Gabriella Miller Kids First Pediatric Research Program: An Integrated Clinical and Genomic Analysis of Treatment Failure in Pediatric Osteosarcoma

Although the survival of children with relapsed osteosarcoma is very poor, little is known about the etiology of treatment failure in this disease. The purpose of this project is to perform whole genome sequencing on serial samples from patients with osteosarcoma obtained before treatment, after treatment, and at relapse/metastasis in order to identify the mutations and pathways that are drivers of drug resistance. If successful, our results may help identify patients at high risk for treatment failure and may yield new treatments for children who cannot currently be cured.

PMID: 33781593.</p> <p>Abstract&nbsp;</p> <p><strong>Objective:&nbsp;</strong>The study objective was to elucidate the mechanisms of left ventricle functional recovery in terms of endocardial contractility and synchronicity after surgical ventricular reconstruction.</p> <p><strong>Methods:&nbsp;</strong>Real-time 3-dimensional transthoracic echocardiography was performed on 20 patients with anterior left ventricle remodeling and ischemic heart failure before surgical ventricular reconstruction and at 6-month follow-up, and on 15 healthy controls matched by age and body surface area. Real-time 3-dimensional transthoracic echocardiography datasets were analyzed through TomTec software (4D LV-Analysis; TomTec Imaging Systems GmbH, Unterschleissheim, Germany): Left ventricle volumes, ejection fraction, and global longitudinal strain were computed; the time-dependent endocardial surface yielded by 3-dimensional speckle-tracking echocardiography was postprocessed through in-house software to quantify local systolic minimum principal strain as a measure of fiber shortening and mechanical dispersion as a measure of fiber synchronicity.</p> <p><strong>Results:&nbsp;</strong>Compared with controls, patients with heart failure before surgical ventricular reconstruction showed lower ejection fraction (P &lt; .0001) and significantly impaired mechanical dispersion (P &lt; .0001) and minimum principal strain (P &lt; .0001); the latter worsened progressively from left ventricle base to apex. After surgical ventricular reconstruction, global longitudinal strain improved from -6.7% to -11.3% (P &lt; .0001); mechanical dispersion decreased in every left ventricle region (P &le; .017) and mostly in the basal region, where computed mechanical dispersion values were comparable to physiologic values (P &ge; .046); minimum principal strain improved mostly in the basal region, changing from -16.6% to -22.3% (P = .0027).</p> <p><strong>Conclusions:&nbsp;</strong>At 6-month follow-up, surgical ventricular reconstruction was associated with significant recovery in global left ventricle function, improved mechanical dispersion indicating a more synchronous left ventricle contraction, and improved left ventricle fiber shortening mostly in the basal region, suggesting the major role of the remote myocardium in enhancing left ventricle functional recovery.</p>

PMID: 32104899.</p> </li> </ul> <p>&nbsp;</p> <p><strong>Abstract</strong></p> <p><strong>Objective: </strong> To assess the prevalence of diffuse idiopathic skeletal hyperostosis and its relationship with vascular risk factors among patients with congestive heart failure.</p> <p><strong>Design: </strong> Population-based cross-sectional study.</p> <p><strong>Participants: </strong> A total of 584 consecutive patients admitted to a Rehabilitative Cardiology Unit.</p> <p><strong>Methods: </strong> Chi-square Automatic Interaction Detector (CHAID) decision tree analysis was used to build a predictive model.</p> <p><strong>Results: </strong> The mean age (standard deviation) of the study population was 68.1 years (standard deviation 12.3), and 77.7% of the subjects were men. The overall prevalence of diffuse idiopathic skeletal hyperostosis in the cohort was 49.8%. Logistic regression analysis showed that age was a predictor of diffuse idiopathic skeletal hyperostosis (odds ratio: 1.034; 95% confidence interval 1.021-1.047, p &lt; 0.001), with increasing odds ratios for increasing age tertiles. The CHAID prediction model identified 2 age &quot;buckets&quot;: &lt; 69 and &ge; 69 years. Patients &ge; 69 years had a diffuse idiopathic skeletal hyperostosis prevalence of 60.1%, compared with 39.2% among those &lt; 69 years. Notably, body mass index was a predictor of diffuse idiopathic skeletal hyperostosis in this younger subset of patients (p = 0.028), with 2 body mass index &quot;buckets&quot;, &le; 23.3 and &gt; 23.3 kg/m2, the latter showing more than twice the prevalence of diffuse idiopathic skeletal hyperostosis (43.2% vs 20%).</p> <p><strong>Conclusion: </strong> Diffuse idiopathic skeletal hyperostosis is extremely frequent among patients with congestive heart failure, with age and body mass index being the strongest predictors.</p>

<p>Abstract</p> <p><strong>Background:&nbsp;</strong>The role of risk scores in heart failure (HF) management has been highlighted by international guidelines. In contrast with HF, which is intrinsically a dynamic and unstable syndrome, all its prognostic studies have been based on a single evaluation. We investigated whether time-related changes of a well-recognized risk score, the MECKI score, added prognostic value. MECKI score is based on peak VO<sub>2</sub>, VE/VCO<sub>2</sub>&nbsp;slope, Na<sup>+</sup>, LVEF, MDRD and Hb.</p> <p><strong>Methods:&nbsp;</strong>A multi-centre retrospective study was conducted involving 660 patients who performed MECKI re-evaluation at least 6 months apart. Based on the difference between II and I evaluation of MECKI values (MECKI II - MECKI I = ∆ MECKI) the study population was divided in 2 groups: those presenting a score reduction (∆ MECKI &lt;0, i.e. clinical improvement), vs. patients presenting an increase (∆ MECKI &gt;0, clinical deterioration).</p> <p><strong>Results:&nbsp;</strong>The prognostic value of MECKI score is confirmed also when re-assessed during follow-up. The group with improved MECKI (366 patients) showed a better prognosis compared to patients with worsened MECKI (294 patients) (p &lt; 0.0001). At 1st evaluation, the two groups differentiated by LVEF, V<sub>E</sub>/VCO<sub>2</sub>&nbsp;slope and blood Na<sup>+</sup>&nbsp;concentration, while at 2nd evaluation they differentiated in all 6 parameters considered in the score. The patients who improved MECKI score, improved in all components of the score but hemoglobin, while patients who worsened the score, worsened all parameters.</p> <p><strong>Conclusions:&nbsp;</strong>This study shows that re-assessment of MECKI score identifies HF subjects at higher risk and that score improvement or deterioration regards several MECKI score generating parameters confirming the holistic background of HF.</p>
