The lower airways microbiota and antimicrobial peptides indicate dysbiosis in sarcoidosis

<p><span><strong>Rationale</strong>: </span><span>The role of the pulmonary microbiome in sarcoidosis is unknown.</span><br><br><span><strong>Objectives</strong>: </span><span>The objectives of the current study was to: 1) </span><span>Examine whether the pulmonary fungal and bacterial microbiota differed in patients with sarcoidosis compared with controls. 2) Examine whether there was an association between the microbiota and levels of the antimicrobial peptides (AMPs) in protected bronchoalveolar lavage (PBAL), indicating an interaction with the innate immune response.</span><br><br><span><strong>Methods</strong>: </span><span>35 sarcoidosis patients and 35 healthy controls underwent bronchoscopy and were sampled with oral wash (OW), protected BAL (PBAL) and left protected sterile brushes (LPSB).  The fungal ITS1 region and the V3V4 region of the bacterial 16SrDNA gene were sequenced. Bioinformatic analyses were performed with QIIME 2. The AMPs secretory leucocyte protease inhibitor (SLPI) and human beta defensins 1 and 2 (hBD-1 &amp; hBD-2), were measured in PBAL by enzyme linked immunosorbent assay (ELISA).</span><br><br><span><strong>Measurements and Main Results</strong>: </span><em><span>Aspergillus</span></em><span> dominated the PBAL samples in sarcoidosis. Differences in bacterial taxonomy were minor. There was no significant difference in fungal alpha diversity between sarcoidosis and controls, but the bacterial  alpha diversity in sarcoidosis was significantly lower in OW (p=0.047) and PBAL (p=0.03) compared with controls. The beta diversity for sarcoidosis compared with controls differed for both fungi and bacteria. AMP levels were significantly lower in sarcoidosis compared to controls (SLPI &amp; hBD-1: p&lt;0.01). No significant correlations were found between </span><span>a</span><span>-diversity and AMPs.</span><br><br><span><strong>Conclusions</strong>: </span><span>The pulmonary fungal and bacterial microbiota in sarcoidosis differed from controls, with lower antimicrobial peptides levels in sarcoidosis.</span></p>

Ultrasound evaluation of the airway in the ED: a feasibility study.

<p>INTRODUCTION Recognition of the difficult airway is a critical element of emergency practice. Mallampati score (MS) and body mass index (BMI) are not always predictive and they may be unavailable in critically ill patients. Ultrasonography provides high-resolution images that are rapidly obtainable, mobile, and non-invasive. Studies have shown correlation of ultrasound measurements with difficult laryngoscopy, however none have been performed in the Emergency Department (ED) using a consistent scanning protocol.</p> <p>OBJECTIVES This study seeks to determine the feasibility of ultrasound measurements of the upper airway performed in the ED by emergency physicians, the interrater reliability of such measurements, and their relationship with Mallampati score and BMI.</p> <p>METHODS A convenience sample of volunteer ED patients with no known airway issues, aged &gt;18 years, had images taken of their airway using a standardized ultrasound scanning protocol by 2 EM ultrasound fellowship trained physicians. Measurements consisted of tongue base, tongue base-to-skin, epiglottic width and thickness, and pre-epiglottic space.. Mean and standard deviation (SD) was used to summarize measurements. Inter-rater reliability was assessed by intraclass correlation coefficients (ICCs). Analysis of variance with linear contrasts was used to compare measurements with Mallampati scores and linear regression with BMI.</p> <p>RESULTS Of 39 participants, 50% were female, 50% white, 42% black, with median age 32.5 years (range 19-90) and BMI 26.0 (range 19-47). Mean ± SD for each measurement (mm) were as follows:  tongue base (44.6 ± 5.1), tongue base-to-skin (60.9 ± 5.3), epiglottic width (15.0 ± 2.8) and thickness (2.0 ± 0.37), and pre-epiglottic space (11.4 ± 2.4). ICCs ranged from 0.76-0.88 for all measurements except epiglottis thickness (ICC=0.57). Tongue base and tongue base-to-skin thickness were found to increase with increasing Mallampati score (p=.04, .01), whereas only tongue-to-skin thickness was loosely correlated with BMI (r=.38).</p> <p>CONCLUSION A standardized ultrasound scanning protocol demonstrates that the airway can be measured by emergency sonologists with good inter-operator reliability in all but epiglottic thickness. The measurements correlate with Mallampati score but not with BMI. Future investigation might focus on ultrasound evaluation of the airway in patients receiving airway management to determine whether ultrasound can predict challenging or abnormal airway anatomy prior to laryngoscopy.</p>

