Exploring the Interaction Between Proprioceptive Sensitivity in the Hip, Knee, and Ankle Joints, Alongside Stability Limits Evaluated Through Computerized Posturography, in Elderly Individuals Afflicted with Fibromyalgia Syndrome.
<p><strong><span>Objectives: </span></strong><span>This study investigates proprioceptive sensitivity and stability limits in patients with Fibromyalgia syndrome (FMS) compared to healthy controls, focusing on joint reposition sense (JRS) and limits of stability (LOS). By delineating differences between the groups, it aims to uncover the proprioceptive deficits that contribute to balance impairments in FM sufferers. Additionally, the study explores correlations between JRS and LOS within the FM group, potentially informing targeted therapeutic strategies to enhance patient outcomes.</span></p> <p><span> </span></p> <p><strong><span>Methods: </span></strong><span>Fifty-four individuals with FM and 54 healthy controls participated in this research. Using precise digital inclinometers, JRS was evaluated at the hip, knee, and ankle joints. LOS was assessed using dynamic posturography with advanced iso-free technology. Various parameters were measured, including reaction time, maximum excursion, and directional control. </span></p> <p><span> </span></p> <p><strong><span>Results:</span></strong><span> Individuals with FM exhibited significant differences in JRS and LOS compared to healthy controls. For JRS errors in the hip, knee, and ankle joints (mean ± SD), the FM group showed 3.2° ± 0.6°, 2.9° ± 0.5°, and 3.5° ± 0.7°, respectively, while the control group exhibited 1.1° ± 0.4°, 1.0° ± 0.3°, and 1.2° ± 0.4°, respectively. In LOS, FM individuals had a reaction time of 1.8 ± 0.3 seconds, maximum excursion of 54.6% ± 6.2%, and directional control of 65.2% ± 5.8%, while healthy controls showed 0.9 ± 0.2 seconds, 72.4% ± 7.1%, and 77.8% ± 6.9%, respectively. </span></p> <p><span> </span></p> <p><strong><span>Conclusions:</span></strong><span> Correlation analysis revealed significant associations between JRS parameters and LOS outcomes within the FMS group, indicating that better proprioception was linked to improved postural stability. These findings have clinical implications for assessing and managing balance impairments in individuals with FMS, potentially contributing to the development of targeted interventions to improve their quality of life.</span></p>

FLUOROQUINOLONE NEUROTOXICITY IN THE ELDERLY
<p>Older patients are taking more and more antibiotics, including fluoroquinolones. Their toxic effects on the<br>nervous system and the psyche are well known but not well described and explained. We sought to<br>determine the number and severity of adverse effects on the nervous system and psyche caused by<br>fluoroquinolones in patients over 65 years of age. We analyzed fluoroquinolone reports submitted to the<br>Canada Vigilance Adverse Reaction Online Database and the Australian Database of Adverse Event<br>Notification. A systematic literature search was also conducted. Confusion and hallucinations were the<br>predominant symptoms caused primarily by ciprofloxacin. The review of the publications indicates an<br>increased incidence of adverse effects on the nervous system in the elderly. Treatment with<br>fluoroquinolones in the elderly should be closely monitored due to the potential for neurotoxicity<br>manifesting as neurological and/or psychiatric symptoms and the risk of misdiagnosis.</p>
Data from: The genetic landscape of axonal neuropathies in the middle aged and elderly: focus on MME (supplementary information)
<p class="BarbaraStandard">OBJECTIVE: To test the hypothesis that monogenic neuropathies such as Charcot-Marie-Tooth disease (CMT) contribute to frequent but often unexplained neuropathies in the elderly, we performed genetic analysis of 230 patients with unexplained axonal neuropathies and disease onset <u>></u>35 years.</p> <p class="BarbaraStandard">METHODS: We recruited patients, collected clinical data and conducted whole-exome sequencing (WES, n=126) and <i>MME</i> single-gene sequencing (n=104). We further queried WES repositories for <i>MME</i> variants and measured blood levels of the <i>MME</i>-encoded protein neprilysin.