Data for: Thriving in a pandemic: determinants of excellent wellbeing among New Zealanders during the 2020 COVID-19 lockdown; a cross-sectional survey
<p><strong>Objective:</strong> The COVID-19 pandemic and associated restrictions are associated with adverse psychological impacts but an assessment of positive wellbeing is required to understand the overall impacts of the pandemic.</p> <p><strong>Methods: </strong>The NZ Lockdown Psychological Distress Survey measured excellent wellbeing categorised by a WHO-Five Well-being Index (WHO-5) score ≥22. The survey also contained demographic and pre-lockdown questions, subjective and objective lockdown experiences, and questions on alcohol use. The proportion of participants with excellent wellbeing is reported with multivariate analysis examining the relative importance of individual factors associated with excellent wellbeing.</p> <p><strong>Results:</strong> Approximately 9% of the overall sample reported excellent wellbeing during the New Zealand lockdown. Excellent wellbeing status was associated with older age, male gender, Māori and Asian ethnicity, and lower levels of education. Excellent wellbeing was negatively associated with smoking, poor physical and mental health, and previous trauma.</p> <p><strong>Conclusion:</strong> A substantial minority of New Zealanders reported excellent wellbeing during severe COVID-19 pandemic restrictions. Demographic and broader health factors predicted excellent wellbeing status. An understanding of these factors may help to enhance wellbeing during any future lockdowns.</p>
Data from: Dispersion of SARS-CoV-2 in air surrounding COVID-19 infected individuals with mild symptoms
<p><span>Since the beginning of the pandemic, the transmission modes of SARS-CoV-2—particularly the role of aerosol transmission—has been much debated. Accumulating evidence suggests that SARS-CoV-2 can be transmitted by aerosols, and not only via larger respiratory droplets. In this study, we quantified SARS-CoV-2 in air surrounding 14 test subjects in a controlled setting. All subjects had SARS-CoV-2 infection confirmed by a recent positive PCR test and had mild symptoms when included in the study. RT-PCR and cell culture analyses were performed on air samples collected at distances of one, two, and four meters from test subjects. Oronasopharyngeal samples were taken from consenting test subjects and analyzed by RT-PCR. Additionally, total aerosol particles were quantified during air sampling trials. Air viral concentrations at one-meter distance were significantly correlated with both viral loads in the upper airways, mild coughing, and fever. One sample collected at four-meter distance was RT-PCR positive. No samples were successfully cultured. The results reported here have potential application for SARS-CoV-2 detection and monitoring schemes, and for increasing our understanding of SARS-CoV-2 transmission dynamics.</span></p>

Characteristics of COVID-19 patients predicting hospital readmission after ED discharge
<p><strong>Objective:</strong> We aimed at identifying baseline predictive factors for emergency department (ED) readmission, with hospitalization/death, in COVID-19 patients previously discharged from the ED. We also developed a disease progression velocity index.</p> <p><strong>Design and setting:</strong> Retrospective cohort study of prospectively collected data. The charts of consecutive COVID-19 patients discharged from the Reggio Emilia (Italy) ED (March 2 - March 31, 2020) were retrospectively examined. Clinical, laboratory, and computed tomography (CT) findings at first ED admission were tested as predictive factors using multivariable logistic models. We divided CT extension by days from symptom onset to build a synthetic velocity index.</p> <p><strong>Participants: </strong>450 patients discharged from the ED with diagnosis of COVID-19.</p> <p><strong>Main outcome measure:</strong> ED readmission within 14 days, followed by hospitalization/death.</p> <p><strong>Results:</strong> Of the discharged patients, 84 (18.7%) were readmitted to the ED and 61 (13.6%) were hospitalized and 10 (2.2%) died. Age (OR=1.05; 95% CI 1.03-1.08), Charlson Comorbidity Index 3 vs 0 (OR=11.61; 95% CI 1.76-76.58), days from symptom onset (OR for one day increase=0.81; 95% CI 0.73-0.90), and CT extension (OR for 1% increase=1.03; 95% CI 1.01-1.06) were associated in a multivariable model for readmission with hospitalization/death. A 2-day lag velocity index was a strong predictor (OR for unit increase=1.21, 95% CI 1.08-1.36); the model including this index resulted in less information loss.</p> <p><strong>Conclusions:</strong> A velocity index combining CT extension and days from symptom onset predicts disease progression in COVID-19 patients. For example, a 20% CT extension 3 days after symptom onset has the same risk as does 50% after 10 days.</p>

