Nerve ultrasound improves detection of treatment-responsive chronic inflammatory neuropathies

<p><b>Objective</b></p> <p>To examine the diagnostic accuracy of nerve ultrasound in a prospective cohort of consecutive patients with a clinical suspicion of chronic inflammatory neuropathies, including chronic inflammatory demyelinating polyneuropathy, Lewis Sumner syndrome and multifocal motor neuropathy, and to determine the added value in the detection of treatment-responsive patients.</p> <p><b>Methods</b></p> <p>Between February 2015 and July 2018, we included 100 consecutive incident patients with a clinical suspicion of chronic inflammatory neuropathy. All patients underwent nerve ultrasound, extensive standardized nerve conduction studies (NCS) and other relevant diagnostic investigations. We evaluated treatment response using predefined criteria. A diagnosis of chronic inflammatory neuropathy was established when NCS were abnormal (fulfilling criteria of demyelination of the EFNS/PNS) or when the degree of nerve enlargement detected by sonography was compatible with chronic inflammatory neuropathy and there was response to treatment.</p> <p><b>Results</b></p> <p>A diagnosis of chronic inflammatory neuropathy was established in 38 patients. Sensitivity and specificity of nerve ultrasound and NCS were 97.4% and 69.4%, and 78.9% and 93.5%, respectively. The added value of nerve ultrasound in detection of treatment-responsive chronic inflammatory neuropathy patients was 21.1% compared to NCS alone.</p> <p><b>Conclusions</b></p> <p>Nerve ultrasound and NCS are complementary techniques with superior sensitivity in the former and specificity in the latter. Addition of nerve ultrasound significantly improves the detection of chronic inflammatory neuropathies. Therefore, it deserves a prominent place in the diagnostic workup of chronic inflammatory neuropathies.</p> <p><b>Classification of Evidence</b></p> <p>This study provides Class I evidence that nerve ultrasound is an accurate diagnostic tool to detect chronic inflammatory neuropathies.</p>

Data from: Multiple simultaneous treatments change plant response from adaptive parental effects to within-generation plasticity, in Arabidopsis thaliana

In general, studies on plant phenotypic plasticity concentrate on plant responses to different levels of a single environmental factor. Under natural conditions, however, multiple environmental factors often vary simultaneously. I studied the consequences for lifetime fitness caused by single treatments or treatment combinations by investigating patterns of phenotypic plasticity within and between generations. The parental plants (3 genotypes of the annual plant Arabidopsis thaliana) received zero, one or two stress treatments at an early life-stage. The treatments included wounding, shading, chilling, and their pairwise combinations. In the second generation, offspring of treated plants received either the parental or no treatment. Offspring of non-treated plants were reared under all treatment conditions. Plants responded strongly to the treatments, especially through delayed reproduction, which positively affected lifetime fitness. Notably, treatment combinations triggered stronger plastic responses on average. Because the delay in reproduction was offset by a delay in senescence, the treatments resulted in a fitness gain instead of a loss. However, under adverse environmental conditions, this delay represents a potential fitness cost, especially when the time for reproduction is limited. The treatments wounding and shading triggered parental effects that increased fitness only in plants that themselves received the treatment. Untreated offspring of wounded or shaded parents performed like control plants. Also, these parental effects were not accompanied by potential fitness costs, such as delayed reproduction. Chilling triggered genotype-specific parental effects that increased or reduced fitness. Of the treatment combinations only wounding and shading resulted in genotype-specific parental effects that increased or reduced fitness independently of offspring treatment. These results suggest that the response of annual plants to treatment combinations triggers predominantly within-generation plastic responses that include potential fitness costs, which cannot be inferred from studies that manipulate environmental factors individually. Therefore, single treatment studies likely underestimate the costs of plasticity in natural environments.

Quantitative Expression of Latent Disease Factors in Individuals Associated with Psychopathology Dimensions and Treatment Response

<p>This repository&nbsp;contains preprocessed resting-state fMRI data&nbsp;including&nbsp;two datasets of obsessive-compulsive disorder (OCD): patients in baseline and&nbsp;patients with capsulotomy.&nbsp;The baseline dataset consists 104 OCD patients and 82 healthy controls&nbsp;were anonymously enrolled from our Institutional database. Part of the data from this cohort has been published previously [1, 2]. In this baseline dataset, 27 refractory patients underwent ventral capsulotomy with complete postoperative MRI scanning and clinical assessment.&nbsp;Exclusion/inclusion criteria, surgical procedure, and detailed clinical information were described in previously published work [1].&nbsp;Clinical assessment included Y-BOCS, HAM-A, and HAM-D (17-item version).&nbsp;The fMRI data were minimally preprocessed using Statistical Parametric Mapping; the preprocessing pipeline details&nbsp;were described in&nbsp;previous studies [1, 2].</p> <p>1.
