016, we collected clinical data from 100 patients with MMN of whom 60 had participated in a nationwide cross-sectional cohort study in 2007. We documented clinical characteristics using standardized questionnaires and performed a standardized neurological examination. We used multiple linear regression analysis to identify factors that correlated with worse outcome.</p> Results <p>We found that age of diagnosis (45.2 <i>vs.</i> 48.6 years, <i>p</i><0.02) significantly increased between 2007 and 2015-2016, whereas diagnostic delay decreased with 15 months. Seven out of ten outcome measures deteriorated over time (all <i>p</i><0.01). Patients who had a lower MRC sum score and absence of one or more reflexes at the baseline visit showed a greater functional loss at follow up (<i>p</i>=0.007 and <i>p</i>=0.016).</p> Conclusions <p>Our study shows that MMN is a progressive disease. Although 87% of patients received maintenance treatment, muscle strength, reflexes, vibration sense, and the Self-Evaluation Scale significantly deteriorated over time. Lower MRC sum score and absence of reflexes predicted a more progressive disease course.</p> Classification of evidence <p>This study provides Class II evidence that lower MRC sum score and the absence of reflexes predict a more progressive disease course in patients with MMN.</p>
