Data from: Performance of primary care in different health care facilities: a cross sectional study of patients' experiences in Southern Malawi

Objective: In most African countries, primary care is delivered through a district health system. Many factors, including staffing levels, staff experience, availability of equipment and facility management, affect the quality of primary care between and within countries. The purpose of this study was to assess the quality of primary care in different types of public health facilities in Southern Malawi. Study design: This was a cross sectional quantitative study. Setting: The study was conducted in 12 public primary care facilities in Neno, Blantyre and Thyolo districts in July 2018. Participants: Patients aged 18 years and above, excluding the severely ill, were selected to participate in the study. Primary outcomes: We used the Malawian primary care assessment tool to conduct face-to-face interviews. ANOVA at 0.05 significance level was performed to compare primary care dimension means and total primary care scores. Linear regression models at 95% CI were used to assess associations between primary care dimension scores, patients' characteristics and healthcare setting. Results: The final number of respondents was 962 representing 96.1% response rate. Patients in Neno hospitals scored 3.77 points higher than those in Thyolo health centers, and 2.87 higher than those in Blantyre health centers in total primary care performance. Primary care performance in health centers and in hospital clinics was similar in Neno (20.9 vs 19.0, p= 0.608) while in Thyolo, it was higher at the hospital than at the health centers (19.9 vs 15.2, p<0.001). Urban and rural facilities showed a similar pattern of performance. Conclusion: These results showed considerable variation in experiences among primary care users in the public health facilities in Malawi. Factors such as funding, policy and clinic level interventions influence patients' reports of primary care performance. These factors should be further examined in longitudinal and experimental settings.

Data from: Association of serum neurofilament light (sNfL) and disease severity in patients with spinocerebellar ataxia type 3

<p><a name="_Hlk22462511"><b>Objective:</b></a> To investigate serum neurofilament light protein (sNfL) levels in patients with spinocerebellar ataxia type 3 (SCA3) and to determine whether they are associated with disease severity.</p> <p><b>Methods: </b>This cross-sectional study enrolled 185 healthy controls and 235 <i>ATXN3</i> mutation carriers (17 asymptomatic stage, 20 preclinical stage, and 198 ataxic stage)<span class="fontstyle01"><span>. We measured </span></span>sNfL levels with the single molecule array (Simoa) platform. Clinical disease severity was assessed using the scale of assessment and rating of ataxia (SARA) and the inventory of non-ataxia signs (INAS). In a subgroup of 50 ataxic stage patients, we further evaluated the gray matter volume and the integrity of white matter fibers by magnetic resonance imaging (MRI).</p> <p><b>Results:</b> sNfL concentrations were elevated in asymptomatic, preclinical,<i> </i>and<i> </i>ataxic<i> ATXN3</i> mutation carriers compared to controls (12.18 [10.20-13.92], 21.84 [18.37-23.45], 36.06 [30.04-45.90] and 8.24 [5.92-10.84] pg/mL, median [interquartile range], respectively, <i>P </i>&lt; 0·001). sNfL correlated with SARA (r=0.406, 95% confidence interval 0.284-0.515, <span class="fontstyle01"><span><i>P </i></span></span><span class="fontstyle01"><span>&lt; 0.0001</span></span>) and INAS (r=0.375, 95% confidence interval 0.250-0.487, <span class="fontstyle01"><span><i>P </i></span></span><span class="fontstyle01"><span>&lt; 0.0001</span></span>), and remained significant after adjustment for age and CAG repeats. In addition, we observed negative correlations of the sNfL with gray matter volume in the left precentral gyrus and the left paracentral lobule as well as with the mean diffusivity in widespread white matter tracts.</p> <p><b>Conclusion: </b>Our results demonstrate that sNfL levels are increased in <span class="fontstyle01"><span>SCA3</span></span> and are associated with clinical disease severity, which supports sNfL as a biomarker for disease severity in SCA3.</p>

Data from: Differential neuropsychological profile of ALS patients with and without C9orf72 mutation