</p> <p class="BarbaraStandard">RESULTS: In the WES cohort, the overall detection rate for assumed disease-causing variants in genes for CMT or other conditions associated with neuropathies was 18.3% (familial cases 26.4%, apparently sporadic cases 12.3%). <i>MME</i> was most frequently involved and accounted for 34.8% of genetically solved cases. The relevance of <i>MME</i> for late-onset neuropathies was further supported by detection of a comparable proportion of cases in an independent patient sample, preponderance of <i>MME</i> variants among patients compared to population frequencies, retrieval of additional late-onset neuropathy patients with <i>MME</i> variants from WES repositories, and low neprilysin levels in patients' blood samples. Transmission of <i>MME</i> variants was often consistent with an incompletely penetrant autosomal dominant trait and less frequently with autosomal recessive inheritance.</p> <p class="BarbaraStandard">CONCLUSIONS: A detectable fraction of unexplained late-onset axonal neuropathies is genetically determined, either by variants in CMT genes or genes involved in other conditions that affect the peripheral nerves and can mimic a CMT phenotype. <i>MME</i> variants can act as completely penetrant recessive alleles but also confer dominantly inherited susceptibility to axonal neuropathies in an aging population.</p> <p> </p>
The elephant in the family: Costs and benefits of elder siblings on younger offspring life-history trajectory in a matrilineal mammal
<p>1. Many mammals grow up with siblings, and interactions between them can influence offspring phenotype and fitness. Among these interactions, sibling competition between different-age offspring should lead to reproductive and survival costs on the younger sibling, while sibling cooperation should improve younger sibling's reproductive potential and survival. However, little is known about the consequences of sibling effects on younger offspring life history trajectory, especially in long-lived mammals.</p> <p>2. We take advantage of a large, multigenerational demographic dataset from semi-captive Asian elephants to investigate how the presence and sex of elder siblings influence the sex, survival until five years old, body condition, reproductive success (i.e. age at first reproduction and lifetime reproductive success), and long-term survival of subsequent offspring.</p> <p>3. We find that elder siblings have heterogenous effects on subsequent offspring life-history traits depending on their presence, their sex, and the sex of the subsequent offspring (named focal calf).</p> <p>4. Overall, the presence of an elder sibling (either sex) strongly increased focal calf long-term survival (either sex) compared to sibling absence. However, elder sisters had higher impact on the focal sibling than elder brothers. Focal females born after a female display higher long-term survival, and decreased age at first reproduction when raised together with an elder sister rather than a brother. Focal males born after a female rather than a male showed lower survival but higher body weight when both were raised together. We did not detect any sibling effects on the sex of the focal calf sex, survival until five years old and lifetime reproductive success.</p> <p>5. Our results highlight the general complexity of sibling effects, but broadly that elder siblings can influence the life-history trajectory of subsequent offspring. Prehabilitation program in elderly patients: a prospective cohort study
<p>The preoperative period may be an opportune period to optimize the patients' physical condition with a multimodal preoperative program. The impact of a "prehabilitation" program in elderly patients is discussed. This mono-centre observational cohort study included consecutively 139 patients planned for major abdominal and thoracic surgery: 44 in the control group (age<65) and 95 in the elderly group (age> 65). All patients followed a "prehabilitation" program including exercise training, nutritional optimization, psychological support, and behavior change. Seventeen patients in the control group and 45 in the elderly group completed the study at six months. The 6-minute walk test (6MWT) increased in both groups from the initial evaluation to the last one (median value of 80 m [interquartile range 51] for those under 65 years; 59 m [34] for the older ones; P=0.114). The 6MWT was also similar after one month of prehabilitation for both populations. The rate of postoperative complications was similar in the two groups. Prehabilitation showed equivalence in patients over 65 years of age compared to younger patients in terms of increase in functional capabilities and of postoperative evolution. This multimodal program represents a bundle of care which can benefit the frailer population.</p>
