Data from: Microglial activation in early Alzheimer trajectory is associated with higher grey matter volume

Objective: To investigate the influence of microglial activation in the early stages of Alzheimer's disease trajectory, we assessed the relationship between microglial activation and grey matter volume and hippocampal volume in MCI patients. Methods: In this study, fifty-five participants (37 early stages MCI and 18 controls) underwent [11C]PBR28 PET, a marker of microglial activation; volumetric MRI to evaluate grey matter and hippocampal volumes as well as clinical and neuropsychometric evaluation. [11C]PBR28 VT (volume of distribution) was calculated using arterial input function and Logan Graphical analysis. Grey matter volume and hippocampal volumes were calculated from MRI for each subject. Statistical parametric mapping software was used to perform voxel-wise correlations and biological parametric mapping analysis. Amyloid status was assessed using [18F]Flutemetamol PET. Results: Higher [11C]PBR28 VT in different cortical areas correlated with higher grey matter volume in both amyloid positive and negative MCI. Additionally, higher hippocampal volume correlated with higher cortical [11C]PBR28 Logan VT. Conclusions: In this in vivo study, we have demonstrated that microglial activation quantified using [11C]PBR28 PET was associated with higher grey matter volume and higher hippocampal volume in MCI patients. This may suggest that microglial activation may not always be associated with neuronal damage, and indeed it may have beneficial effect in early stages of Alzheimer's trajectory. While further longitudinal studies are necessary, these findings have significant implications on therapeutic strategies targeting microglial activation.

Data from: Comparison of cerebral blood volume and plasma volume in untreated intracranial tumors

Purpose: Plasma volume and blood volume are imaging-derived parameters that are often used to evaluation intracranial tumors. Physiologically, these parameters are directly related, but their two different methods of measurements, T1-dynamic contrast enhanced (DCE)- and T2-dynamic susceptibility contrast (DSC)-MR utilize different model assumptions and approaches. This poses the question of whether the interchangeable use of T1-DCE-MRI derived fractionated plasma volume (vp) and relative cerebral blood volume (rCBV) assessed using DSC-MRI, particularly in glioblastoma, is reliable, and if this relationship can be generalized to other types of brain tumors. Our goal was to examine the hypothetical correlation between these parameters in three most common intracranial tumor types. Methods: Twenty-four newly diagnosed, treatment naïve brain tumor patients, who had undergone DCE- and DSC-MRI, were classified in three histologically proven groups: glioblastoma (n=7), meningioma (n=9), and intraparenchymal metastases (n=8). The rCBV was obtained from DSC after normalization with the normal-appearing anatomically symmetrical contralateral white matter. Correlations between these parameters were evaluated using Pearson (r), Spearman's (ρ) and Kendall's tau-b (τB) rank correlation coefficient. Results: The Pearson, Spearman and Kendall's correlation between vp with rCBV were r=0.193, ρ=0.253 and τB=0.33 (p-Pearson=0.326, p-Spearman=0.814 and p-Kendall=0.823) in glioblastoma, r=-0.007, ρ=0.051 and τB=0.135 (p-Pearson=0.970, p-Spearman=0.765 and p-Kendall=0.358) in meningiomas, and r= 0.289, ρ=0.228 and τB= 0.239 (p-Pearson=0.109, p-Spearman=0.210 and p-Kendall=0.095) in metastasis. Conclusion: Results indicate that no correlation exists between vp with rCBV in glioblastomas, meningiomas and intraparenchymal metastatic lesions. Consequently, these parameters, as calculated in this study, should not be used interchangeably in either research or clinical practice.