<p><b>Objective</b>. To determine whether the neuropsychological profiles of ALS patients with (ALSC9+) and without (ALSC9-) <i>C9orf72</i> expansion are different we administered a battery of neuropsychological tests to 741 ALS patients (68 ALSC9+ and 673 ALSC9-) and 129 controls.</p> <p><b>Methods</b>. The study population includes 741 ALS patients who were consecutively diagnosed at the Turin ALS expert center in the period 2010-2018 and who underwent both cognitive/behavioral and genetic testing. Patients' neuropsychological patterns were compared (a) at the same degree of cognitive and behavioral deficit according to the revised ALS-FTD Consensus Criteria; and (b) at the same level of motor impairment according to the King's staging system.</p> <p><b>Results</b>. Despite being about 7 years younger, ALSC9+ patients had significant lower scores in tests exploring executive functions, and verbal memory both when classified as cognitively normal and when diagnosed in the intermediate cognitive categories. Considering the clinical perspective, ALSC9+ patients showed significantly lower scores compared to ALSC9- patients at King's stage 1 and 3 in almost all the examined neuropsychological domains, while at King's stage 2 ALSC9+ patients were more severely affected only in the verbal memory domain. Behavioral function was comparably impaired in the two cohorts.</p> <p><b>Conclusions</b>. ALSC9+ patients show a different neuropsychological profile compared to ALSC9- ones, being more impaired in executive functions and verbal memory domains at all King's stages. Verbal memory emerged as a particularly vulnerable function in ALSC9+, with worse performances even when patients were still classified as cognitively normal.</p>

Data for: Assessment of quality of life (QoL) in cancer patients

<p><strong>Background</strong>: A cancer patient's quality of life (QoL) is the perception of their physical, functional, psychological, and social well-being as well as their mental and emotional state. QoL is one of the most important factors to consider when a person is being treated for cancer and during follow-up. The present study aimed to understand the status of QoL of cancer patients and determine the factors affecting it.</p> <p><strong>Methods</strong>: This cross-sectional study was conducted among 210 cancer patients attending the oncology unit of a medical college, within a 4-month consecutive time period in 2022. Data were collected by using the Bengali version of the European Organization for Research and Treatment of Cancer questionnaire.</p> <p><strong>Results</strong>: The present study reported a high number of female cancer patients (67.6%). Breast cancer was more common among females (31.43%) while lung and upper respiratory tract cancer was among males (19.05). Most of the patients in the present study were diagnosed with cancer in the past year (86.19%). The functional scales' overall mean scores varied from 54.92 for physical functioning to 38.89 for social functioning. The highest symptom scale score was for financial issues (63.02), while the lowest was for diarrhea (33.01). The overall QoL of cancer patients in the present study was 47.98 which was 45.71 for males and 49.10 for females respectively.  </p> <p><strong>Conclusion</strong>: The overall QoL was poor in cancer patients in the present study compared to the developed countries. There was a low score for QoL for social and emotional function. Financial difficulty was the primary reason behind low QoL in the symptom scale. If the government supports cancer patients by providing subsidies for treatment and health insurance policies, cancer patients will benefit and QoL will improve.</p>

Data related to the olfactory evaluation of leprosy patients

<p>Background: The effect of leprosy on the sense of smell is not yet fully established. Studies that have relied only on patients' perceptions may have under- or over-estimated the change in smell perception. A validated and psychophysical method is necessary to avoid these errors in assessment.</p> <p>Objectives: This study aimed to validate the existence of olfactory involvement in leprosy patients.</p> <p>Methods: A cross-sectional, controlled study was conducted, in which individuals with leprosy (exposed individuals) and individuals without leprosy (control patients) were recruited. For each exposed individual, we selected two control patients. A total of 108 patients (72 control patients and 36 exposed individuals) with no history of infection with the new coronavirus (COVID-19) took the University of Pennsylvania Smell Identification Test(UPSIT).</p> <p>Results: Most exposed individuals had olfactory dysfunction [n = 33, 91.7% (CI 95%: 77.5%–98.3%)] when compared with the control patients [n = 28, 38.9% (CI 95%: 27.6%-51.1%)], but only two (5.6%) had olfactory complaints. The olfactory function was significantly worse among exposed individuals [UPSIT leprosy = 25.2 (CI 95%: 23.1–27.3) when compared with the UPSIT control patients = 34.1 (CI 95%: 33.0–35.3); p&lt;0.001]. The risk of olfactory loss was higher among the exposed individuals [OR: 19.5 (CI 95%: 5.18–105.70; p&lt; 0.001)].</p> <p>Conclusions: Olfactory dysfunction was highly prevalent among exposed individuals, although they had little or no self-knowledge of the disorder. The results show that it is important to assess the sense of smell in exposed individuals.</p>

