Clinical usefulness of library and information services in Japan: The detailed use and value of information in clinical settings

Abstract

Objectives

Considering that there is a lack of evidence regarding the contribution of library and information services to evidence-based medicine in actual clinical practice in Japan, the purpose of the study is to explore the current status of use and value of library and information services in clinical settings to examine the usefulness of information in implementing evidence-based medicine (EBM) into practice.

Methods

A Web-based survey was conducted at seven sites (hospitals with 300–1,200 beds) and interviews conducted at five sites to investigate information behavior among health professionals (physicians, residents, and nurses) in 2016, replicating the Value Study carried out in the United States in 2010 and 2011. Using a critical incident technique, respondents answered questions about their information topics, information resources used, search location, access points, and evaluation of the information.

Results

Analysis from 598 valid responses (275 physicians, 55 residents, and 268 nurses) revealed the characteristics of information use and recognition of the value of information. Physicians and residents showed their information needs regarding clinical care using PubMed (80.4%, 65.5%), Ichushi-Web (61.8%, 63.6%), and UpToDate (40.4%, 65.5%). While physicians rely more on electronic journals (37.8%), residents use more hybrid resources including Japanese print books (38.2%) and online books (30.9% for Japanese, 32.7% for English) to confirm their knowledge. Nurses need more information close to patients and explore a wider variety of information resources such as Japanese print books (60.4%), Ichushi -Web (40.3%), Japanese online books (20.5%), and websites of academic organizations (19.0%). Although the overall recognition of the value of information was relatively modest, concrete changes in clinical practice were found in some areas. Environments with insufficient information and availability of electronic resources should be improved to increase the use of library and information services for implementing EBM.

Dataset - Assessing the Added Value of the Intermediate Complexity Atmospheric Research Model (ICAR) for Precipitation in Complex Topography

Abstract. The coarse grid spacing of global circulation models necessitates the application of downscaling techniques to investigate the local impact of a changing global climate. Difficulties arise for data sparse regions in complex topography which are computationally demanding for dynamic downscaling and often not suitable for statistical downscaling due to the lack of high quality observational data. The Intermediate Complexity Atmospheric Research Model (ICAR) is a physics-based model that can be applied without relying on measurements for training and is computationally more efficient than dynamic downscaling models. This study presents the first in-depth evaluation of multi-year precipitation time series generated with ICAR on a 4 × 4 km2 grid for the South Island of New Zealand for an eleven-year period, ranging from 2007 until 2017. It focuses on complex topography and evaluates ICAR at 16 weather stations, eleven of which are situated in the Southern Alps between 700 m MSL and 2150 m MSL. ICAR is assessed with standard skill scores and the effect of model top elevation, topography, season, atmospheric background state and synoptic weather patterns on these scores are investigated. The results show a strong dependence of ICAR skill on the choice of the model top elevation, with the highest scores obtained for 4 km above topography. Furthermore, ICAR is found to provide added value over its ERA-Interim reanalysis forcing data set for alpine weather stations, improving mean squared errors (MSE) by up to 53 % and 30 % on median. It performs similarly during all seasons with an MSE minimum during winter, while flow linearity and atmospheric stability were found to increase skill scores. ICAR scores are highest during weather patterns associated with flow perpendicular to the Southern Alps and lowest for flow parallel to the alpine range. While measured precipitation is underestimated by ICAR, these results show the skill of ICAR in a real-world application, and may be improved upon by further observational calibration or bias correction techniques.
Based on these findings ICAR shows the potential to generate downscaled fields for long term impact studies in data sparse regions with complex topography.

Data for: Beyond signal quality: The value of unmaintained pH, dissolved oxygen, and oxidation-reduction potential sensors for remote performance monitoring of on-site sequencing batch reactors

Sensor maintenance is time-consuming and is a bottleneck for monitoring on-site wastewater treatment systems. Hence, we compare maintained and unmaintained sensors to monitor the biological performance of a small-scale sequencing batch reactor (SBR). The sensor types are ion-selective pH, optical dissolved oxygen (DO), and oxidation-reduction potential (ORP) with platinum electrode. We created soft sensors using engineered features: ammonium valley for pH, oxidation ramp for DO, and nitrite ramp for the ORP. Four soft sensors based on unmaintained pH sensors correctly identified the completion of the ammonium oxidation (89 to 91 out of 107 cycles), about as many times as soft sensors based on a maintained pH sensor (91 out of 107 cycles). In contrast, the DO soft sensor using data from a maintained sensor showed slightly better (89 out of 96 cycles) detection performance than that using data from two unmaintained sensors (77, respectively 82 out of 96 correct). Furthermore, the DO soft sensor using maintained data is much less sensitive to the optimisation of cut-off frequency and slope tolerance than the soft sensor using unmaintained data. The nitrite ramp provided no useful information on the state of nitrite oxidation, so no comparison of maintained and unmaintained ORP sensors was possible in this case. We identified two hurdles when designing soft sensors for unmaintained sensors: i) Sensors' type- and design-specific deterioration affects performance. ii) Feature engineering for soft sensors is sensor type specific, and the outcome is strongly influenced by operational parameters such as the aeration rate. In summary, the results with the provided soft sensors show that frequent sensor maintenance is not necessarily needed to monitor the performance of SBRs. Without sensor maintenance monitoring smalls-scale SBRs becomes practicable, which could improve the reliability of unstaffed on-site treatment systems substantially.
