Low-dose biliatresone treatment of pregnant mice causes subclinical biliary disease in their offspring: Evidence for a spectrum of neonatal injury

<p>Biliary atresia is a neonatal disease characterized by damage, inflammation, and fibrosis of the liver and bile ducts and by abnormal bile metabolism. It likely results from a prenatal environmental exposure that spares the mother and affects the fetus. Our aim was to develop a model of fetal injury by exposing pregnant mice to low-dose biliatresone, a plant toxin implicated in biliary atresia in livestock, and then to determine whether there was a hepatobiliary phenotype in their pups.<strong> </strong>Pregnant mice were treated orally with 15 mg/kg/d biliatresone for 2 days. Histology of the liver and bile ducts, serum bile acids, and liver immune cells of pups from treated mothers were analyzed at P5 and P21. Pups had no evidence of histological liver or bile duct injury or fibrosis at either timepoint. In addition, growth was normal. However, serum levels of glycocholic acid were elevated at  P5, suggesting altered bile metabolism, and the serum bile acid profile became increasingly abnormal through P21, with enhanced glycine conjugation of bile acids. There was also immune cell activation observed in the liver at P21. These results suggest that prenatal exposure to low doses of an environmental toxin can cause subclinical disease including liver inflammation and aberrant bile metabolism even in the absence of histological changes. This finding suggests a wide potential spectrum of disease after fetal biliary injury.</p>

Assessment of Self-Medication Practices and Safety Profile of Medicines Utilisation among Pregnant Women attending Antenatal Clinics in Freetown, Sierra Leone: A Multicentre Cross-Sectional Descriptive Study

<p><strong><span>Background: </span></strong><span>Despite the potential fetal and maternal risks of self-medication, studies on self-medication practice and the safety profile of medicines used during pregnancy are scarce in our setting. This study determined the self-medication practice and safety profile of medicines used among pregnant women.</span></p> <p><strong><span>Methods: </span></strong><span>This cross-sectional study was conducted in face-to-face interviews among 345 pregnant women at three hospitals in Sierra Leone. Data were analyzed using descriptive statistics and binary logistic regression to <span>determine the prevalence and associated factors of self-medication. </span></span></p> <p><strong><span>Results: </span></strong><span>A total of 345 pregnant women participated in the study. The prevalence of self-medication among pregnant women with conventional and/or herbal medicine was </span><span>132 (38.3%)</span><span>. Also, 93 (75%) of the conventional medicines (CMs) were categorized as probably safe, of which paracetamol 36 (29.0%) was commonly used, followed by amoxicillin 23 (18.5%) and antimalarials 22 (17.7%) for common illnesses such as </span><span>headache 30 (25.4%), </span><span>urinary tract infection </span><span>23 (19.4%)<span> and malaria 22 (18.6%). The common reason for self-medication was previous experience with the disease 24 (27.3%). </span><em>Luffa acutangula</em> 19 (30.2%) <span>was the most used herbal medicine (HM), </span>and Oedema 30 (47.6%) was the most reported ailment. <span>Among the HM users,</span> 34 (54.0%)<span> believe they are more effective than CMs. Secondary school education (AOR = 2.128, 95%CI = 1.191 &ndash; 3.804, p = 0.011), tertiary education (AOR = 2.915, 95%CI = 1.104 &ndash; 7.693, p = 0.031), monthly income of greater than NLe 1,000 (AOR = 4.084, 95% CI = 1.269 &ndash; 13.144, p = 0.018), and perceived maternal illness (AOR = 0.367, CI = 0.213 &ndash; 0.632, p = &lt;0.001) were predictors of self-medication.</span></span></p> <p><strong><span>Conclusion: </span></strong><span>Self-medication practice was highly prevalent and was associated with educational status, monthly income, and maternal illness during pregnancy. Therefore, intervention programs should be designed and implemented to minimize the practice and risk associated with self-medication among pregnant women.</span></p>

Raw data fort the article: FLUOROSCOPY-GUIDED BILIARY PROCEDURES IN A PREGNANT, LIVER TRANSPLANT PATIENT: FETUS RADIATION PROTECTION

<p>We report three cases of clinically necessary, fluoroscopy-guided, percutaneous biliary procedures performed safely in a pregnant, liver transplant recipient using three different angiography suites. The uterine cumulative equivalent dose was 0.25 mSv, a value obtained by adding the doses of the three procedures described above, and which is relatively low when compared with the naturally occurring background radiation exposure for a 9-month pregnancy (~0.5-1 mSv). Our experience shows that staff knowledge, awareness and liaison promote the application of all dose reduction strategies possible while still achieving the clinical aim despite using different angiographic equipment.</p>

In vivo noninvasive systemic myography of acute systemic vasoactivity in female pregnant mice

<p>Altered vasoactivity is a major characteristic of cardiovascular and oncological diseases, and many therapies are therefore targeted to the vasculature. Therapeutics which are selective for the diseased vasculature are ideal, but whole-body selectivity of a therapeutic is challenging to assess in practice. Vessel myography is used to determine the functional mechanisms and evaluate pharmacological responses of vascularly-targeted therapeutics. However, myography can only be performed on ex vivo sections of individual arteries. We have developed methods for implementation of spherical-view photoacoustic tomography for non-invasive and in vivo myography. Using photoacoustic tomography, we demonstrate the measurement of acute vascular reactivity in the systemic vasculature and the placenta of pregnant mice in response to two vasodilators. Photoacoustic tomography simultaneously captured the significant acute vasodilation of major arteries and detected the selective vasoactivity of the maternal-fetal vasculature. Photoacoustic tomography has the potential to provide invaluable preclinical information on vascular response that cannot be obtained by other established methods.</p>
