<p><strong>Introduction</strong></p> <p>&nbsp;Multiple sclerosis (MS) is a chronic inflammatory disease of the central nervous system (CNS). T helper (Th) 17 lymphocytes play a role in the pathogenesis of MS. Indeed, Th17 cells are abundant in the cerebrospinal fluid and peripheral blood of MS patients and promote pathogenesis in the mouse model of MS. To gain insight into the function of Th17 cells in MS, we tested whether Th17 cells polarized from na&iuml;ve CD4 T cells of healthy donors and MS patients display different features</p> <p><strong>Methods</strong></p> <p>We analysed several parameters that typify the Th17 profile during the differentiation process of na&iuml;ve CD4 T cells obtained from relapsing-remitting (RR)-MS patients and healthy donors (HD), such as expression of surface receptors by flow cytometry.</p> <p><strong>Results</strong></p> <p>The analysis revealed that expression of IL-1R1 is significantly increased in Th17 cells derived from MS patients compared to healthy donor-derived cells. Interestingly, IL-1R1 expression is also increased in Th17 cells circulating in the blood of MS patients compared to healthy donors. Since IL-1R1 play a crucial role in the activation of immune cells, our data indicate that high expression of these molecules in Th17 cells from MS patients could be related to their high inflammatory status.</p>

Data from: Neurological and neuroimaging findings in COVID-19 patients: a retrospective multicenter study

Objective To describe neuroimaging findings and to report the epidemiological and clinical characteristics of COVID-19 patients with neurological manifestations. Methods In this retrospective multicenter study (10 Hospitals), we included 64 confirmed COVID-19 patients with neurologic manifestations who underwent a brain MRI. Results The cohort included 43 men (67%), 21 women (33%), and the median age was 66 years (range: 20-92). 36 (56%) brain MRIs were considered abnormal, possibly related to SARS-CoV-2. Ischemic strokes (27%), leptomeningeal enhancements (17%), and encephalitis (13%) were the most frequent neuroimaging findings. Confusion (53%) was the most common neurological manifestation, following by impaired consciousness (39%), presence of clinical signs of corticospinal tract involvement (31%), agitation (31%), and headache (16%). The profile of patients experiencing ischemic stroke was different from the other patients with abnormal brain imaging since the former had less frequently acute respiratory distress syndrome (p=0·006) and more frequently corticospinal tract signs (p=0·02). Patients with encephalitis were younger (p=0·007), whereas agitation was more frequent for patients with leptomeningeal enhancements (p=0·009). Conclusions COVID-19 patients may develop a wide range of neurological symptoms, which can be associated with severe and fatal complications, such as ischemic stroke or encephalitis. Concerning the meningoencephalitis involvement, even if a direct effect of the virus cannot be excluded, the pathophysiology rather seems to involve an immune and/or inflammatory process given the presence of signs of inflammation in both cerebrospinal fluid and neuroimaging but the lack of virus in cerebrospinal fluid.

Data from: Long‐term follow‐up, quality of life and survival of Lambert‐Eaton myasthenic syndrome patients

Objective To study survival and characterize long-term functional impairments as well as health-related quality of life (HRQOL) of Lambert-Eaton myasthenic syndrome (LEMS) patients. Methods In this observational study, survival of LEMS patients, separately for non-tumor (NT) and small-cell lung cancer (SCLC), was compared to the Dutch general population and to patients with SCLC. Disease course in LEMS patients was recorded retrospectively. Several scales for functional impairments and health-related quality of life were assessed. Results We included 150 LEMS patients. Survival was similar to the general population in 65 NT-LEMS patients. Tumor survival was significantly longer in 81 SCLC-LEMS patients compared to non-LEMS SCLC patients (overall median survival 17 vs. 7.0 months, p&lt;0.0001). At diagnosis, 39 patients (62%) of 63 patients with complete follow-up data were independent for ADL activities, improving to 85% at 1-year follow-up. Physical HRQOL composite score (55.9) was significantly lower than in the general population (76.3, p&lt;0.0001) and comparable to myasthenia gravis (60.5) Mental HRQOL composite score was 71.8 in LEMS patients, comparable to the general population (77.9, p=0.19) and myasthenia gravis (70.3). Conclusions This study shows NT-LEMS patients have normal survival. SCLC-LEMS patients have an improved tumor survival, even after correcting for tumor stage. A majority of LEMS patients report a stable disease course and remain or become independent for self-care after treatment.

Data from: Intravenous thrombolysis in patients with chronic kidney disease: A systematic review and meta-analysis

