Comprehensive assessment of postoperative mobility during the first days after mini-invasive lung surgery: A prospective observational study

Study Objective: Postoperative physical therapy and early mobilization are major elements of enhanced recovery after surgery. In contrast with supervised physical therapy sessions that can be monitored, self-mobilization is not easily quantifiable and has so far been estimated mainly through patient auto-reports. This study aimed to perform a comprehensive and objective evaluation of postoperative mobility.

Design: Prospective observational study.

Setting: Postoperative setting.

Patients: Patients undergoing mini-invasive lung surgery.

Interventions: Measurement of postoperative mobility during the five first postoperative days using an accelerometer (ActiGraph GT3X).

Measurements: The primary outcome was the number of daily steps. Secondary outcomes included physical activity duration and intensity, sedentary time, number of breaks in sedentary time, sedentary patterns, daily evaluation by physiotherapists, postoperative complications, and acceptability of wearing the accelerometer.

Main Results: Sixty patients were included in the study, of whom 56 provided at least one day of valid accelerometry data. There was no significant change during the first four PODs concerning the number of daily steps nor the mean cadence. One-minute cadence peak, total activity counts, and duration of light-intensity physical activity increased over time (P = 0.032, P = 0.001 and P = 0.001, respectively). Sedentary patterns changed favorably over time, with a decrease in prolonged sedentary bouts (≥ 60 consecutive min), and an increase in shorter bouts (< 10 min) (P < 0.001). Similar results were observed when analysis was adjusted for the day of the week when the surgery took place except for daily sedentary time (P = 0.055). The median acceptability of wearing the accelerometer was excellent (median 10 [9-10] on a 10-point Likert scale). Three patients had major complications.

Conclusions: Our findings suggest that daily steps may not be the most relevant indicator of early mobility following thoracic surgery and that accelerometry is suitable to follow patient's early postoperative activity.

EFFECTIVENESS OF PELVIC FLOOR EXERCISE TO PREVENT LARS (LOW ANTERIOR RESECTION SYNDROME) AFTER MINI-INVASIVE LOW ANTERIOR RESECTION IN PATIENTS WITH RECTAL CANCER. Adherence to prescribed home exercise. Quality of life after low anterior resection.

Advances in the surgical treatment of rectal diseases lead to better oncological results, a higher chance of preserving the sphincters, and thus a lower number of permanent stomas. However, the preserved anus does not always have to perform its original function fully. All patients after a low anterior resection are at risk of developing functional disorders - low anterior resection syndrome (LARS). The prevalence of LARS ranges from 41-80% and is a significant factor in reducing the quality of life. According to the available data, it is possible to prevent LARS through postoperative pelvic floor exercises, however, relevant studies are missing. The pelvic floor is a ligament-muscle system that provides dynamic support for the organ systems located in the small pelvis. Strengthening the muscles can serve as a follow-up treatment after surgical procedures including prevention of LARS.

The main goal of this randomized study will be to determine the effectiveness of pelvic floor exercises on the incidence or severity of LARS in patients after mini-invasive rectal resection. Secondary goal is to identify the adherence of patients to prescribed home exercise and quality of life.

Researchers will compare the group of patients with pelvic floor exercises to those without. Under the professional guidance of a physiotherapist, participants will be educated the day before surgery and in the first 4 postoperative days to exercise the pelvic floor exercise and continue at home for a month. The resulting knowledge will have a fundamental impact on clinical practice and patient management.

Mini Nutritional Assessment tool validation project

Background: The health status of older people is usually overlooked in many low-income countries like Ethiopia. Appropriate nutritional assessment improves the health of old age people. The Mini Nutritional Assessment tool is a noninvasive and cheap practical evaluation tool that provides a simple and quick method of evaluating the nutritional status of old age people. The tool has multiple versions of confirmed validity in diverse languages, except in Amharic. Furthermore, the tool has still not been properly validated for Ethiopian old age people.

Objective: The purpose of this study was to translate the Mini Nutritional Assessment tool into the Amharic language and validate it among old age people in Bahir Dar City.

Methods: This tool translation and validation study followed normal COSMIN Study design and reporting guidelines. The study was conducted in three stepwise phases from January 16 to March 13, 2021. The first phase was reviewing and selecting nutritional assessment tools for old age people. In the second phase, the selected Mini Nutritional Assessment tool was translated and reviewed by experts. Using the heterogenous purposive sample, ten healthcare specialists with professional experience in the care of old age people were chosen for this step. The experts examined the face and content validity of the Amharic version of the instrument in two rounds of the Delphi technique. Finally, after incorporating the experts' suggestions and comments, a cross-sectional study was conducted among old age people for psychometric validation. The participant-to-variables ratio of the 10:1 rule of thumb was followed for the minimum sample size. Since the Mini Nutritional Assessment tool has 18 items, 180 community-dwelling old-age people were selected in multistage cluster sampling from Belay Zeleke, one of the sub-cities of Bahir Dar City. Principal component analysis was used to measure construct validity while Cronbach's alpha was employed to assess internal consistency reliability.

Results: As experts reviewed, all items in the translated tool are socially acceptable and have no taboo or sensitive words. The translated tool's content validity ratio was 0.93, and its scale validities (S-CVI/Ave and S-CVI/AU) were 0.97 and 0.83, respectively. Moreover, 180 community-dwelling old age people aged 60 to 90 years old participated in a psychometric evaluation study. Construct validity of the tool was confirmed with factor loadings ranging from 0.47 to 0.89 with a Cronbach's alpha of 0.65. The tool had a sensitivity and specificity of 97% and 83%, respectively.

Conclusion: The Amharic version of the Mini Nutritional Assessment tool showed good cross-cultural adaption, internal consistency reliability, and construct validity in Bahir Dar community-dwelling old age people. The tool can be used in regular care activities for aged people.

Monitoring mesophotic scleractinian corals using an underwater mini-ROV to sample eDNA

Mesophotic coral ecosystems (MCEs) are light-dependent tropical or subtropical communities occurring at depths of 30 to 150 m. We recently devised a coral-specific environmental DNA (eDNA) barcoding method that can identify 36 scleractinian genera in shallow reefs by sampling ~1L of surface seawater. If eDNA barcoding is combined with sampling using underwater mini-Remote Operated Vehicles (mini-ROVs), it may be possible to survey mesophotic corals more easily and broadly. Around the Zamami Islands, in Okinawa, Japan, seawater was collected 1–2 m above the bottom at six locations 20–80 m below the surface and subjected to coral-specific eDNA amplification. Metabarcoding analyses showed that (a) eDNA from ~0.5 L seawater was sufficient to identify genera and to yield comparative ratios of genera at these sites; (b) Acropora dominates shallow reefs and upper ridges of slopes, while other genera including Porites, Pocillopora and Polyphyllia are more abundant at mesophotic sites; (c) one site showed a gradient in which Acropora was replaced by Plesiastrea at increasing depths. Although further technical improvements are required, the use of eDNA and underwater mini-ROVs may permit monitoring of mesophotic corals more broadly and easily.
