HLA epitopic mismatch and risk of developing Donor Specific Antibodies in pediatric kidney transplantation
<p>The aim of this study was to evaluate the correlation between epitopic HLA mismatches and the risk of developing DSA in pediatric kidney recipients. There are very few publications concerning epitope HLA mismatch in pediatric kidney transplantation although this topic has been growing in scientific articles in recent years and is promising in adult recipients. The majority of children with chronic renal failure will require multiple kidney transplants during their lifetime and it is therefore important that we can limit the risk of antibody development in order to give them the best chance of survival for their future kidney transplants.</p>

Educational Material For: Development of video-based educational materials for kidney-transplant patients
<p><span><span><b>Introduction</b></span></span></p> <p><span><span><span>Treatment adherence has been evaluated as a major predictor of long-term outcome, and education has been suggested to improve adherence. Considering the characteristics of adult learners, it is necessary to implement educational programs that meet the needs of transplant patients. Multimedia education may be well-suited for this. This study aims to develop video education materials in accordance with transplant patients' self-care needs.</span></span></span></p> <p><span><span><span><span><b>Methods</b></span></span></span></span></p> <p><span><span><span><span>This study includes a literature review and patient interviews aimed at developing video education materials for the self-care needs of patients who underwent renal transplant surgery at a university hospital in Seoul. Ten patients were interviewed about the desired educational content, accessibility, and other preferences. After verifying the validity of the data, the video scenarios were produced and satisfaction surveys were conducted.</span></span></span></span></p> <p><span><span><span><span><b>Results </b></span></span></span></span></p> <p><span><span><span><span>Eleven self-care education items were identified through interviews with 10 kidney transplant patients. The expert validation of video-based educational content result was high (mean CVI = 0.94). The mean score of the patients' satisfaction evaluation of the completed 7-minute video instructional materials was also high (4.55 on a 5-point Likert scale).</span></span></span></span></p> <p><span><span><span><span><b>Conclusion</b></span></span></span></span></p> <p><span><span><span><span>Findings indicate that the video education materials will meet the needs of adult learners and mitigate the limitations of the existing education programs by increasing interest and motivation and may contribute to increased treatment adherence and ultimately, positively effect self-care for new transplant patients.</span></span></span></span></p>
Data from: Provision of care for chronic kidney disease by non-nephrologists in a developing nation: a national survey
Objectives The prevalence of chronic kidney disease (CKD) in developing countries has increased dramatically. This study aimed to explore the practice patterns of non-dialysis-dependent CKD care in an affluent developing country. Settings Primary and specialised healthcare facilities of public and private sectors in the United Arab Emirates. Participants 159 non-nephrologist physicians practising in the United Arab Emirates. Interventions A 28-item online self-administered questionnaire based on CKD clinical practice guidelines. Primary and secondary outcome measures The physicians' approach to identifying and managing patients with CKD. Results The survey was completed by 159 non-nephrologists, of whom 135 reported having treated patients with CKD. Almost all the respondents screen patients with hypertension and diabetes for CKD, but one-third of them do not screen patients with cardiovascular disease and elderly patients for CKD. The use of accurate CKD screening tests (estimated glomerular filtration rate and albumin/creatinine ratio) was suboptimal (77% and 59% of physicians used the procedures, respectively). One-third of the physicians do not offer treatment with inhibitors of the renin–angiotensin system to patients with CKD, and only 66% offer antilipid treatment. In general, the primary healthcare physicians are more familiar than secondary healthcare physicians with the diagnosis and management of patients with CKD. Conclusions We identified substantial physician-declared deficiencies in the practice of identifying and managing early CKD. Integration of quality CKD care within the healthcare system is required to face the increasing burden of CKD in the United Arab Emirates and possibly in other developing nations.
