Functional capacity and risk of falls in the elderly with metabolic syndrome.

<p>This is a database for the study:&nbsp;<strong>Functional capacity and risk of falls in the elderly with metabolic syndrome.</strong></p> <p><strong>Method: </strong>A cross-sectional study was carried out with 126 elderly subjects, 58 presented MetS and 68 without MetS, according to the criteria of the International Diabetes Federation. Katz Index, Tinetti Index, Timed Up and Go test and Berg Scale were applied to evaluate the functional capacity. Falls Efficacy Scale-International scale (FES-I-Brazil) was used to evaluate the fear of falling. For statistical analysis, the Mann-Whitney, Chi-square and Spearman correlation tests were used.</p>

Predictive biomarkers of individual trajectories in elderly persons with subtle cognitive decline: APOE genotype data

<p>The mentalizing network (MN) treats social interactions based on our understanding of other people's intentions and includes the medial prefrontal cortex (mPFC), temporoparietal junction (TPJ), posterior cingulate cortex (PCC), precuneus (PC) and amygdala. Not all elders are equally affected by the aging-related decrease of mentalizing abilities. Personality has recently emerged as a strong determinant of functional connectivity in MN areas. However, its impact on volumetric changes across the mentalizing network in brain aging is still unknown. To address this issue, we explored the determinants of volume decrease in MN components including amyloid burden, personality, and APOE genotyping in a previously established cohort of 130 healthy elders with a mean follow-up of 54 months. Personality was assessed with the Neuroticism Extraversion Openness Personality Inventory-Revised. Regression models corrected for multiple comparisons were used to identify predictors of volume loss including time, age, sex, personality, amyloid load, presence of APOE epsilon 4 allele and cognitive evolution. In cases with higher Agreeableness scores, there were lower volume losses in posterior cingulate cortex (PCC), precuneus (PC) and amygdala bilaterally. This was also the case for right medial prefrontal cortex (mPFC) in elders displaying lower Agreeableness and Conscientiousness. In multiple regression models, the effect of Agreeableness was still observed in left PC and right amygdala and that of Conscientiousness in right mPFC volume loss (26.3% of variability, significant age, sex). Several Agreeableness (Modesty) and Conscientiousness (order, dutifulness, achievement striving and self-discipline) facets were positively related to increased volume loss in cortical components of the MN. In conclusion, these data challenge the beneficial role of higher levels of Agreeableness and Conscientiousness in old age showing that they are associated with an increased rate of volume loss within the mentalizing network.</p>

Data from: Oral bisphosphonates are associated with increased risk of atypical femoral fractures in elderly women

Objectives: To evaluate the association between bisphosphonate use and the risk of atypical femoral fractures among women aged 65 or older. Design: Nested case–control study. Setting: General practice research database in Spain. Exposures Use of oral bisphosphonates before the occurrence of atypical fractures among cases or the corresponding index date among controls. Bisphosphonate use was categorised as ever versus never users. Ever users were divided according to the total time since first prescription. Main outcome measures: Cases were defined as women aged 65 years or older with a first diagnosis of subtrochanteric or diaphyseal fracture, recorded in the BIFAP database between 1 January 2005 and 31 December 2008, and with at least 1 year of follow-up before the index date. For each case, five age-matched and calendar-year-matched controls without a history of hip or atypical fracture were randomly selected from the database. Statistical analysis: OR of atypical femoral fracture by bisphosphonate use was determined using conditional logistic regression. Models were adjusted for comorbidities and use of other medications. Results: The analysis included 44 cases and 220 matched controls (mean age, 82 years). Ever use of bisphosphonates was more frequent in cases than controls (29.6% vs 10.5%). In multivariate analyses, OR (95% CI) of atypical femoral fracture was 4.30 (1.55 to 11.9) in ever versus never users of bisphosphonates. The risk increased with long-term use, with an OR of 9.46 (2.17 to 41.3) comparing those using bisphosphonates over 3 years versus no users (p for trend=0.01). Conclusions: Bisphosphonate use was associated with an increased risk of subtrochanteric or diaphyseal fractures in elderly women in a low fracture risk population, with a higher risk among long-term bisphosphonate users.

