Data from: A systematic review of comorbidities and outcomes of adult patients with pleural infection
Background Pleural infection remains an important cause of mortality. This study aimed to investigate worldwide patterns of pre-existing comorbidities and clinical outcomes of patients with pleural infection. Methods Studies reporting on adults with pleural infection between 2000-2017 were identified from a search of Embase and Medline. Papers reporting exclusively on tuberculous, fungal or post pneumonectomy infection were excluded. Two reviewers assessed 20980 records for eligibility. Results 211 studies met the inclusion criteria. 134 papers (227,898 patients, mean age 52.8 years) reported comorbidity and/or outcome data. The majority of studies were retrospective observational cohorts (n=104, 78%) and the most common region of reporting was East Asia (n=33, 24%) followed by North America (n=27, 20%). 85 papers (50,756 patients) reported comorbidity. The median percentage prevalence of any comorbidity was 72% (IQR 58-83%), with respiratory illness (20%, 16-32%) and cardiac illness (19%, 15-27%) most commonly reported. 125 papers (192,298 patients) reported outcome data. The median length of stay was 19 days (IQR 13-27) and median in-hospital or 30-day mortality was 4% (IQR 1-11%). In regions with high-income economies (n=100, 74%) patients were older (mean 56.5 vs. 42.5 years, p<0.0001), but there were no significant differences in prevalence of pre-existing comorbidity nor in length of hospital stay or mortality. Conclusion Patients with pleural infection have high levels of comorbidity and long hospital stays. Most reported data are from high-income economy settings. Data from lower-income regions is needed to better understand regional trends and enable optimal resource provision going forward.
The benefit of kidney transplantation versus remaining on the waitlist depends on patients' baseline comorbidities: a retrospective study
<p>Background:<b> </b>Patients with end-stage renal disease present comorbidities that increase mortality risk on the waitlist and after kidney transplantation. This study aimed to determine the impact of baseline comorbidities on the risk of death after kidney transplantation compared to remaining on the waitlist.</p> <p>Methods: Records of waitlisted patients were retrospectively analyzed. Multivariable time-dependent Cox-regression model was used to determine the initial and the lowest relative hazard-risk of death (HRD) after exposure to kidney transplantation.</p> <p>Results: 1692 patients were included in the study. Patients without comorbidities had the relative HRD 3-fold higher, immediately after kidney transplantation, but it reduced progressively, reaching equality to waitlist at 75 days and a HRD of 0.28 thereafter (p<0.001). Patients with Diabetes and coronary artery disease had an initial relative HRD of 2.41 (p=0.009) and 2.68 (p=0.009) and matched to the waitlist at 175 and 200 days, respectively. Patients with peripheral vascular disease, had an initial relative HRD 3.6-fold in the first 240 days (p=0.008) and patients with congestive heart failure a relative HRD of 7.47 (p=0.07). Both declined rapidly, reaching equality at 110 days. Finally, all comorbidities, except cerebrovascular disease, presented lower HRD on long-term follow-up.</p> <p>Conclusions: This study shows that the time and the range of relative HRD between kidney transplantation and waitlist varies depending upon the patients´ baseline comorbidities.</p>

Supplemental material: Time trends in incidence, comorbidity, and mortality of ischemic stroke in Denmark, 1996-2016
<p><i>Objective:</i> To examine whether the incidence, comorbidity, and mortality of first-time ischemic stroke changed in Denmark between 1996-2016 overall and according to age and sex using a nationwide cohort design.</p> <p><i>Methods: </i>In this cohort study, 224,617 individuals <u>></u>18 years admitted with first-time ischemic stroke between 1996-2016 were identified using Danish nationwide registries. We calculated annual age-standardized incidence rates and absolute 30-day and 1-year mortality risks. Further, we calculated annual incidence rate ratios using Poisson regression, odds ratios for 30-day mortality using logistic regression, and hazard ratios for 1-year mortality using Cox regression.</p> <p><i>Results</i><b>:</b> The overall age-standardized incidence rates of ischemic stroke per 1,000 person-years increased from 1996 (2.70 [95%CI,2.65-2.76]) to 2002 (3.25 [95%CI,3.20-3.31]) and then gradually decreased to below the initial level until 2016 (1.99 [95%CI,1.95-2.02]). Men had higher incidence rates than women in all age groups except 18-34 and <u>></u>85 years. Absolute mortality risk decreased between 1996-2016 (30-day mortality from 17.1% to 7.6% and 1-year mortality from 30.9% to 17.3%). Women between 55-64 and <u>></u>85 years had higher mortality than men. Similar trends were observed for all analyses after multivariable adjustment. The prevalence of atrial fibrillation, hypertension, diabetes mellitus, and use of lipid-lowering medication increased during the study period.</p> <p><i>Conclusions:</i> <a name="_Hlk36885003">The age-standardized incidence of first-time hospitalization for ischemic stroke increased from 1996-2002 and then gradually decreased to below the initial level until 2016. </a>Absolute 30-day and 1-year mortality risks decreased between 1996-2016. <a name="_Hlk36741776">These findings correspond to increased stroke prevention awareness and introduction of new treatments during the study period.</a></p>
