COVID – 19 Pandemics: Impact, Interventions, Progress and Future Direction

Though the global health and socioeconomic crisis created by COVID–19withitsrapid spreading nature, high infectivity and fatality rate leading to impairment in human health, psychosocial life and world economy, mass fear of COVID – 19 (corona-phobia) appears forgotten in many countries of the world. Yet consciously or unconsciously the high economic burden, mass emotional distress and abundance of psychiatric manifestations are still being felt including the fear of future similar occurrence. This review investigated the health, psycho-socioeconomic impact and interventions aimed at addressing the menace forced upon the globe by COVID- 19 in order to bring to the fore, the future direction in combating any epidemic or pandemic in the world. We searched for literatures in PubMed, Google, Google scholar etc. using key words: COVID – 19, Pandemic, psycho-socioeconomic, Technology-utilization, Telemedicine for articles addressing the above subject matter. From over a thousand articles on COVID -19, we selected 58 articles that addressed the subject matter with global, regional and local perspectives included and utilizing same in writing this paper. We conclude that future pandemics can be tackled effectively by adopting a three-pronged approach: - public health, early preparedness, and involve-ment of the entire society as a unitary force.

Data from: HLA-G 14 bp variant is associated with exercise-induced oxygen desaturation in the post-COVID-19 condition

The HLA-G molecule has been related to the lung inflammatory process in respiratory diseases. A 14 pb insertion/deletion in the gene encoding HLA-G has been associated with differential levels of the HLA-G molecule in plasma and with the severity of infectious diseases. Therefore, this study evaluates the association of the HLA-G variant 14 pb insertion/deletion (rs371194629) with COVID-19 susceptibility, severity, and oxygen desaturation in the post-COVID condition. We included 621 patients with severe COVID-19 hospitalized in a third-level hospital; 520 subjects Non-COVID (recruited previous to the COVID-19 pandemic); and 101 patients that were followed after discharge of COVID-19 (46.4% presenting exercise-induced oxygen desaturation). The severity of COVID-19 was evaluated through the invasive mechanical ventilation (IMV) requirement. We found that the DD genotype was more frequent among patients requiring IMV than non-IMV (0.372 vs 0.290, p= 0.046, OR =1.45, 95%CI = 1.01-2.08, dominant model). This genotype was also associated with exercise-induced oxygen desaturation in post-COVID patients (0.435 vs 0.196, p=0.011, OR=3.15, CI 95%=1.28-7.79, dominant model). The allele and genotype frequencies were similar when COVID-19 and non-COVID subjects were compared. Thus, the DD genotype of rs371194629 HLA-G was associated with COVID-19 severity and post-COVID condition.

Temporal variability of microparticles under the Seattle Aquarium, WA: Documenting the global Covid‐19 pandemic

Anthropogenic debris including microparticles (MP; <5mm) are ubiquitous in marine environments. The Salish Sea experiences seasonal fluctuations in precipitation, river discharge, sewage overflow events, and tourism– all variables previously thought to have an impact on MP transport and concentrations. Our goals are two-fold: 1) Describe long-term MP contamination data including concentration, type, and size and 2) Determine if seasonal MP concentrations are dependent on environmental or tourism variables in Elliott Bay, Salish Sea. We sampled 100 L of seawater at depth (~9 m) at the Seattle Aquarium approximately every two weeks 2019 – 2020 and used an oil extraction protocol to separate MP. We found MP concentrations ranged from 0 – 0.64 particles L⁻¹ and fibers were the most common type observed. Microparticle concentration exhibited a breakpoint on April 10, 2020, where estimated slope and associated MP concentration significantly declined. Further, when considering both environmental as well as tourism variables, temporal MP concentration was best described by a mixed-effects model with tourism as the fixed effect and the person counting MP as the random effect. While monitoring efforts presented here set out to identify effects of seasonality and interannual differences in MP concentrations, it instead captured an effect of decreased tourism due to the global Covid-19 pandemic. Long-term monitoring is critical to establish temporal MP concentrations and to help researchers understand if there are certain events, both seasonal and sporadic (e.g. rain events, tourism, or global pandemics), when the marine environment is more at risk from anthropogenic pollution.

Associated factors of Burnout Syndrome in Peruvian health professionals before Covid 19 pandemic: A systematic review

Introduction: The Burnout Syndrome (BS) constitutes a prevalent occupational health problem in the group of health professionals.

Objective: To describe the prevalence and factors associated with BS in Peruvian health professionals.

Method: A systematic review and meta-analysis of the literature were accomplished. Key terms were used: "burnout", "professional exhaustion", with words related to Peru. The databases consulted were LILACS/ Virtual health library, Medline/PubMed, Science Direct, EBSCO, Scopus, SciELO, and RENATI-SUNEDU, published from 2000 to 2020. The methodological quality assessment was accomplished using the Newcastle Ottawa instrument.

Results: 33 studies were identified (11 scientific articles and 22 graduate theses), the median of the participants included in each study was 76 (IQR: 47-106). The median total score was 55.8 points (IQR: 44-61). The meta-analysis identified the global prevalence of moderate BS by subgroups of regions of Peru, it was 53% (95% CI: 36% to 70%); by profession 54% (95% CI: 28% to 79%); by general areas 61% (95% CI: 39% to 70%) and emergency 40% (95% CI: 18% to 64%). Most of the factors associated with BS are related to work (work environment, dysfunctional group behavior, time of service, satisfaction, and workload); and sociodemographic (age, sex, and profession).

Conclusions: The BS and its three dimensions are more frequently associated with work, sociodemographic and individual factors. SB and the dimensions of emotional exhaustion and depersonalization are associated with factors outside of work. It is the first systematic review of the subject in Peru, being useful to generate proposals for public policies in occupational health and health intervention programs.