Data from: Increasing compliance with low tidal volume ventilation in the ICU with two nudge-based interventions: evaluation through intervention time-series analyses

Objectives: Low tidal volume (TVe) ventilation improves outcomes for ventilated patients, and the majority of clinicians state they implement it. Unfortunately, most patients never receive low TVes. 'Nudges' influence decision-making with subtle cognitive mechanisms and are effective in many contexts. There have been few studies examining their impact on clinical decision-making. We investigated the impact of 2 interventions designed using principles from behavioural science on the deployment of low TVe ventilation in the intensive care unit (ICU). Setting: University Hospitals Bristol, a tertiary, mixed medical and surgical ICU with 20 beds, admitting over 1300 patients per year. Participants: Data were collected from 2144 consecutive patients receiving controlled mechanical ventilation for more than 1 hour between October 2010 and September 2014. Patients on controlled mechanical ventilation for more than 20 hours were included in the final analysis. Interventions: (1) Default ventilator settings were adjusted to comply with low TVe targets from the initiation of ventilation unless actively changed by a clinician. (2) A large dashboard was deployed displaying TVes in the format mL/kg ideal body weight (IBW) with alerts when TVes were excessive. Primary outcome measure: TVe in mL/kg IBW. Findings: TVe was significantly lower in the defaults group. In the dashboard intervention, TVe fell more quickly and by a greater amount after a TVe of 8 mL/kg IBW was breached when compared with controls. This effect improved in each subsequent year for 3 years. Conclusions: This study has demonstrated that adjustment of default ventilator settings and a dashboard with alerts for excessive TVe can significantly influence clinical decision-making. This offers a promising strategy to improve compliance with low TVe ventilation, and suggests that using insights from behavioural science has potential to improve the translation of evidence into practice.

Data from: The complex aerodynamic footprint of desert locusts revealed by large-volume tomographic particle image velocimetry

Particle image velocimetry has been the preferred experimental technique with which to study the aerodynamics of animal flight for over a decade. In that time, hardware has become more accessible and the software has progressed from the acquisition of planes through the flow field to the reconstruction of small volumetric measurements. Until now, it has not been possible to capture large volumes that incorporate the full wavelength of the aerodynamic track left behind during a complete wingbeat cycle. Here, we use a unique apparatus to acquire the first instantaneous wake volume of a flying animal's entire wingbeat. We confirm the presence of wake deformation behind desert locusts and quantify the effect of that deformation on estimates of aerodynamic force and the efficiency of lift generation. We present previously undescribed vortex wake phenomena, including entrainment around the wing-tip vortices of a set of secondary vortices borne of Kelvin–Helmholtz instability in the shear layer behind the flapping wings.

Data from: Thalamic volume is reduced in cervical and laryngeal dystonias

Background: Dystonia, a debilitating movement disorder characterized by abnormal fixed positions and/or twisting postures, is associated with dysfunction of motor control networks. While gross brain lesions can produce secondary dystonias, advanced neuroimaging techniques have been required to identify network abnormalities in primary dystonias. Prior neuroimaging studies have provided valuable insights into the pathophysiology of dystonia, but few directly assessed the gross volume of motor control regions, and to our knowledge, none identified abnormalities common to multiple types of idiopathic focal dystonia. Methods: We used two gross volumetric segmentation techniques and one voxelwise volumetric technique (voxel based morphometry, VBM) to compare regional volume between matched healthy controls and patients with idiopathic primary focal dystonia (cervical, n = 17, laryngeal, n = 7). We used (1) automated gross volume measures of eight motor control regions using the FreeSurfer analysis package; (2) blinded, anatomist-supervised manual segmentation of the whole thalamus (also gross volume); and (3) voxel based morphometry, which measures local T1-weighted signal intensity and estimates gray matter density or volume at the level of single voxels, for both whole-brain and thalamus. Results: Using both automated and manual gross volumetry, we found a significant volume decrease only in the thalamus in two focal dystonias. Decreases in whole-thalamic volume were independent of head and brain size, laterality of symptoms, and duration. VBM measures did not differ between dystonia and control groups in any motor control region. Conclusions: Reduced thalamic gross volume, detected in two independent analyses, suggests a common anatomical abnormality in cervical dystonia and spasmodic dysphonia. Defining the structural underpinnings of dystonia may require such complementary approaches.

