Characterizing Subjects Exposed to Humidifier Disinfectants Using Computed Tomography-based Latent Traits: A Deep Learning Approach
<p>Around nine million people had been exposed to toxic humidifier disinfectants (HDs) in Korea. HD-exposure may lead to HD-associated lung injuries (HDLI). However, many people who claimed HD exposure were not diagnosed of HDLI but still felt discomfort possibly due to the unknown effects of HD. Therefore, this study examined HD-exposed subjects with normal appearing lungs as well as unexposed subjects in clusters (subgroups) with distinct characteristics classified by deep-learning-derived computed-tomography (CT)-based tissue-pattern latent traits. Among the major clusters, cluster 0 (C0) and cluster 5 (C5) were dominated by HD-exposed and unexposed subjects, respectively. C0 was characterized by features attributable to lung inflammation or fibrosis compared with C5. The computational fluid and particle dynamics (CFPD) analysis suggested that the smaller airway sizes observed in the C0 subjects led to greater airway resistance and particle deposition in the airways. Accordingly, women appeared more vulnerable to HD-associated lung abnormality than men.</p>

Low-cost, local production of a safe and effective disinfectant for resource-constrained communities
<p>Improved hygiene depends on the accessibility and availability of effective disinfectant solutions. These disinfectant solutions are unavailable to many communities worldwide due to resource limitations, among other constraints. Safe and effective chlorine-based disinfectants can be produced via simple electrolysis of salt water, providing a low-cost and reliable option for on-site, local production of disinfectant solutions to improve sanitation and hygiene. This study reports on a system (herein called "Electro-Clean") that can produce concentrated solutions of hypochlorous acid (HOCl) using readily available, low-cost materials. With just table salt, water, graphite welding rods, and a DC power supply, the Electro-Clean system can safely produce HOCl solutions (~1.5 liters) of up to 0.1% free chlorine (i.e.,1000 ppm) in less than two hours at low potential (5 V DC) and modest current (~5 A). Rigorous testing of free chlorine production and durability of the Electro-Clean system components, described here, has been verified to work in multiple locations around the world, including microbiological tests conducted in India and Mexico to confirm the biocidal efficacy of the Electro-Clean solution as a surface disinfectant. Cost estimates are provided for making HOCl locally with this method in the USA, India, and Mexico. Findings indicate that Electro-Clean is an affordable alternative to off-the-shelf commercial chlorinator systems in terms of first costs (or capital costs), and cost-competitive relative to the unit cost of the disinfectant produced. By minimizing dependence on supply chains and allowing for local production, the Electro-Clean system has the potential to improve public health by addressing the need for disinfectant solutions in resource-constrained communities.</p>

Data from: Stethoscope disinfection is rarely done in Ethiopia: what are the associated factors?
Introduction: The stethoscope, which is non-critical medical devices and a symbol of healthcare, is likely to be contaminated by pathogenic microorganisms and can play a contributory role in the transmission of hospital-acquired infection. And regular cleaning of the diaphragm of the stethoscope with a suitable disinfectant is decisive. However, in the resource-constrained setting like many healthcare facilities in Ethiopia healthcare provider's stethoscope disinfection practice and its associated factors have not been well studied so far. Therefore, this study sought to determine stethoscope disinfection practice and associated factors among the healthcare providers in Addis Ababa, Ethiopia. Methods: A facility-based cross-sectional study was carried out between April and May 2016. For this study, 576 healthcare providers (physicians, health officers, nurses, midwives, and anesthesiologist) were included from 21 healthcare facilities in Addis Ababa. A pre-tested structured questionnaire was used for data collection. Descriptive statistics were computed. Bivariate and multivariable logistic regression analyses were used to identify factors that were significantly associated with stethoscope disinfection after every use. Results: A total of 546 healthcare providers participated in this study, for a response rate of 94.7%. Two-fifths, 39.7% (95%CI: 35.9, 44.0%) of healthcare providers disinfecting their stethoscope after every use. And a significant number of participants 34.6% (95%CI: 30.8, 38.5%) never disinfect their stethoscope. Three out of four (76.0%) healthcare providers believe that stethoscope contamination can contribute to the transmission of infections. Safe infection prevention practice (AOR = 3.79, 95%CI: 2.45–5.84), awareness on infection prevention guideline (AOR = 1.93; 95%CI: 1.31, 2.82), and favorable attitude towards infection prevention (AOR = 1.73, 95%CI: 1.02, 2.93) were significantly associated with stethoscope disinfection after every use. The study also found that the odds of stethoscope disinfection were likely to be reduced by 79% among physicians than nurses (AOR = 0.21; 95%CI: 0.09, 0.49).Conclusions: Only a small proportion of healthcare providers disinfect their stethoscopes after every use. Factors such as safe infection prevention practice, awareness on infection prevention guidelines, and favorable attitude towards infection prevention were the independent predictors of stethoscopes disinfection after every use. Hence, promotion of stethoscope hygiene along with an educational program to enhance disinfection compliance in healthcare facilities may have a positive effect.
Taxonomic abundance of bacterial community of Bactrocera dorsalis as affected by antibiotics treatments and eggs disinfection
<p><span>In order to understand the role of symbionts for their insect hosts, it is customary to treat them with antibiotics or to sterilize eggs (treatments), resulting in aposymbiotic and axenic insects, respectively. Such axenic insects can then be compared to untreated controls. Fruit flies often bear complex communities which are greatly reduced by such treatments. However, the bacterial community is not completely eliminated. Here, we examine the effect of these procedures on the structure of the remaining bacterial communities of <i>Bactrocera dorsalis</i> (Diptera: Tephritidae) and on the insect longevity. The antibiotics (Norfloxacin and Ceftazedime) were administered to 1 day old adult flies through sugar meal for seven days, and eggs were surface sterilized and dechorionated to produce axenic lines. The flies were starved from protein before they were offered full diets or diets containing non-essential amino acids only. Antibiotic and egg disinfection treatments resulted in significant reduction of the vast majority of gut bacterial populations, especially Proteobacteria, Firmicutes and Bacteroidetes. On the other hand, it allowed the persistence of Actinobacteria, Cyanobacteria and Acidobacteria populations. In untreated control flies, longevity was extended irrespective of diet quality in comparison to treated flies. Conversely, when gut bacteria were largely reduced (aposymbiotic and axenic flies), longevity was reduced in the non-essential amino acids diet treatment versus slightly improved in the presence of a protein diet. We discuss these results in an ecological–life history perspective.</span></p>
