Not only pulmonary rehabilitation for critically ill patients with COVID-19

<p><strong>Background</strong></p> <p>Prolonged immobility in COVID-19 patients in mechanical ventilation combined with the infection-mediated harmful immune response can be responsible of peripheral nervous system complications, such as Intensive Care Unit Acquired Weakness (ICU-AW). We report preliminary findings of our monocentric prospective study.</p> <p><strong>Method</strong></p> <p>Prospective observational study of PCR-confirmed COVID-19 patients in mechanical ventilation in the Intensive Care Unit of Azienda Ospedaliero Universitaria Careggi (Firenze).&nbsp;We evaluated: 1. muscle power grading on motor response to nociceptive stimuli by means of MRC; 2. nerve conduction velocities, compound muscle action potentials (CMAP) and sensory nerve action potentials (SNAP) of six motor and four sensitive nerves in bilateral upper and lower limbs; 3. spontaneous muscle activity in bilateral tibialis anterior and biceps by needle. The presence of combined critical illness polyneuropathy and myopathy (CIPNM) was defined by very low amplitude of CMAP and/or SNAP on neurophysiological evaluation with normal or mildly reduced nerve conduction velocities, combined with myopathic features on needle electromyography.&nbsp;Data about pre-existing comorbidities such as diabetes and hypertension were also collected.</p> <p><strong>Findings</strong></p> <p>Between March 23 March 2020 to 10 April 2020, we performed ENG studies in 9 patients. Based on clinical examination&nbsp;ICU-AW (MRC score &lt;48/60) was present in all 9 patients. Four of them had diagnosis of CIPNM at neurophysiological evaluation, and 3 of the remaining 5 patients presented neurophysiological findings consistent with common peroneal nerve compression</p> <p><strong>Interpretation</strong></p> <p>The present findings suggest that in critically ill patients affected by COVID-19 functional motor deficits are highly prevalent and should be searched for. ICU-AW have been found to negatively affect weaning from mechanical ventilation long-term outcome&nbsp;and increase in-hospital mortality of critical patients. Early recognition of these complications could help clinicians to plan an appropriate neuromotor rehabilitation (e.g. by early passive limbs mobilization and posture changes) for improving respiratory function and clinical outcome.</p> <p><strong>Funding</strong></p> <p>None</p> <p>&nbsp;</p>

Data from: "Dead birds flying": Can North American rehabilitated raptors released into the wild mitigate anthropogenic mortality?

<p>As the human footprint expands to meet societal energy needs, so do the impacts on wildlife. Raptors in particular are highly susceptible to anthropogenic caused mortality. Industry sectors are encouraged to offset these causes of mortality. Several options to mitigate these losses have been proposed, including raptor rehabilitation. However, its role as a conservation tool is untested. Currently, no peer-reviewed demographic analyses exist using post-release data from rehabilitated raptors to evaluate its effectiveness at continental scales. Our objectives were to estimate annual survival of rehabilitated and wild raptors, and then use those estimates in demographic models to assess potential effects at individual and population levels. We hypothesized that rehabilitated raptors would survive similarly to their wild counterparts after an acclimation period, and that longer-lived species (<em>K-</em>selected) would benefit most from these releases. We used U.S. Geological Survey Bird Banding Lab band-recovery data (1974 – 2018) from 20 raptor species for modeling survival of rehabilitated individuals (<em>n </em>= 125,740) in comparison to wild birds (<em>n </em>= 1,913,352). Results from 17 species with adequate recovery data indicated that 5 species rehabilitated ≠ wild survival, 2 species had uncertain estimates, and 10 species rehabilitated ≈ wild survival by years 2 and 3 post-release. We acquired admission (<em>n </em>= 69,707) and release (<em>n = </em>25,740) data from 24 rehabilitation centers across the U.S. (2012-2021). We integrated survival, fecundity, and numbers of releases into demographic models. These models quantified the extent to which rehabilitated raptors may contribute to broader conservation efforts, especially in the context of individual take. All but two species had measurable numbers of individuals added to the population regardless of the number of releases. The general pattern was for <em>K</em>-selected species to yield larger benefits from rehabilitated supplementation to the population. These results provide evidence that rehabilitation may serve as a mitigation tool to offset incidental take.</p>

The role of rehabilitation and vitamin D supplementation on motor and psychological outcomes in poststroke patients.