Data from: Phylogenetic signal, feeding behaviour, and brain volume in Neotropical bats

Comparative correlational studies of brain size and ecological traits (e.g. feeding habits and habitat complexity) have increased our knowledge about the selective pressures on brain evolution. Studies conducted in bats as a model system assume that shared evolutionary history has a maximum effect on the traits. However, this effect has not been quantified. In addition, the effect of levels of diet specialization on brain size remains unclear. We examined the role of diet on the evolution of brain size in Mormoopidae and Phyllostomidae using two comparative methods. Body mass explained 89% of the variance in brain volume. The effect of feeding behaviour (either characterized as feeding habits, as levels of specialization on a type of item or as handling behaviour) on brain volume was also significant albeit not consistent after controlling for body mass and the strength of the phylogenetic signal (λ). Although the strength of the phylogenetic signal of brain volume and body mass was high when tested individually, λ values in phylogenetic generalized least squares models were significantly different from 1. This suggests that phylogenetic independent contrasts models are not always the best approach for the study of ecological correlates of brain size in New World bats.

Data from: Paternal B vitamin intake is a determinant of growth, hepatic lipid metabolism and intestinal tumor volume in female Apc1638N mouse offspring

Background The importance of maternal nutrition to offspring health and risk of disease is well established. Emerging evidence suggests paternal diet may affect offspring health as well. Objective In the current study we sought to determine whether modulating pre-conception paternal B vitamin intake alters intestinal tumor formation in offspring. Additionally, we sought to identify potential mechanisms for the observed weight differential among offspring by profiling hepatic gene expression and lipid content. Methods Male Apc1638N mice (prone to intestinal tumor formation) were fed diets containing replete (control, CTRL), mildly deficient (DEF), or supplemental (SUPP) quantities of vitamins B2, B6, B12, and folate for 8 weeks before mating with control-fed wild type females. Wild type offspring were euthanized at weaning and hepatic gene expression profiled. Apc1638N offspring were fed a replete diet and euthanized at 28 weeks of age to assess tumor burden. Results No differences in intestinal tumor incidence or burden were found between male Apc1638N offspring of different paternal diet groups. Although in female Apc1638N offspring there were no differences in tumor incidence or multiplicity, a stepwise increase in tumor volume with increasing paternal B vitamin intake was observed. Interestingly, female offspring of SUPP and DEF fathers had a significantly lower body weight than those of CTRL fed fathers. Moreover, hepatic trigylcerides and cholesterol were elevated 3-fold in adult female offspring of SUPP fathers. Weanling offspring of the same fathers displayed altered expression of several key lipid-metabolism genes. Hundreds of differentially methylated regions were identified in the paternal sperm in response to DEF and SUPP diets. Aside from a few genes including Igf2, there was a striking lack of overlap between these genes differentially methylated in sperm and differentially expressed in offspring. Conclusions In this animal model, modulation of paternal B vitamin intake prior to mating alters offspring weight gain, lipid metabolism and tumor growth in a sex-specific fashion. These results highlight the need to better define how paternal nutrition affects the health of offspring.

Data from: Long-term follow-up results in patients undergoing percutaneous coronary intervention (PCI) with drug-eluting stents: results from a single high-volume PCI center

