Facility Information

Entry 1

No pain, distress, or use of pain-relieving drugs (Column C)

Required fields
  • Institution name is required.
  • USDA registration number is required.
  • Reporting period start must be YYYY-MM-DD format.
  • Reporting period end must be YYYY-MM-DD format.
  • Entry 1: protocol number is required.
  • Entry 1: protocol title is required.
  • Entry 1: PI name is required.
  • Entry 1: animal count must be at least 1.

When to use

  • USDA annual report (Form 7023) preparation
  • IACUC annual program review summaries
  • Grant-level animal usage attribution reports
  • Institutional compliance documentation

Do not use for

  • Prospective animal number planning (use IACUC Budget Estimator)
  • Real-time census tracking (use Census Forecaster)
  • Protocol amendment tracking (use Protocol Expiration Tracker)

Pearl

Enter each species ×\times pain category ×\times outcome as a separate row. A single protocol with 20 mice in Category C and 10 in Category D needs two entries.

Pearl

Include grant numbers for every entry — the per-grant attribution table helps PIs track usage against approved IACUC numbers for each funding source.

Pitfall

Do not double-count animals that were held (Column B) and later used (Column C/D/E). Report each animal in the HIGHEST category experienced during the period.

Pitfall

Animals that die during a procedure should be counted in the appropriate pain category (C/D/E), not separately. The outcome field tracks disposition, not pain category.

1

Method

Protocol entries are aggregated three ways: by species (with USDA pain category columns B–E and covered/non-covered classification), by protocol (with PI, species, and outcome tracking), and by grant (with protocol and species attribution). USDA-covered species are identified by matching against the Animal Welfare Act's covered species list.

Animal Welfare Act (7 U.S.C. § 2131–2159) and 9 CFR Parts 1–4.

USDA APHIS Form 7023: Annual Report of Research Facility.

Understanding USDA Reporting Requirements

The Animal Welfare Act (AWA) requires registered research facilities to file an annual report (Form 7023) with USDA APHIS. Key requirements:

Reporting period: Fiscal year (October 1 – September 30) for most institutions, though calendar year reporting is allowed with USDA approval.
Pain categories: Animals must be classified into columns B–E based on the highest category of pain/distress experienced during the reporting period. An animal that starts in Column B and is later used in a Column D procedure should be counted in Column D only — no double-counting.
Covered species: The AWA covers warm-blooded animals except rats (Rattus) and mice (Mus) bred for research, birds bred for research, horses not used for research, and other farm animals used for food/fiber. Wild rodents and wild-caught animals ARE covered even if they are rats or mice.
IACUC oversight: Beyond USDA reporting, most institutions require annual animal use summaries for IACUC program review. These typically include all vertebrate species (including mice and rats), broken down by protocol, PI, grant, and outcome.
