Cognitive development among children in a low-income setting: cost-effectiveness analysis of a maternal nutrition education intervention in rural Uganda

<p><span>Inadequate nutrition and insufficient stimulation in early childhood can lead to long-term deficits in cognitive and social development. Evidence for policy and decision-making regarding the cost of delivering nutrition education is lacking in low and middle-income countries (LMIC). In rural Uganda, we conducted a cluster-randomized controlled trial (RCT) examining the effect of a maternal nutrition education intervention on developmental outcomes among children aged 6–8 months. This intervention led to significantly improved cognitive scores when the children reached the age of 20–24 months. When considering the potential for this intervention's future implementation, the desired effects should be weighed against the increased costs.</span><span> This study therefore aimed to assess the cost-effectiveness of this education intervention compared with current practice.</span><span> Health outcome data were based on the RCT. Cost data were collected via interviews with researchers involved in processing the intervention. This study considered a healthcare provider perspective for an 18-month time horizon.</span><span> The control group was considered as the current practice for the future large-scale implementation of this intervention. </span><span>A cost-effectiveness analysis was performed, including calculations of incremental cost-effectiveness ratios (ICERs). In addition, uncertainty in the results was characterized using one-way and probabilistic sensitivity analyses. The ICER for the education intervention compared with current practice was USD ($) 16.50 per cognitive composite score gained, with an incremental cost of </span><span>$265.79 and an incremental cognitive composite score of 16.11. The sensitivity analyses indicated the robustness of these results. The ICER was sensitive to changes in cognitive composite score and the cost of personnel, but the education intervention can be considered cost-effective compared with the current practice. The outcome of this study, including the cost analysis, health outcome, cost-effectiveness, and sensitivity analysis, can be useful to inform policymakers and stakeholders about effective resource allocation processes in Uganda and possibly other LMIC.</span></p>

Differential DNA methylation may provide a mechanistic understanding of these long-term associations. We aimed to quantify the associations of MEA with offspring DNA methylation levels at birth, in childhood and in adolescence. Using 37 studies from high-income countries, we performed meta-analysis of epigenome-wide association studies (EWAS) to quantify the associations of completed years of MEA at the time of pregnancy with offspring DNA methylation levels at birth (n&thinsp;=&thinsp;9 881), in childhood (n&thinsp;=&thinsp;2 017), and adolescence (n&thinsp;=&thinsp;2 740), adjusting for relevant covariates. MEA was found to be associated with DNA methylation at 473 cytosine-phosphate-guanine sites at birth, one in childhood, and four in adolescence. We observed enrichment for findings from previous EWAS on maternal folate, vitamin-B12 concentrations, maternal smoking, and pre-pregnancy BMI. The associations were directionally consistent with MEA being inversely associated with behaviours including smoking and BMI. Our findings form a bridge between socio-economic factors and biology and highlight potential pathways underlying effects of maternal education. The results broaden our understanding of bio-social associations linked to differential DNA methylation in multiple early stages of life. The data generated also offers an important resource to help a more precise understanding of the social determinants of health.</p>

A pragmatic randomized maternal education trial in rural Uganda - Eight years of sustained improvement in child development

<p><strong><span>Background</span> </strong></p> <p><span>Nutrition and stimulation interventions may promote early childhood development, but little is known about the long-term benefits of such interventions, particularly in low- and middle-income countries. We now conducted a follow-up study on a cluster-randomized maternal education trial that was conducted when their children were 6<span>–</span>8 months old, to assess the sustainability of the developmental benefits eight years after the intervention.</span></p> <p><strong><span>Methods and findings</span></strong></p> <p><span>The intervention consisted of education in nutrition, hygiene, oral sanitation and child stimulation. In the current study, we assessed child processing and cognitive abilities using the Kaufman Assessment Battery for Children Second Edition (KABC-II) and attention and inhibitory control using the </span><span>Test of Variables of Attention (TOVA)</span><span>. Using Stata version 17.0, t-tests and multi-level regression were employed to compare scores from KABC-II and </span><span>TOVA tests</span><span> in the two study groups adjusting for clustering effect. The data were analyzed by intention-to-treat. The original trial included 511 mother-child pairs (intervention n=263, control n=248), whereas in the current study, 361 (71%; intervention n=185, control n=176) pairs were available for analyses. T</span><span>he intervention group scored higher than the controls (all P-values&lt;0.001) on all five KABC-II sub-scales, as well as on the KABC-II global score</span><span> (mean difference: </span><span>14, 95% CI: 12–16; P&lt;0.001). </span><span>Furthermore, </span><span>for all five TOVA variables, the intervention group scored higher than the controls in both the visual and auditory tasks (all P-values &lt;0.05).</span> <span> The main limitation is that we were unable to determine the exact individual contribution of each component (nutrition, hygiene and stimulation) of the intervention to the developmental benefits since they were all delivered as a combined package.</span></p> <p><strong><span>Conclusions </span></strong></p> <p><span>The intervention group consistently scored markedly higher on both psychometric tests. Thus, even eight years after the original maternal education intervention, the developmental benefits that we observed at child age of one, two and three years, were sustained.</span></p>