Objective: To assess both short and long-term prognosis in consecutive patients with coronary heart disease (CHD) treated with drug-eluting stents (DES) in a high-volume percutaneous coronary intervention (PCI) center. Design: Observational cohort study. Setting: A hospital in the Henan province, China, between 2009 and 2011. Participants: A total of 2,533 patients were enrolled. Patients with ST-elevation myocardial infarction (STEMI) treated with urgent PCI accounted for 3.9% of cases; patients with STEMI treated with delayed PCI accounted for 20.5% of cases; patients with stable angina (SA) accounted for 16.5% of cases; and patients with non-ST elevation acute coronary syndromes (NSTE-ACS) accounted for 58.6% of cases. Primary outcomes: Death, major adverse cardiac and cerebrovascular events (MACCE: death/myocardial infarction/stroke), and target vessel revascularization. Results: Follow-up after a median of 29.8 months was obtained for 2,533 patients (92.6%). The mortality rate during hospitalization was highest in the urgent PCI group (p <0.001). During follow-up, although the incidences of death and MACCE were highest in the urgent PCI group, no significant differences were observed between the different groups. The incidences of cardiac death and myocardial infarction were significantly higher in the paclitaxel-eluting stent (PES) group than in the sirolimus-eluting stent (SES) group. Independent predictors of death during follow-up were: age, left ventricular ejection function (LVEF) < 40%, diabetes mellitus, prior coronary artery bypass graft (CABG), and chronic total occlusion. Conclusions: PCI patients with STEMI had the worst hospital and long-term prognosis. The mortality rate after hospital increased markedly in NSTE-ACS patients. SES seems to be more effective than PES.

Data from: A critical evaluation of the volume, relevance and quality of evidence submitted by the tobacco industry to oppose standardised packaging of tobacco products

Objectives: To examine the volume, relevance and quality of transnational tobacco corporations' (TTCs) evidence that standardised packaging of tobacco products 'won't work', following the UK government's decision to 'wait and see' until further evidence is available. Design Content analysis. Setting: We analysed the evidence cited in submissions by the UK's four largest TTCs to the UK Department of Health consultation on standardised packaging in 2012. Outcome measures: The volume, relevance (subject matter) and quality (as measured by independence from industry and peer-review) of evidence cited by TTCs was compared with evidence from a systematic review of standardised packaging . Fisher's exact test was used to assess differences in the quality of TTC and systematic review evidence. 100% of the data were second-coded to validate the findings: 94.7% intercoder reliability; all differences were resolved. Results: 77/143 pieces of TTC-cited evidence were used to promote their claim that standardised packaging 'won't work'. Of these, just 17/77 addressed standardised packaging: 14 were industry connected and none were published in peer-reviewed journals. Comparison of TTC and systematic review evidence on standardised packaging showed that the industry evidence was of significantly lower quality in terms of tobacco industry connections and peer-review (p&lt;0.0001). The most relevant TTC evidence (on standardised packaging or packaging generally, n=26) was of significantly lower quality (p&lt;0.0001) than the least relevant (on other topics, n=51). Across the dataset, TTC-connected evidence was significantly less likely to be published in a peer-reviewed journal (p=0.0045). Conclusions: With few exceptions, evidence cited by TTCs to promote their claim that standardised packaging 'won't work' lacks either policy relevance or key indicators of quality. Policymakers could use these three criteria—subject matter, independence and peer-review status—to critically assess evidence submitted to them by corporate interests via Better Regulation processes.

Data from: Minimally invasive estimation of ventricular dead space volume through use of Frank-Starling curves

This paper develops a means of more easily and less invasively estimating ventricular dead space volume (Vd), an important, but difficult to measure physiological parameter. Vd represents a subject and condition dependent portion of measured ventricular volume that is not actively participating in ventricular function. It is employed in models based on the time varying elastance concept, which see widespread use in haemodynamic studies, and may have direct diagnostic use. The proposed method involves linear extrapolation of a Frank-Starling curve (stroke volume vs end-diastolic volume) and its end-systolic equivalent (stroke volume vs end-systolic volume), developed across normal clinical procedures such as recruitment manoeuvres, to their point of intersection with the y-axis (where stroke volume is 0) to determine Vd. To demonstrate the broad applicability of the method, it was validated across a cohort of six sedated and anaesthetised male Pietrain pigs, encompassing a variety of cardiac states from healthy baseline behaviour to circulatory failure due to septic shock induced by endotoxin infusion. Linear extrapolation of the curves was supported by strong linear correlation coefficients of R = 0.78 and R = 0.80 average for pre- and post- endotoxin infusion respectively, as well as good agreement between the two linearly extrapolated y-intercepts (Vd) for each subject (no more than 7.8% variation). Method validity was further supported by the physiologically reasonable Vd values produced, equivalent to 44.3-53.1% and 49.3-82.6% of baseline end-systolic volume before and after endotoxin infusion respectively. This method has the potential to allow Vd to be estimated without a particularly demanding, specialised protocol in an experimental environment. Further, due to the common use of both mechanical ventilation and recruitment manoeuvres in intensive care, this method, subject to the availability of multi-beat echocardiography, has the potential to allow for estimation of Vd in a clinical environment.