<p>The uploaded zip-file entails the data needed for and obtained through the dissertation titled &#39;Topic modelling for the stratification of neurological patients&#39; as part of the programme &#39;MSc. in Statistical Data Analysis&#39; at Ghent University. The study aimed at exploring the applicability of hierarchical stochastic block models on resting-state functional magnetic resonance imaging (RS-fMRI) to cluster participants with known neurological disorders.</p> <p>The data is structured in different folders&nbsp;and aligns with the folder structure of the GitHub-repository that contains the analysis scripts (https://github.com/wvechelp/hsbm_on_fmri). The GitHub-repository already covers some example data, while additional data and results can be found in this zip-file. Additional comments on the analyses are also provided in the analysis scripts.</p> <p>The original raw data is obtained through&nbsp;the OpenFMRI project (<a href="http://openfmri.org/">http://openfmri.org/</a>, with label <em>ds000030</em>) and as a Stanford Digital Repository (<a href="https://purl.stanford.edu/mg599hw5271">https://purl.stanford.edu/mg599hw5271</a>) (Bilder<em> et al.</em>, 2016). It is obtained from the NIH Roadmap Initiative, as a result of the Consortium for Neuropsychiatric Phenomics (CNP) study (Poldrack<em> et al.</em>, 2016). Throughout the study, data was collected through interviews and rating scales, self-report measures, neurocognitive exams (using both paper-pencil and computerised tests), and a variety of neuroimaging data.&nbsp;More specific information on the selection procedure of the participants can be found in the description of the data (Bilder<em> et al.</em>, 2016) and the associated article (Poldrack<em> et al.</em>, 2016).&nbsp;Among the available neuroimaging data, the RS-fMRI data&nbsp;have been collected by asking participants to remain relaxed, while keeping their eyes open (with scans lasting 304 s and an image being collected every 2 seconds (Poldrack<em> et al.</em>, 2016)).&nbsp;This&nbsp;raw data was pre-processed by Rasero<em> et al.</em> (2019) to correct for motion and temporal alignment. Smoothing (6-mm full width at half-maximum Gaussian kernel), intensity normalisation, and a band-pass filter (between 0.01 and 0.08 Hz) were applied prior to the removal of linear and quadratic trends. Motion time courses, average CSF signal, and the average white matter signal were regressed out prior to data transformation into voxels with a volume of 3 mm x 3 mm x 3 mm. Ultimately, the functional atlas of Shen<em> et al.</em> (2013) was used to average the voxel signals per anatomical region of interest (ROI), resulting in a parcellation&nbsp;of 278 ROIs (and associated time series consisting of 152 measurements). From these ROI-specific time series, Rasero<em> et al.</em> (2019) generated 278 x 278 matrices with Pearson coefficients.</p> <p>References:<br> Bilder, R. M., Poldrack, R. Data from: Analysis of altered level of blood-based biomarkers in the prognosis of COVID-19 patients

<p><strong>Introduction:</strong> Immune and inflammatory responses developed by the patients with Coronavirus Disease 2019 (COVID-19) during rapid disease progression result in an altered level of biomarkers. Therefore, this study aimed to analyze levels of blood-based biomarkers that are significantly altered in patients with COVID–19.</p> <p><strong>Methods: </strong>A cross-sectional study was conducted among COVID-19 diagnosed patients admitted to the tertiary care hospital. Several biomarkers – biochemical, hematological, inflammatory, cardiac, and coagulatory – were analyzed and subsequently tested for statistical significance at P&lt;0.01 by using SPSS version 17.0.</p> <p><strong>Results:</strong> A total of 1,780 samples were analyzed from 1,232 COVID-19 patients (median age 45 years [IQR 33-57]; 788 [63.96%] male). The COVID-19 patients had significantly (99% CI, p&lt;0.001) elevated glucose, urea (p=0.001), alanine transaminase (ALT), aspartate aminotransaminase (AST), alkaline phosphatase (ALP), lactate dehydrogenase (LDH) levels, total white blood cell count (WBC), C-reactive protein (CRP), procalcitonin (PCT), interleukin-6 (IL-6), ferritin, D-Dimer, and creatinine phosphokinase-MB (CPK-MB, p=0.004) as compared to control group. However, the levels of total protein, albumin, and platelets were significantly lowered in COVID-19 patients as compared to control group. The elevated levels of glucose, urea, direct bilirubin, WBC, CRP, prothrombin time (PT), D-Dimer, and LDH were significantly associated with in-hospital mortality among COVID-19 patients.</p> <p><strong>Conclusions:</strong> Assessing and monitoring the elevated levels of glucose, urea, ALT, AST, ALP, ferritin, DB, WBC, CRP, PCT, LDH, D-Dimer, PT, and CPK-MB and the lowered levels of total protein, albumin, and platelet could provide a basis for evaluation of improved prognosis and effective treatment in patients with COVID-19.</p>
