Outcomes and adverse events of pre- and extensively drug-resistant tuberculosis patients in Kinshasa, Democratique Republic of the Congo: retrospective cohort study

<p><strong>Abstract</strong></p> <p><strong>Background</strong>: Extensively drug-resistant tuberculosis (XDR TB) is a very serious form of tuberculosis that is burdened with a heavy mortality toll, especially before the advent of new TB drugs. The Democratic Republic of the Congo (DRC) is among the countries most affected by this new epidemic.</p> <p><strong>Methods:</strong> A retrospective analysis was performed of the records of all patients with pre- and extensively drug-resistant tuberculosis hospitalized from January 1, 2015 to December 31, 2017 and monitored for at least 6 months to one year after the end of their treatment in Kinshasa; an individualized therapeutic regimen with bedaquiline for 20 months was built for each patient. The adverse effects were systematically monitored.</p> <p><strong>Results:</strong> Of the 40 laboratory-confirmed patients, 32 (80%) patients started treatment, including 29 preXRB and 3 XDR TB patients. In the eligible group, 3 patients (9.4%) had HIV-TB coinfections. The therapeutic success rate was 53.2%, and the mortality rate was 46.8% (15/32); there were no relapses, failures or losses to follow-up. All coinfected HIV–TB patients died during treatment. The cumulative patient survival rate was 62.5% at 3 months, 53.1% at 6 months and 53.1% at 20 months. The most common adverse events were vomiting, Skin rash, anemia and peripheral neuropathy.</p> <p><strong>Conclusion: </strong>Bedaquiline based treatment improves patient survival in the DRC despite the still high mortality rate. The new anti-tuberculosis<br> drugs are a real hope for the management of Drug Resistant tuberculosis patient and other new therapeutic combinations may improve favorable outcomes.</p>

Data from: One and two year visual outcomes from the Moorfields age-related macular degeneration database: a retrospective cohort study and an open science resource

Objectives: To analyse treatment outcomes and share clinical data from a large, single-center, well-curated database (8174 eyes / 6664 patients with 120,756 single entries) of patients with neovascular age related macular degeneration (AMD) treated with anti-vascular endothelial growth factor (VEGF). By making our depersonalised raw data openly available, we aim to stimulate further research in AMD, as well as setting a precedent for future work in this area. Setting: Retrospective, comparative, non-randomised electronic medical record (EMR) database cohort study of the UK Moorfields AMD database with data extracted between 2008 and 2018. Participants: Including one eye per patient, 3357 eyes/patients (61% female). Extraction criteria were ≥ 1 ranibizumab or aflibercept injection, entry of "AMD" in the diagnosis field of the EMR, and a minimum of one year of follow-up. Exclusion criteria were unknown date of first injection and treatment outside of routine clinical care at Moorfields before the first recorded injection in the database. Main outcome measures: Primary outcome measure was change in VA at one and two years from baseline as measured in Early Treatment Diabetic Retinopathy Study (ETDRS) letters. Secondary outcomes were the number of injections and predictive factors for VA gain. Results: Mean VA gain at one-year and two years were +5.5 (95%CI:5.0,6.0) and +4.9 (95%CI:4.2,5.6) letters respectively. Fifty-four percent of eyes gained ≥5 letters at two years, 63% had stable VA (±≤14 letters), forty-four percent of eyes maintained good VA (≥70 letters). Patients received a mean of 7.7 (95%CI:7.6,7.8) injections during year one and 13.0 (95%CI:12.8,13.2) injections over two years. Younger age, lower baseline VA, and more injections were associated with higher VA gain at two years. Conclusion: This study benchmarks high quality EMR study results of real life AMD treatment and promotes open science in clinical AMD research by making the underlying data publicly available.