Data from: Diagnostic accuracy of presepsin in predicting bacteremia in elderly patients admitted to the emergency department: prospective study in Japan

<p class="CxSpFirst"><span><b>Objective</b></span></p> <p class="CxSpMiddle"><span><a name="_Hlk1399924">Early detection of bacteremia in the elderly is needed in the emergency department (ED). </a></span></p> <p class="CxSpMiddle"><span><a name="_Hlk11878757"><b>Design, Setting, and Participants</b></a></span></p> <p class="CxSpMiddle"><span>Prospective study in Japan, single center trial in patients who satisfied the sepsis criteria was conducted between September 2014 and March 2016. Forty-six elderly patients aged ≥70 years were included.</span></p> <p class="CxSpMiddle"><span><b>Interventions</b></span></p> <p class="CxSpMiddle"><span>Blood sampling to evaluate C-reactive protein (CRP), procalcitonin (PCT) and presepsin plasma levels; two sets of blood sampling for bacterial cultures; and evaluations of the Sequential Organ Failure Assessment (SOFA) and Acute Physiology and Chronic Health Evaluation (APACHE II) scores were performed upon arrival at the ED. The results were compared between patients with bacteremia and those without bacteremia.</span></p> <p class="CxSpMiddle"><span><a name="_Hlk11878769"><b>Main Outcome Measure</b></a></span></p> <p class="CxSpMiddle"><span>The accuracy of detecting bacteremia</span></p> <p class="CxSpMiddle"><span><b>Results</b></span></p> <p class="CxSpMiddle"><span>The presepsin value was significantly higher in the bacteremia group than in the non-bacteremia group (866.6 ± 184.6 pg/mL vs. 639.9 ± 137.1 pg/mL, p = 0.03). The PCT and CRP did not significantly differ between the groups. The area under the receiver-operating-characteristic curve (AUC) values were not significantly different among presepsin (0.69), PCT (0.61), and CRP (0.53). Multivariate analysis showed that presepsin was independently associated with bacteremia (odds ratio, 8.84; 95% confidence interval, 0.95–81.79; p = 0.02).</span></p> <p class="CxSpMiddle"><span><b>Conclusion<a name="_Hlk1399945"></a></b></span></p> <p class="CxSpMiddle"><span>Presepsin could be a good biomarker to predict bacteremia in elderly patients with sepsis criteria admitted to the ED.</span></p>

Data from: Disparity in preemptive end-of-life conversation experience caused by subjective economic status among general Japanese elderly people: a cross-sectional study with stratified random sampling

Objectives: Preemptive conversations (PCs) about end-of-life (EOL) preferences are beneficial for both elderly people and their families to understand and share the preferences. However, the factors which promote/inhibit PCs have yet to be clarified. We therefore aimed to determine the factors related to having PCs with hypothesis that age, subjective economic status and subjective health status are associated with having PC experience. Design: A cross-sectional study administering a questionnaire and using stratified random sampling by gender and region. Setting: Residents aged 65 years or older who were not receiving nursing care as of November 1, 2016, were extracted from the Japanese long-term care insurance system registry in Koriyama City, Fukushima Prefecture, Japan. Participants: 1,575 participants (717 males and 858 females). Outcome: Presence or absence of PC experience with family or friends (yes/no). Results: The mean age of the participants was 74.0 years. A multivariable logistic-regression analysis revealed that having PC experience was significantly associated with gender (OR = 1.907; 95% CI = 1.556, 2.337; p &lt; 0.001), subjective economic status (OR = 0.832; 95% CI = 0.716, 0.966; p=0.016), and subjective happiness (OR = 0.926; 95% CI = 0.880, 0.973; p=0.003). Conclusions: Poor subjective economic status of elderly people may result in absence of EOL conversation experience with their families and friends, hindering the elderly from sharing and understanding the EOL preferences. To promote PCs about EOL, gerontology and public health professionals should give special consideration to the subjective economic status of elderly people.
