Nurses coping with daily stressors

<p><b>Background:</b> During their workday, nurses face a variety of stressors that are dealt with using different coping strategies. One criticism of the contextual models of work stress is that they fail to focus on individual responses like coping with stress. Neverthless, little is know about the momentary determinants of coping in nurses.</p> <p><b>Objectives</b>: To identify the momentary predictors of problem-focused approaching coping and emotion-focused approaching coping, as well as those for seeking social support and refusal coping strategies, during the working day in nurses.</p> <p><b>Design:</b> This study uses descriptive, correlational, two-level design with repeated measures.</p> <p><b>Settings: </b>Wards of two University hospitals.</p> <p><b>Participants: </b>A random cohort of 113 nurses was studied.</p> <p><b>Methods</b>: An ecological momentary assessment was made of demand, control, effort, reward, nursing task, coping, mood and fatigue, and of coping style by questionnaire. Multilevel two-level statistical analyses were performed in order to identify both within person and between person relationships.</p> <p><b>Results: </b>Different momentary types of coping were associated with different tasks. The problem-focused coping could be explained by the direct care and medication tasks, demand, planning coping style, mood, and negatively by acceptation coping style. Emotion-focused coping could be explained by documentation and medication tasks (negatively), mood, demand, distraction, and disengagement coping styles. Seeking social support coping could be explained by the task of communication, mood, fatigue (negatively), and seeking emotional support as a coping style. Refusal coping could be explained by mood, and the coping style of focusing and venting emotions. Refusal coping is not specific to any task.</p> <p><span><b>Conclusions: </b>The choice of the coping strategy depends on the task, of their appraisal and on the different styles of coping.</span></p>

Data from: Comparing safety climate for nurses working in operating theatres, critical care and ward areas in the UK: a mixed methods study

Objectives: The main aim of the study was to explore the potential sources of variation and understand the meaning of safety climate for nursing practice in acute hospital settings in the UK. Design: A sequential mixed methods design included a cross-sectional survey using the Safety Climate Questionnaire (SCQ) and thematic analysis of focus group discussions. Confirmatory Factor Analysis (CFA) was used to validate the factor structure of the SCQ. Factor scores were compared between nurses working in operating theatres, critical care and ward areas. Results from the survey and the thematic analysis were then compared and synthesised. Setting: A London University. Participants: 319 registered nurses working in acute hospital settings completed the SCQ and a further 23 nurses participated in focus groups. Results: CFA indicated that there was a good model fit on some criteria (χ2 = 1683.699, df 824, p &lt; 0.001; χ2/ df = 2.04; RMSEA = 0.058) but a less acceptable fit on Comparative Fit Index (CFI) = 0.804. There was a statistically significant difference between clinical specialisms in Management Commitment (F [4,266] = 4.66, p = 0.001). Nurses working in operating theatres had lower scores compared with ward areas and they also reported negative perceptions about management in their focus group. There was significant variation in scores for Communication across clinical specialism (F [4,266] = 2.62, p = 0.035) but none of the pair-wise comparisons achieved statistical significance. Thematic analysis identified themes of Human Factors, Clinical Management and Protecting Patients. The System and the Human Side of Caring was identified as a meta-theme. Conclusions: The results suggest that the SCQ has some utility but requires further exploration. The findings indicate that safety in nursing practice is a complex interaction between safety systems and the social and interpersonal aspects of clinical practice.

Data from: Nurse-based restoration of degraded tropical forests with tussock grasses: experimental support from the Andean cloud forest