<p><b><span>Objective</span></b><span> We sought to determine the association of chronic kidney disease (CKD) with the safety and efficacy of intravenous thrombolysis (IVT) among acute ischemic stroke (AIS) patients.</span></p> <p><b><span>Methods</span></b><span> Systematic review and pairwise meta-analysis of studies involving patients with CKD undergoing IVT for AIS were conducted to evaluate the following outcomes: symptomatic intracranial hemorrhage (sICH), asymptomatic and any ICH, in-hospital and 3-month mortality, </span>3-month favorable functional outcome (FFO, mRS 0-1) and <span>3-month functional independence (FI, mRS 0-2). CKD was defined using estimated glomerular filtration rate (eGFR) ranging from mild (eGFR: 60-89ml/min), moderate (eGFR: 30-59ml/min) and severe (eGFR: 15-29ml/min). </span></p> <p><b><span>Results</span></b><span> We identified 20 studies comprising </span>60,486 AIS <span>patients treated with IVT. In unadjusted analyses, CKD was associated with </span>sICH according to NINDS (7 studies; OR=1.41, 95%CI: 1.19–1.67) and ECASS-II (9 studies; OR=1.37, 95%CI: 1.01–1.85) definitions, any ICH (8 studies; OR=1.42, 95%CI: 1.18–1.70), 3-month mortality (9 studies; OR = 2.20, 95%CI: 1.72–2.81)<span>, </span>3-month FFO (8 studies; OR=0.58, 95%CI: 0.47–0.72) and <span>3-month FI</span> (8 studies; OR=0.57, 95%CI: 0.46–0.71). In adjusted analyses, <span>CKD</span> was associated with sICH according to NINDS (4 studies; OR<sub>adj</sub>=1.34, 95%CI: 1.01–1.79) and ECASS-II (3 studies; OR<sub>adj</sub>=2.08, 95%CI 1.27–3.43) definitions, any ICH (6 studies; OR<sub>adj</sub>=1.41, 95%CI 1.01–1.97), in-hospital mortality (2 studies; OR<sub>adj</sub> =1.19; 95%CI, 1.09-1.30) and 3-month FFO (6 studies; OR<sub>adj</sub>=0.80, 95%CI 0.70–0.92).  </p> <p><b><span>Conclusions</span></b><span> After adjustment for confounders in this pairwise meta-analysis, moderate-severe CKD is associated with increased risks of intracranial hemorrhage and worse functional outcomes among AIS patients treated with IVT. </span></p>

Intensive care unit patients' opinion on enrollment in clinical research: a multicenter survey - Study data bank

<p><span><span><span><span><span><span><span><span><span><span><span><b>Background:</b> In most emergency situations or severe illness, patients are unable to consent for clinical trial enrollment. In such circumstances, the decision about whether to participate in a scientific study or not is made by a legally designated representative.</span></span></span></span></span></span></span></span></span></span></span></p> <p><span><span><span><span><span><span><span><span><span><span><span><b>Objective: </b>To address the willingness of patients admitted to the intensive care unit (ICU) to be enrolled in a scientific study as volunteers, and to assess the agreement between patients' and their legal representatives' opinion concerning enrollment in a scientific study.</span></span></span></span></span></span></span></span></span></span></span></p> <p><span><span><span><span><span><span><span><span><span><span><span><b>Methods:</b><b> </b>This survey was conducted in two hospitals in São Paulo, Brazil. Patients (≥18 years) with preserved cognitive functions accompanied by a surrogate admitted to the ICU were eligible for this study. A survey containing 28 questions for patients and 8 questions for surrogates was applied within the first 48h from ICU admission. The survey for patients comprised three sections: demographic characteristics, opinion about participation in clinical research and knowledge about the importance of research. The survey for legal representatives contained two sections: demographic characteristics and assessment of legal representatives' opinion in authorizing patients to be enrolled in research.</span></span></span></span></span></span></span></span></span></span></span></p> <p><span><span><span><span><span><span><span><span><span><span><span><b>Results:</b><b> </b>Between January 2017 and May 2018, 208 pairs of ICU patients and their respective legal representatives answered the survey. Out of 208 ICU patients answering the survey, 73.6% (153/208) were willing to be enrolled in the study as volunteers. Of those patients, 65.1% (97/149) would continue participating in a research even if their legal representative did not support their enrollment.  Agreement between patients' and surrogates' opinion concerning participation was poor [Kappa=0.11 (IC95% -0.02 to 0.25)]. If a consent for study participation had been obtained, 69.1% (103/149) of patients would continue participating in the study until its conclusion, and 23.5% (35/149) would allow researchers to use data collected to date, but would withdraw from the study on that occasion.</span></span></span></span></span></span></span></span></span></span></span></p> <p><span><span><span><span><span><span><span><span><span><span><span><b>Conclusion:</b><b> </b>The majority of patients admitted to the ICU were willing to be enrolled in a scientific study as volunteers, also after a deferred informed consent procedure has been used. Nevertheless, contradictory opinions between patients and their and their legal representatives' concerning enrollment in a scientific study were often observed.</span></span></span></span></span></span></span></span></span></span></span></p>

Data from: A systematic review of comorbidities and outcomes of adult patients with pleural infection

Background Pleural infection remains an important cause of mortality. This study aimed to investigate worldwide patterns of pre-existing comorbidities and clinical outcomes of patients with pleural infection. Methods Studies reporting on adults with pleural infection between 2000-2017 were identified from a search of Embase and Medline. Papers reporting exclusively on tuberculous, fungal or post pneumonectomy infection were excluded. Two reviewers assessed 20980 records for eligibility. Results 211 studies met the inclusion criteria. 134 papers (227,898 patients, mean age 52.8 years) reported comorbidity and/or outcome data. The majority of studies were retrospective observational cohorts (n=104, 78%) and the most common region of reporting was East Asia (n=33, 24%) followed by North America (n=27, 20%). 85 papers (50,756 patients) reported comorbidity. The median percentage prevalence of any comorbidity was 72% (IQR 58-83%), with respiratory illness (20%, 16-32%) and cardiac illness (19%, 15-27%) most commonly reported. 125 papers (192,298 patients) reported outcome data. The median length of stay was 19 days (IQR 13-27) and median in-hospital or 30-day mortality was 4% (IQR 1-11%). In regions with high-income economies (n=100, 74%) patients were older (mean 56.5 vs. 42.5 years, p&lt;0.0001), but there were no significant differences in prevalence of pre-existing comorbidity nor in length of hospital stay or mortality. Conclusion Patients with pleural infection have high levels of comorbidity and long hospital stays. Most reported data are from high-income economy settings. Data from lower-income regions is needed to better understand regional trends and enable optimal resource provision going forward.