Data from: Adiposity is related to cerebrovascular and brain volumetry outcomes in the RUN DMC Study

Objective: Adiposity predictors, body mass index (BMI), waist circumference (WC), and blood leptin and total adiponectin levels were associated with components of cerebral small vessel disease (CSVD) and brain volumetry in 503 adults with CSVD age ≥50 years and enrolled in the Radboud University Nijmegen Diffusion tensor and Magnetic resonance imaging Cohort (RUN DMC). Methods: RUN DMC participants were followed for 9 years (2006-2015). BMI, WC, brain imaging and dementia diagnoses were evaluated at baseline and follow-up. Adipokines were measured at baseline. Brain imaging outcomes included CSVD components, white matter hyperintensities, lacunes and microbleeds; and gray and white matter, hippocampal, total brain, and intracranial volumes. Results: Cross-sectionally among men at baseline, higher BMI, WC and leptin were associated with lower gray matter and total brain volumes, and higher BMI and WC were associated with lower hippocampal volume. At follow-up 9 years later, higher BMI was cross-sectionally associated with lower gray matter volume, and an obese WC (&gt;102cm) was protective for &gt;1 lacune or &gt;1 microbleed in men. In women, increasing BMI and overweight or obesity (BMI &gt;25 kg/m2 or WC &gt;88 cm) were associated with &gt;1 lacune. Longitudinally, over 9 years, a baseline obese WC was associated with decreasing hippocampal volume particularly in men, and increasing WMH volume in women and men. Conclusions: Anthropometric and metabolic adiposity predictors were differentially associated with CSVD component and brain volumetry outcomes by sex. Higher adiposity is associated with a vascular-neurodegenerative spectrum among adults at-risk for vascular forms of cognitive impairment and dementias.

PREDICTORS OF POST-OPERATIVE OUTCOME IN EMERGENCY LAPAROTOMY FOR PERFORATION PERITONITIS, A PROSPECTIVE CROSS-SECTIONAL STUDY

<p>Introduction <span>Surgical mortality is high in hollow viscus perforation. Numerous efforts have been undertaken to preoperatively identify high-risk individuals and to optimize and manage these patients with greater vigour. To assess the predictors of outcome for patients undergoing emergency laparotomy due to perforation peritonitis, this study was conducted. Techniques: This prospective cross-sectional study will be carried out on patients with perforation peritonitis who are admitted to tertiary care hospitals' Department of General Surgery. We assessed the relationship between preoperative patient characteristics and mortality, anastomotic leakage, postoperative problems, and the necessity for intensive care unit (ICU) admission. The study concludes that patients receiving emergency laparotomy for perforation peritonitis are more likely to have high mortality after surgery if they have hypotension, azotemia, coagulopathy, or a delay in treatment. The identification of preoperative determinants of the requirement for ICU admission is aided by this investigation. Finding preoperative indicators linked to a higher risk of postoperative problems is made easier by this study.</span></p>

Breaking the ice to improve motor outcomes in patients with chronic stroke: a retrospective clinical study on neuromodulation plus robotics

<p>Stroke is one of the main causes of impairment affecting daily activities and quality of life. There is a growing effort to potentiate the recovery of functional gait and to enable stroke patients to walk independently. To estimate the effects of dual-site transcranial direct current stimulation (dstDCS) on gait recovery in chronic stroke patients provided with robot-aided gait training (RAGT). Thirty-seven patients were included in this retrospective clinical study. Nine patients were provided with dstDCS during the first 10 min of RAGT by using Lokomat&reg;Pro (on-RAGT), 15 patients immediately after RAGT (post-RAGT), and 13 patients immediately before RAGT (pre-RAGT). Each group improved over time concerning disability burden and lower limb strength. on-RAGT and post-RAGT experienced better improvement in balance (p&thinsp;&lt;&thinsp;0.001) and, moderately, gait endurance (p&thinsp;=&thinsp;0.04) as compared to pre-RAGT. Furthermore, all treatments decreased the facilitation of the unaffected hemisphere (p&thinsp;&lt;&thinsp;0.001) and the inhibition of the affected hemisphere (p&thinsp;&lt;&thinsp;0.001). The duration of such aftereffects was found to be greater for post-RAGT. This is the first trial with dstDCS coupled with RAGT in chronic stroke patients with gait impairment. When timely coupled with RAGT, dstDCS may be considered an effective tool for the recovery of lower limb function in patients with first unilateral stroke in the chronic phase. Moreover, our data suggest the ductility of dstDCS concerning RAGT timing, thus making this intervention suitable in a neurorehabilitation setting and well adaptable to patients&rsquo; needs.&nbsp;</p>