1. The degradation of the Andean cloud forest raises strong biological conservation issues and threatens the sustainability of a crucial water resource. The idea that nurse-based restoration can accelerate the recovery of these forests is underexplored, despite its promise as a restoration technique. Recent conceptual models predict that facilitation among plants may be an important mechanism, but there is a lack of strong empirical support. We gathered experimental data to test this prediction and explore the relevance of using nurse-based forest restoration in these environments. 2. A 20-month factorial experimental design in the Andean tropical cloud forest was established. We measured the survival and estimated the biomass production of transplanted seedlings of a keystone canopy forest species, Ceroxylon echinulatum (Arecaceae), in a deforested area in the presence/absence of herbivory, a potential nurse plant (the tussock grass Setaria sphacelata, Poaceae), and artificial shade. 3. The joint effects of deforestation and herbivory led to the death of all seedlings, whereas most seedlings survived in the adjacent forest, which was used as the control. The presence of nurse plants led to significantly higher survival and growth of Ceroxylon seedlings throughout the experiment, regardless of herbivore presence. 4. The nurse effects were explained by a reduction of the relative abiotic stress experienced by the seedlings outside the forest, i.e. the consistently decreasing maximum vapour pressure deficit. Furthermore, nurse tussocks delayed and reduced the effects of herbivory by offering physical protection and a refuge for seedlings against detection by herbivores. However, the effects of herbivory and abiotic stress on facilitation were not additive. 5. Synthesis and applications: Facilitation in degraded cloud forest can be intense as soon as the beneficiary plants are driven away from their physiological optimum (relative abiotic stress) and/or are confronted by herbivory. Using pre-established exotic tussock grasses as a nurse-based restoration technique in degraded cloud forest is a low-cost, non-detrimental (to biodiversity) option, especially in the absence of nurse trees and shrubs. The success of this method requires transplanting seedlings at the base of tussocks.

Data from: Nurse species and indirect facilitation through grazing drive plant community functional traits in tropical alpine peatlands

Facilitation among plants mediated by grazers occurs when an unpalatable plant extends its protection against grazing to another plant. This type of indirect facilitation impacts species coexistence and ecosystem functioning in a large array of ecosystems worldwide. It has nonetheless generally been understudied so far in comparison with the role played by direct facilitation among plants. We aimed at providing original data on indirect facilitation at the community scale to determine the extent to which indirect facilitation mediated by grazers can shape plant communities. Such experimental data are expected to contribute to refining the conceptual framework on plant–plant–herbivore interactions in stressful environments. We set up a 2-year grazing exclusion experiment in tropical alpine peatlands in Bolivia. Those ecosystems depend entirely on a few, structuring cushion-forming plants (hereafter referred to as "nurse" species), in which associated plant communities develop. Fences have been set over two nurse species with different strategies to cope with grazing (direct vs. indirect defenses), which are expected to lead to different intensities of indirect facilitation for the associated communities. We collected functional traits which are known to vary according to grazing pressure (LDMC, leaf thickness, and maximum height), on both the nurse and their associated plant communities in grazed (and therefore indirect facilitation as well) and ungrazed conditions. We found that the effect of indirectly facilitated on the associated plant communities depended on the functional trait considered. Indirect facilitation decreased the effects of grazing on species relative abundance, mean LDMC, and the convergence of the maximum height distribution of the associated communities, but did not affect mean height or cover. The identity of the nurse species and grazing jointly affected the structure of the associated plant community through indirect facilitation. Our results together with the existing literature suggest that the "grazer–nurse–beneficiary" interaction module can be more complex than expected when evaluated in the field.

Data from: Fine nurse variations explain discrepancies in the stress-interaction relationship in alpine regions

Despite a large consensus on increasing facilitation among plants with increasing stress in alpine regions, a number of different outcomes of interaction have been observed, which impedes the generalisation of the 'stress-gradient hypothesis' (SGH). With the aim to reconcile the different viewpoints on the stress-interaction relationship in alpine environments we hypothesized that fine nurse variations within a single life form (cushion) may explain this pattern variability To test this hypothesis, we compared the magnitude of the stress-interaction relationship in a single study area with that observed in existing studies involving cushions, worldwide. We characterized the nurse effects of cushions on the whole plant community at inter-specific, intra-specific and intra-individual levels along a stress gradient in the dry, alpine tropics of Bolivia (4400 m, 4700 m and 4900 m a.s.l). Using a relative index of interaction (RII) we included our data in a meta-analysis on the nurse effects of cushions along alpine gradients, worldwide. At inter-specific level, the loose cushion Pycnophyllum was a better nurse than the compact Azorella compacta. However, at intra-individual level facilitation was higher at the periphery than at the centre of cushions, exceeding in magnitude the variation observed at inter-specific level. This pattern was associated with higher minimum temperature and lower mortality at the periphery of cushions. The net effects of cushions on plant communities became more positive at higher elevation, corroborating the SGH. Within our single site in Bolivia, fine morphological nurse variations captured a similar variability in the stress-interaction relationship as that observed in a subset of studies on cushions on a worldwide scale. This suggests that fine variations in nurse traits, in general those not considered in protocols dealing with facilitation or in restoration/conservation management plans, explain in part the current discrepancies among SGH studies in alpine regions.
