How the first years of motherhood impact the cardiac autonomic profile of female healthcare professionals: a study by heart rate variability analysis.

PMID: 33854133; PMCID: PMC8047021.</p> <p>Abstract</p> <p>The conciliation between career and family is a relevant issue for working women, in particular during the first years of motherhood. Data about the state of the cardiac autonomic regulation in working women with preschoolers are lacking. Aim of this study was to compare the cardiac autonomic profile of female healthcare professionals with and without preschoolers via the analysis of the variability of the time distance between two consecutive R-wave peaks (RR) from standard 24-h Holter electrocardiogram (ECG). Fifty healthy active female healthcare professionals were enrolled: 25 with at least one preschooler (W_KID) and 25 without (W_NOKID). A standard Holter ECG was obtained during a regular working day. Segments of 5000 consecutive RRs were selected during daytime (DAY) and nighttime (NIGHT). Heart rate variability analysis was performed and the following parameters were considered for comparison between the two groups: mean (μ<sub>RR</sub>), variance (σ<sup>2</sup><sub>RR</sub>), and the absolute power in high frequency component (HF) of RR (HF<sub>RR</sub>) series. HF<sub>RR</sub> was considered as a marker of vagal cardiac modulation. Only µ<sub>RR</sub> significantly increased from DAY to NIGHT in both groups (699 ± 88 vs 887 ± 140 ms in W_KID and 728 ± 90 vs 942 ± 166 ms in W_NOKID). Instead, σ<sup>2</sup><sub>RR</sub> and HF<sub>RR</sub> increased from DAY to NIGHT only in W_NOKID (from 3334 ± 2153 to 4816 ± 4063 ms<sup>2</sup> and from 356 ± 334 to 1397 ± 1629 ms<sup>2</sup>, respectively). W_KID showed lower σ<sup>2</sup><sub>RR</sub> and HF<sub>RR</sub> during NIGHT, compared to W_NOKID (2336 ± 3170 vs 4816 ± 4063 ms<sup>2</sup> and 556 ± 950 vs 1397 ± 1629 ms<sup>2</sup>, respectively). The perceived stress according to the visual analogue scale was similar in the two groups (4.7 ± 2.1 in W_KID, 5.7 ± 2.1 in W_NOKID). The presence of preschoolers lowered nocturnal cardiac vagal modulation in female healthcare professionals. This might represent an adaptation with a finalistic purpose, scilicet the facilitation of a prompt reaction in case of a child's need.</p>

PMID: 33660030; PMCID: PMC7928172.</p> <p>Abstract</p> <p><strong>Purpose: </strong>The purpose of the present cross-sectional study is to investigate the role of perceived COVID-19-related organizational demands and threats in predicting emotional exhaustion, and the role of organizational support in reducing the negative influence of perceived COVID-19 work-related stressors on burnout. Moreover, the present study aims to add to the understanding of the role of personal resources in the Job Demands-Resources model (JD-R) by examining whether personal resources-such as the professionals' orientation towards patient engagement-may also strengthen the impact of job resources and mitigate the impact of job demands.</p> <p><strong>Methods: </strong>This cross-sectional study involved 532 healthcare professionals working during the COVID-19 pandemic in Italy. It adopted the Job-Demands-Resource Model to study the determinants of professional's burnout. An integrative model describing how increasing job demands experienced by this specific population are related to burnout and in particular to emotional exhaustion symptoms was developed.</p> <p><strong>Results: </strong>The results of the logistic regression models provided strong support for the proposed model, as both Job Demands and Resources are significant predictors (OR = 2.359 and 0.563 respectively, with p < 0.001). Moreover, healthcare professionals' orientation towards patient engagement appears as a significant moderator of this relationship, as it reduces Demands' effect (OR = 1.188) and increases Resources' effect (OR = 0.501).</p> <p><strong>Conclusions: </strong>These findings integrate previous findings on the JD-R Model and suggest the relevance of personal resources and of relational factors in affecting professionals' experience of burnout.</p>
The COVID-related mental health load of neonatal healthcare professionals: a multicenter study in Italy
<p>This database includes the raw data linked with the paper “The COVID-related mental health load of neonatal healthcare professionals: a multicenter study in Italy”. This study is part of the Staff and Parental Adjustment to COVID-19 Epidemics – Neonatal Experience in Tuscany” (SPACE-NET) multicenter project. In this paper, we report data on the mental health load experienced by physicians, nurses and other healthcare professionals who work in neonatal wards (NWs) and neonatal intensive care units (NICUs).</p> <p><strong>Procedures</strong></p> <p>All healthcare personnel of seven level-3 and six level-2 neonatal units in Tuscany (Italy) were invited to complete an online survey. We measured the level of physical exposure to COVID-19 risk, self-reported pandemic-related stress, and mental health load outcomes (i.e., anxiety, depression, burnout, psychosomatic, and post-traumatic symptoms) using validated, self-administered questionnaires. </p> <p><strong>Analytical plan</strong></p> <p>For variables depression, anxiety, burnout, post-traumatic symptoms a dichotomous variable [above the clinical cutoff/below the cutoff] was computed, and binomial regressions were used to estimate the probability of scoring above cutoff, including the following predictors in the model: setting, job, job experience, and <em>pandemic-related stress index</em>. As all mental health domains were correlated, in order to reduce the number of statistical comparisons and obtain an overall index, we used a principal component analysis (PCA) to calculate a global <em>mental health load index </em>(<em>MHLI</em>).</p> <p><strong>Findings in brief</strong></p> <p>Scores above the clinical cutoff were reported by 91% of participants for symptoms of anxiety, 29% for post-traumatic symptoms, 13% for burnout, and 3% for symptoms of depression. Pandemic-related stress was significantly associated with all the measured mental health load outcomes, with a Risk Ratio of 3.33 for clinically relevant anxiety, 2.39 for post-traumatic symptoms, 1.79 for emotional exhaustion, and 2.51 for depression.</p> <p> </p>

