Sensitivity analysis of a mathematical model simulating the post-hepatectomy hemodynamics response
<p>Recently a lumped-parameter model of the cardiovascular system was proposed to simulate the hemodynamics response to partial hepatectomy and evaluate the risk of portal hypertension (PHT) due to this surgery. Model parameters are tuned based on each patient data. This work focuses on a global sensitivity analysis (SA) study of such model to better understand the main drivers of the clinical outputs of interest. The analysis suggests which parameters should be considered patient-specific and which can be assumed constant without losing in accuracy in the predictions. While performing the SA, model outputs need to be constrained to physiological ranges. An innovative approach exploits the features of the polynomial chaos expansion method to reduce the overall computational cost. The computed results give new insights on how to improve the calibration of some model parameters. Moreover the final parameter distributions enable the creation of a virtual population available for future works. Although this work is focused on partial hepatectomy, the pipeline can be applied to other cardiovascular hemodynamics models to gain insights for patient-specific parameterization and to define a physiologically relevant virtual population.</p>

Raw fMRI data from 12 rats used in the manuscript "Mapping of hemodynamic responses to the sensorymotor stimulation in a rodent model: a BOLD fMRI study" submitted to PlosOne for publication
<p>Raw data are from twelve male adult Wistar rats (Charles River Laboratories, Paris-France) weighing 300 ± 20g.</p> <p>Rats were initially anesthetized (induction) with 3% isoflurane and were maintained under 0,7-0.8% (sedation along with a muscle relaxation) during fMRI session. </p> <p>Each rat was submitted to two fMRI sessions: one with TE of 30 ms and and other with TE of 40 ms.</p> <p>For fMRI acquisition, electrodes were inserted subcutaneously in the palmar surface of the right hindpaw of each rat and electrical stimulation (current pulses with a 1.7 mA amplitude, 10 ms duration and 8 Hz frequency) was applied in a block-design starting with a resting period of 25s as a baseline followed by 25s stimulation, repeated 8 times.</p> <p>Ten 1-mm thick contiguous axial slices, from -6.36 mm to +2.64 mm to Bregma, were acquired with a two-shot gradient echo planar imaging (GE EPI) pulse sequence (2.56 cm2 FOV; 64x64 matrix size; a TR of 1000 ms; a flip angle of 50°) resulting in the pixel size of 0.4 mm.</p> <p>All imaging experiments were performed on a 4.7T Bruker (Biospec 47/40, Bruker, GmbH, Ettlingen,Germany) with a horizontal bore magnet equipped with a 12 cm gradient coil (Bruker BGA12, 400 mT/m) and interfaced to AVANCE III console. Two actively decoupled RF coils were used: a 7.2-cm diameter volume coil for transmission and a 2-cm diameter surface coil (Rapid Biomedical, Rimpar, Germany) positioned on the top of the animal's head for reception.</p>
Comparative Study of Intravenous Esmolol & Magnesium Sulphate in Attenuating Hemodynamic Response during Laryngoscopy & Endotracheal Intubation in Patients Undergoing Valvular Heart Surgery: a Randomised Clinical Trial
<p>The present study aimed to compare the effectiveness of esmolol & magnesium sulphate in attenuating the hemodynamic response<br> to endotracheal intubation and to note any signifi cant side effects caused by these drugs.<br> Background: Induction with endotracheal intubation in patients undergoing valvular heart replacement pose lot of hemodynamic<br> variations. Obtunding hemodynamic response which can be deleterious in such patients pose a challenge to the cardiac anaesthetist. We<br> hypothesized that using esmolol as compared to magnesium sulphate will attenuate the hemodynamic response during laryngoscopy &<br> endotracheal intubation in patients undergoing valvular heart replacement.<br> Methods: This was a double blind, randomised, single centre, interventional, prospective study. In this study 96 patients were divided<br> into two groups with 48 patients each (n = 48) by sealed enveloped method of randomisation. Group E received esmolol 1.5 mg/ kg i.v<br> and Group M received magnesium sulphate 50 mg/ kg i.v each diluted in normal saline to make up a volume of 50 ml & given via infusion<br> slowly over 5 minutes by a burette set. Hemodynamic parameters like Heart Rate (HR), Mean Arterial Pressure (MAP), Systolic Blood<br> Pressure (SBP), Diastolic Blood Pressure (DBP) at baseline, 5 minutes after premedication, just before intubation 3, 5, 10 & 15 minutes<br> post intubation were recorded. The last observation at the end of 15 minutes post intubation was considered as the end of study.<br> Results: All the enrolled patients were analyzed. The esmolol group showed a decrease in the H.R from baseline (86.13 ± 15.87)<br> as compared to magnesium sulphate (98.51 ± 16.81 with a 95% CI, 4.62-4.69, p value < 0.001) 5 minute after premedication. There was<br> statistically signifi cant difference in H.R between both groups 5 minutes after drug administration.<br> Conclusion: Administration of esmolol before intubation in valvular heart patients undergoing valve replacement surgery controls the<br> hemodynamic response much better as compared to magnesium sulphate.</p>