<p>Post-Stroke depression affects between 12% and 72% of patients who have suffered a stroke. The association between low serum levels of 25-hydroxyvitamin D (25(OH) D) and increased risk of depression is reported in both stroke and non-stroke patients. Similarly, high 25(OH) D levels might be associated with greater functional improvement during rehabilitation program.</p> <p>We wanted to investigate the effects of an intensive rehabilitation on poststroke outcomes. We wondered if the daily rehabilitation of motor and cognitive functions could also have an effect on mood and functional abilities in addition to or as an alternative to vitamin D supplementation.</p> <p>We conducted a 12-week, randomized trial, double blind, parallel, monocentric clinical trial of 40 patients undergoing intensive neuro-rehabilitation treatment at a specialized care facility for ischemic or hemorrhagic brain stroke. Participants were randomly assigned, in a 1:1 ratio, to 1 of 2 parallel groups: in the experimental group, 2000 IU/day of oral cholecalciferol was administered; in the control group patients were not taking vitamin D supplementation. Patients underwent a text evaluation to investigate psychological and motor outcomes.</p> <p>Significant intra-group difference in outcomes measures was found but not between control group and experimental group. In the vitamin D group, we highlighted significant differences between T0 and T1 in calcium (<em>P</em> &lt; .001), vitamin D (<em>P</em> &lt; .001), in Montgomery Aasberg Depression Rating Scale (<em>P</em> = .001), and in Functional Independent Measures (<em>P</em> &lt; .001). In the health control group, we found a significant difference in calcium (<em>P</em> = .003), vitamin D (<em>P</em> &lt; .001), Montgomery Aasberg Depression Rating Scale (<em>P</em> = 0.006), in general self-efficacy (<em>P</em> = .009), and in Functional Independent Measures (<em>P</em> &lt; .001).</p> <p>Our results show that the beneficial effect on mood and functional recovery is mainly due to neurorehabilitation rather than vitamin D supplementation.</p>

The impact of the SARS-COV2 infection on the disorder of consciousness rehabilitation unit

<p>Disorders of consciousness include coma (cannot be aroused, eye remain closed), vegetative state&mdash;VS (can appear to be awake, but unable to purposefully interact) and minimally conscious state&mdash;MCS (minimal but definite awareness). The objective of this study is to assess the impact of the SARS-CoV-2 infection on the Disorder of Consciousness (DOC) Rehabilitation Unit.This is a retrospective, longitudinal, descriptive, observational, pilot study. We consecutively enrolled 18 patients (age range: 40&ndash;72 years, 9 females and 9 males), from three to five months after a brain injury. They were grouped into VS (n = 8) and MCS (n = 10). A confirmed case of COVID-19 was defined as a positive result on high-throughput sequencing or real-time reverse-transcription polymerase chain reaction analysis of throat swab specimens. We collected data of lung Computed Tomography (CT) and laboratory exams. DOC patients who were positive for SARS-CoV-2 were classified into severe and no severe infected group, according to the American Thoracic Society guidelines.A total of 18 hospitalized patients with (16) and without confirmed (2) SARS-CoV-2 infection were included in the analysis. After one month, a follow-up clinical evaluation reported that one patient died, one patient was transferred from Covid Unit to Emergency Unit and 3 patients were resulted negative to double swab and they returned to Rehabilitative Unit. Significant differences were reported about hypertension, cardiac disease and respiratory problems between the patients with severe infection and patients without severe infection (P&lt; 0.001). The laboratory findings, such as blood cell counts (<em>P</em> &lt; 0.001), C-reactive protein, D-dimer, potassium and vitamin D levels, seemed to be considered as useful prognostic predictors.To our knowledge, this is the first longitudinal study on a sample of chronic DOC patients affected by SARS-CoV-2. This study may offer important new clinical information on COVID-19 for management of DOC patients. Our findings showed that for the subjects with severe infection due to COVID-19, rapid clinical deterioration or worsening could be associated with clinical and laboratory findings, which could contribute to high mortality rate. During the COVID-19 epidemic period, the clinicians should consider all the reported risk factors to avoid delayed diagnosis or misdiagnosis and to prevent the infection transmission in DOC Rehabilitation Unit.</p>

Cortical Reorganization after Rehabilitation in a Patient with Conduction Aphasia Using High-Density EEG

<p>Conduction aphasia is a language disorder occurred after a left-brain injury. It is characterized<br> by fluent speech production, reading, writing and normal comprehension, while speech repetition is<br> impaired. The aimof this study is to investigate the cortical responses, induced by language activities,<br> in a sub-acute stroke patient affected by conduction aphasia before and after an intensive speech therapy<br> training. The patientwas examined by usingHigh-Density Electroencephalogram(HD-EEG) examination,<br> while was performing language tasks. The patient was evaluated at baseline and after two months<br> after rehabilitative treatment. Our results showed that an intensive rehabilitative process, in sub-acute<br> stroke, could be useful for a good outcome of language deficits. HD-EEG results showed that left<br> parieto-temporol-frontal areas were more activated after 2 months of rehabilitation training compared<br> with baseline. Our results provided evidence that an intensive rehabilitation process could contribute to<br> an inter- and intra-hemispheric reorganization.</p>