Covid-19 and Cancer Consortium (CCC19) breast cancer and racial disparities outcomes study

<p><span>Background</span>: Limited information is available for patients with breast cancer (BC) and coronavirus disease 2019 (COVID-19), especially among underrepresented racial/ethnic populations.</p> <p><span>Methods</span>: This is a COVID-19 and Cancer Consortium (CCC19) registry-based retrospective cohort study of females with active or history of BC and laboratory-confirmed severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) infection diagnosed between March 2020 and June 2021 in the US. Primary outcome was COVID-19 severity measured on a five-level ordinal scale, including none of the following complications, hospitalization, intensive care unit admission, mechanical ventilation, and all-cause mortality. Multivariable ordinal logistic regression model identified characteristics associated with COVID-19 severity.</p> <p><span>Results</span>: 1,383 female patient records with BC and COVID-19 were included in the analysis, the median age was 61 years, and median follow-up was 90 days. Multivariable analysis revealed higher odds of COVID-19 severity for older age (aOR per decade, 1.48 [95% CI, 1.32–1.67]); Black patients (aOR 1.74; 95 CI 1.24–2.45), Asian Americans and Pacific Islander patients (aOR 3.40; 95 CI 1.70–6.79) and Other (aOR 2.97; 95 CI 1.71–5.17) racial/ethnic groups; worse ECOG performance status (ECOG PS ≥2: aOR, 7.78 [95% CI, 4.83–12.5]); pre-existing cardiovascular (aOR, 2.26 [95% CI, 1.63–3.15])/pulmonary comorbidities (aOR, 1.65 [95% CI, 1.20–2.29]); diabetes mellitus (aOR, 2.25 [95% CI, 1.66–3.04]); and active and progressing cancer (aOR, 12.5 [95% CI, 6.89–22.6]). Hispanic ethnicity, timing, and type of anti-cancer therapy modalities were not significantly associated with worse COVID-19 outcomes. The total all-cause mortality and hospitalization rate for the entire cohort were 9% and 37%, respectively; however, it varied according to the BC disease status.</p> <p><span>Conclusions</span>: Using one of the largest registries on cancer and COVID-19, we identified patient- and BC-related factors associated with worse COVID-19 outcomes. After adjusting for baseline characteristics, underrepresented racial/ethnic patients experienced worse outcomes compared to Non-Hispanic White patients.</p>