Genetic and immunological evaluation of children with inborn errors of immunity and hypoxemic COVID-19 pneumonia
<p><strong><em>Background:</em></strong> SARS-CoV-2 has infected more than 450 million people around the world. In about 10% of cases, the infection leads to hypoxemic pneumonia, which is much rarer in children, while multisystem inflammation is seen almost only in children.</p> <p><strong><em>Methods:</em></strong> We evluated 31 young patients (0.5-19 years) who had both pre-existing inborn errors of immunity (IEI) and were later diagnosed with COVID-19 complications. Whole-exome sequencing was performed, SARS-CoV-2 specific antibodies and autoantibody against type I interferons were measured and plasma inflammatory factors were profiled. We also reviewed COVID-19 disease severity and outcome in IEI patients in the literature.</p> <p><strong><em>Results:</em></strong> A potential genetic cause of the IEI was identified in 28 patients (90.3%). The IgM and IgG-specific antibody response against the spike protein of the virus was positive in 14 (66.6%) of the IEI patients tested. Autoantibodies neutralizing type I interferons were identified in the plasma of two patients (6.8% of tested samples). Five patients fulfilled the diagnostic criteria of multisystem inflammatory syndrome in children (MIS-C). Eleven patients (35.4%) died due to COVID-19 complications showing a 3000 fold higher mortality rate compared to normal COVID-19 death rate in children (0.01%). For the additional 696 IEI patients reported till date, severe presentation of COVID-19 was observed in 23.3% of cases. Furthermore, a 5-fold higher mortality rate was observed, when compared with global infection-fatality reports.</p> <p><strong><em>Conclusions:</em></strong> Elucidating the genetic basis of IEI patients with severe/critical COVID-19 may help to better understand the human innate and adaptive immune response against SARS-CoV-2, which may assist to develop better preventive and therapeutic strategies.</p>

Seroprevalence of COVID-19 in Carabayllo Peru
<div> <div> <div> <div> <p><strong>Objectives:</strong> To measure the SARS-CoV-2 seroprevalence in the Carabayllo district of Lima, Peru at the end of the first wave of the pandemic.</p> <p><strong>Methods:</strong> This was a cross-sectional study of the district with random bi-stage cluster sampling. We collected socio-demographic and clinical data as well as serum samples for SARS-CoV2 antibody testing.</p> <p><strong>Results:</strong> Among 2,014 subjects sampled, 59.93% were men. SAR-CoV-2 seropositivity was 24.88% (501/2014), and after adjusting for the sampling design effects and test performance, the seroprevalence in November/December 2020 was 22.4% (95% CI 22.35 - 27.63). Seropositivity varied by age, with participants aged 30-59 years old having the highest prevalence. Seropositivity in symptomatic patients (32%) was 1.5 times more than in asymptomatic ones (23%), and work away of home, age >18, COVID-19 home contact, obesity were risk factors of SARS-CoV-2.</p> <p><strong>Conclusions:</strong> The low seroprevalence after the first wave of pandemic in this community indicates that a high proportion of people remained susceptible. Infection was important among people who never reported symptoms. People with obesity had a higher risk of having SARS-Cov-2 infection.</p> </div> </div> </div> </div>

Measurement report: The importance of biomass burning in light extinction and direct radiative effect of urban aerosol during the COVID-19 lockdown in Xi'an, China
<p>To explore the impacts of anthropogenic emissions on aerosol optical properties and direct radiative effect (DRE), a series of real-time measurements was conducted in an urban area of China before and during the lockdown of Coronavirus Disease 2019. The generalized addictive model analysis shows that the light extinction coefficient (<em>b</em><sub>ext</sub>) decreased largely during the lockdown due to the sharp reductions in anthropogenic emissions. Organic aerosol was the largest contributor to <em>b</em><sub>ext</sub> based on the ridge regression analysis. A hybrid environmental receptor model combining with chemical and optical variables shows that <em>b</em><sub>ext</sub> from biomass burning increased in the lockdown due to the undiminished needs of residential cooking and heating in winter. Biomass burning contributed the largest positive effect to aerosol DRE in the atmosphere during the lockdown, highlighting the importance of controlling biomass burning for mitigating climate change in China in the future.</p>