In this paper, we report data on potential predictors of emotional distress of healthcare professionals who work in neonatal wards (NWs) and neonatal intensive care units (NICUs).</p> <p><strong>Procedures</strong> - Healthcare professionals of seven level-3 and six level-2 neonatal units in Tuscany (Italy) were invited to complete an online survey. Emotional distress (i.e., anxiety, depression, psychosomatic, post-traumatic stress symptoms and emotional exhaustion), Behavioral Inhibition System (BIS) and Behavioral Approach System (BAS) sensitivity, coping strategies and safety culture were assessed through well-validated, self-reported questionnaires.</p> <p><strong>Analytical plan</strong> - Differences in mean levels of personality, coping and safety between professionals from NICUs or NWs were determined by Student’s t tests. Forward stepwise multivariate regression analyses were performed to identify significant predictors of Emotional Distress for the total sample and separately for professionals from NWs and NICUs. Furthermore, we performed a two-step cluster analysis to exploratorily identify specific profiles of professionals in terms of personality, coping strategies and safety culture and their relationship with emotional distress.</p> <p><strong>Findings in brief</strong> - Greater BIS/BAS sensitivity, avoidance coping strategies and a sub-dimension of safety culture (i.e., stress recognition) were all associated with greater risk of emotional distress, whereas job satisfaction emerged as a protective factor. Neonatal wards and NICUs personnel presented different associations between personality, coping and safety culture.</p>

PMID: 36008506; PMCID: PMC9411539.</p> <p>Abstract</p> <p>A reduced nocturnal cardiac vagal modulation has been observed in working women with preschoolers. Whether this adaptation also occurs in men remains an open question. The aim of this study was to analyze the cardiac autonomic profile of two groups of healthcare male professionals, one with and one without preschoolers, to be compared to females. Twenty-five working men with preschoolers (M_KID, age 35.41 ± 4.01 years) and 25 without (M_NOKID, 34.48 ± 6.00 years) were compared with 25 working women with preschoolers (W_KID, 37.7 ± 5.6 years) and 25 without (W_NOKID, 35.4 ± 7.2 years). A 24-h Holter electrocardiogram was performed for time and frequency domain analysis of the beat-to-beat variations of RR interval (RR) variability, during daytime (DAY) and nighttime (NIGHT). The power of RR variability in the high frequency band (HF<sub>RR</sub>) was considered as an index of cardiac vagal modulation. RR variability indices were similar in M_KID and M_NOKID during both DAY and NIGHT. In contrast, W_KID showed a reduced nocturnal HF<sub>RR</sub> compared to W_NOKID. The comparison of working men with and without preschoolers revealed no differences in the cardiac autonomic profile, in contrast with women. This suggests that sex and/or gender may represent a crucial factor in the cardiac neural control in the parental condition.</p>
