Data from: Comparison of Nottingham Prognostic Index and Adjuvant Online prognostic tools in young women with breast cancer: review of a single-institution experience
Objective: Accurately predicting the prognosis of young patients with breast cancer (<40 years) is uncertain since the literature suggests they have a higher mortality and that age is an independent risk factor. In this cohort study we considered two prognostic tools; Nottingham Prognostic Index and Adjuvant Online (Adjuvant!), in a group of young patients, comparing their predicted prognosis with their actual survival. Setting: North East England. Participants: Data was prospectively collected from the breast unit at a Hospital in Grimsby between January 1998 and December 2007. A cohort of 102 young patients with primary breast cancer was identified and actual survival data was recorded. The Nottingham Prognostic Index and Adjuvant! scores were calculated and used to estimate 10-year survival probabilities. Pearson's correlation coefficient was used to demonstrate the association between the Nottingham Prognostic Index and Adjuvant! scores. A constant yearly hazard rate was assumed to generate 10-year cumulative survival curves using the Nottingham Prognostic Index and Adjuvant! predictions. Results: Actual 10-year survival for the 92 patients who underwent potentially curative surgery for invasive cancer was 77.2% (CI 68.6% to 85.8%). There was no significant difference between the actual survival and the Nottingham Prognostic Index and Adjuvant! 10-year estimated survival, which was 77.3% (CI 74.4% to 80.2%) and 82.1% (CI 79.1% to 85.1%), respectively. The Nottingham Prognostic Index and Adjuvant! results demonstrated strong correlation and both predicted cumulative survival curves accurately reflected the actual survival in young patients. Conclusions: The Nottingham Prognostic Index and Adjuvant! are widely used to predict survival in patients with breast cancer. In this study no statistically significant difference was shown between the predicted prognosis and actual survival of a group of young patients with breast cancer.
Dataset: Co-administration of adjuvanted recombinant Ov-103 and Ov-RAL2 vaccines confer protection against natural challenge in a bovine O. ochengi infection model of human onchocerciasis
<p class="MsoNormal"><span>[Abstract from accompanying manuscript]</span></p> <p class="MsoNormal"><span>Onchocerciasis (river blindness), caused by the filarial nematode <em>Onchocerca volvulus</em>, is a neglected tropical disease endemic to sub-Saharan Africa. Worldwide, an estimated 20.9 million individuals live with infected and a further 205 million are at risk of disease. Current control methods rely on mass drug administration of ivermectin to suppress microfilarial development. Presently, there are no viable treatment options effective against the long-lived adult worms. The identification and development of efficacious vaccine candidates as complementary tools to support ongoing elimination efforts are therefore a major objective of onchocerciasis research. </span></p> <p class="MsoNormal"><span>We evaluated the immunogenicity and protective effects of co-administering two leading <em>O. volvulus</em>-derived recombinant vaccine candidates (<em>Ov</em>-103 and <em>Ov</em>-RAL2) in cattle with subsequent natural exposure to <em>Onchocerca ochengi, </em>a close phylogenetic relative of <em>O. volvulus</em>.</span></p> <p class="MsoNormal"><span>Immunisation induced strong IgG responses against both vaccine candidates. Following natural exposure to <em>O. ochengi</em>, vaccination status and resulting serum antibody response was associated with delayed nodule development and onset of microfilaridermia. Peptide arrays identified several <em>Ov</em>-103 and <em>Ov</em>-RAL2-specific epitopes recognised by IgG from immunised cattle which warrant further investigation. Our data provide supporting evidence for ongoing development of a recombinant vaccine for control of human onchocerciasis.</span></p>

Dabrafenib plus trametinib versus anti-PD-1 monotherapy as adjuvant therapy in BRAF V600-mutant stage III melanoma after definitive surgery: a prospectively-powered multicenter, retrospective cohort study
