Illness perception mediates the relationship between the severity of symptoms and perceived health status in patients with Behcet's disease.
<p><strong>Abstract</strong></p> <p><em>Objective</em>. To investigate the relationship between psychological representations of illness, perceived health status and self-assessment of symptom severity in patients with <strong>Behçet's</strong>Disease – a rare long-term incurable condition with unclear etiology.</p> <p><em>Methods</em>. Using cross-sectional survey design, data on self-administered questionnaires on illness perception, health status, symptoms severity and demographic characteristics were collected from 273 patients with <strong>Behçet's</strong>Disease (age range 18 – 65+). The data were subjected to mediation analysis to test whether cognitive and emotional components of illness perception mediate the relationship between the severity of symptoms and heath status.</p> <p><em>Results</em>. The results support our hypotheses that cognitive components of illness perception (perceived consequences and identity of the illness) mediate the link between symptom activity and pain, whereas emotional components of the illness (emotional representations about the illness) mediate the relationship between disease activity and perceived energy level.</p> <p> <em>Conclusion</em>. The robustness of these mediation effects suggests potential directions for clinical psychologists and health care practitioners in developing support programmes. We supplement our study with Open Access database containing information about type of medication, comorbid mood disorder, detailed measurement of the severity of BD symptomsfor sharing and accumulating multidisciplinary knowledge aiming to support the development of interventions. Addressing psychological aspects ofBD will help to manage complex patients effectively.</p> <p>Database of survey (complete)</p>

Data from: An exploratory study of staff perceptions of shift safety in the critical care unit and routinely available data on workforce, patient and organisational factors
<p><b>Objectives </b></p> <p>To explore: bedside professional reported (BPR) perceptions of safety in intensive care staff; and the relationships between BPR safety, staffing, patient and work environment characteristics.</p> <p><b>Design </b>An exploratory study of self-recorded staff perceptions of shift safety and routinely collected data.</p> <p><b>Setting </b>A large teaching hospital comprising 70 critical care beds.</p> <p><b>Participants </b>All clinical staff working in adult critical care.</p> <p><b>Interventions </b>Staff recorded whether their shift felt "safe, unsafe or very unsafe" for 29 consecutive days. We explored these perceptions and relationships between these and routine data on staffing, patient and environmental characteristics.</p> <p><b>Outcome measures</b> Relationships between BPR safety and staffing, patient and work environment characteristics.</p> <p><b>Results</b> 2836 BPR scores were recorded over 29 consecutive days (response rate 57.7%). Perceptions of safety varied between staff, including within the same shift. There was no correlation between perceptions of safety and two measures of staffing; care hours per patient day (r= 0.13 p=0.108) and Safecare Allocate (r= -0.19 p=0.013). We found a significant, positive relationship between perceptions of safety and the percentage of Level 3 (most severely ill) patients (r=0.32, p=0.0001). There was a significant inverse relationship between perceptions of safety and the percentage of Level 1 patients on a shift (r= -0.42, p=<0.0001). Perceptions of safety correlated negatively with increased numbers of patients (r= -0.44, p=0.0006) and higher percentage of patients located side rooms (r=0.63, p<0.0001). We found a significant relationship between perceptions of safety and the percentage of staff with a specialist critical care course (r=0.42. p=0.0001).</p> <p><b>Conclusion </b>Existing staffing models, which are primarily influenced by staff:patient ratios may not be sensitive to patient need. Other factors may be important drivers of staff perceptions of safety and should be explored further.</p> <p><b>Trial registration</b>- UK Health Research Authority approval was obtained (ID249248).</p>
Data from: Understanding the role of physician attire on patient perceptions: a systematic review of the literature— targeting attire to improve likelihood of rapport (TAILOR) investigators
Objectives: Despite a growing body of literature, uncertainty regarding the influence of physician dress on patients' perceptions exists. Therefore, we performed a systematic review to examine the influence of physician attire on patient perceptions including trust, satisfaction and confidence. Setting, participants, interventions and outcomes: We searched MEDLINE, Embase, Biosis Previews and Conference Papers Index. Studies that: (1) involved participants ≥18 years of age; (2) evaluated physician attire; and (3) reported patient perceptions related to attire were included. Two authors determined study eligibility. Studies were categorised by country of origin, clinical discipline (eg, internal medicine, surgery), context (inpatient vs outpatient) and occurrence of a clinical encounter when soliciting opinions regarding attire. Studies were assessed using the Downs and Black Scale risk of bias scale. Owing to clinical and methodological heterogeneity, meta-analyses were not attempted. Results: Of 1040 citations, 30 studies involving 11 533 patients met eligibility criteria. Included studies featured patients from 14 countries. General medicine, procedural (eg, general surgery and obstetrics), clinic, emergency departments and hospital settings were represented. Preferences or positive influence of physician attire on patient perceptions were reported in 21 of the 30 studies (70%). Formal attire and white coats with other attire not specified was preferred in 18 of 30 studies (60%). Preference for formal attire and white coats was more prevalent among older patients and studies conducted in Europe and Asia. Four of seven studies involving procedural specialties reported either no preference for attire or a preference for scrubs; four of five studies in intensive care and emergency settings also found no attire preference. Only 3 of 12 studies that surveyed patients after a clinical encounter concluded that attire influenced patient perceptions. Conclusions: Although patients often prefer formal physician attire, perceptions of attire are influenced by age, locale, setting and context of care. Policy-based interventions that target such factors appear necessary.
