Basaloid Squamous Cell Carcinoma: A Case Report
<p><strong>Background:</strong> The upper aerodigestive tract is where BSCC (basaloid squamous cell carcinoma), a rare variation of conventional SCC, is most frequently found. The hypopharynx, tonsil, supraglottic larynx, tongue (base), and head-neck regions are particularly susceptible to BSCC. Clinically, the presentation of BSCC is similar to that of conventional SCC, but it has a poor prognosis than traditional SCC. BSCC is distinguished histopathologically by a dimorphic-pattern, a distinctive basal cell component paired with a squamous component, and a squamous component. Compared to traditional SCC, the prognosis for BSCC is worse. However, clinically it shows similar features like conventional SCC which makes it difficult to diagnose. Therefore, histopathology and immunohistochemistry have a crucial role in diagnosing such tumors. We here present a case of a seventy-year male diagnosed with BSCC involving the tongue.</p> <p><strong>Keywords</strong>: Basaloid squamous cell carcinoma, dimorphic pattern, basaloid cells, comedo necrosis.</p>

<p>The data included in this record constituted the input for the case study reported in the manuscript "DREAM: an R package for druggability evaluation of human complex diseases", by Antonio Federico, Michele Fratello, Alisa Pavel, Lena Möbus, Giusy del Giudice, Angela Serra, Dario Greco. The data derive from transcriptomics experiments executed on lesional skin from atopic dermatitis patients and unaffected skin counterparts. The data consists of two files in ".txt" format reporting gene expression data in tabular format, where on the rows are reported genes and on the columns are reported samples. The data is an aggregated and batch-corrected collection of datasets originally downloaded by Gene Expression Omnibus (GEO, https://www.ncbi.nlm.nih.gov/geo/). The file "GE_Mic_AD_Pamr_MAARS.txt" reports gene expression estimates of lesional skin of atopic dermatitis patients, while the file "GE_Mic_AD_Pamr_nl_MAARS.txt" reports gene expression estimates of non-lesional skin of atopic dermatitis patients.</p>

A rare entity - percutaneous lead extraction in a very late onset pacemaker endocarditis - case report and review of literature
<p><strong>Video 1.</strong> Transthoracic two-dimensional echocardiography apical 4-chamber view: large hypoechogenic hyper-pedunculated mobile mass at the level of tricuspid valve; <strong>Video 2.</strong> Transesophageal two-dimensional echocardiography: low echogenicity pedunculated mass attached to the pacemaker lead, with multiple sites of binding, with no supplementary involvement of the tricuspid valve and myxomatous appearance of the posterior leaflet with hypermobility and rupture of chordae; <strong>Video 3. </strong>Fluoroscopy during the lead extraction procedure, after freeing the lead from adhesions: traction of the lead from the right ventricular apex, through the tricuspid valve, right atrium, superior vena cava and left subclavian vein, to complete lead removal; <strong>Video 4.</strong> Postprocedural transesophageal two-dimensional echocardiography: posterior leaflet chordae rupture, no residual vegetation, no pericardial effusion; <strong>Video 5.</strong> Transthoracic two-dimensional echocardiography parasternal short axis, no additional cardiac masses, posterior leaflet chordae rupture of the tricuspid valve; <strong>Video 6.</strong> Transthoracic two-dimensional echocardiography parasternal short axis, color Doppler: mild tricuspid regurgitation, with two small thin jets.</p>
Leptomeningeal metastasis from Adrenocortical carcinoma: a case report
<p>Adrenocortical carcinoma (ACC) is an uncommon endocrine malignancy with limited treatment options. While overall 5-year survival rate in patients with ACC is 35%, the disease is often rapidly progressive with long-term survival in only 5% of patients. Although tumor stage, grade and excess hormonal activity predict unfavorable prognosis, additional biomarkers are needed to identify patients with aggressive disease.</p> <p>A 23-year-old woman presented with rapidly progressing signs and symptoms of Cushing's syndrome, with associated abdominal pain and fullness. Evaluation revealed a large left adrenal mass which had developed over 8 months. En bloc surgical resection was performed by an endocrine surgeon, and pathology revealed adrenocortical carcinoma with Ki67 of 60%. Despite adjuvant treatment with mitotane and etoposide-doxorubicin-carboplatin chemotherapy, the patient had rapid disease progression with metastatic spread to liver, lung, bone, brain and leptomeningies and she died eleven months after the initial diagnosis. Subsequent analysis of patient's tumor revealed mutations in TP53 and MEN1. RNA sequencing was compared against the The Cancer Genome Atlas data set and clustered with the high steroid, proliferative subtype, associated with the worst prognosis. The tumor also demonstrated a low BUB1B/PINK1 ratio as well as G0S2 hypermethylation, both predictive of very aggressive ACC.</p> <p>This case represents a subset of ACC characterized by rapid and fatal progression. Clinically available predictors as well as recently reported molecular signatures and biomarkers correlated with this tumor's aggressiveness, suggesting that development and validation of combinations of biomarkers may be useful in guiding personalized approaches to patients with ACC.</p>
Nivolumab-associated acute glomerulonephritis: Case report and literature review
<p><strong>Background:</strong> Immune checkpoint inhibitors are changing the landscape of oncologic treatment as they are significantly improving treatment of multiple malignancies. Nivolumab, anti PD-1 antibody, is FDA-approved for treatment of melanoma, non-small cell lung cancer and kidney cancer and can cause a spectrum of autoimmune reaction. The adverse effect can occur at any organ system in the body, but most commonly affects colon, lung, liver or endocrine system. Although rare, kidneys also can be involved.</p> <p><strong>Case presentation: </strong>A 70 year old male with clear cell kidney cancer was admitted with acute kidney injury while he was receiving nivolumab treatment. A kidney biopsy revealed diffuse tubular injury and immune complex mediated glomerulonephritis. Electron microscopic exam of the specimen demonstrated hump-like subepithelial deposits. Nivolumab was discontinued and the patient was started on high dose steroid. After 5 months of systemic corticosteroid and hemodialysis, patient’s kidney function improved to his baseline. In spite of the prolonged treatment interruption, immunosuppressive therapy did not compromise the anticancer effect of nivolumab.</p> <p><strong>Conclusion: </strong>Immune-related adverse effect in kidney can cause autoimmune glomerulonephritis as well as tubulo-interstitial injury. In previous literature, immune-related nephritis generally responded well to systemic corticosteroid. Based our experience, prolonged course of high dose of steroid and hemodialysis may be required to achieve adequate treatment effect. </p>
