<p><i>Background</i>: Antimalarial drug resistance is a major challenge hampering malaria control and elimination. <i>Plasmodium falciparum</i>, the leading causative parasite species, has developed resistance to basically all antimalarials. Continued surveillance of drug resistance using genetic markers provides important molecular data for treatment policies. This study sought to verify the genetic mechanism of resistance to sulfadoxine-pyrimethamine and assess the occurrence of point mutations associated with artemisinin resistance in <i>P. falciparum</i> clinical isolates from Eritrea.</p> <p><i>Methods</i>: Nineteen dried blood spot samples were collected from patients visiting Adi Quala, Keren and Gash Barka Hospitals, Eritrea. The patients were followed up after receiving treatment with first line artesunate-amodiaquine. Nested polymerase chain reaction and Sanger sequencing techniques were employed to genotype point mutations in the <i>P. falciparum</i> bifunctional dihydrofolate reductase-thymidylate synthase (<i>Pfdhfr</i>, PF3D7_0417200), dihydropteorate synthase (<i>Pfdhps</i>, PF3D7_0810800) and kelch 13 (<i>PfK13</i>, PF3D7_1343700) genes.</p> <p><i>Results</i>: Eight of nineteen (42%) of the dried blood spot samples were successful for PCR-amplification. Data analyses of the PCR-positive isolates revealed the following point mutations: <i>Pfdhfr </i>N51I in four isolates, C59R in one isolate, S108N in four isolates, a rare non-synonymous substitution V45A in four isolates and <i>Pfdhps</i> K540E in four isolates. No <i>PfK13</i> point mutations were reported.</p> <p><i>Conclusions</i>: <i>Pfdhfr</i> C59R and <i>Pfdhps</i> K540E point mutations are reliable markers for the sulfadoxine-pyrimethamine quintuple mutant haplotype combination. These findings highlight first reports in Eritrea, which verify the underlying genetic mechanism of antifolate resistance. Continuous monitoring of the <i>PfK13</i> marker is recommended.</p>

<p>A retrospective records-based study design was conducted in student dental clinics at Tygerberg Oral Health Centre, University of the Western Cape. Information was collected from patient files who received complete dentures in the period 2014-2019. Data collected included the patient's age, gender, medical conditions, denture experience, presence of oral candidiasis, site of lesion and treatment for the condition. Patients were further classified using the Uniform Patient Fee Schedule (UPFS). Patients treated in government hospitals are charged according to the UPFS, which sets a tariff according to the level of hospital providing the treatment, category of medical staff providing the treatment and the income of the patient.A retrospective records-based study design was conducted in student dental clinics at Tygerberg Oral Health Centre, University of the Western Cape. Comparing the first and second waves of COVID-19 in a tertiary university hospital in Barcelona
<p><strong>Objectives</strong></p> <p>To compare the flow of COVID-19 patients in emergency rooms and hospital wards between the pandemic; first and second waves; at the University Hospital of Vall d'Hebron (Barcelona, Spain), and to compare the profiles, severity and mortality of COVID-19 patients between the two waves.</p> <p><strong>Methods</strong></p> <p>Retrospective observational analysis of COVID-19 patients attending the hospital from February 24 to April 26, 2020 (first wave) and from July 24, 2020, to May 18, 2021 (second wave). We analysed the data of the Electronic Medical Records on patient demographics, comorbidity, severity and mortality.</p> <p><strong>Results</strong></p> <p>The daily number of COVID-19 patients entering the ER dropped by 65% during the second wave compared to the first wave. During the second wave, patients entering the ER were significantly younger (61 vs 63 y.o. p<0.001) and less severely affected (39% vs 48% with a triage level of resuscitation or emergency; p<0.001). ER mortality declined during the second wave (1% vs 2%; p<0.000). The daily number of hospitalised COVID-19 patients dropped by 75% during the second wave. Those hospitalised during the second wave were more severely affected (20% vs. 10%; p<0.001) and were derived to the intensive care unit (ICU) more frequently (21% vs 15%; p<0.001). Inpatient mortality showed no significant difference between the two waves.</p> <p><strong>Conclusions</strong></p> <p>Changes in the flow, severity and mortality of COVID-19 patients entering this tertiary hospital during the two waves may reflect a better adaptation of the health system and the improvement of knowledge on the disease.</p>
Prevalence of oral manifestations in patients with lupus erythematosus in a sample of Egyptian population, a hospital based cross-sectional study
<p>Background: Several systemic diseases manifest themselves in the oral cavity. Oral manifestations of lupus erythematosus (LE) are associated with a significantly increased risk of cancer. Dentists who are unaware of these lesions will possibly miss them. This cross-sectional study aimed to assess the prevalence of oral manifestations in patients with LE in a sample of the Egyptian population.</p> <p>Methods: A descriptive study was performed on 189 patients attending the Internal Medicine Department, Rheumatology clinic in EL Qasr EL Ainy Hospital, Cairo University. Every patient was examined clinically after completing a questionnaire. Moreover, patients' medical records were also evaluated. The oral manifestations were recorded according to the WHO guide to physical examination of the oral cavity and classified according to their morphologic aspects and localization.</p> <p class="MsoBodyText">Results [DSL1] : Out of 198 patients, 182 females (96.3%) and 7 males (3.7%).</p> <p class="MsoBodyText">The prevalence of oral lesions in LE patients was 55.6%. The most affected site was the tongue 25.7%. The most common clinical aspect was patches, 53%. About 77.1% of the lesions were asymptomatic. 74.3% of the patients had oral candidiasis. The prevalence of skin lesions in LE patients was 37.6%. The most common finding was malar rash 79%.</p> <p>Conclusions: The present study emphasizes the importance of early diagnosis of oral lesions to recognize patients with LE as the <span>WHO considers oral manifestations of LE a widespread state associated with an increased risk of cancer. Also, </span>implementation of oral hygiene measures and treatment to improve patients' nutritional state and health-related quality of life are recommended.</p>

Supplementary Documentation - Epilepsy risk among survivors of intensive care unit hospitalization for sepsis
Objective <p>Our objectives were to determine if survivors of intensive care unit (ICU) hospitalizations with sepsis experience higher epilepsy risk than survivors of ICU hospitalizations without sepsis, and to identify sepsis survivors at highest risk.</p> Methods <p>We used linked, administrative healthcare databases to conduct a population-based, retrospective matched cohort study of adult Ontario residents discharged from an ICU between January 1, 2010 and December 31, 2015, identified using the Discharge Abstract Database. We used propensity scores to match patients who experienced sepsis during their index ICU hospitalization with up to four patients who did not experience sepsis. We applied marginal Cox proportional hazards regression to estimate the risk of epilepsy within two years following the index ICU hospitalization. Among sepsis survivors, Cox proportional hazards regression was used to identify factors associated with epilepsy.</p> Results <p>143,892 patients were included, 32,252 (22.4%) of whom were exposed. Sepsis survivors were at significantly higher epilepsy risk (HR=1.44, 95% CI=1.15-1.80). The risk of epilepsy marginally decreased with increasing age (HR=0.97, 95% CI=0.96-0.99), while those with chronic kidney disease (HR=2.25, 95% CI=1.48-3.43) were at highest risk.</p> Conclusions <p>In this real-world analysis, sepsis survivors, particularly those who are younger and have chronic kidney disease, are at significantly higher epilepsy risk. These findings indicate that sepsis may be an unrecognized epilepsy risk factor.</p>

<p>The dataset contains information about the attitudes of 1000 Italian citizens toward the use of customer-oriented internet of things technologies in hotel settings.
