Modelling data for: Short-course combination treatment for experimental chronic Chagas disease

Chagas disease, caused by the protozoan parasite Trypanosoma cruzi, affects millions of people in the Americas and across the world leading to considerable morbidity and mortality. Current treatment options, benznidazole (BNZ) and nifurtimox, offer limited efficacy and often lead to adverse side effects due to long treatment durations. Better treatment options are therefore urgently required. Here we describe a pyrrolopyrimidine series, identified through phenotypic screening, that offers a clear opportunity to improve on current treatments. In vitro cell-based washout assays demonstrate that compounds in the series are incapable of killing all parasites, however, combining these pyrrolopyrimidines with a sub-efficacious dose of BNZ can clear all parasites in vitro after five days. Importantly, these findings were replicated in a clinically predictive in vivo model of chronic Chagas disease, where five days of treatment with the combination was sufficient to prevent parasite relapse. Comprehensive mechanism of action studies, supported by ligand-structure modelling, show that compounds from this pyrrolopyrimidine series inhibit the Qi active site of T. cruzi cytochrome b, part of the cytochrome bc1 complex of the electron transport chain. Knowledge of the molecular target enabled a cascade of assays to be assembled to evaluate selectivity over the human cytochrome b homologue. As a result, a highly selective and efficacious lead compound was identified. The combination of our lead compound with BNZ rapidly clears T. cruzi parasites, both in vitro and in vivo, and shows great potential to overcome key issues associated with currently available treatments.

Identifying biomarkers for chronic obstructive pulmonary disease in the salivary metabolome.

Background: The lack of specificity in spirometry and accuracy in ‘pre-disease states’, and elderly indicates the need for other accurate, inexpensive and non-invasive tests for diagnosis, triage severity and particularly for choosing and monitoring response to treatments in chronic obstructive pulmonary disease (COPD). We compared salivary metabolomic signatures from patients with COPD and controls across a range of severity of airflow obstruction.

Methods

45 people with COPD (mean age 66.1 ± 23 years, forced expiratory volume [FEV1] 47+15% predicted) and 48 healthy controls (mean age 56.0 +16.2 years, FEV1 90+23 % predicted) provided saliva that was assessed by flow infusion electrospray mass spectrometry (FIE-MS). Spectra were interrogated using an online library package. Data (patient data and metabolomic outputs) are available from this site.

Results

Four potential biomarkers identified the presence of COPD with a sensitivity of 73%, specificity of 72%. Six metabolites predicted FEV1 % in the COPD cohort (P < 0.001, R2 > 0.3, AUC > 0.7) whilst a range of multivariate approaches targeted six metabolites linked to GOLD stage (P < 0.001, AUC > 0.7). Hematopoietic Tumors in a Mouse Model of X-linked Chronic Granulomatous Disease after Lentiviral Vector-Mediated Gene Therapy

Chronic granulomatous disease (CGD) is a rare inherited disorder due to loss-of-function mutations in genes encoding the NADPH oxidase subunits. Hematopoietic stem and progenitor cell (HSPC) gene therapy (GT) using regulated lentiviral vectors (LVs) has emerged as a promising therapeutic option for CGD patients. We performed non-clinical Good Laboratory Practice (GLP) and laboratory-grade studies to assess the safety and genotoxicity of LV targeting myeloid specific Gp91phox expression in X-linked chronic granulomatous disease (XCGD) mice. We found persistence of gene-corrected cells for up to 1 year, restoration of Gp91phox expression and NADPH oxidase activity in XCGD phagocytes, and reduced tissue inflammation after LV-mediated HSPC GT.
Although most of the mice showed no hematological or biochemical toxicity, a small subset of XCGD GT mice developed
T cell lymphoblastic lymphoma (2.94%) and myeloid leukemia (5.88%). No hematological malignancies were identified in C57BL/6 mice transplanted with transduced XCGD HSPCs. Integration pattern analysis revealed an oligoclonal composition with rare dominant clones harboring vector insertions near oncogenes in mice with tumors. Collectively, our data support the long-term efficacy of LV-mediated HSPC GT in XCGD mice and provide a safety warning because the chronic inflammatory XCGD background may contribute to oncogenesis.

CHRONIC KIDNEY DISEASE SCREENING IN COMMUNITY PHARMACY: CRIERFAC PROJECT

Chronic kidney disease (CKD) is a major health problem. Early detection improves the prognosis of these patients, but the disease does not show its face until it is more advanced. The aim is to screen for the disease in community pharmacies in order to refer patients to primary care physicians.

Data from the 2116 patients recruited are presented in a single Excel table where each line corresponds to one patient. It contains patient demographics data, risk factors for chronic kidney disease, creatinine and glomerular filtration data, pharmacist intervention and physician response.
