The benefit of kidney transplantation versus remaining on the waitlist depends on patients' baseline comorbidities: a retrospective study
<p>Background:<b> </b>Patients with end-stage renal disease present comorbidities that increase mortality risk on the waitlist and after kidney transplantation. This study aimed to determine the impact of baseline comorbidities on the risk of death after kidney transplantation compared to remaining on the waitlist.</p> <p>Methods: Records of waitlisted patients were retrospectively analyzed. Multivariable time-dependent Cox-regression model was used to determine the initial and the lowest relative hazard-risk of death (HRD) after exposure to kidney transplantation.</p> <p>Results: 1692 patients were included in the study. Patients without comorbidities had the relative HRD 3-fold higher, immediately after kidney transplantation, but it reduced progressively, reaching equality to waitlist at 75 days and a HRD of 0.28 thereafter (p<0.001). Patients with Diabetes and coronary artery disease had an initial relative HRD of 2.41 (p=0.009) and 2.68 (p=0.009) and matched to the waitlist at 175 and 200 days, respectively. Patients with peripheral vascular disease, had an initial relative HRD 3.6-fold in the first 240 days (p=0.008) and patients with congestive heart failure a relative HRD of 7.47 (p=0.07). Both declined rapidly, reaching equality at 110 days. Finally, all comorbidities, except cerebrovascular disease, presented lower HRD on long-term follow-up.</p> <p>Conclusions: This study shows that the time and the range of relative HRD between kidney transplantation and waitlist varies depending upon the patients´ baseline comorbidities.</p>
Data from: Long-term use benefits of personal frequency-modulated systems for speech in noise perception in patients with stroke with auditory processing deficits: a non-randomised controlled trial study
Objectives: Approximately one in five stroke survivors suffer from difficulties with speech reception in noise, despite normal audiometry. These deficits are treatable with personal Frequency Modulated systems (FMs). This study aimed to evaluate long term benefits in speech reception in noise, after daily 10 week use of personal FMs, in non-aphasic stroke patients with auditory processing deficits. Design: This was a prospective non randomised controlled trial study. Patients were allocated to an intervention care group or standard care subjects group according to their willingness to use the intervention or not. Setting: Tertiary care setting. Participants: Nine non-aphasic subjects with ischemic stroke, normal/near normal audiometry, and auditory processing deficits and with reported difficulties understanding speech in background noise were recruited in the subacute stroke stage (3-12 months after stroke). Interventions: Four patients (intervention care subjects) used the FMs in their daily life over 10 weeks. Five patients (standard care subjects) received standard care. Primary outcome measures: All subjects were tested at baseline (visit 1) and 10 weeks later (visit 2) on a sentences in noise test with the FMs (aided) and without the FMs (unaided). Results: Speech reception thresholds showed clinically and statistically significant improvements in intervention but not in standard care subjects at 10 weeks in both aided and unaided conditions. Conclusions: 10 week use of FM systems by adult stroke patients may lead to benefits in unaided speech in noise perception. Our findings may indicate auditory plasticity type changes and require further investigation.
How patients with mild dementia living in a nursing home benefit from dementia cafés: a case-control study focusing on psychological and behavioural symptoms and caregiver burden
<p><strong>Background: </strong>Dementia is a syndrome, mainly due to neurodegeneration, affecting cognition, behaviour, feelings and relationships. Pharmacological treatment is still challenging and thus different ways to improve/slow down the disease are necessary.</p> <p><strong>Methods: </strong>Twenty-five subjects with mild dementia, living in a nursing home, and their relatives were invited to attend a dementia cafe, a community group which provides support for families affected by dementia. Each patient was evaluated by a neuropsychologist, through the administration of a specific neuropsychological battery, before and at the end of the study. Their outcomes were compared to a matched group of patients with dementia receiving psycho-counselling.</p> <p><strong>Results: </strong>After the dementia cafe meetings, patients showed higher significant changes in mood (P < 0.01), behavioural symptoms (P < 0.001), quality of life (P < 0.001), and caregiver burden (P < 0.001). The control group significantly improved only in quality of life with a reduction of caregiver burden.</p> <p><strong>Conclusions: </strong>Our findings confirm that patients with dementia may benefit from the dementia cafe, especially concerning behavioural symptoms. Moreover, caregivers find these cafés to be welcoming, relaxed places to socialise and access support and information. Future dementia cafés should create programs and comfortable environments answering to the different needs of the patients.</p>