Impact of a pulmonary rehabilitation program on social disadvantage and physical activity data of postCOVID19 patients: A North-African pilot study

<p><strong>WHAT IS ALREADY KNOWN</strong></p> <ul> <li>In post-acute COVID19, many patients still manifest symptoms for weeks and months.</li> <li>Psychological manifestations are prevalent and persisting in recovered COVID19 patients.</li> <li>Indeed, COVID19 was reported to possibly lead to symptoms of depression and anxiety, and to alter health-related quality of life (HRQoL).</li> <li>Physical activity has been proven to be effective in improving both mental and physical health.</li> <li>Thus, COVID19 patients could benefit from pulmonary rehabilitation program (PRP).</li> <li>Many studies related the effects of a standard PRP on physical activity and HRQoL in post COVID19 patients. However, to the best of the authors&rsquo; knowledge, no study evaluated the physical activity and social disadvantage of patients who were hospitalized for COVID19 in a low income country.</li> <li>This paper is the second part of a cross sectional study that aimed to evaluate the impact of a PRP on social disadvantage (physical activity, hospital anxiety and depression (HAD), HRQoL) data of Tunisian patients with postCOVID19.</li> </ul> <p><strong>WHAT THIS PAPER ADDS</strong></p> <ul> <li>The present North African study demonstrated that a PRP improved HRQoL, HAD, and physical activity data of Tunisian COVID19 patients.</li> <li>For instance, the means of the functional, psychological and relation dimensions, and the VQ11 total score (ie; main outcome) decreased significantly by 1.79, 2.00, 1.57, and 5.36 points. In addition, the means of the HAD anxiety and depression scores decreased significantly by 2.07, and 2.57 points, respectively, and the means of the physical activity and total scores improved significantly by 1.75 and 1.78 points, respectively.</li> <li>To the best of the authors&rsquo; knowledge, this is the first African and Arab study investigating the impact of a PRP on postcovid19 patients in terms of social disadvantages and physical activity.</li> <li>&nbsp;</li> </ul> <ul> <li>This study confirms that an outpatient PRP program improves physical activity, psychological status and HRQoL in the management of postCOVID19 patients.</li> </ul>

Effectiveness of the use of virtual reality rehabilitation in chil-dren with dyslexia: follow up after one year.

<p><span>Dyslexia is a common learning disorder that hinders reading fluency and comprehension. Traditional treatments can be tedious for children, limiting their effectiveness. This study investigated the one-year effects of rehabilitation treatment with <a name="_Hlk163731836"></a>Virtual Reality Rehabilitation System (VRRS) on children with dyslexia. 24 children were divided into a control (CG) and experimental (EG) group. The CG underwent conventional neuropsychological treatment (CNT) while the EG underwent VR neurorehabilitation training (VRNT) using the VRRS. Neuropsychological evaluation was conducted before treatment, after six months, and again after one year for both groups. Compared to the control group, children who received VR training showed significant improvement in reading skills, especially in non-word reading and reading speed, even after one year without further VR intervention. VRRS can improve treatment adherence and minimize symptoms by offering engaging activities for children. These findings suggest VRRS may be a valuable tool for dyslexia rehabilitation with long-lasting effects.</span></p>

Prediction and Rehabilitation of Highway Embankment Slope Failures in Changing Climate

<p>Corresponding data set for Tran-SET Project No. 17GTLSU04. Abstract of the final report is stated below for reference:</p> <p>&quot;Highway slopes constructed with clayey soil are prone to desiccation cracks due to wetting and drying weather cycle, which allows greater moisture infiltration into the embankment from precipitation. Fissures formed due to extended wetting and drying cycles allow water to seep deeper into the soil than surficial wetting and increase the water content. This increases the moisture content in the soil and results in reduction in shear strength to the fully softened strength. On the other hand, development of desiccation cracks and reduction of the soil matric suction ultimately result in higher hydraulic conductivity value which causes development of higher pore water pressure. As the moisture content of the clayey soil increases, the strength reduces to a fully softened shear strength that causes frequent shallow and medium slope failures that are oriented approximately parallel to the surface of the embankment. Hence, the fully softened strength of Louisiana and Texas soils need to be quantified to develop a predictive tool for identifying high-risk zones of highway embankments. Given the documented failures in Texas and Louisiana, this research project is focused on investigating past failures to develop lessons learned and guidelines that can be implemented in the predictive framework.&quot;</p>

Bilateral Stand-alone Robotic Motion-Assisted Finger Exoskeleton for Home Rehabilitation

