Development and validation of a machine learning model for use as an automated artificial intelligence tool to predict mortality risk in patients with COVID-19
<p><strong>Background</strong></p> <p>New York City quickly became an epicenter of the COVID-19 pandemic. Due to a sudden and massive increase in patients during COVID-19 pandemic, healthcare providers incurred an exponential increase in workload which created a strain on the staff and limited resources. As this is a new infection, predictors of morbidity and mortality are not well characterized.</p> <p><strong>Methods</strong></p> <p>We developed a prediction model to predict patients at risk for mortality using only laboratory, vital and demographic information readily available in the electronic health record on more than 3000 hospital admissions with COVID-19. A variable importance algorithm was used for interpretability and understanding of performance and predictors.</p> <p><strong>Findings</strong></p> <p>We built a model with 84-97% accuracy to identify predictors and patients with high risk of mortality, and developed an automated artificial intelligence (AI) notification tool that does not require manual calculation by the busy clinician. Oximetry, respirations, blood urea nitrogen, lymphocyte percent, calcium, troponin and neutrophil percentage were important features and key ranges were identified that contributed to a 50% increase in patients’ mortality prediction score. With an increasing negative predictive value (NPV) starting 0.90 after the second day of admission, we are able more confidently able identify likely survivors. This study serves as a use case of a model with visualizations to aide clinicians with a better understanding of the model and predictors of mortality. Additionally, an example of the operationalization of the model via an AI notification tool is illustrated.</p>
Data from: Altered fronto-temporal functional connectivity in individuals at ultra-high-risk of developing psychosis
Background: The superior temporal gyrus (STG) is one of the key regions implicated in psychosis, given that abnormalities in this region are associated with an increased risk of conversion from an at-risk mental state to psychosis. However, inconsistent results regarding the functional connectivity strength of the STG have been reported, and the regional heterogeneous characteristics of the STG should be considered. Methods: To investigate the distinctive functional connection of each subregion in the STG, we parcellated the STG of each hemisphere into three regions: the planum temporale, Heschl's gyrus, and planum polare. Resting-state functional magnetic resonance imaging was obtained from 22 first-episode psychosis (FEP) patients, 41 individuals at ultra-high-risk for psychosis (UHR), and 47 demographically matched healthy controls. Results: Significant group differences (in seed-based connectivity) were demonstrated in the left planum temporale and from both the right and left Heschl's gyrus seeds. From the left planum temporale seed, the FEP and UHR groups exhibited increased connectivity to the bilateral dorsolateral prefrontal cortex. In contrast, the FEP and UHR groups demonstrated decreased connectivity from the bilateral Heschl's gyrus seeds to the dorsal anterior cingulate cortex. The enhanced connectivity between the left planum temporale and right dorsolateral prefrontal cortex was positively correlated with positive symptom severity in individuals at UHR (r = .34, p = .03). Conclusions: These findings corroborate the fronto-temporal connectivity disruption hypothesis in schizophrenia by providing evidence supporting the altered fronto-temporal intrinsic functional connection at earlier stages of psychosis. Our data indicate that subregion-specific aberrant fronto-temporal interactions exist in the STG at the early stage of psychosis, thus suggesting that these aberrancies are the neural underpinning of proneness to psychosis.
Data from:Corruption, development and governance indicators predict invasive species risk from trade
Invasive species have an enormous global impact, with international trade being the leading pathway for their introduction. Current multinational trade deals under negotiation will dramatically change trading partnerships and pathways. These changes have considerable potential to influence biological invasions and global biodiversity. Using a database of 47 328 interceptions spanning 10 years, we demonstrate how development and governance socio-economic indicators of trading partners can predict exotic species interceptions. For import pathways associated with vegetable material, a significantly higher risk of exotic species interceptions was associated with countries that are poorly regulated, have more forest cover and have surprisingly low corruption. Corruption and indicators such as political stability or adherence to rule of law were important in vehicle or timber import pathways. These results will be of considerable value to policy makers, primarily by shifting quarantine procedures to focus on countries of high risk based on their socio-economic status. Further, using New Zealand as an example, we demonstrate how a ninefold reduction in incursions could be achieved if socio-economic indicators were used to select trade partners. International trade deals that ignore governance and development indicators may facilitate introductions and biodiversity loss. Development and governance within countries clearly have biodiversity implications beyond borders.
Data from: Female mosquitoes disperse further when they develop under predation risk
Predation is one of the strongest selective forces in nature. Organisms characterized by a complex life cycle, undergoing an ontogenetic niche shift, can reduce predation risk on natal stages by metamorphosing earlier. Yet, this anti-predatory response may incur numerous life-history-related costs. Interestingly, the consequence of larval predation risk on adult dispersal, a key trait dictating the persistence of spatially-structured populations, is largely understudied. Here, we explored the effect of larval predation risk on the life-history and dispersal characteristics of female adults in the mosquito Culex pipiens. As predicted, mosquito larvae reared in the presence of a caged larvivorous-fish metamorphosed earlier, while also suffering from reduced survival. Despite this reduction in development time, the body size of emerging females was larger, implying that more resources were allocated to increase the growth rate, probably at the expense of reduced maintenance and storage. This shift in energy allocation translated into decreased pupa and adult survival. Remarkably, the respective dispersal distance of these larger bodied females was greater. We suggest that the increase in dispersal distance allows these females to cover larger areas, while searching for oviposition sites that are safer than their natal aquatic habitat. Exploring the effects of larval conditions on adult dispersal is central for understanding the distribution of organisms with a complex life cycle in spatially heterogeneous environments, and specifically for disease transmission by mosquitoes.