<p><strong>Summary </strong></p> <p>Background Both dabrafenib/trametinib (D/T) and anti-PD-1 monotherapy (PD-1) are approved adjuvant therapies for patients with stage III BRAF V600-mutant melanoma. However, there is still a lack of head-to-head comparative data. We aimed to describe efficacy and toxicity outcomes for these two standard therapies across melanoma centers.</p> <p>Methods This prospectively-powered multicenter, retrospective cohort study was conducted in 15 melanoma centers in Australia, China, Germany, Italy, Japan, UK, and US. We included adult patients with resected stage III BRAF V600-mutant melanoma who received either adjuvant D/T or PD-1. The primary endpoint was recurrence-free survival (RFS). Secondary endpoints included overall survival (OS), recurrence pattern and toxicity.</p> <p>Findings We included 598 patients with stage III BRAF V600-mutant melanoma who received either adjuvant D/T (n=393 [66%]) or PD-1 (n=205 [34%]) post definitive surgery between Jul 2015 and Oct 2022. At a median follow-up of 33 months (IQR 21-43), the median RFS was 51∙0 months (95%CI 41∙0-not reached [NR]) in the D/T group, significantly longer than 44∙8 (95%CI 28∙5-NR) in the PD-1 group (univariate: HR 0∙658, 95%CI 0∙498-0∙869, P=0∙003; multivariate: HR 0∙687, 95% CI 0∙512-0∙922, P = 0∙012), with comparable OS with PD-1 (multivariate, HR 1∙011, 95%CI 0∙637-1∙604, P>0∙95). Similar findings were observed using a restricted-mean-survival-time model. D/T survival benefits were consistent in most subgroups, except for older, male patients and those with V600K mutation. Among those who experienced recurrence, the proportion of distant metastases was higher in the D/T cohort. D/T had a higher incidence of treatment modification due to adverse events (AEs) than PD-1, but fewer persistent AEs.</p> <p>Interpretation In patients with stage III BRAF V600-mutant melanoma post definitive surgery, D/T yielded better RFS than PD-1, with higher transient but lower persistent toxicity, and comparable OS. D/T is potentially a superior adjuvant option compared with PD-1. </p>
Comparison of post operative analgesia between Nalbuphine and Fentanyl as adjuvant with intrathecal Levobupivacaine for lower abdominal gynaecological surgeries
<p>For lower abdominal gynaecological procedures, spinal anaesthetic is the recommended anaesthetic approach because it offers the advantages of a quick onset of effect, full motor blocking, and ease of application. It also requires a low amount of local anaesthetic medication and has a lower frequency of unsuccessful blocks, but it is insufficient to give post-operative analgesia. Adjuvants, such as dexmeditomedine, fentanyl, and nalbuphine, have a synergistic effect and amplify sensory block without increasing sympathetic block when added to local anaesthetics for intrathecal administration. This leads to better hemodynamic stability with fewer adverse effects, a reduction in the dosage of local anaesthetic medication, and aids in post-operative analgesia.</p> <p>Fentanyl belongs to phenypiperidine derivative synthetic opiod. It is structurally similar to Meperidine. It is a pure mu receptor agonist. It is lipophilic in nature, has rapid onset of action and does do not cause respiratory depression following intrathecal administration.. It improves anaesthetic quality without much complications. It prolongs duration of anesthesia as well as depression of ventilation after intrathecal administartion. The typical adverse effects of mu agonists include drowsiness, nausea, vomiting, urine retention, and respiratory depression. Nalbuphine is a mixed agonist and antagonist opioid drug. Agonist to kappa and antagonist to mu receptor. It is similar with oxymorphone and naloxone in terms of structure. It offers analgesia without the negative side effects associated with mu receptor agonist activity. Almost all general anaesthesia and regional anaesthesia techniques work with it. Nalbuphine causes analgesia by attaching to kappa receptors found throughout the spinal cord and brain.(1)</p> <p>Levobupivacaine is a local anaesthetic medication from the amide family. It is bupivacaine's S-enantiomer, which is less caridotoxic and neurotoxic and equally potent as compared to bupivacaine. It also shows early motor recovery. Many studies have compared intrathecal nalbuphine and fentanyl with levobupvacaine and its effects on post-operative analgesia.</p> <p>The goal of this research is to compare the efficacy of fentanyl & nalbuphine as adjuvants with intrathecal levobupivacaine for post-operative analgesia following lower abdominal gynaecological surgeries.</p>