Data from: Multiple regression modelling for estimating endocranial volume in extinct Mammalia

The profound evolutionary success of mammals has been linked to behavioral and life-history traits, many of which have been tied to brain size. However, studies of the evolution of this key trait have yet to explore the full potential of the fossil record, being limited by the difficulty of obtaining endocranial data from fossils. Using measurements of endocranial volume, length, height, and width of the braincase in 503 adult specimens from 199 extant species, representing 99 of 133 extant mammalian families, we expand upon a simple method of using multiple regression to develop a formula for estimating brain size from external skull measurements. We also examined non- mammalian synapsids to assess the phylogenetic limits of our model's application. Model-predicted volume correlates strongly with measured volume (R2 = 0.993) and prediction error is between 16% and 19%. Error decreases if models developed for well-sampled subclades such as primates or rodents are used, demonstrating that some differential evolution of the relationship between brain size and skull size has occurred. However, reanalysis using phylogenetically independent contrasts demonstrates weak phylogenetic dependency, indicating that our model is appropriate for estimating the endocranial volume of species of unknown phylogenetic affinity. Thus, the model represents a generally applicable, fast and cost-efficient way to dramatically expand the taxonomic and temporal scope of mammalian brain size data sets. Even endocranial volumes of taxa with highly derived crania, such as cetaceans and monotremes, can be estimated confidently. However, the model works best for generalized placental crania. Fundamental differences in cranial architecture suggest that the model cannot provide accurate estimates of endocranial volume in non-mammalian synapsids more basal than Morganucodon (ca. 200 Ma). Therefore, use of the model for taxa phylogenetically distant from the mammalian crown group is not warranted, but it might be used to establish relative brain sizes between closely related subgroups.

Data from: Corrections for rooting volume and plant size reveal negative effects of neighbour presence on root allocation in pea

Plants are able to detect the presence of their neighbours belowground. The associated root responses may affect plant performance, plant-plant interactions and community dynamics, but the extent and direction of these responses is heavily debated. Some studies suggest that plants will over-proliferate roots in response to neighbours at the expense of reproduction, which was framed as a "tragedy of the commons". Others proposed an "ideal free distribution" hypothesis, stating that plants produce roots simply as a function of the amount of available nutrients. However, experimental evidence for either hypothesis that is unbiased by confounding effects of rooting volume and plant size in their experimental setups is still lacking. We grew split-root pea plants in the presence or absence of a belowground neighbour at a range of rooting volumes, while providing equal amounts of nutrients per plant. Path analyses were used to disentangle the direct effects of neighbour presence on allocation patterns from the confounding effects of rooting volume and plant size. Within the chosen range of rooting volumes, the presence of a belowground neighbour generally reduced plant root mass by 21% and total mass by 9%. A doubling of rooting volume generally increased plant root mass by 18% and total mass by 11%. Pod mass was only directly and positively correlated with vegetative mass. The presence of a belowground neighbour induced less root allocation but more pod allocation, whereas increased rooting volume caused a reduction in reproductive allocation. A large part of these effects, however, was indirectly mediated through the influence on plant total mass. Synthesis: Not considering the effects of rooting volume and plant size may lead to misinterpretations of plant growth strategies in response to neighbours. Accounting for these factors, we found pea allocating less mass to roots in the presence of a belowground neighbour. The obtained results can help to reconcile the various responses to belowground neighbours as they are published in the literature.