<p><span>This presents a novel exoskeleton robot which can be used at home for rehabilitating the index fingers of stroke-affected patients. This exoskeleton is designed as a one degree-of-freedom four bar mechanism able to guide the human index finger motion. The proposed device is the only lateral, stand-alone mechanism built till date which can carry the weight of the human hand, thus making the user free from wearing it. The design starts by tracing the trajectory of the index finger using &lsquo;Angulus&rsquo; software. &lsquo;SALAR&rsquo; software is used for dimensional synthesis of four bar mechanism. Using additive manufacturing technology, a prototype of the proposed device is developed. Static force analysis is done to select the most appropriate actuator for producing the required torque to manipulate the fingers effectively. The kinematics of the index finger while performing finger curl exercise is obtained. The proposed linkage mechanism can drive the index fingers of both the hands. Simulation and experimental results proved feasibility and effectiveness of the proposed design to be used for index finger rehabilitation for a wide range of users and applications by making simple minor alterations in the design. Also, the device can be used for rehabilitating the middle fingers together with the index fingers.</span></p>

Building better conservation media for primates and people: A case study of orangutan rescue and rehabilitation YouTube videos

<p>1. Conservation organizations rely on social/internet media platforms to raise awareness and fundraise. Social media is a double-edged sword: it can be a wide-reaching and effective tool for education and fundraising, but can also have counter-productive impacts on public views toward wildlife and understanding of wildlife conservation.</p> <p>2. For example, depicting humans interacting with wildlife in media may increase video popularity, but animals shown in anthropogenic contexts are also viewed as appealing pets. We are interested in understanding whether this is true for social media posts (YouTube videos) by orangutan rescue and rehabilitation organizations, which rely on social media for fundraising and awareness-raising. Our goal is to provide data and recommendations to guide these organizations in building media with positive conservation impact while minimizing potential negative effects.</p> <p>3. Using YouTube analytics and sentiment analysis of comments on 118 videos, we ask how viewer responses to videos vary with 1) the amount of human-orangutan interaction depicted, 2) the ages of the orangutans featured, and 3) the mention of threats to orangutans.</p> <p>4. Videos with longer human-orangutan interaction time were viewed more, but comments on them were significantly more likely to be negative toward Indonesian/Malaysian people. Comments on orangutan rescue/rehabilitation videos were more likely to be categorized as negative for orangutan conservation compared to videos about orangutans generally, and within these, so were comments on videos featuring infant and juvenile orangutans.</p> <p>5. Based on our findings, we recommend that orangutan rescue and rehabilitation organizations feature adult and mixed age groups of orangutans rather than infants and juveniles, minimize the amount of human-orangutan interaction shown, and talk about conservation threats to orangutans in their videos. We also recommend that, as a precaution, other primate rescue and rehabilitation groups also abide by these suggestions.</p>

A case report on intensive, robot-assisted rehabilitation program for brainstem radionecrosis

<p>Radiotherapy is a valid treatment option for nasopharyngeal carcinoma. However, complications can occur following irradiation of the closest anatomical structures, including brainstem radionecrosis (BRN). The rehabilitation is poorly described in patients with BRN, despite its usefulness in improving functional independence in patients with brain tumors. We aimed at testing the usefulness of intensive, robot-assisted neurorehabilitation program to improve functional independence in a 57-year-old male with BRN.</p> <p>Patient concerns:&nbsp;</p> <p>A 57-year-old male diagnosed with a nasopharyngeal carcinoma, received a radiation total dose of 72 Gy. Owing to the appearance of a severe symptomatology characterized by dysphagia, hearing loss, and left sided hemiparesis, the patient was hospitalized to be provided with intensive pharmacological and neurorehabilitation treatment.</p> <p>Diagnosis:&nbsp;</p> <p>Follow-up brain magnetic resonance imaging disclosed no residual cancer, but some brainstem lesions compatible with BRN areas were appreciable.</p> <p>Intervention:&nbsp;</p> <p>The patient underwent a 2-month conventional, respiratory, and speech therapy. Given that the patient only mildly improved, he was provided with intensive robot-aided upper limb and gait training and&nbsp;virtual reality-based cognitive rehabilitation for other 2 months.</p> <p>Outcomes:&nbsp;</p> <p>The patient reported a significant improvement in functional independence, spasticity, cognitive impairment degree, and balance.</p> <p>Conclusion:&nbsp;</p> <p>Our case suggests the usefulness of neurorobotic intensive rehabilitation in BRN to reduce functional disability. Future studies should investigate whether an earlier, even multidisciplinary rehabilitative treatment could lead to better functional outcome in patients with BRN.</p>
