Early parenting intervention promotes 24-month psychomotor development in preterm children
<p><strong>Introduction</strong>. In the present study, we report on the effects of an early NICU psychoeducational parenting intervention including both mothers and fathers on the psychomotor outcome of preterm infants at 24 months of CA.</p> <p><strong>Methods</strong>. Between January 2015 and December 2016, 42 preterm children from the NICU of the Fondazione IRCCS Policlinico San Matteo, Pavia, Italy, and their parents were enrolled in this study. All preterm infants with gestational age ≤32 weeks or birth weight ≤1500 grams were eligible for the study. Subjects were not considered eligible to the study if at least one parent was unable to converse in Italian, had psychiatric morbidities or reported alcohol/drug abuse, was less than 18 years old, and if the infants had any genetic syndrome. Twenty-one children and their parents were allocated to the early intervention arm, whereas twenty-one were allocated to the care as usual arm. The following socio-demographic variables were collected: infants’ sex, gestational age, birthweight, Apgar score at the first and fifth minute, parental age and educational level measured as years of study. The clinical characterization of the children included the following variables: vaginal or Caesarean delivery, being small for gestational age, intrauterine growth restriction, bronchopulmonary dysplasia, retinopathy of prematurity, periventricular leukomalacia, ultrasound scan results and clinical outcomes of neurological examinations at discharge, term-equivalent age and 3 months of corrected age. Parents were asked to complete the parental closeness diary starting from the day of enrollment up to discharge, to control for differences related to the amount of skin-to-skin and holding between the two groups. Mothers and fathers completed the diary separately. The parental closeness diary allows the collection of quantitative data on the presence of parents, time spent holding the infant and time spent in skin-to-skin contact. A clinician blind to the random allocation administered Griffiths Mental Developmental Scale (GMDS) at 24 months of corrected age. A standardized developmental quotient (DQ) score (range: 50-150) is obtained for each domain (mean= 100, standard deviation= 16) and for the global score (mean= 100, standard deviation =12). Both groups were compared for socio-demographic and clinical variables as well as for holding and skin-to-skin percentage scores by means of pair-wise mean comparisons, non-parametric Mann-Whitney test and chi-square test. General linear models were used to assess the presence of significant group differences in the total and domain DQ scores of the GMDS.</p> <p><strong>Results</strong>. A significant difference emerged for the total GMDS score (t(40)=2.40,P= .02) Children from parents in the early intervention arm had higher global DQ compared to those in the care as usual arm. Moreover, a significant multivariate effect was detected (F(5,36)=4.49, P=.003, η2p =0.38). Significant univariate differences emerged for two domains: personal-social (F(1,40) =14.93, P<.001,η2p=0.27) and hearing-speech (F (1,40)=5.36, P=.026,η2p=0.12). For both the domains, children of parents in the early intervention group had higher DQ scores compared to those in the care as usual group.</p>
Neurodevelopmental outcome of preterm very low birth weight infants admitted to an Italian tertiary center over an 11-year period
<p><strong>This database includes</strong> the raw data linked with the paper “<em>Neurodevelopmental outcome of preterm very low birth weight infants admitted to an Italian tertiary center over an 11-year period</em>”.</p> <p><strong>In this paper, we reported</strong> the data of perinatal history and neurodevelopmental outcome of a single center cohort of Italian preterm VLBWi born between 2005 and 2015.</p> <p>T<strong>he aim of the study </strong>is to describe neurodevelopmental outcome at 24 months of corrected age and to identify associations between perinatal risk factors and neurodevelopmental outcome.</p> <p><strong>502 VLBWi completed the follow up</strong>. 75.3% showed a normal outcome, 13.9% minor sequelae and 10.8% major sequelae (3.8% cerebral palsy). Male gender (OR 1.59 CI 95% 1.0-2.5 p value 0.043), bronchopulmonary dysplasia (OR 1.7 CI 95% 1.1-2.9 p value 0.026), abnormal neonatal neurological assessment (OR 2.1 CI 95% 1.3-3.4 p value 0.002) and severe brain ultrasound abnormalities (OR 6.1 IC 95% 2.4-15.1 p value <0.001) were independently associated with poor outcome on multivariate ordered logistic regression.</p>

The dual nature of hypothalamic-pituitary-adrenal axis regulation in dyads of very preterm infants and their mothers
<p>Raw data of the study "The dual nature of hypothalamic-pituitary-adrenal axis regulation in dyads of very preterm infants and their mothers" In this study, 3-month-old (corrected age) VPT infants and FT counterparts with their mothers took part in a well-validated stress-inducing laboratory task (i.e., double Face-to-Face Still-Face, FFSF paradigm). Salivary cortisol samples were obtained before (Baseline) and after (Early reactivity, Late reactivity and Recovery) the FFSF procedure. Dyadic HPA axis co-regulation was assessed at each sample time-point (i.e., <em>in-moment</em> coupling) as well as across samples (i.e., <em>in-time synchrony</em>). Significant <em>in-moment</em> coupling emerged at Baseline, Late reactivity and Recovery for FT infants’ dyads only. An overlying pattern of salivary cortisol trajectories emerged between mothers and infants in the VPT group, whereas a more complex pattern of reciprocal and complementary co-regulation was found for FT infants’ dyads. Although both groups gave evidence of HPA axis co-regulation, dyads of VPT infants appear to be less able to adapt reciprocally and dynamically to stressful conditions</p>
Telomere length and salivary cortisol stress reactivity in very preterm infants
<p>Raw data of the study "Telomere length and salivary cortisol stress reactivity in very preterm infants" In this study, we prospectively examined the relationship between NICU pain-related stress and HPA axis reactivity to an age-appropriate socio-emotional condition (i.e., the Still-Face Procedure, SFP) in healthy VPT infants at 3-month corrected age. NICU pain-related stress was computed as the ratio between the number of skin-breaking procedures and length of NICU stay. A differential score (i.e., ∆TL) was obtained subtracting TL at birth from TL at discharge. A normalized (log10) cortisol reactivity index (CRI) was obtained by averaging post-stress (20 min after SFP) salivary cortisol sample on baseline value. A regression model controlling for neonatal and socio-demographic confounders showed that ∆TL was the only significant predictor of CRI. Although preliminary, these findings contribute to our knowledge of the mechanisms linking early exposures to adversity and later in life regulation of the HPA axis in VPT infants.</p>