The analysis of the gathered survey data provided strong evidence that the NRGF, including its process-guiding Web-platform (available at <a href="http://nanorigo.europeanprojects.net/platform/login">http://nanorigo.europeanprojects.net/platform/login</a>), represents a beneficial resource collection (data, tools, information) to support knowledge acquisition and enhancement of the understanding of a best practice approach of risk governance, going beyond conventional risk assessment and covering the whole life cycle of risk.</p> <p><strong>2_NanoRIGO_CS2 Setting up an In Vitro Aerosol Exposure Workplace</strong></p> <p>This case study aimed at testing whether the NanoRIGO risk governance framework (NRGF) is helpful for setting up a new workplace with potential hazards, obeying mandatory regulations and the principles of best practice. The test is valuated as successful if the facility setup and the definition of user practices was better, cheaper (<em>e.g.,</em> by faster and easier development) or safer than would have been possible without following the RGF guidance. For this case study PLUS applied as workplace a specific new facility, including the NAVETTA device for delivering airborne micro- and nanoparticles with high efficiency to lung cell cultures in an air-liquid interface (ALI) setup, using lateral flow and electrostatically enhanced deposition techniques.</p> <p><strong>3_NanoRIGO_CS3 Occupational Risk Management for Nanomaterials Production</strong></p> <p>The focus of the case study is to analyze, from the point of view of nanomaterial risk management, how useful is the NRGF framework developed, and its implementation in the NanoRIGO web platform. The scenarios considered have been defined according to key parameters such as: common types of processes in the handling of nanomaterials, most common types of nanomaterials and their characteristics, normal risk mitigation and containment conditions applied. In addition, risk communication tools have been tested.</p> <p><strong>4_NanoRIGO_CS4 Rubber Tyre Tear &amp; Wear </strong></p> <p>This case study tested the usefulness and added value of the NRGF with the real-life use of tyres: the use of manufactured nanomaterials in the rubber tyre tread and the environmental release of tyre wear particles (TWP) from this tread during car driving. With a focus on the release and hazards of nano-TWP, a selected group of real-life stakeholders defined an approach for a risk governance process, and a group of scientific experts assessed the usefulness of the available tools and methods.</p> <p><strong>5_NanoRIGO_CS5 Carbon Nanofibers in Production, Use and End-of-life</strong></p> <p>This case study focused on the production, use and end of life of two Carbon Nanofibers (CNFs) produced by Grupo Antolin. These CNFs are the only known industrially produced carbon nanofilaments grown by chemical vapour deposition based on the floating catalyst technique, using a Ni compound as catalyst. They are used in different applications: automotive, sport equipment, aeronautics, etc. Due to their high aspect ratio, they are usually grouped with carbon nanotubes thus, raising some concerns about their safety at production, use and end of life. The main goal of the Grupo Antolin case study is to verify the RGF in a real industrial setting, following the main steps defined for the NRGF.</p> <p><strong>6_NanoRIGO_CS6 TiO2 Risk Classification and Data Readiness</strong></p> <p>This case study aimed to showcase the Classification, Labelling and Packaging (CLP) classification process of TiO<sub>2</sub> as a way to test the applicability and operability of the NRGF: (i) how the NRGF could support the CLP regulatory process in the future, (ii) whether any steps of the NRGF are missing or not conducted optimally, and (iii) how a council-like body could participate in operationalizing the NRGF. This issue has been examined by applying the Data Readiness Levels (DRLs; referred to as Knowledge, information, and data Readiness Levels [KaRLs] here) elaborated within the project to each step of the NRGF process.</p> <p><strong>7_NanoRIGO_CS7 Economic Benefits from the Insurance Industry Perspective</strong></p> <p>This case study aimed at the insurance industry as they are clearly an important stakeholder focusing on the economic perspectives, <em>i.e.</em> benefits vs. cost analysis. The &ldquo;underwriting moment&rdquo; has a quasi-regulatory function. It may also reside in with the &ldquo;soft law&rdquo; mechanisms suggested by Marchant (2014, <a href="https://doi.org/10.1080/13669877.2014.889200">https://doi.org/10.1080/13669877.2014.889200</a>). The NRGF allows better information flows, more effective risk communication (which has been largely absent) and crucially a better understanding of liability regimes on the part the nanotechnology sector. These are all important advances from the perspective of the insurance industry.</p>

Studying the association between Gitcoin's issues and resolving outcomes

<p>This dataset provides a replication package of our paper submission titled &quot;Studying the association between Gitcoin bounties and issue-addressing outcomes&quot;.</p> <p><strong>Abstract</strong></p> <p>The development of open-source software (OSS) projects usually have been driven through collaborations among contributors and strongly relies on volunteering. Thus, allocating software practitioners (e.g., contributors) to a particular task is non-trivial and draws attention away from the development. Therefore, a number of bug bounty platforms have emerged to address this problem through bounty rewards. Especially, Gitcoin, a new bounty platform, introduces a bounty reward mechanism that allows individual issue owners (backers) to define a reward value using cryptocurrencies rather than using crowdfunding mechanisms. Although a number of studies have investigated the phenomenon on bounty platforms, those rely on different bounty reward systems. Our study thus investigates the association between the Gitcoin bounties and their outcomes (i.e., success and non-success). We empirically study over 4,000 issues with Gitcoin bounties using statistical analysis and machine learning techniques. We also conducted a comparative study with the Bountysource platform to gain insights into the usage of both platforms. Our study highlights the importance of factors such as the length of the project, issue description, type of bounty issue, and the bounty value, which are found to be highly correlated with the outcome of bounty issues. These findings can provide useful guidance to practitioners.</p>
