PMID: 31891712.</li> </ul> <p> </p> <p><strong>Abstract</strong><br> <strong>Objective: </strong>To investigate the incremental prognostic significance of malnutrition in patients with severe poststroke disability.<br> <strong>Design: </strong>Retrospective cohort study. The patients were recruited from 3 specialized inpatient rehabilitation facilities. Nutritional status was assessed using the Prognostic Nutritional Index (PNI), which is calculated from serum albumin and total lymphocyte count. Scores >38 points reflect normal nutrition status, scores of 35-38 indicate moderate malnutrition, and scores <35 indicate severe malnutrition. The association of PNI categories with outcomes was assessed using multivariable regression analyses.<br> <strong>Setting: </strong>Inpatient rehabilitation facility.<br> <strong>Participants:</strong> Patients (NZ668) with ischemic stroke admitted to inpatient rehabilitation within 90 days from stroke occurrence and classified as Case-Mix Groups 0108, 0109, and 0110 of the current Medicare case-mix classification system.<br> <strong>Interventions: </strong>Not applicable.<br> <strong>Main Outcome Measures:</strong> Three outcomes were examined: (1) the combined outcome of transfer to acute care and death within 90 days from admission to rehabilitation; (2) 2-year mortality; and (3) FIM motor effectiveness, calculated as (FIM motor change/maximum FIM motoradmission FIM motor score)100.<br> <strong>Results: </strong>Overall, the median time to rehabilitation admission was 18 days (range, 12-26 days). The prevalence of moderate and severe malnutrition was 12.7% and 11.5%, respectively. Ninety-one patients (13.6%) experienced the combined outcome. After adjusting for independent predictors including sex, atrial fibrillation, dysphagia, FIM cognitive score, and hemoglobin levels, neither moderate (PZ.280) nor severe malnutrition (PZ.482) were associated with the combined outcome. Similar results were observed when looking at 2-year mortality. Overall, FIM motor effectiveness was 30%24%. After adjusting for independent predictors, severe malnutrition (b coefficient 0.4580.216; PZ.034) was associated with FIM motor effectiveness.<br> <strong>Conclusions:</strong> Approximately 1 in every 9 patients presented severe malnutrition. On top of the independent predictors, severe malnutrition did not provide additional prognostic information concerning risk of the combined outcome or 2-year mortality. Conversely, severe malnutrition was associated with poorer functional outcome as expressed by FIM motor effectiveness.</p>
Intracranial carotid artery calcification morphology differs in patients with lacunar and non-lacunar acute ischemic strokes
<p><strong>Introduction. </strong>This database includes the raw data linked with the paper "Intracranial carotid artery calcification morphology differs in patients with lacunar and non-lacunar acute ischemic strokes".</p> <p>In this paper, we reported the association of diverse pattern of intracranial carotid artery calcifications (ICAC) and etiologic subtypes of ischemic stroke.</p> <p><strong>Methods.</strong> We retrospectively analyzed a single center cohort of patients admitted for ischemic stroke with known etiology. Patients with a first ischemic stroke with lacunar, cardioembolic or atherothrombotic large artery disease etiology were included. Each carotid artery was evaluated separately on NCCT scans to define the ICAC pattern. The association of ICAC patterns and stroke etiology was investigated using two logistic regression models adjusting for confounders.</p> <p><strong>Results.</strong> A total of 300 patients were included in the analysis, mean age 78 years (+-10), 158 (53%) female, median NIHSS 6 (IQR 9). Prevalence of medial, intimal, and mixed calcification was: 73%, 17% and 11% in cardioembolic stroke, 66%, 25% and 9 % in large artery atherosclerosis stroke and 37%, 44% and 19% in lacunar stroke (p<0.001). After adjustment for confounders, intimal pattern (OR=2.14; 95%CI=1.07-4.30), and medial pattern (OR=0.33; 95CI%=0.16-0.67) remained independently associated with lacunar stroke etiology. In conclusion, our study suggests that intimal ICAC pattern is associated with lacunar stroke, and, if validated in further studies, it may serve as a marker for lacunar stroke etiology.</p>

Rapid Risk Stratification of Acute Ischemic Stroke Patients in the Emergency Department: The Incremental Prognostic Role of Left Atrial Reservoir Strain
<p><strong>Objectives</strong>: To determine the prognostic value of positive global left atrial strain (LA-GSA+), measured by two-dimensional speckle tracking echocardiography (2D-STE) in a population of acute ischemic stroke (AIS) patients without atrial fibrillation (AF), in the setting of Emergency Department (ED).</p> <p><strong>Methods</strong>: All consecutive AIS patients with sinus rhythm on ECG and without AF history entered this prospective study. All patients underwent complete blood tests and transthoracic echocardiography implemented with 2D-STE analysis of LA strain parameters within 6-12 hours after symptoms onset. At 6-months follow-up, we evaluated the composite endpoint of all-cause mortality plus cardiovascular re-hospitalizations.</p> <p><strong>Results</strong>: A total of 102 AIS patients (76.4±10.8 yrs, 47% males) were prospectively included. LA-GSA+ was markedly reduced in AIS patients (20.8±7.7%), without any statistically significant difference between the stroke subtypes. At 6-months follow-up, 7 deaths and 27 re-hospitalizations occurred. On multivariate Cox regression analysis, variables independently associated with outcome were: LA-GSA+ (per unit) (HR 0.29, 95% CI 0.19-0.39) and C-reactive protein (CRP) (per 0.1 mg/dl) (HR 1.45, 95% CI 1.15-1.75) as continuous variables; statin therapy (HR 0.45, 95%CI 0.28-0.62), and type 2 diabetes (HR 1.65, 95% CI 1.15-2.35) as categorical variables. A LA-GSA+ ≤20.0% predicted the occurrence of the above-mentioned outcome at 6-months follow-up with 94% sensitivity and 81% specificity (AUC=0.84). Interestingly, GSA+ showed a strong inverse correlation with CRP levels (r=-0.86).</p> <p><strong>Conclusions</strong>: A LA-GSA+ ≤20% reflects a more advanced atrial cardiomyopathy and might provide a rapid and reliable prognostic risk stratification of AIS patients without AF history in the setting of ED.</p>