CUCO Database: A voice and speech corpus of patients who underwent upper airway surgery in pre- and post-operative states

<p>The data set comprises 3,800 speech audio files of 3 types of upper respiratory tract surgeries and 1 control set. The dataset has an average of <strong>35.51 +- 5.91</strong> audio recordings per patient. It provides valuable resources to the scientific community to systematically investigate the objective effects of upper respiratory tract surgery on voice and speech. &nbsp;</p> <p>This data set is a complete corpus comprising data from 107 Spanish Castilian speakers. This corpus encompasses voice and speech recordings from both control speakers and patients who underwent upper airway surgical procedures in pre- and post-operative stages. The surgeries in focus include <strong>Tonsillectomy</strong>, <strong>Functional Endoscopic Sinus Surgery</strong>, and <strong>Septoplasty</strong>, all consistently performed by a single surgeon.</p> <p>This corpus has been the basis for different previous studies to evaluate changes in voice and its quality due to surgery. The results do not suggest significant changes in the most relevant acoustic parameters studied for the voice, which is consistent with the initial hypothesis. However, the analysis of speech recordings remains open, with a special focus on the nasalised segments, which are expected to change due to surgical intervention.</p> <p>This data set also opens the way to study the effect of upper airway surgery on the performance of speaker recognition and identification methods, as well as to be used to test anti-spoofing methodologies to make them more robust.&nbsp;</p> <p>Please, if you use this database, cite this open-access paper where the data acquisition is explained and detailed:</p> <p>Hern&aacute;ndez-Garc&iacute;a, E., Guerrero-L&oacute;pez, A., Arias-Londo&ntilde;o, J. High-speed video microscopy analysis of cilia before and after airway cell culture

<p>High-speed video microscopy analysis (HSVA) is a diagnostic tool used within the UK Primary Ciliary Dyskinesia (PCD) Service to access airway ciliary function on nasal brushing biopsies.&nbsp; The Southampton PCD group is based at the University of Southampton, Faculty of Medicine and the&nbsp;University Hospital Southampton NHS Foundation Trust and is led by Professor Jane Lucas.&nbsp;&nbsp;We also use&nbsp;air-liquid interface (ALI) culture to differentiate airway epithelial cells to regrow healthy cilia to repeat&nbsp;standard PCD tests (including HSVA, immunofluoresence labelling of cilia proteins, transmission electron microscopy and functional genomics) and provide&nbsp;PCD research samples,&nbsp;which also allow us to develop new diagnostic approaches.&nbsp; ALI-culture can restore normal ciliary movement when secondary damage (due to infection or poor cell health) temporarily&nbsp;causes of abnormal cilia movement or a lack of cilia. ALI-culture can also&nbsp;re-confirm when ciliary defects and abnormal ciliary function are permanent and cause by inherited PCD (a ciliopathy).&nbsp;&nbsp;</p>

MicroCT scan of the nasal airway of a domestic short hair cat

<p>The peripheral structures of mammalian sensory organs often serve to support their functionality, such as alignment of hair cells to the mechanical properties of the inner ear. Here, we examined the structure-function relationship for mammalian olfaction by creating an anatomically accurate computational nasal model for the domestic cat (<em>Felis catus</em>) based on high resolution microCT and sequential histological sections. Our results showed a distinct separation of respiratory and olfactory flow regimes, featuring a high-speed dorsal medial stream that increases odor delivery speed and efficiency to the ethmoid olfactory region without compromising the filtration and conditioning purpose of the nose. These results corroborated previous findings in other mammalian species, which implicates a common theme to deal with the physical size limitation of the head that confines the nasal airway from increasing in length infinitely as a straight tube. We thus hypothesized that these ethmoid olfactory channels function as parallel coiled chromatograph channels, and further showed that the theoretical plate number, a widely-used indicator of gas chromatograph efficiency, is more than 100 times higher in the cat nose than an "amphibian-like" straight channel fitting the similar skull space, at restful breathing state. The parallel feature also reduces airflow speed within each coil, which is critical to achieving the high plate number, while feeding collectively from the high-speed dorsal medial stream so that total odor sampling speed is not sacrificed. The occurrence of ethmoid turbinates is an important step in the evolution of mammalian species that correlates to their expansive olfactory function and brain development. Our findings reveal novel mechanisms on how such structure may facilitate better olfactory performance, furthering our understanding of the successful adaptation of mammalian species, including <em>F. catus</em>, a popular pet, to diverse environments.</p>
