Data from: Joint effect of education and main lifetime occupation on late life health: a cross-sectional study of older adults in Xiamen, China
Background: The effects of education and occupation on health have been well documented individually, but little is known about their joint effect, especially their cumulative joint effect on late life health. Methods: We enrolled 14,292 participants aged 60+ years by multistage sampling across 173 communities in Xiamen, China, in 2013. Heath status was assessed by the ability to perform six basic activities of daily life. Education was classified in four categories: 'Illiterate', 'Primary', 'Junior high school' and 'Senior high school and beyond'. Main lifetime occupation was also four categorized: 'Employed', 'Farmer', 'Jobless' and 'Others'. Odds ratios (ORs) were estimated by random-intercept multilevel models regressing health status on education and main lifetime occupation with or without their interactions, adjusting by some covariates. Results: Totally, 13,880 participants had complete data, of whom 12.5% suffered from disability, and 'Illiterate' and 'Farmer' took up the greatest proportion (33.01% and 42.72%, respectively). Participants who were higher educated had better health status (ORs = 0.62, 0.46, and 0.44 for the 'Primary', 'Junior high school', and 'Senior high school and beyond', respectively, in comparison with 'Illiterate'). Those who were long term jobless in early life had poorest heath (ORs = 1.88, 95% CI 1.47 to 2.40). Unexpectedly, for the farmers, the risk of poor health gradually increased in relation to higher education level (ORs = 1.26, 1.28, 1.40 and 2.24, respectively). For the 'Employed', similar ORs were obtained for the 'Junior high school' and 'Senior high school and beyond' educated (both ORs = 1.01). For the 'Farmer' and 'Jobless', participants who were 'Illiterate' and 'Primary' educated also showed similar ORs. Conclusions: Both education and main lifetime occupation were associated with late life health. Higher education was observed to be associated with better health, but such educational advantage was mediated by main lifetime occupation.

<p><b>Background and Objective:</b> Fish intake may prevent cerebrovascular disease (CVD), yet the mechanisms are unclear, especially regarding its impact on subclinical damage. Assuming that fish may have pleiotropic effect on cerebrovascular health, we investigated the association of fish intake with global CVD burden based on brain MRI markers.</p> <p><b>Methods:</b> This cross-sectional analysis included participants from the Three-City Dijon population-based cohort study (aged ≥65 years) without dementia, stroke, or history of hospitalized cardiovascular disease, who underwent brain MRI with an automated assessment of white matter hyperintensities, visual detection of covert infarcts, and grading of dilated perivascular spaces. Fish intake was assessed through a frequency questionnaire and the primary outcome measure of CVD burden was defined as the first component of a factorial analysis of mixed data applied to MRI markers. The association of fish intake with the CVD burden indicator was studied using linear regressions.</p> <p><b>Results:</b> In total, 1,623 participants (mean age, 72.3 years; 63% women) were included. The first component of factorial analysis (32.4% of explained variance) was associated with higher levels of all three MRI markers. Higher fish intake was associated with lower CVD burden. In a model adjusted for total intracranial volume, compared to participants consuming fish less than once a week, those consuming fish 2 to 3 times per week and ≥4 times per week had a β = -0.19 units (95% CI, -0.37; -0.01) and β = -0.30 (-0.57; -0.03) lower indicator of global CVD burden, respectively (<i>P</i> for trend <0.001). We found evidence of effect modification by age, so that the association of fish to CVD was stronger in younger participants (65-69 years) and not significant in the older age group (≥75 years). For comparison, in the younger age group, consuming fish 2-3 times a week was roughly equivalent (in the opposite direction) to the effect of hypertension, and consuming fish ≥4 times had double that effect.</p> <p><b>Discussion:</b> In this large population-based study, higher frequency of fish intake was associated with lower CVD burden, especially among participants younger than 75 years, suggesting a beneficial effect on brain vascular health before manifestation of overt brain disease.</p>
Gastric emergencies in older adults: not always the same old story! A tertiary care emergency department observational cohort study
<p><b>Objective</b>:<b> </b>To analyze and compare the clinical outcomes between patients ≥80 years and patients 65-79 years, presented to our Emergency Department (ED) with the two most common Gastric Emergencies (GE) clinical presentation, i.e. gastric bleeding and gastric perforation.</p> <p><b>Design</b>: Single-center retrospective cohort study.</p> <p><b>Participants:</b> A total of 1011 patients were enrolled: 421 patients aged ≥80 years were compared to a group of 590 patients aged 65-79 years.</p> <p><b>Primary and secondary outcome measures</b>:<b> </b>The primary outcome was to compare the overall mortality. Secondary outcomes included major complications, in-hospital length of stay (LOS), and need for surgical procedures.</p> <p><b>Results</b>: Patients ≥80 years with GE had different presentation at Emergency Department compared to younger patients: they had higher gastrointestinal bleeding (24.9% vs. 16.3%; <i>p </i>= 0.001), anemization (14.5% vs. 8.8%; <i>p= </i>0.005), and shock (10.9% vs. 5.1%; <i>p</i> = 0.001) rates. Median LOS, cumulative major complications and mortality rates were similar among the two groups.</p> <p>Multivariate analysis identified shock, gastric malignancy, and gastric fistula as independent predictors for survival.</p> <p><b>Conclusions</b>: Patients ≥80 years with GE did not have a higher mortality rate and cumulative major complications as compared to younger elderly patients. Invasive treatments were not associated to a different prognosis in this age group.</p>
A multidisciplinary Telehealth approach for community dwelling older adults
<p>Telemedicine may be used for the continuity of care in several chronic conditions. Sixty frail old people were enrolled along with their primary caregivers, and randomly divided into two groups: 30 of them received a multi-specialist telemedicine care, whilst the other 30 were treated in their usual territory care. All of the patients were evaluated through a clinical and psychometric battery at baseline (T0), after 6 months (T1), and at the end of the study (T2). It was found that telemedicine was more effective than the traditional approach in mood improvement (p < 0.001), behaviour (p < 0.01) and ADL/IADL (p < 0.01/0.04), as well as nutritional status. These changes increased over time (from T0 to T1), the caregivers' burden decreased, and system usability was rated as good. Telemedicine could be considered an important tool to improve the psychological health and quality of the life of older frail patients living at home.</p>
