<p>Tuberculosis is the most common multi-systemic infection, with various types of manifestation and clinical description, lung is the most common location for developing tuberculosis disease (WHO, 2018). As many as 58% of cases of pulmonary TB occur in 3 (three) countries such as Southeast Asia. Based on profile Service Health Maluku Province, in 2021 number of invention Pulmonary TB cases Service Health (Health Office) of Maluku Province to end in 2019 it reached 6,379 people or by 0.35 percent of total population in the province the largest as 1.8 million people.This research uses a quantitative observational design with a cross sectional approach. With sample use systematic type Purposive sampling of 83 respondents. Data analysis using linear regression. Results test statistics show the most influential variable to level pulmonary tuberculosis treatment is a level indicator compliance with level significance sig 0.000. Based on the results of the Multiple Linear Regression analysis, it shows that with a p-value of 0.00 0 &lt; 0.05, H 1 is accepted, so it is concluded that there is a simultaneous influence quality of life as well as the level of compliance of TB lung sufferers And body mass index on post treatment tb lung at DR M Haulussy Ambon Regional Hospital.</p>

Whole blood RNA-seq demonstrates an increased host immune response in individuals with cystic fibrosis who develop nontuberculous mycobacterial pulmonary disease

<p><strong>Background </strong></p> <p>Individuals with cystic fibrosis have an elevated lifetime risk of colonization, infection, and disease caused by nontuberculous mycobacteria. A prior study involving non-cystic fibrosis individuals reported a gene expression signature associated with susceptibility to nontuberculous mycobacteria pulmonary disease (NTM-PD). In this study, we determined whether people living with cystic fibrosis who progress to NTM-PD have a gene expression pattern similar to the one seen in the non-cystic fibrosis population. <strong> </strong></p> <p><strong>Methods</strong></p> <p>We evaluated whole blood transcriptomics using bulk RNA-seq in a cohort of cystic fibrosis patients with samples collected closest in timing to the first isolation of nontuberculous mycobacteria. The study population included patients who did (n = 12) and did not (n = 30) develop NTM-PD following the first mycobacterial growth. Progression to NTM-PD was defined by a consensus of two expert clinicians based on reviewing clinical, microbiological, and radiological information. Differential gene expression was determined by DESeq2.</p> <p><strong>Results</strong></p> <p>No differences in demographics or composition of white blood cell populations between groups were identified at baseline. Out of 213 genes associated with NTM-PD in the non-CF population, only two were significantly different in our cystic fibrosis NTM-PD cohort. Gene set enrichment analysis of the differential expression results showed that CF individuals who developed NTM-PD had higher expression levels of genes involved in the interferon (α and γ), tumor necrosis factor, and IL6-STAT3-JAK pathways. <strong> </strong></p> <p><strong>Conclusion</strong></p> <p>In contrast to the non-cystic fibrosis population, the gene expression signature of patients with cystic fibrosis who develop NTM-PD is characterized by increased innate immune responses.</p>

Dataset for: Laser absorption spectroscopy measurements of different pulmonary oxygen gas concentrations in transmittance and remittance geometry – phantom study

<p><strong>Significance</strong></p> <p>GASMAS technique has the potential for continuous, clinical monitoring of pre-term infant lung function, removing the need to X-ray diagnosis and reliance on indirect and relatively slow measurement of blood oxygenation.</p> <p><strong>Aim</strong></p> <p>To determine optimal source-detector configuration for reliable path lengths calculation and to estimate the oxygen gas concentration inside the lung cavities filled with humidified gas with four different oxygen gas concentrations ranging between 21% and 100%.</p> <p><strong>Approach</strong></p> <p>Anthropomorphic optical phantoms of neonatal thorax with two different geometries were used to acquire Gas in Scattering Media Absorption Spectroscopy (GASMAS) signals, for 30 source-detector configurations in transmittance and remittance geometry of phantoms in two sizes.</p> <p><strong>Results</strong></p> <p>The results show that an internal light administration is more likely to provide a high GASMAS signal-to-noise ratio (SNR). In general, better SNRs were obtained with the smaller set of phantoms. The values of path length and O<sub>2</sub> concentrations calculated with signals from the phantoms with optical properties at 820 nm, exhibit higher variations than signals from the phantoms with optical properties at 764 nm.</p> <p><strong>Conclusion</strong></p> <p>The study shows that by moving the source and detector over the thorax, most of the lung volumes can potentially be assessed using GASMAS technique.</p>

Not only pulmonary rehabilitation for critically ill patients with COVID-19

<p><strong>Background</strong></p> <p>Prolonged immobility in COVID-19 patients in mechanical ventilation combined with the infection-mediated harmful immune response can be responsible of peripheral nervous system complications, such as Intensive Care Unit Acquired Weakness (ICU-AW). We report preliminary findings of our monocentric prospective study.</p> <p><strong>Method</strong></p> <p>Prospective observational study of PCR-confirmed COVID-19 patients in mechanical ventilation in the Intensive Care Unit of Azienda Ospedaliero Universitaria Careggi (Firenze).&nbsp;We evaluated: 1. muscle power grading on motor response to nociceptive stimuli by means of MRC; 2. nerve conduction velocities, compound muscle action potentials (CMAP) and sensory nerve action potentials (SNAP) of six motor and four sensitive nerves in bilateral upper and lower limbs; 3. spontaneous muscle activity in bilateral tibialis anterior and biceps by needle. The presence of combined critical illness polyneuropathy and myopathy (CIPNM) was defined by very low amplitude of CMAP and/or SNAP on neurophysiological evaluation with normal or mildly reduced nerve conduction velocities, combined with myopathic features on needle electromyography.&nbsp;Data about pre-existing comorbidities such as diabetes and hypertension were also collected.</p> <p><strong>Findings</strong></p> <p>Between March 23 March 2020 to 10 April 2020, we performed ENG studies in 9 patients. Based on clinical examination&nbsp;ICU-AW (MRC score &lt;48/60) was present in all 9 patients. Four of them had diagnosis of CIPNM at neurophysiological evaluation, and 3 of the remaining 5 patients presented neurophysiological findings consistent with common peroneal nerve compression</p> <p><strong>Interpretation</strong></p> <p>The present findings suggest that in critically ill patients affected by COVID-19 functional motor deficits are highly prevalent and should be searched for. ICU-AW have been found to negatively affect weaning from mechanical ventilation long-term outcome&nbsp;and increase in-hospital mortality of critical patients. Early recognition of these complications could help clinicians to plan an appropriate neuromotor rehabilitation (e.g. by early passive limbs mobilization and posture changes) for improving respiratory function and clinical outcome.</p> <p><strong>Funding</strong></p> <p>None</p> <p>&nbsp;</p>

Data from: Association between total dose of ritodrine hydrochloride and pulmonary edema in twin pregnancy: a retrospective cohort study in Japan

Objective Pulmonary edema is recognized as a severe side effect of ritodrine hydrochloride. Recently, the number of twin pregnancies has been increasing. Few studies have reported the association between total dose of ritodrine hydrochloride prior to delivery and pulmonary edema in twin pregnancy. We aimed to examine this association and determine the optimal cutoff threshold of total ritodrine hydrochloride dose to predict the incidence of pulmonary edema in twin pregnancy, based on obstetric records. Design Retrospective cohort study. Setting Yamanashi Prefectural Central Hospital, Japan Participants Two hundred twenty-six women with twin pregnancy who delivered at Yamanashi Prefectural Central Hospital between September 2009 and November 2016 Methods The obstetric records of the participants were analyzed. We defined one unit of ritodrine hydrochloride as 72 mg per 24 h continuous transfusion at 50 µg/min to calculate the dose of ritodrine used for tocolysis. Outcome measures Multivariable logistic regression analysis was performed to examine the association between total dose of ritodrine hydrochloride used for threatened preterm labor and pulmonary edema, while controlling for potential confounding factors. Then, a receiver-operating characteristic curve was used to determine the optimal cutoff of total ritodrine dose to predict pulmonary edema incidence. Results Mean maternal age was 32 (range, 18-46) years; 143 participants were nulliparous (63.3%), 109 had (48.2%) term deliveries, and 194 (85.8%) had cesarean deliveries. The overall incidence of pulmonary edema was 13.7% (31/226). Multivariable analysis showed that the total dose of ritodrine was significantly associated with pulmonary edema (adjusted odds ratio 1.02; 95% confidence interval, 1.004-1.03). The best cut-off point to predict the incidence of pulmonary edema was 26 units (1872 mg) (sensitivity, 61.3%; specificity, 87.8%). Conclusion Our results suggest that consideration of the total dose of ritodrine hydrochloride is helpful in the management of patients with threatened preterm labor in twin pregnancy.

Identifying biomarkers for chronic obstructive pulmonary disease in the salivary metabolome.

<p><strong><span>Background:</span></strong><span> The lack of specificity in spirometry and accuracy in &lsquo;pre-disease states&rsquo;, and elderly indicates the need for other accurate, inexpensive and non-invasive tests for diagnosis, triage severity and particularly for choosing and monitoring response to treatments in chronic obstructive pulmonary disease (COPD). We compared salivary metabolomic signatures from patients with COPD and controls across a range of severity of airflow obstruction. </span></p> <p><a name="_61k4y1plsv5"></a><strong><span>Methods</span></strong></p> <p><span>45 people with COPD (mean age 66.1 &plusmn; 23 years, forced expiratory volume [FEV<sub>1</sub>] 47<u>+</u>15% predicted) and 48 healthy controls (mean age 56.0 <u>+</u>16.2 years, FEV<sub>1</sub> 90<u>+</u>23 % predicted) provided saliva that was assessed by flow infusion electrospray mass spectrometry (FIE-MS). Spectra were interrogated using an online library package. Data (patient data and metabolomic outputs) are available from this site.&nbsp;</span></p> <p><a name="_j99pcjszp0an"></a><strong><span>Results</span></strong></p> <p><span>Four potential biomarkers identified the presence of COPD with a sensitivity of 73%, specificity of 72%.<span>&nbsp; </span>Six metabolites predicted FEV1 % in the COPD cohort (<em>P </em>&lt; 0.001, R<sup>2</sup> &gt; 0.3, AUC &gt; 0.7) whilst a range of multivariate approaches targeted six metabolites linked to GOLD stage (<em>P </em>&lt; 0.001, AUC &gt; 0.7). Identification of the metabolites suggested changes in pterin biosynthesis, lipid processing, nucleotide metabolism and melatonin in COPD patients. </span></p> <p><a name="_2n4ucvxs4l1n"></a><strong><span>Conclusions</span></strong></p> <p><span>Metabolic fingerprinting of saliva samples could differentiate COPD from age matched controls and inform COPD severity of airflow obstruction.
